Pelayanan pemberian informasi obat dan penyerahan obat yang dilakukan oleh unit kefarmasian tidak lepas dari risiko kesalahan pemberian obat pada pasien. Analisis risiko kesalahan pemberian obat dapat dilakukan dengan metode FMEA. Failure Mode Effect Analysis atau FMEA adalah metode perbaikan kinerja dengan cara mengidentifikasi dan mencegah adanya potensi kegagalan atau kesalahan sebelum terjadi dengan tujuan untuk meningkatkan keselamatan pasien. Penelitian ini bertujuan untuk mengetahui peran apoteker dalam menganalisis risiko kesalahan yang dapat terjadi dalam pelayanan resep dan menganalisis risiko kesalahan yang dapat yang terjadi dalam pelayanan resep pada pasien di Puskesmas Kecamatan Matraman. Penelitian dilakukan dengan cara observasi lalu menentukan risiko yang terdapat pada setiap tahapan pelayanan resep. Risiko yang telah diidentifikasi kemudian dinilai Risk Priority Number (RPN) berdasarkan tingkat keparahan, kemungkinan risiko, dan kemudahan terdeteksi. Hasil penelitian menunjukkan apoteker berperan dalam manajemen risiko pada pelayanan resep untuk menjamin keberhasilan terapi pasien. Analisis risiko kesalahan dalam pelayanan resep yang diperoleh yaitu kegagalan dalam proses penyiapan obat merupakan kegagalan dengan nilai RPN (Risk Priority Number.) yang paling tinggi, sehingga proses ini menjadi prioritas pertama yang perlu diperbaiki untuk mencegah atau meminimalkan kegagalan pada tahapan pelayanan resep di pelayanan farmasi Puskesmas Kecamatan Matraman, Jakarta Timur.

The service of providing drug information and drug delivery carried out by the pharmaceutical unit cannot be separated from the risk of drug administration errors to patients. Analysis of the risk of drug administration errors can be done with the FMEA method. Failure Mode Effect Analysis or FMEA is a performance improvement method by identifying and preventing potential failures or errors before they occur with the aim of improving patient safety. This study aims to determine the role of pharmacists in analyzing the risks of errors that can occur in prescribing services and to analyze the risks of errors that can occur in prescribing services to patients at the Matraman District Health Center. The research was carried out by means of observation and then determining the risks involved in each stage of the prescription service. The risks that have been identified are then assessed by a Risk Priority Number (RPN) based on the level of severity, likelihood of risk, and ease of detection. The results showed that pharmacists play a role in risk management in prescription services to ensure the success of patient therapy. Analysis of the risk of errors in prescription services obtained, namely failure in the drug preparation process is a failure with the highest RPN (Risk Priority Number.) value, so this process is the first priority that needs to be improved to prevent or minimize failure at the prescription service stage in pharmaceutical services.
