Penyakit stroke adalah penyebab kematian utama di dunia dengan 15,2 juta kematian pada tahun 2016 dan tetap menjadi penyebab utama kematian secara global dalam 15 tahun terakhir. Di Negara Indonesia, Kementrian Kesehatan RI mengemukakan bahwa Survei Sample Regristration System (SRS) pada tahun 2014 di Indonesia menunjukkan bahwa stroke merupakan penyebab kematian tertinggi dan disusul oleh Penyakit Jantung Koroner (PJK) sebanyak 12,9% kematian dalam. Tingginya angka kematian yang disebabkan oleh penyakit ini membuat para tenaga medis lebih berhati - hati dalam melakukan pengobatan dan perawatan karena kesalahan pemberian obat. Tujuan dilakukan Pemantauan Terapi Obat (PTO) terhadap pasien secara berkala yaitu untuk menurunkan resiko masalah terkait obat. Kompleksitas penyakit dan penggunaan obat, serta respons pasien yang sangat individual meningkatkan munculnya masalah terkait obat.
