Stroke iskemik merupakan obstruksi yang terjadi di pembuluh darah menuju otak akibat adanya sumbatan thrombus ataupun emboli. Pasien dengan Stroke iskemik mengalami penurunan sistem neurologi, salah satu tanda gejalanya adalah terganggunya kemampuan kognitif. Studi kasus ini bertujuan untuk menganalisis penerapan intervensi reminiscence therapy pada pasien stroke iskemik dengan gangguan kognitif. Metode yang diberikan adalah memberikan tiga sesi terapi reminiscence dengan tema yang dapat menstimulasi memori pasien. Waktu pemberian 15-60 menit sesuai dengan ambang focus pasien dalam terapi.  Hasil yang di dapatkan adalah terdapat perbaikan prognosis kognitif pada pasien ditunjukkan oleh peningkatan nilai MoCA dari 15 menjadi 23 setelah intervensi.  Efektifitas yang ditunjukkan reminiscence therapy ini dapat dijadikan sumber informasi dalam penerapan evidence based care pada praktik mandiri perawat dalam mengatasi masalah gangguan kognitif pada pasien stroke iskemik.

Ischemic stroke is an obstruction of blood vessel to the brain due to blockage of thrombus or embolism. Patient with ischemic stroke experience neurological deficit, one of major symptoms is cognitive impairment. The purpose of this paper is to analyze application of reminiscence therapy in ischemic stroke patient with cognitive impairment. The study case method is provide three session of therapy with a theme that can stimulate pastient memory. The administration time 15 – 60 minute according to patient focus. The result shows there is an improvement in the cognitive prognosis in patients as indicated by increasing MoCA score from 15 to 23 after intervenstion. The effectiveness shown by reminiscence therapy can be used as evidence based nursing care in patients ischemic stroke with cognitive impairment.
