Medication error adalah peristiwa yang dapat dicegah yang dapat menyebabkan atau mengarah pada penggunaan obat yang tidak tepat atau membahayakan pasien saat pengobatan berada dalam kendali profesional perawatan kesehatan, pasien, atau konsumen. Di dalam depo farmasi, seorang apoteker dapat mengalami medication error pada tahap prescribing (peresepan), dispensing (penyiapan), dan penyerahan obat. Ketika terjadi kesalahan dalam salah satu tahap maka dapat terjadi secara berantai dan menimbulkan kesalahan pada tahap selanjutnya. Penelitian ini bertujuan untuk Mengidentifikasi persentase medication error yang terjadi pada fase prescribing aspek administratif, farmasetik, dan klinis dalam pelayanan depo rawat jalan pasien BPJS mandiri Rumah Sakit Pusat Angkatan Darat Gatot Soebroto. Metodologi penelitian yang digunakan adalah pengumpulan data resep rawat jalan pasien BPJS mandiri di setiap temuan medication error pada fase prescribing. Analisis data dilakukan secara deskriptif besaran persentase kejadian medication error dari fase prescribing aspek administratif, farmasetik, dan klinis. Didapatkan kesimpulan sebagai berikut: Ditemukan 28,67% resep yang tidak memenuhi persyaratan pada aspek administrative dengan rincian nama pasien sebanyak 6,54%, umur pasien sebanyak 62,09%, berat badan pasien 100%, tinggi badan pasien 100%, dan jenis kelamin pasien 13,07%; Ditemukan 100% resep memenuhi persyaratan farmasetik; Ditemukan 2,62% resep yang tidak memenuhi persyaratan klinis dengan rincian duplikasi pengobatan 3,27% dan interaksi obat 1,96%.

Medication error is a preventable event that could cause or lead to inappropriate medication use or patient harm while medication is within the control of the healthcare professional, patient, or consumer. In a pharmacy depot, a pharmacist can experience medication errors at the stages of prescribing, dispensing, and drug delivery. When an error occurs in one of the stages, it can occur in a chain and cause errors in the next stage. This study aims to identify the percentage of medication errors that occur during the prescribing phase of administrative, pharmaceutical, and clinical aspects in outpatient depot services for BPJS Mandiri patients at Central Army Hospital Gatot Soebroto. The research methodology used was the collection of independent BPJS outpatient prescription data for each medication error found during the prescribing phase. Data analysis was carried out descriptively on the percentage of medication error incidents from the prescribing phase of administrative, pharmaceutical, and clinical aspects. The following conclusions were drawn: There were 28.67% of prescriptions that did not meet the requirements in the administrative aspect with details of the patient's name as much as 6.54%, the patient's age as much as 62.09%, the patient's weight 100%, the patient's height 100%, and the type patient sex 13.07%; Found 100% of prescriptions meet pharmaceutical requirements; It was found that 2.62% of prescriptions did not meet clinical requirements with 3.27% details of duplication of treatment and 1.96% of drug interactions.
