Ekspresi LMP1 pada Epitel Lesi Prakanker Nasofaring yang Berhubungan dengan Karsinoma Nasofaring

 Abstrak

Latar belakang : Etiopatogenesis karsinoma nasofaring (KNF) sampai sekarang masih terus diselidiki. Faktor yang dianggap sebagai penyebab timbulnya KNF antara lain virus Epstein-Barr (VEB), faktor genetik dan faktor lingkungan. Latent Membrane protein 1 (LMP1) sebagai produk protein pada fase laten infeksi VEB diduga mempunyai peranan mulai dari lesi prakanker sampai terjadinya KNF. Penelitian ini mencoba menganalisis ekspresi LMP1 pad a epitellesi prakanker nasofaring dan ekspresi LMP1 KNF. Ruang lingkup dan cara penelitian : telah dilakukan penelitian potong lintang pada 16 kasus lesi prakanker nasofaring dan 16 kasus KNF yang berasal dari pasien yang sama, dari Bagian Patologi Anatomik FK UII RSUPN eM selama 4 tahun (1997-2000) dengan melihat umur, jenis kelamin dan tipe histologik. Selanjutnya dilakukan pulasan imunohistokimia LMP1 pada kedua lesi tersebut dengan metode streptavidin-biotin. Kemudian dinilai intensitas pewarnaan LMP1 baik pad a lesi prakanker maupun pada KNF dan frekuensi epitel yang terpulas pada lesi prakanker dan lesi KNF. Background: the etiopathogenesis of nasopharyngeal carcinoma (NPC) is still under investigation. The Epstein-Barr virus (EBV) infection as well as genetic and environmental factors are suggested to be the etiology of this disease. Latent membrane protein 1 (LMP1) as a protein product of EBV in latent phase, may play an active role in tumorigenesis from precancerous lesion to NPC. In this study the expression LMP1 in nasopharyngeal epithelium of precancerous lesion and in NPC was evaluated. Scope and method of study: a cross sectional study was applied to 16 cases of nasopharyngeal precancerous lesion that progressed to NPC from Department of Anatomic Pathology, Faculty of Medicine, University of Indonesia. Immunohistochemical staining with LMP1 using streptavidin-biotin method was performed. The intensity and frequency of the immunostaining was evaluated. A score system was used based on frequency and intensity of the staining. Statistical analysis using non-parametric test : Wilcoxon signed rank test, Mann-Whitney and Spearman correlation test were performed. Result and conclusion : the study revealed that LMP1 was positive in 81% of precancerous lesion and in 87% of NPC. The score of LMP1 + was found in 6 cases of precancerous lesion and 6 cases of NPC, while the score of LMP1 ++ was found in 10 cases of precancerous lesion and 10 cases of NPC. This study showed that there was no difference in the intensity of LMP1 in precancerous lesion and in NPC, although the frequency of immunostaining from precancerous lesion to NPC tended to increase. However, statistical . analysis showed no correlation between expression of LMP1 in the epithelium of nasopharyngeal precancerous and NPC.
