Analisis tarif Puskesmas ditinjau dari tingkat kemampuan masyarakat bertujuan untuk mendapatkan gambaran berapa tarif puskesmas yang realistis untuk suatu wilayah kerja tertentu dengan mempertimbangkan biaya satuan dan kemampuan serta kemauan masyarakat di wilayah kerja tersebut. Tarif' yang berlaku sekarang untuk suatu daerah tingkat II diberlakukan sama. Satu tahun sebalumnya berlaku sama untuk suatu daerah tingkat I. Penelitian dilakukan secara purposive di wilayah kerja Puskesmas Kalitanjung, Kotamadya Daerah Tingkat II Cirebon, yang merupakan salah satu wilayah kerja lapisan masyarakat perkotaan golongan menengah. Analisis Biaya Puskesma menggunakan data tahun anggaran:1995/1996, sedangkan kemampuan dan kemauan masyarakat dilakukan dengan cara survei. Sampel adalah rumah tangga yang berada dalam radius satu kilometer dari Puskesmas. Instrumen yang digunakan berupa kuesioner, yang pengumpulan datanya dilakukan dengan wawancara. Hasil penelitian analisis biaya dideskripsikan dalam biaya operasional dan pemeliharaan serta biaya satuan 5 program yaitu BP, KIAIKB, Kesehatan Gigi, imunisasi dan Laboratorium. Hasil survei kemampuan dan kemauan masyarakat dideskripsikan melalui analisis persentase masyarakat yang "tersingkir" pada tarif tertentu. Hasil temuan dalam penelitian antara lain: Tarif Puskesmas BP/KIA - KB sebesar Rp. 800,- yang baru diberlakukan bulan Oktober 1996 berada di bawah biaya satuan dan di bawah kemampuan masyarakat Kemauan membayar masyarakat meningkat, kalau kualitas pelayanan di tingkatkan. Dengan hasil terscebut kiranya tarif Puskesmas yang realistis harus disesuaikan dengan kemampuan dan kemauan masyarakat setempat, sehingga pemerataan pelayanan kesehatan dasar dapat dicapai. Peraturan Daerah Tingkat II mengenai tarif Puskesmas yang akan datang sebaiknya berlaku secara regional sesuai dengan kelompok lapisan masyarakat di wilayah kerja Puskesmas, dengan mengacu pada kemampuan dan kemauan masyarakatnya. Dengan demikian dapat terjadi lebih dari satu pola tarif Puskesmas dalam satu Daerah Tingkat II.

The objective of analyzing Puskesmas health care fee viewed from the level of people's affordability is to obtain the description on realistic fee in certain area with regard to the unit cost and the affordability and willingness of the people to pay for the fees in that area. The current fee of all Puskesmas throughout Dati II is applied the same. In the previous year, the fee was applied the same too for the Puskesmas in Dati I. The research is conducted purposively in the area of Puskesmas Kalitanjung, Kotamdya Dati II Cirebon, which is one of the municipals' areas of the middle class. The analysis of Puskesmas cost used in the research is taken from the fiscal year of 19951 1996, while the affordability and willingness of the people is obtained by doing a survey. The sample of the research are the households live approximately one kilometer from the Puskesmas. The tool of the research is in the form of questionnaires, the data gathering of which is carried out through interviews. The results of the cost analysis is described in the operational cost and maintenance and the unit cost at 5 programs i.e. BP, KIA/KB, Tooth care, Immunization, and Laboratory.

The result of surveying the affordability and willingness of the people is described by means of the analysis of percentage of individuals "rejected" at a certain fee. One of the findings of the research is as the following. The health care fee at Puskesmas BP/KIA-KB as much as Rp 800.-, which was set in October 1996, is below the unit cost and is also below the people's affordability. The people's willingness to pay for the fees increases when the quality of health service is improved. Based upon the finding, the realistic Puskesmas health care fee should be adjusted to the affordability and willingness of local people so that the even distribution of health service can be achieved. The regulation of the local government on the Puskesmas health care fee in the future time had better be applied regionally in accordance with the classification of people in the area of the Puskesmas considering the affordability and willingness of the to pay for the fees. Thus, there can be more than one structure of Puskesmas health care fees in a Dati II.
