Karya Ilmiah Akhir ini merupakan laporan dari rangkaian kegiatan praktik residensi Ners Spesialis Keperawatan Medikal Bedah, dengan penerapan Teori Care, Cure, Core Lydia E Hall pada proses keperawatan pasien gangguan sistem kardiovaskular, penerapan praktik keperawatan berdasarkan pembuktian, dan melakukan inovasi keperawatan. Pengkajian pasien dengan teori Lydia Eloise Hall berfokus pada hubungan dan reaksi antara perawat-pasien dan juga keluarga yang terdiri dari tiga aspek care, cure, core. Pelaksanaan praktik keperawatan berdasarkan pembuktian, dilakukan untuk pengelolaan pasien yang menggunakan alat kateter intravena perifer agar dapat terhindari dari phlebitis dan infiltrasi serta komplikasinya. Pelaksanaan inovasi keperawatan berupa pelaksanaan praktik konsultasi keperawatan bagi pasien gagal jantung di unit rawat jalan RSJPDHK.

This scientific work is report of a series of practice residency Nurse Medical Surgical Nursing Specialist, with the application of Theory Care, Cure, Core Lydia E Hall on the nursing process with patients cardiovascular disorder, application of evidence-based nursing practice, and nursing innovation. Assessment of patients with Lydia Eloise Hall theory focuses on the relationship between nurse and patient reactions and also a family, that consisting of three aspects of care, cure, cores. Implementation of evidence-based nursing practice, performed for the management of patients with peripheral intravenous catheters using the tool of phlebitis and infiltration in order to avoid its complications. Implementation of nursing innovations in the in the outpatient unit of RSJPDHK and implementation of nursing practice consultations for patients with heart failure.
