Perubahan paradigma perumahsakitan di dunia juga membuat rumah sakit di Indonesia dapat sebagai subyek hukum, khususnya mulai tahun 2003 yang menandai kesepakatan pemberlakuan perdagangan bebas antar negara-negara ASEAN (AFTA).RS Mitra Kemayoran adalah rumah sakit swasta yang penyelenggaraan rekam medisnya telah menggunakan sistem kearsipan sentralisasi dan tahun 2002 ini mendapat sekitar 130 komplain medis pasien, yang terdiri dari berbagai macam kasus. Semua komplain tersebut cepat atau lambat akan berujung pada tuntutan hukum terhadap rumah sakit. Untuk mengantisipasi hal ini maka hal yang berkaitan dengan legal aspek dari komplain tersebut hams sudah tersusun dengan baik, Berkaitan dengan itu peneliti mengangkat masalah penelitian berupa belum diketahuinya tingkat kelengkapan berkas rekam medis di RS Mitra Kemayoran tahun 2002.Tujuan penelitian ini adalah untuk mengetahui tingkat kepatuhan dokter dalam mengisi rekam medis di RS Mitra Kemayoran 'tahun 2002, dalam hal ini diukur dengan kelengkapan pengisian rekam medis, keakuratan, ketepatan waktu dan pemenuhan pengisian aspek legal rekam medis. Kerangka pikir penelitian ini adalah sistem untuk menggali informasi pengisian berkas rekam medis mulai dari komponen Input, Proses dan Output, tetapi yang diteliti disini komponen Proses dan Output.Penggalian data diambil secara sekunder yaitu dari rekam medis yang sudah selesai diisi oleh dokter dan sudah dikembalikan ke bagian rekam medik.Melihat dari tujuannya, penelitian ini termasuk dalam penelitian kualitatif dengan wawancara mendalam dan penelitian kuantitatif dengan pendekatan cross sectional.Hasil penelitian ini adalah dari 130 rekam medis didapatkan pada lembar harian dan instruksi dokter tanda tangan/paraf dokter 63% lengkap, nama dokter hanya 12%-lengkap, sedang jam pemeriksaan pasien 18% lengkap,. Pada informed refusal didapatkan 40% lengkap, pulang paksa 62% lengkap, anamnesis dan pemeriksaan fisik 77% lengkap sedangkan informed consent operasi 92% lengkap. Dari in-depth interview baik pada kelengkapan pengisian rekam medis, keakuratan, ketepatan waktu dan aspek legalnya memang masih ada kekurangan dalam pengisian/pendokumantasian rekam medis.Kesimpulan penelitian ini adalah dari analisis kelengkapan rekam medis yang telah dilakukan serta in-depth interview pada para informan dapat disimpulkan bahwa para dokter di RS Mitra Kemayoran cukup patuh dalam mengisi rekam medis, namun memang masih banyak ketidak lengkapan pada berkas rekam medis tertentu. Hal ini dikarenakan kesadaran dokter akan pentingnya kegunaan rekam medis bagi dokter sendiri masih kurang, sehingga perlu pihak management RS Mitra Kemayoran mensosialisasikan pada dokter untuk mengisi rekam medis dengan lengkap dan pelaksanaannya perlu mendapat kontrol secara terus menerus oleh panitia rekam medik. Belum adanya No Blame Culture di RS Mitra Kemayoran.Saran yang diberikan oleh peneliti: Perlu ditinjau kembali isi informed consent, karena tampaknya hanya seperti surat persetujuan saja, yang sebenarnya harus ditulis pula apa yang sudah dijelaskan/diterangkan keterangan yang sudah diberikan pada pasien. Perlu dipikirkan pembuatan cap nama dokter yang spesifik dan sangat khas serta dipegang oleh dokter sendiri, yang dibuat oleh pihak rumah sakit untuk memecahkan salah satu masalah ketidaklengkapan rekam medis.Daftar Pustaka : 30 buah ( 1985- 2003)

Completeness Analysis of Medical Record on Complain Cases During the Year 2002 at Mitra Kemayoran HospitalHospital paradigm shift in the world has placed the present hospitals in Indonesia as a legal subject, especially starting the year 2003 which indicates the agreement of world free trade between ASEAN countries (AFTA).

Mitra Kemayoran Hospital is a private hospital that used Medical Record Centralized Document System. In the year 2002 this hospital has accepted 130 patient complains which consist of variety cases. All complains whether fast or slowly will end on law suit to the hospital. To anticipate this matter, all the connected legal aspect of that complains must be in a good arrangement. In connection with the complains researcher takes the research problem as the level of completeness of medical record is not yet known at Mitra Kemayoran Hospital in the year 2002.

The goal of this research is to get the information about doctor's compliance in completeness of writing medical record at Mitra Kemayoran Hospital in the year 2002, at this point to be measured with completeness of medical records, aquracy, punctuality and fulfillment medical record with fill in legal aspect. The framework of this research is system to dig information about the fill in medical record starting from Input, process and output component, however the process and output component to be researched in this study.

Data collection from secondary data which were finished medical record where the doctors already written and the already returned to the medical record department.

To see the goal of this research, this study is qualitative study with in-depth interview and cross sectional approach.

The result of this research was from 130 medical records can be found in loose paper daily report and doctor's instruction, doctor's signature 63% complete, doctor's name 12 % complete, while hours patient's examination 18% complete. In informed refusal can be found 40% complete. force discharged 62% complete, assessment and physical examination 77%' complete, informed consent for an operation 92% complete. From in-depth interview whether in completeness of medical records, aquracy, punctuality in time and legal aspect are still there are lacking in writing medical records documentation.

The conclusion of this research is from the analysis of completeness of medical records which has been done and with in-depth interview to the informant and can be concluded that the doctors at Mitra Kemayoran Hospital fair compliance in writing medical records, but there are many uncompleted medical records at certain parts. Perhaps this is due to the lack of awareness of the important of medical records and the use of them for the doctors themselves. It is needed for the management of Mitra Kemayoran Hospital to socialized to the doctors to write medical records and the implementation can be controlled continuously from the medical record's committee. At Mitra Kemayoran Hospital there is not yet established "No Blame Culture".

Suggestion given to the hospital there is a need to make an informed consent because it seems only as an agreement letter_ It must be written also what the doctor?s explanation, the information given to the patient. There is a need to be concerned about to have a medical doctor's name stamp that are specific and given to the doctor's him or herself. This stamp is made by the hospital to solve among others the problem of not complete medical records.
