COVID-19 dapat menimbulkan beberapa komplikasi, salah satunya gangguan neurologis yaitu stroke iskemik. Disfagia dan penurunan kemampuan batuk menjadi gejala yang dapat terjadi pada penyakit ini. Kedua hal tersebut dapat memperburuk kondisi pasien yang sudah mengalami infeksi pada saluran pernapasan. Keterbatasan alat suction di ruang perawatan membuat perawat perlu memberikan intervensi modifikasi untuk menjaga kepatenan dan bersihan jalan napas pasien, yaitu dengan memberikan posisi lateral dan moral hygiene. Studi kasus ini akan menggambarkan pemberian posisi lateral dan oral hygiene pada seorang lansia, 82 tahun, yang mengalami hipersekresi mukus, disfagia, penurunan kemampuan batuk akibat stroke iskemik dan COVID-19. Hasil intervensi selama tujuh hari, akumulasi sekret pada pasien berkurang, suara napas, respiratory ratesaturasi oksigen dalam rentang normal, kondisi oral bersih dan lembab. Hal tersebut menunjukan bahwa pemberian posisi lateral dan melakukan tindakan oral hygiene secara rutin pada pasien COVID-19 dengan stroke iskemik dapat mempertahankan bersihan jalan napas pada pasien. Pemberian posisi lateral dapat diberikan setiap dua jam sekali dan oral hygiene dilakukan minimal tiga kali sehari akan memberikan perubahan pada bersihan jalan napas pasien. Untuk mendapatkan hasil yang optimal, kedua intervensi tersebut dapat dikombinasikan dan dilakukan secara rutin selama minimal tujuh hari.

COVID-19 can cause several complications, one of which is neurological disorders, namely ischemic stroke. Dysphagia and decreased ability to cough are symptoms that can occur in this disease. Both of these things can worsen the condition of patients who already have an infection in the respiratory tract. The limitations of suction devices in the treatment room make nurses need to provide modification interventions to maintain patent and clean patient's airway, namely by providing lateral positions and oral hygiene. This case study will describe the provision of lateral positions and oral hygiene in an elderly, 82 years old, who has mucus hypersecretion, dysphagia, decreased ability to cough due to ischemic stroke, and COVID-19. The results of the intervention for seven days, the accumulation of secretions in the patient was reduced, breath sounds, respiratory rate, oxygen saturation were in the normal range, oral conditions were clean and moist. This shows that giving a lateral position and performing routine oral hygiene measures in COVID-19 patients with ischemic stroke can maintain airway clearance in patients. Giving the lateral position can be given every two hours and oral hygiene is done at least three times a day will give changes to the patient's airway clearance. To get optimal results, the two interventions can be combined and carried out routinely for a minimum of seven days.
