Gambaran kejadian medication error pada rumah sakit di berbagai negara: Literature review = Overview of medication error incidence in hospitals in various countries: Literature review

 Abstract

Pendahuluan: Keselamatan pasien merupakan isu penting dalam aspek pelayanan rumah sakit. Medication error merupakan kesalahan medis yang paling umum terjadi. Terdapat sekitar 0,1 juta orang meninggal setiap tahun akibat kesalahan medis di rumah sakit dan angka kematian akibat kesalahan pengobatan lebih tinggi dibandingkan cedera di tempat kerja. Diperkirakan dari tahun 2013 sebanyak 9.5% kematian diakibatkan oleh medication error. Biaya yang dikeluarkan terkait kesalahan pengobatan secara global dapat mencapai USD 42 miliar per tahunnya.
Metode: Studi menggunakan metode literature review dengan penelusuran studi melalui database ProQuest, ScienceDirect, Pubmed, Google Scholar, dan Garuda RistekBRIN dengan tahun publikasi pada rentang 2011 hingga 2021.
Hasil dan Diskusi: Dari total 11 studi terinklusi didapatkan angka kejadian medication error dengan persentase yang bervariatif dimulai dari 6,10% hingga 87,25%. Fase yang paling banyak terjadi ialah prescribing error dengan urutan tertinggi, selanjutnya disusul oleh administration error, transcribing error, dan dispensing error. Strategi pencegahan terjadinya medication error di rumah sakit dapat berupa penerapan sistem informasi berupa CPOE dan melakukan pelatihan pada staf yang dinilai mampu menurunkan dan mencegah terjadinya kesalahan pengobatan dan meningkatkan keselamatan pasien.

Introduction: Patient safety is an important issue in the aspects of hospital services. Medication errors are the most common medical errors. There are about 0.1 million people die each year due to medical errors in hospitals and the mortality rate from medication errors is higher than injuries in the workplace. It is estimated that from 2013 9.5% of deaths were caused by medication errors. The costs associated with medication errors globally can reach USD 42 billion per year.
Methods: The study uses the literature review method with study searches through the ProQuest, ScienceDirect, Pubmed, Google Scholar, and Garuda RistekBRIN databases with the publication year in the range of 2011 to 2021.
Results and Discussion: From a total of 11 included studies, the incidence of medicine error was found with varying proportions starting from 6.10% to 87.25%. The phase that occurs the most is prescribing error with the highest order, followed by administration error, transcribing error, and dispensing error. Strategies to prevent medication errors in hospitals can be in the form of implementing information systems in the form of CPOE and conducting training for staff who are considered capable of reducing and preventing medication errors and improving patient safety.
