520 Ringkasan/Abstrak/IntisariLatar Belakang: Stunting dan permasalahan kesehatan periodontal merupakan masalah kesehatan masyarakat di Indonesia. Stunting adalah terganggunya proses tumbuh kembang tubuh anak yang turut berperan dalam mempengaruhi bakteri di rongga mulut karena berkurangnya daya tahan terhadap bakteri patogen yang menjadi faktor pembentukan biofilm pada gigi yang berujung pada permasalahan kesehatan periodontal. Biofilm merupakan kolonisasi mikrobiom oral yang ditandai dengan pembentukan pelikel. Salah satu bakteri yang berperan dalam permasalahan kesehatan periodontal, yakni Treponema denticola. Treponema denticola merupakan salah satu bakteri ?red complex? late colonizer yang melekat di biofilm dan ditemukan saat anak mengalami periode mixed dentition. Permasalahan jaringan periodontal yang sering terjadi pada anak adalah gingivitis sehingga diperlukan pengendalian bakteri yang dapat dilakukan menggunakan bahan yang bersifat antibakteri. Salah satunya adalah pemakaian bahan kumur berbahan dasar minyak atsiri atau essential oils dan chlorhexidine gluconate 0,2%. Namun, belum banyak penelitian yang membahas efektivitas bahan kumur tersebut terhadap proporsi bakteri Treponema denticola dan kesehatan periodontal pada anak stunting. Tujuan: Penelitian ini bertujuan untuk menganalisis efektivitas sebelum dan setelah pemberian jangka pendek bahan kumur chlorhexidine gluconate 0,2% dan essential oils terhadap Treponema denticola dan kesehatan periodontal pada anak stunting. Metode: Sejumlah 28 subjek yang telah memenuhi kriteria inklusi dan eksklusi penelitian dibagi secara acak dalam 3 kelompok perlakuan, yakni kontrol negatif (tanpa pemberian bahan kumur), kontrol positif (chlorhexidine gluconate 0,2%), dan essential oils. Subjek penelitian diminta berkumur selama 30 detik, dua kali sehari pagi dan sebelum tidur selama 2 minggu. Sebelum dan setelah perlakuan subjek diminta untuk meludahkan saliva ke tube 15mL dan dilakukan pemeriksaan kesehatan periodontal dengan Simplified Basic Periodontal Examination (sBPE). Saliva yang telah dikumpulkan akan dilakukan ekstraksi DNA dan diuji menggunakan qPCR. Data yang terkumpul dianalisis menggunakan Shapiro-Wilk untuk uji normalitas, Wilcoxon signed-rank test untuk uji hipotesis non-parametrik, dan Spearman untuk uji korelasi. Hasil: Hasil penelitian menunjukkan bahwa berkumur dengan essential oils dan chlorhexidine gluconate 0,2% mampu menurunkan proporsi bakteri Treponema denticola dalam saliva (p ? value < 0,05). Pemberian chlorhexidine gluconate 0,2% efektif mampu meningkatkan kesehatan periodontal secara signifikan (p ? value < 0,05). Pemberian essential oils mampu meningkatkan kesehatan periodontal, namun tidak signifikan (p ? value > 0,05). Terlihat adanya kecenderungan apabila terjadi penurunan proporsi bakteri Treponema denticola akan terjadi penurunan persentase parameter klinis kesehatan periodontal (sBPE) pada kelompok intervensi essential oils dan chlorhexidine gluconate 0,2%. Penurunan persentase parameter klinis kesehatan periodontal (sBPE) menandakan terjadinya peningkatan kesehatan periodontal. Kesimpulan: Terdapat efektivitas sebelum dan setelah pemberian bahan kumur chlorhexidine gluconate 0,2% dan essential oils terhadap proporsi Treponema denticola. Terdapat efektivitas sebelum dan setelah pemberian bahan kumur chlorhexidine gluconate 0,2% terhadap peningkatan kesehatan periodontal dan terbukti signifikan, namun pemberian bahan kumur essential oils tidak signifikan. Tidak terdapat korelasi antara proporsi bakteri dan kesehatan periodontal pada dengan chlorhexidine gluconate 0,2% dan essential oils, namun terlihat adanya tren korelasi positif. ......Background: Stunting and periodontal health problems are public health problems in Indonesia. Stunting is a disruption in the growth and development of children's bodies which also plays a role in influencing bacteria in the oral cavity due to reduced resistance to pathogenic bacteria which are a factor in the formation of biofilms on teeth which lead to periodontal health problems. Biofilm is a colonization of the oral microbiome which is characterized by the formation of pellicle. One of the bacteria that plays a role in periodontal health problems is Treponema denticola. Treponema denticola is one of the "red complex" late colonizer bacteria that adheres to biofilms and is found when children experience mixed dentition. Periodontal tissue problems that often occur in children are gingivitis so that bacterial control is needed which can be done using antibacterial materials. One of them is the use of mouthwash made from essential oils or essential oils and 0.2% chlorhexidine gluconate. However, there has not been much research discussing the effectiveness of these mouthwashes on the proportion of Treponema denticola bacteria and periodontal health in stunted children. Objective: This study aims to analyze the effectiveness before and after short-term administration of 0.2% chlorhexidine gluconate mouthwash and essential oils on Treponema denticola and periodontal health in stunted children. Method: A total of 28 subjects who met the inclusion and exclusion criteria of the study were randomly divided into 3 treatment groups, namely negative control (without administration of mouthwash), positive control (0.2% chlorhexidine gluconate), and essential oils. The study subjects were asked to gargle for 30 seconds, twice a day in the morning and before going to bed for 2 weeks. Before and after treatment, the subjects were asked to salivate into a 15 mL tube and undergo a periodontal health examination with the Siplified Basic Periodontal Examination (sBPE). The saliva that had been collected would be subjected to DNA extraction and tested using Real-time PCR. The data collected were analyzed using the Shapiro-Wilk for normality testing, the Wilcoxon Signed-Rank Test for non-parametric hypothesis testing, and Spearman for correlation testing. Results: The results showed that gargling with essential oils and 0.2% chlorhexidine gluconate was able to reduce the proportion of Treponema denticola bacteria in saliva (p-value < 0.05). Administration of 0.2% chlorhexidine gluconate was effective in significantly improving periodontal health (p-value < 0.05). Administration of essential oils was able to improve periodontal health, but not significantly (p-value> 0.05). There was a tendency that if there was a decrease in the proportion of Treponema denticola bacteria, there would be a decrease in the percentage of clinical parameters of periodontal health (sBPE) in the intervention group of essential oils and 0.2% chlorhexidine gluconate. The decrease in the percentage of clinical parameters of periodontal health (sBPE) indicated an increase in periodontal health. Conclusion: There was effectiveness before and after administration of 0.2% chlorhexidine gluconate mouthwash and essential oils on the proportion of Treponema denticola. There is effectiveness before and after administration of 0.2% chlorhexidine gluconate mouthwash on improving periodontal health and it is proven to be significant, but administration of essential oil mouthwash is not significant. There is no correlation between the proportion of bacteria and periodontal health with 0.2% chlorhexidine gluconate and essential oils, but there is a positive correlation trend.
