ABSTRAK
Proses Konversi lsquo;Very Large Crude Carrier rsquo; Tanker menjadi lsquo;Floating Storage Offloading rsquo; FSO merupakan satu acuan pilihan sebagai sarana eksplorasi dalam industri Minyak dan Gas Bumi. Pada pelaksanaan proses konversi proyek ini teridentifikasi beberapa tahapan yang memiliki resiko dan mempengaruhi kinerja proyek secara keseluruhan,berdasarkan pada temuan kondisi tersebut penulis melakukan penelitian melalui suatu evaluasi proses konversi proyek dengan berbasis risiko.Penelitian ini menggunakan data primer dan sekunder baik dari kuesioner maupun arsip dokumen yang dianalisa menggunakan qualitative risk analysis untuk mendapatkan level risiko dari faktor risiko yang berasal dari setiap kegiatan pelaksanaan proses konversi proyek FSO dimana ditemukan bahwa Pengadaan Long Lead Item, Prakualifikasi EPCI Kontraktor dan Studi kelayakan enjiniring permulaan merupakan risiko dominannya. Selanjutnya dilakukan evaluasi dan diketahui bahwa mitigasi risiko merupakan respon prefentive yang tepat. Sementara, tindakan korektif yang tepat adalah melakukan pembagian paket pengadaan , merevisi syarat prakualifikasi dan melibatkan pakar enjiniring. Sayangnya respon risiko tersebut masih belum berjalan optimal bahkan terdapat respon yang belum diterapkan untuk itu dilakukanlah beberapa improvisasi dalam pengembangan sistem konversi.

ABSTRACT
The 39 Very Large Crude Carrier 39 Tanker Converting Process to 39 Floating Storage Offloading 39 FSO is a reference choice as a means of exploration in the Oil and Gas industry. In the implementation of this project conversion process identified several stages that have risks and affect the overall performance of the project, based on the findings of these conditions the authors conduct research through an evaluation of project conversion process with risk based methode. This research using primary and secondary data from both the questionnaire and archive documents and analyzed using qualitative risk analysis to obtain the risk level of risk factors derived from each activity of FSO project conversion process where it was found that the Procurement of Long Lead Item, Prequalification of EPCI Contractor and Feasibility Study of Engineering is most dominant risk factor. Further evaluations are made and it is known that risk mitigation is an appropriate prefentive response. Meanwhile, appropriate corrective action is to procure procurement packages, revise the prequalification requirements and involve engineering experts. Unfortunately the risk response is still not running optimally even there is a response that has not been applied for that done some improvisation in the development of FSO conversion system.
