 Abstract

ABSTRAK
Penatalaksanaan histerektomi dapat menjadi indikasi medis pada kasus mioma uteri sehingga dapat menyebabkan perubahan konsep diri pada perempuan. Studi kasus ini bertujuan menganalisis penerapan model adaptasi Roy dan teori transisi Meleis dalam asuhan keperawatan klien post histerektomi atas indikasi mioma uteri. Justifikasi penerapan teori tersebut untuk memfasilitasi klien dalam proses transisi yang membutuhkan rentang waktu tertentu agar dapat mencapai koping yang adaptif. Masalah keperawatan yang muncul post operasi antara lain gangguan perfusi jaringan perifer, nyeri berhubungan dengan insisi bedah, kesiapan meningkatkan konsep diri, dan risiko disfungsi seksual. Implementasi yang dilakukan antara lain memonitor tanda-tanda vital, manajemen nyeri farmakologis dan non farmakologis, mengeksplorasi aspek positif klien, dan pendidikan kesehatan mengenai gejala menopause dini. Pendekatan model adaptasi Roy dan teori transisi Meleis dapat membantu klien dalam penguasaan dan peningkatan kepercayaan diri pada klien apabila perawat melakukan asuhan keperawatan berkelanjutan untuk meninjau proses transisi klien dalam rentang waktu tertentu.

ABSTRACT
Abstract Hysterectomy could be a medical indication in cases of uterine myoma that may lead to changes in self concept in women. This case study aims to analyze the application of Roy 39 s adaptation model and Meleis transition theory in nursing care of post hysterectomy clients on the indication of uterine myoma. Justify the application of the theory is to facilitate clients in the transition process that requires a certain time in order to achieve an adaptive coping. Post operative nursing problems include peripheral tissue perfusion ineffective, pain associated with surgical incision, readiness to enhance self concept, and risk of sexual dysfunction. Nursing intervention include monitoring vital signs, pharmacological and nonpharmacologic pain management, exploring positive aspects of the client, and health education about early menopause symptoms. Roy 39 s model adaptation and Meleis transition approach can assist clients in the mastery and enhancement of confidence if the nurse carries continuum of care to review the client 39 s transition process in a range of time.
