 Abstract

COVID-19 adalah wabah virus yang baru muncul pada akhir tahun 2019 di Wuhan, China. Karena kasus yang begitu progresif, diagnosis penyakit COVID-19 hampir selalu dijumpai pada banyak rumah sakit saat ini, baik yang derajat sakit sedang hingga berat dan kritis. Sebagian besar pasien yang terkonfirmasi COVID-19 juga mengalami gangguan koagulasi sehingga sangat berisiko terjadi thrombosis dan tromboemboli vena maupun arteri. Kami melaporkan tiga dari kasus pasien terkonfirmasi COVID-19 derajat berat mengalami masalah koagulasi dan telah mendapatkan terapi antikoagulan (heparin). Namun, penggunaan heparin tentunya tidak jarang memberikan efek samping, seperti memar, nyeri, dan hematoma pada area injeksi. Hal ini kemungkinan disebabkan karena teknik injeksi yang tidak tepat, misalnya durasi injeksi yang terlalu cepat. Laporan kasus ini membutuhkan penelitian lebih lanjut untuk membuktikan manfaat penerapan injeksi heparin dengan teknik slow injection (durasi injeksi 30 detik dan jeda 10 detik) dalam menurunkan kejadian memar dan intensitas nyeri lokal pada pasien Covid-19 derajat berat dengan abnormalitas parameter koagulasi.

COVID-19 is a viral outbreak that emerged at the end of 2019 in Wuhan, China. Because cases are so progressive, the diagnosis of COVID-19 is almost always found in many hospitals today, both those with moderate to severe and critical illness. Most patients with confirmed COVID-19 also have coagulation disorders, so they are at high risk for venous and arterial thrombosis and thromboembolism. We report that three of the confirmed cases of severe COVID-19 had coagulation problems and were receiving anticoagulant therapy (heparin). However, the use of heparin is certainly not uncommon to have side effects, such as bruising, pain, and hematoma at the injection site. This may be due to improper injection technique, for example the injection duration is too fast. This case report requires further research to prove the benefits of applying heparin injection with a slow injection technique (injection duration of 30 seconds and a pause of 10 seconds) in reducing the incidence of bruising and local pain intensity in severe Covid-19 patients with abnormal coagulation parameters.
