"Untuk membantu kegiatan/aktivitas operasional pengeboran minyak antara lain digunakan komponen yang bernama centralizer. Centralizer adalah spring steel guides yang diikatkan pada casing untuk membantu menjaga posisi agar terpusat di dalam sumur (well) sehingga dengan demikian memberikan uniform cement sheath disekitar casing pipe. Pada centralizer tersebut telah terjadi failure, dimana pada beberapa bagian lengannya mengalami fracture. Untuk mengetahui sumber penyebab (root of cause) failure pada centralizer ini dilakukan tahapan-tahapan failure analysis, antara lain: (1) initial observations, (2) background data, (3) laboratory studies, dan (4) synthesis of failure.Serangkaian pengamatan dan pengujian dilakukan untuk mengumpulkan data fakta dan bukti yang mengarah kepada penyebab terjadinya kegagalan material centralizer, diantaranya yaitu: pengamatan visual, pengukuran ketebalan plat, pengujian komposisi kimia, pengamatan fraktografi dengan foto makro, pengamatan metalografi dengan foto mikro, pengamatan Scanning lectron Microscope (SEM), pengujian Energy Dispershe X-ray (EDAX) dan pengujian X­ray Diffaction (XRD).Berdasarkan hasil pengujian dan pengamatan, diperoleh kesimpulan bahwa penyebab failure adalah corrosion fatigure, yaitu fracture akibat beban siklik (fllactuating stress) pada material serta lingkungan korosif yang menghasilkan surface-origin fatigue cracking."

Pemeliharaan merupakan proses yang dilakukan untuk menjaga kehandalan aset perseorangan/kelompok. Pompa jenis perpindahan positif lazim digunakan pada sistem drainase tertutup sebagai pompa transfer di instalasi pengolahan minyak dan gas bumi. Namun, ada kalanya pompa tersebut mengalami kegagalan yang membutuhkan usaha lebih lanjut untuk mencegah kegagalan tersebut terulang kembali. Usaha yang dapat dilakukan untuk menanggulangi hal tersebut adalah dengan menggunakan metode Analisis Sebab Akar Kegagalan (Root Cause Failure Analysis). Metode RCFA menggunakan rekoleksi barang bukti terkait desain, pengadaan, hingga pengoperasian sebuah alat hingga ditemukan sebab akar dan sebab laten terjadinya permasalahan. Akibat ditemukannya kesenjangan antara proses desain, pengadaan yang menjadi sebab akar kegagalan pompa, penelitian ini merekomendasikan kepada perusahaan pengguna pompa untuk lebih memperhatikan instalasi perpipaan penunjang kinerja pompa dan koordinasi antara pihak owner, design, dan procurement untuk mencegah kegagalan ini terulang kembali.

ABSTRACTMaintenance is a process carried out to maintain the reliability of individual/group assets. Positive displacement type pumps are commonly used in closed drainage systems as transfer pumps in oil and gas processing plants. However, there are times when the pump fails which requires further effort to prevent the failure from recurring. Efforts that can be made to overcome this are using the method of Analysis of the Root Failure (Root Cause Failure Analysis). The RCFA method uses recollection of evidence related to design, procurement, and operation of a tool to find root causes and latent causes of problems. As a result of gaps found between the design process, procurement which leads to the root cause of pump failure, this study recommends pump-operating companies to pay more attention to piping installation to support pump performance and coordination between owner, design, and procurement to prevent this failure from recurring.
