Latar belakang: Nyeri pasca bedah merupakan fenomena yang subyektif. Penelitian ini untuk membandingkan efek analgetik NSAID dan PCA dengan Elektroakupunktur dan PCA pada nyeri pasca bedah Caesar.
Metodologi: 38 wanita yang mendapatkan anestesi spinal selama menjalani bedah Caesar di Departemen Obstetrik dan Ginekologi di RSUPN Cipto Mangunkusumo, dibagi secara acak menjadi kelompok NSAID dan PCA serta kelompok Elektroakupunktur dan PCA. Setelah selesai menjalani pembedahan subyek diberikan NSAID atau mendapat stimulasi Elektroakupunktur dan kemudian dipasang PCA. Waktu pertama kali membutuhkan morfin dan dosis PCA yang digunakan dicatat.
Hasil: Hasilnya menunjukkan bahwa kelompok Elekroakupuntur dan PCA (205 menit) menunda waktu kebutuhan untuk morfin 25 menit lebih lama dibandingkan dengan kelompok NSAID dan PCA (180 menit). Dosis total PCA pada 24 jam pertama berkurang 25 % pada kelompok Elektroakupunktur dan PCA (4,5 mg) dibanding kelompok NSAID dan PCA (6 mg), sehingga tidak terdapat perbedaan bermakna. Pada kelompok NSAID dan PCA maupun kelompok Elektroakupunktur dan PCA tidak didapatkan efek samping yang berhubungan dengan opioid seperti pusing.
Kesimpulan: Tidak terdapat perbedaan dalam waktu kebutuhan analgetik tambahan pasca bedah Caesar dan dosis PCA 24 jam pada kelompok NSAID dan PCA dengan Elektroakupunktur dan PCA.
Background: Post-operation pain is a very subjective phenomenon. The aim of this study was to compare the analgesic effects of NSAID and PCA or Electro-acupunture and PCA on post-cesarean pain.Methods: 38 women, who had had spinal anesthesia during cesarean section at the Department of Obstetrics of Cipto Mangunkusumo Hospital, were randomly assigned to the NSAID and PCA group and the electro-acupuncture and PCA group. After the operation, we applied subjects with NSAID or Electro-acupuncture, and the patient controlled analgesia (PCA). The first time of requesting morphine and the doses of PCA used were recorded.Results: The results showed that the Electro-acupuncture and PCA group (205 minutes) could delay the time of requesting morphine up to 25 minutes when compared with the NSAID and PCA group (180 minutes). The total dose of PCA used within the first 24 hours was 25 % less in the Electro-acupuncture and PCA group (4,5 mg) when compared with the NSAID and PCA group (6 mg), which no significant difference between the NSAID and PCA group and the Electro-acupuncture and PCA group. Finally, the incidence of opioid-related side effects, such as dizziness, was not record in the NSAID and PCA group or Electro-acupuncture and PCA group.Conclusion: There was no different in the time of requesting pain relief medication after cesarean section and the PCA doses used within the first 24 hours in NSAID and PCA group or Electro-acupuncture and PCA group.
