"Skripsi ini membahas pengelolaan Rekam Medis rawat inap di RSUD Kota Bekasi pada tahun 2012. Proses pengelolaan rekam medis rawat inap yang terdiri dari yang terdiri dari pencatatan, pengelolaan data medis, penyimpana RM, dan pengembalian RM. Proses tersebut digunakan untuk menghasilkan informasi yang cepat dan lengkap. Penelitian ini dilakukan dengan menggunakan penelitian kualitatif dan kuantitatif (univariat). Sampel yang digunakan adalah 100 berkas rekam medis rawat inap. Adapun hasil data yang diperoleh waktu pengembalian berkas rekam medis periode Januari-Oktober 2012 dari ruangan perawatan adalah Tepat Waktu sebesar 20% (8961 RM) dan Tidak Tepat Waktu sebesar 80% (4892 RM). Sedangkan hasil evaluasi kelengkapan berkas rekam medis rawat inap periode Januari-Oktober 2012 adalah Lengkap sebesar 84% (84 RM) dan Tidak Lengkap 16% (16 RM).

This thesis discusses the management of inpatient Medical Record Bekasi District Hospital in 2012. Process management of inpatient medical records consisting of consisting of records, medical records management, RM last save, and return RM. The process used to produce information quickly and completely. The research was conducted using qualitative and quantitative research (univariate). "ABSTRAKPenilaian unstructured data sebagai bagian dari rekod Perpustakaan UI dipilih sebagai topik berdasarkan perubahan terbaru dalam dinamika manajemen informasi seiring dengan perkembangan teknologi informasi. penggunaan media sosial sebagai cara untuk berkkomunikasi dengan publik menambahkan elemen baru untuk dipertimbangkan dalam pengambilan keputusan di perpustakaan. Tujuan dari topik ini adalah untuk menganalisis fungsi penilaian unstructured data sebagai bagian dari rekod yang dipertimbangkan di dalam pengambilan keputusan. Metode yang digunakan dalam tulisan ini adalah analisis. Data akan didapatkan melalui wawancara dan studi literatur. Penilaian unstructured data berdasar pada apakah unstructured data diperlakukan sama seperti rekod yang mana digunakan sebagai memori kolektif organisasi dan pertimbangan mereka dalam pengambilan keputusan.

The activity of using social media as a mean to communicate with the public adds a new element to be considered in the decision making of the library. The objective of this topic is to analyze the function of appraising unstructured data as a part of record that is considered in the decision making. The method used in this paper is analytical. The data will be acquired through interview and literature studies. The appraisal of the unstructured data is based on whether it is treated the same as record which is used as a collective memory of an organization and their assessment in the decision making. "

"Waktu tunggu pelayanan rawat jalan merupakan salah satu indikator kualitaspelayanan yang ditawarkan oleh Rumah Sakit Sumber Waras Jakarta. Denganmenggunakan metode kualitatif, penelitian ini melakukan analisis sistempenyediaan berkas rekam medis RM terhitung ketika pasien registrasi awalsampai berkas tersedia di poliklinik rawat jalan yang dituju. Hasil penelitianmenunjukkan waktu penyediaan berkas RM belum sesuai dengan standarpelayanan minimal SPM yang ditetapkan oleh Rumah Sakit. Beberapa kendalayang ditemui antara lain pemberkasan tidak lengkap, nomor antrian tidakberurutan sesuai loket, sistem error, jarak antar rak penyimpanan tidak sesuaistandar, jarak antara instalasi rekam medis ke rawat jalan jauh, berkas RM belumdikembalikan ke rak, SPO penyediaan berkas rekam medis belum ada.

Outpatient clinic waiting time is one of the important indicators of quality ofservices offered by Sumber Waras Hospital. Using qualitative approach, thisresearch aims to analyze the time needed to provide outpatient medical record MR starting at patient initial registration at the front desk to medical record rsquo savailable at the hospital outpatient clinic. The result of the research indicates thatMR provision time has not fulfilled the minimum serviced standard as suggestedby the hospital. Obstacles include incomplete files, queue numbering sequence,system error, distance between shelves, distance between medical record storageand outpatient clinic, MR files have not been returned to the shelf, and absence ofprocedures for provision of MR files."

"ABSTRAKKesantunan dapat digunakan di segala situasi untuk membangun dan memelihara hubungan yang baik di masyarakat, termasuk hubungan dalam keluarga. Dalam buku anak Hallo baby! yang ditulis oleh Carry Slee dan Dagmar Stam, beragam bentuk strategi kesantunan ditemukan dalam percakapan antara kakak-adik, orang tua, serta antara orang tua dan anak. Jurnal ini mengidentifikasi bagaimana strategi kesantunan bald on record, on record, dan off record digunakan, serta jenis strategi kesopanan manakah yang paling mendominasi dalam buku ini. Jurnal ini menggunakan metode deskriptif kualitatif dan tiga teori strategi kesantunan Brown dan Levinson yaitu; bald on record, on record, dan off record, serta teori face-keeping (menjaga-muka) Goffman.Hasil menunjukkan bahwa strategi bald on record kebanyakan ditemukan dalam pertengkaran kakak-adik, on record dalam permintaan disertai imbalan hadiah, dan off record, dalam perintah tak langsung. Bald on record diidentifikasi sebagai strategi kesantunan yang mendominasi buku ini karena adanya beberapa faktor sosial, yaitu; hubungan yang dekat antara anggota keluarga dan usia yang masih balita.

ABSTRACT

Politeness can be used in any situation to build and maintain good relationship in the society, including within a family. In Hallo baby! a children book written by Carry Slee and Dagmar Stam, some various forms of politeness strategies are found in conversation between siblings, parents, and also between parents and children. This journal identifies how the politeness strategies such as bald on record, on record, and off record are used, and which kind of politeness strategy is dominant in this book. This journal uses the descriptive qualitative method and the three politeness strategies of Brown and Levinson namely; bald on record, on record and off record, and also Goffman‟s face-keeping theorie.

The outcomes show that bald on record strategy are found mostly in quarrels between siblings, on record strategy in request with rewards, and off record in indirect instruction. Bald on record identified as the most dominant politeness strategy in the book because of the social factors: the close relationship among family members and the age of the toddlers."

"Rumah sakit sebagai penyedia jasa pelayanan kesehatan harus terus meningkatkan mutu dari pelayanan kesehatannya salah satunya dapat dilihat dari mutu rekam medis. Mutu rekam medis yang baik dapat dilihat dari kelengkapan pengisian berkas rekam medis. Tujuan dari penelitian ini adalah untuk mengetahui gambaran kelengkapan pengisian berkas rekam medis pasien rawat inap di RSAU dr.Esnawan Antariksa Tahun 2014. Penelitian ini bersifat deskriptif observasional dengan pendekatan kuantitatif dan kualitatif. Sampel penelitian ini adalah berkas rekam medis pasien rawat inap bulan Januari dan Februari tahun 2014 yang telah dikembalikan ke Bagian Rekam medis sebanyak 90 berkas rekam medis. Teknik sampling yang digunakan dengan sampel acak sistematis. Hasil penelitian menunjukkan bahwa rata - rata kelengkapan pengisian rekam medis sudah cukup lengkap namun masih kurang dari standar kelengkapan yang ditetapkan Depkes RI sebesar 100%. Disamping itu, faktor pendukung kelengkapan pengisian rekam medis yang terdiri dari sumber daya manusia, sarana dan prasarana, bahan, metode dan biaya masih kurang mendukung kegiatan kelengkapan pengisian berkas rekam medis pasien rawat inap. Oleh karena itu, untuk mendapatkan angka kelengkapan pengisian rekam medis 100% sesuai dengan standar Depkes maka perlu dilakukan pengembangan kemampuan tenaga medis dan paramedis dengan pengadaan pelatihan mengenai kelengkapan pengisian rekam medis, penambahan jumlah petugas rekam medis, sosialisasi SOP kelengkapan pengisian rekam medis pemberian sanksi dan reward serta pengembangan teknologi rekam medis yang terkomputerisasi untuk meningkatkan mutu dari rekam medis pasien rawat inap.

Hospitals as health care providers should continue to improve the quality of health care one of which can be seen from the quality of medical records. Good quality of medical record can be seen from filling completeness of medical record document. The purpose of this study is to describe the completeness filling of the inpatients medical record document in RSAU dr.Esnawan Antariksa of 2014. This is a descriptive observational study with quantitative and qualitative approaches. The sample was medical record document of inpatients in January and February of 2014 that has been restored a total of 90 medical records. The sampling technique used by systematic random sampling.

The results showed that the averages of completeness filling inpatients medical records are quite complete, but still less than the standard set by Ministry of Health Republic of Indonesia completeness of 100%. In addition, the supporting factors of completeness filling inpatients medical records consisting of human resources, facilities, materials, methods and costs are still lacking. Therefore, to obtain medical record numbers 100% of filling completeness in accordance with the standards of the Department of Health is necessary for the development of medical and paramedical personnel capability with procurement training on completeness filling of medical records, increasing the number of medical records clerk, socialization SOP completeness filling of medical records, sanctions and reward as well as the development of a computerized medical record technology to improve the quality of inpatients medical record."

"Mengacu pada kata milik didalam Permenkes No. 269 Tahun 2008 dikatakan bahwa dokumen rekam medis merupakan milik rumah sakit adalah sebagai hak eigendom penuh sedangkan yang dimaksud dengan isi rekam medis milik rumah sakit adalah sebagai hak eigendom terbatas. Sehingga kosekuensinya adalah rumah sakit dapat menolak permintaan pasien untuk memiliki atau mengkopi rekam medis tersebut karena dengan dikuasainya suatu benda berdasarkan hak eigendom penuh, maka seseorang pemegang hak milik diberikan kewenangan untuk menguasainya secara tentram dan untuk mempertahankannya terhadap siapapun yang bermaksud untuk menganggu ketentramannya dalam menguasai, memanfaatkan serta mempergunakan benda tersebut.

In article 12 Permenkes No. 269/2008 said that patient is owner contents of the medical records while the medical record documents will still be owned by the hospital. Referring to the word belongs here, the meaning of the document medical records which belong to a hospital is a full eigendom rights while the meaning of the content of medical record which hospital-owned is a limited egeindom rights. The consequence are, hospital can refuse a patient's request to have a copy of medical records or because the mastering of medical records are based on full egeindom rights, then the rights holder is authorized to master it peacefully and to defend it against anyone who intends to disturb its tranquility on mastering, utilizing and using these objects."

"Pengelolaan rekam medis adalah untuk menunjang tertib administrasi dalam rangka upaya peningkatan pelayanan kesehatan di rumah sakit yang didukung oleh sistem pengelolaan rekam medis yang cepat, tepat, bernilai, dan dapat dipertanggung jawabkan. Waktu penyediaan rekam medis adalah waktu yang diperlukan pada saat pasien mendaftarkan sampai dengan berkas rekam medis pasien diterima oleh petugas rawat jalan. Standar waktunya ≤ 10 menit. Tujuan penelitian ini untuk menganalisis waktu penyediaan rekam medis rawat jalan di bagian rekam medis RS Tugu Ibu Depok tahun 2015. Jenis Penelitian ini adalah cross sectional dengan menggunakan pendekatan kuantitatif dan di dukung dengan metode kualitatif. Hasil penelitian, penyedianan rekam medis rawat jalan belum berjalan secara optimal, hal ini ditunjukan dengan output yang dihasilkan, masih terdapat banyak keluhan tentang rekam medis yang terlambat oleh paien, perawat poliklinik, dan dokter, berdasarkan hasil observasi peneliti berkas rekam tidak sesuai standar mencapai 48%. Faktor-faktor utama mengenai terkait terlambat berkas rekam medis adalah dalam proses pencarian rekam medis, yaitu banyak berkas rekam medis yang dipinjam oleh bagian lain kembali tidak tepat waktu dan berkas rekam medis tidak tersusun dengan rapi di ruang penyimpanan. Saran dilakukan evaluasi kordinasi terkait dengan penyediaan berkas rekam medis dengan melibatkan pihak-pihak dan bagian yang terkait, perlu dipertimbangkan tentang penambahan ruang dan rak penyimpanan berkas rekam medis dan rekomendasi standar penyedian rekam medis rawat jalan menjadi dasar keputusan manajemen.

Medical record management functions as an assistance for systematical administrative in order to increase hospital’s health service which is supported by fast, accurate, qualified, and certified medical record management. Medical record provision time is a time starting from when the patient registers till medical record file is received by the outpatient’s nurse, the standard time is around 10 minutes. This research is focused on analyzing the outpatient medical record provision time in medical record unit at Tugu Ibu Depok Hospital (2015). The research method is cross sectional with quantitative approach and also supported by qualitative method.

The result of the research is that the provision of medical record has not operated optimally. This can be proven from the generated output such as there are a lot of complaints by patient, polyclinic nurses, and doctors regarding the delayed medical records and medical records that are not suitable are almost 48%, based on researchers’ observation result. The main factors regarding the delayed medical record files are many medical record files while in process of searching the medical record, which is borrowed by other parts are not returned on time and the file of medical records are not neatly arranged in the storage room.

Suggestion an evaluation of coordination associated with the provision of medical record file by involving the parties and the relevant parts, need to be considered on the addition of space and file storage of medical records and also the standard recomendation of medical records outpatient become the base of making decisions for hospital management."
