Formularium nasional merupakan daftar obat terpilih yang dibutuhkan dan digunakan sebagai acuan penulisan resep pada pelaksanaan pelayanan kesehatan dalam penyelenggaraan program jaminan kesehatan. Pembuatan formularium nasional diharapkan dapat menjadi acuan untuk tiap tingkatan fasilitas kesehatan memilih obat yang tepat. Oleh karena itu, diperlukan suatu evaluasi untuk melihat kesesuaian penggunaan obat yang digunakan di Puskesmas Kecamatan Kalideres dengan daftar obat yang disarankan pada formualrium nasional. Metode penelitian yang digunakan dalam pembuatan tugas khusus ini adalah penelitian observasional dengan design cross sectional. Data diambil secara retrospektif terhadap data sekunder yaitu data penggunaan obat yang diperoleh dari instalasi farmasi Puskesmas Kecamatan Kalideres periode Januari-Februari 2022. Pengambilan data dilakukan dengan metode convenience sampling terhadap 5 resep yang mewakili setiap harinya. Secara keseluruhan, terdapat 295 pasien yang dijadikan sampel dalam penelitian ini dengan total resep yang diperoleh adalah 902 resep. Berdasarkan hasil penelitian dapat disimpulkan bahwa sebanyak 91,67% obat yang diresepkan oleh dokter pada bulan Januari dan 94,55% di bulan Februari tahun 2022 di Puskesmas Kecamatan Kalideres telah memenuhi kesesuaian obat yang terdaftar di Fornas tahun 2021.

The national formulary is a list of selected drugs needed and used as a reference for writing prescriptions in the implementation of health services in the administration of health insurance programs. The creation of a national formulary is expected to be a reference for each level of health facility to choose the right drug. Therefore, an evaluation is needed to see the suitability of the use of drugs used in the Kalideres District Health Center with the list of recommended drugs in the national formulary. The research method used in making this special assignment is an observational study with a cross-sectional design. Data were collected retrospectively from secondary data, namely data on drug use obtained from the pharmacy installation of the Kalideres District Health Center for the period January-February 2022. Data was collected using the convenience sampling method for 5 representative prescriptions every day. Overall, there were 295 patients who were sampled in this study with a total of 902 prescriptions obtained. Based on the results of the study, it can be concluded that 91.67% of the drugs prescribed by doctors in January and 94.55% in February 2022 at the Kalideres District Health Center met the suitability of drugs registered at Fornas in 2021.
