Penyakit jantung koroner dan stroke, termasuk Penyakit Tidak Menular, merupakan komplikasi dari hipertensi, salah satu penyebab kematian utama di dunia. Hipertensi dapat ditangani lebih efektif dengan kolaborasi interprofesi. Pelayanan kesehatan primer di DKI Jakarta menerapkan kolaborasi interprofesi dalam menangani hipertensi salah satunya dengan PIS PK KPLDH. Tujuan penelitian untuk mengetahui gambaran kolaborasi interprofesi pada pelayanan kesehatan hipertensi dengan pendekatan keluarga. Penelitian dilakukan pada 6 Puskesmas di Jakarta Timur dengan responden tenaga kesehatan yang tergabung dalam tim PIS PK KPLDH, yakni dokter, perawat, dan bidan. Desain penelitian menggunakan deskriptif cross-sectional dengan sampel 219 responden. Instrumen yang digunakan adalah Collaborative Practice Assessment Tool (CPAT) versi Bahasa Indonesia. Hasil penelitian dikategorikan menjadi dua yaitu kolaborasi baik (52,05%) dan kurang baik (47,95). Hasil penelitian dapat menjadi data dasar yang digunakan dalam refleksi dan evaluasi untuk meningkatkan praktik kolaborasi interprofesi dalam pelayanan kesehatan.

The coronary heart disease and stroke, including non-communicable diseases, are the complications of hypertension, one of the deadliest diseases in the world. Hypertension can be effectively treated by an interprofession collaboration. The primary health service in DKI Jakarta employed the interprofession collaboration in treating hypertension, such as by PIS, PK, KPLDH. The aim of this study is to know the description of the interprofession collaboration in the hypertension health service by using the family approach. This study was conducted in 6 (Public Health Centre) in East Jakarta with the respondents of health workers included in the team of PIS, PK, KPLDH; the workers were doctors, nurses, and midwives. The design of this research employed the cross-sectional descriptive with the sample of 219 respondents. Collaborative Practice Assessment Tool (CPAT) Bahasa Indonesia version was used as the research instrument. The results of this study are divided into two categories; good collaboration (52,05%) and mediocre collaboration (47,95%). This results can be the basic data which are used as a reflection and evaluation to improve the interprofession collaboration practicum in health services.
