Penetapan tarif Puskesmas selama ini belum mengacu pada suatu hasil analisis tingkat kernampuan & kemauan masyarakat dan biaya satuan pelayanan. Apakah dengan tarif yang sekarang berlaku sudah mendekati dengan biaya satuan pelayanan dan tingkat kemampuan & kemauan masyarakat membayar, maka dilakukanlah suatu penelitian/analisis tentang tarif ini di wilayah kerja Puskesmas Pesisir Kotamadya DT II Cirebon yang secara pusposif dipilih mewakili daerah yang kumuh dan miskin dengan tingkat penghasilan masyarakat yang rendah. Penelitian ini merupakan analisis diskriptif dengan rancangan cross sectional. Hasil penelitian menunjukkan bahwa dihitung dari total biaya Puskesmas biaya satuan untuk BP adalah Rp.7.488,77, KIA/KB adalah Rp.18.666,86, Imunisasi adalah Rp.1.789,98 dan Laboratorium adalah Rp.2.654,94. Sedangkan kalau dihitung dari biaya operasional & pemeliharaan (tanpa Annualized Fixed Cost + gaji) biaya satuan untuk BP Rp.6.251,40, KIA/KB Rp.15.038,71, Imunisasi Rp.836,21 dan Laboratorium Rp.943,14. Mengenai kemampuan masyarakat, pengeluaran bukan makan sebesar Rp.123.959,83, pengeluaran bukan makanan tanpa pasta sebesar Rp.120.326,00, dan pengeluaran non esensial sebesar Rp.31.648,93. Dilihat dari 5% pengeluaran bukan makanan, kemampuan masyarakat adalah Rp.6.197.99. Kemauan masyarakat pada pengandaian sakit rata-rata adalah Rp.995,83 dan pengandaian periksa KIA rata-rata sebesar Rp.813,21. Titik temu dengan kemampuan berdasarkan pengeluaran non esensial pada nilai Rp.800,00 dengan menyingkirkan 28% masyarakat. Sedangkan kemauan masyarakat dilihat dari pengandaian peningkatan kualitas pelayanan Puskesmas rata-rata sebesar Rp.1844,00, titik temu dengan kemampuan masyarakat berdasarkan pengeluaran non esensial pada nilai Rp.1000,00 dengan menyingkirkan 28% masyarakat. Tarif Rp.800,00 ini ternyata merupakan tarif maksimal di Puskesmas Pesisir. Tarif dapat ditingkatkan menjadi Rp.1000,00 asakkan kualitas pelayanan ditingkatkan tetapi dengan tetap menyingkirkan sebagian masyarakat. Masyarakat yang tersingkir perlu didukung dengan program Kartu Sehat. Hasil penelitian ini dapat dijadikan pertimbangan oleh Pemenntah Daerah Kotamadya Daerah Tingkat II Cirebon dalam menetapkan tarif pelayanan di Puskesmas.

The Analysis of Health Care Fee Viewed from the Level of Peoples Affordability in The Job Area of Puskesmas Pesisir Kotamadya DT II CirebonThe determining of health care fee has not so far been referred to the result of analyzing the level of affordability/ability and people's willingness to pay and the unit cost of service. To find are wether the current fee has approached the unit cost of health service and the affordability and willingness of the people, the research on the health care fee is conducted in the area of Puskesmas Pesisir Kotamadya DT II Cirebon, which is purposively selected to represent the vile and poor people with their very low income.

This research is a descriptive analysis using cross sectional design. The research points out that the unit cost of BP, KIAIKB, Immunization and laboratory are respectively Rp.7.488,77, Rp.18.666,86, Rp.1.789,98, Rp.2.654,94 counted from the total cost of the Puskesmas. On the other hand, if it is counted from the operational cost and maintenance (without Annualized Fixed Cost t salaries), the unit cost of BP, KIAIKB, Immunization and laboratory are Rp.6.251,40, Rp.15.038,71, Rp.836,21, Rp. 943,14 respectively. Concerning to the people's affordability, they spend Rp.123.959,83 for their non food need, Rp.120.326,00 for non food excluding their need for entertainment, and Rp.31,648,93 for the essential need. Viewed from 5% of the non food budget, the affordability is Rp.6.197,99.

The people's willingness to pay for health care service in ill condition is Rp.995,83 at the average and Rp.813,21 for KIA examination, The meeting point with the affordability based upon the non essential budget is at the value Rp.800,00 by rejecting 28% of the people. Mean while, the people's willingness viewed from the condition that the quality of health services is improved is Rp.1.844,00 at the average. The meeting point with the affordability based upon the non essential budget is at the value of Rp.1000,00 by rejecting 28% of the people. The health care fee as much as Rp.800,00 proved to be maximum in Puskesmas Pesisir. This fee can be amounted to Rp.1000,00 provided that the quality of health service is improved. Nevertheless, a number of the people are kept to be rejected. The rejected people are necessarily supported with the program of Health card (Kartu Sehat). The result of the research can be a consideration for the local goverment in determining the health care fee in Puskesmas.
