Frekuensi dan Kualitas Pelayanan Antenatal Care terhadap Kejadian Bayi Berat Lahir Rendah (BBLR): Komparasi Demographic and Health Survey di Empat Negara ASEAN = Frequency and Quality of Antenatal Care Services on Low Birth Weight (LBW) Incidence: The Comparison of Demographic and Health Survey in Four ASEAN Countries

 Abstract

Asia Tenggara penyumbang kedua kejadian BBLR tertinggi di Benua Asia yaitu sebesar 12,3%. Antenatal Care (ANC) yang berkualitas menjadi salah satu mitigasi dalam penurunan kejadian BBLR. Hasil Penelitian Fonseca dkk (2014) didapatkan hasil risiko kejadian BBLR pada ibu dengan pelayanan ANC yang tidak berkualitas 4,13 kali lebih tinggi dibandingkan pelayanan ANC yang berkualitas. Tujuan penelitian untuk mengetahui pengaruh K1 murni dan kualitas pelayanan ANC terhadap kejadian BBLR setelah dikontrol variabel kovariat yaitu karakteristik maternal, kesehatan dan kebiasaan maternal, faktor bayi, karakteristik rumah tangga dan pemberdayaan perempuan. Penelitian menggunakan data DHS pada Empat Negara ASEAN yaitu Indonesia (2017), Kamboja (2014), Myanmar (2015−2016), dan Filipina (2017). Penelitian menggunakan desain cross sectional (potong lintang) dengan analisis regresi logistik ganda dengan model pada semua negara dan komparasi setiap negara. Hasil semua negara dan komparasi setiap negara didapatkan K1 murni tidak memiliki pengaruh terhadap kejadian BBLR. Namun, seluruh negara baik Indonesia, Kamboja, Myanmar, dan Filipina didapatkan kualitas pelayanan ANC memiliki pengaruh dengan kejadian BBLR. Odds tertinggi kejadian BBLR adalah Negara Myanmar dimana odds kejadian BBLR dengan kualitas pelayanan ANC tidak sesuai standar (< 4 kunjungan, 6 komponen tidak lengkap) 1,7 kali lebih tinggi mengalami kejadian BBLR dibandingkan dengan kualitas pelayanan ANC sesuai standar (≥ 4 kunjungan, 6 komponen lengkap) (aOR = 1,7; CI 95%: 1,0−2,7) setelah dikontrol kovariat. Jika hanya memperhatikan K1 murni belum cukup dalam menurunkan risiko kejadian BBLR, masih dibutuhkan peningkatan kualitas pelayanan ANC yang didapatkan ibu hamil agar menjadi mitigasi dalam menurunkan risiko kejadian BBLR.

Southeast Asia is the second highest incidence of low birth weight (LBW) in Asia, accounting for approximately 12.3% of LBW cases in Asia. Quality of Antenatal Care (ANC) is one of the most important preventive measures in reducing the incidence of LBW. The results of the study by Fonseca et al. (2014) showed that the risk of LBW for mothers that received poor quality ANC services was 4.13 times higher than the risk of LBW for mothers that received good quality ANC services. The study aimed to analysis the association of the timing of the first antenatal visit and quality of ANC services, towards LBW incidence, after adjustment with several covariats: maternal characteristics, health and maternal habits, infant factors, household characteristics and women's empowerment. This study used Demographic and Health Survey (DHS) data in four ASEAN countries, which are Indonesia (2017), Cambodia (2014), Myanmar (2015-2016), and the Philippines (2017). This study used a cross-sectional design with multiple logistic regression analysis, an analysis for each country was done, and an analysis to compare the results between the different countries was also done. The results of each country and comparisons between each country showed that early antenatal care visit did no effect on the incidence of LBW. However, in each country it was found that the quality of ANC services had an association with LBW incidence. Myanmar had the highest odds ratio of LBW incidence, in which the odds of LBW for mothers that received poor quality ANC service (< 4 visits, 6 incomplete components) was 1.7 times higher compared with mothers that received good quality ANC services (≥ 4 visits, 6 complete components) (OR = 1.7; 95% CI: 1.0-2.7). Early antenatal care visit is not enough to reduce the risk of LBW incidence, it’s important to improve the quality of ANC services for reducing the risk of LBW incidence.
