"Tuberkulosis adalah penyakit yang disebabkan oleh Mycobacterium tuberculosis dan menjadi sangat berbahaya karena kemudahannya untuk menginfeksi orang lain. Rifampisin merupakan salah satu OAT lini pertama yang menjadi dasar obat tuberculosis dan terjadinya resistensi terhadap rifampisin menjadi salah satu kendala pemberantasan TB di Indonesia.
Penelitian ini bertujuan menentukan pola resistensi M. tuberculosis terhadap Rifampisin serta mengetahui perbandingan monoresisten rifampisin, multi drug resistance (MDR), serta multiresisten lain tuberkulosis. Penelitian ini dilakukan dengan menganalisis data sekunder sebanyak 676 sampel dengan kultur positif dari Departemen Mikrobiologi FKUI pada September 2005 sampai Desember 2007 dan telah menjalani pemeriksaan resistensi sesuai dengan panduan WHO/IUATLD.
Dari hasil analisis didapatkan pola resistensi terhadap rifampisin sebanyak 23.96% dimana monoresisten rifampisin sebesar 7,24%, MDR TB sebesar 8,73%, serta multiresisten lain sebesar 7,99%.

"Obat anti tuberkulosis sudah ditemukan lebih dari 50 tahun akan tetapi sampai sekarang tuberkulosis masih saja menjadi permasalahan kesehatan di Indonesia. Hal ini membuktikan masih ada kelemahan dari metode yang selama ini digunakan. Hal tersebut bisa berupa salah pemberian jenis obat, dosis yang kurang ataupun waktu pengobatan yang tidak cukup. Permasalahan yang diakibatkan karena kesalahan dalam pengobatan ini adalah resistensi Mycobacterium tuberculosis terhadap obat anti tuberkulosis termasuk salah satunya etambutol sebagai obat anti tuberkulosis lini pertama.
Penelitian ini bertujuan menentukan pola resistensi Mycobacterium tuberculosis terhadap etambutol. Penting untuk mengetahui besarnya resistensi kuman tuberkulosis terhadap etambutol untuk meningkatkan keberhasilan terapi. Penelitian ini dilakukan dengan menganalisis data sekunder sebanyak 676 sampel dengan kultur positif dari Departemen Mikrobiologi FKUI pada September 2005 sampai Desember 2007 dan telah menjalani pemeriksaan resistensi sesuai dengan panduan WHO/IUATLD. Dari hasil analisis didapatkan bahwa pemeriksaan kultur lebih sensitif bila dibandingkan dengan pemeriksaan BTA serta didapatkan pola resistensi terhadap etambutol sebanyak 16,7%.

Anti tuberculosis medicine had been discovered for more than 50 years; however tuberculosis still exists as one of Indonesia?s greatest health concerns. This proves that there are weaknesses of the current medication method, such as: prescription of incorrect medicine, lack of dosage, or insufficient medication period. The problem arising from these erroneous medications is the resistance of Mycobacterium tuberculosis towards anti tuberculosis medication including ethambutol as first-line anti tuberculosis medicine.
This research aims to determine the resistance pattern of Mycobacterium tuberculosis towards ethambutol. It is of great importance to ascertain the resistance degree of tuberculosis bacteria towards ethambutol to enhance therapy success. This research was conducted by analyzing secondary data of 676 samples with positive cultures from the Department of Microbiology of FKUI on September 2005 to December 2007 and had gone through resistance examination in compliance with WHO/IUATLD guidelines. The analysis result shows that culture examination is more sensitive compared to BTA examination, while the resistance pattern towards ethambutol is at 16.7%."

"Tuberkulosis merupakan penyakit saluran pernapasan yang disebabkan oleh Mycobacterium tuberculosis dan menjadi sangat berbahaya karena kemampuan penularannya dan mortalitas yang cukup tinggi. Diperlukan metode standar dengan sensitivitas yang tinggi untuk mendeteksi dan pengobatan dini sehingga rantai transmisi dapat dihentikan. Salah satu OAT lini pertama yang menjadi dasar pengobatan tuberkulosis adalah streptomisin. Resistensi terhadap streptomisin menjadi salah satu kendala pemberantasan TB di Indonesia. Penelitian ini bertujuan untuk mengetahui perbandingan sensitivitas antara pemeriksaan Basil Tahan Asam (BTA) dan kultur dalam mendiagnosis TB serta pola sensitivitas M. tuberculosis terhadap streptomisin. Penelitian ini dilakukan dengan menganalisis data sekunder di Departemen Mikrobiologi FKUI pada periode September 2005 hingga Desember 2007, sejumlah 676 sampel. Sampel tersebut merupakan sampel dengan hasil kultur positif dan telah dilakukan uji sensitivitas sesuai dengan panduan WHO/IUATLD. Dari hasil analisis, didapatkan bahwa angka resistensi Mycobacterium tuberculosis terhadap streptomisin adalah sebesar 22.9%, pemeriksaan kultur lebih sensitif bila dibandingkan dengan pemeriksaan BTA dalam mendiagnosis penyakit TB, dan pola sensitivitas terhadap streptomisin menunjukkan penurunan angka resistensi setiap tahun dari tahun 2005 hingga 2007.

Standard method with high sensitivity was needed to early detect and treatment for stopping the transmission. One of the first line drugs that has been basically used to treat tuberculosis is streptomycin. Resistance against this drug has been the obstacle in stopping tuberculosis in Indonesia. This research was aimed to determine the sensitivity of acid fast bacilli method compared to culture in tuberculosis diagnosis and the resistance pattern of Mycobacterium tuberculosis against streptomycin. This research was done by collecting and analyzing 676 secondary samples from Microbiology Department Medical Faculty University of Indonesia in September 2005 until December 2007. These samples criteria are positive culture and had been undergone resistance tests based on WHO/IUATLD guidelines. The results of analysis were the resistance of streptomycin was 22.9%, culture test was more sensitive than acid fast bacilli method in tuberculosis diagnosis, and there was decline of resistance against streptomisin from 2005 until 2007."

"Tuberkulosis adalah penyakit yang disebabkan oleh Mycobacterium tuberculosis dan merupakan suatu penyakit global yang menjadi perhatian di dunia karena kemudahan transmisi ke orang lain. Isoniazid merupakan salah satu OAT lini pertama yang menjadi dasar obat tuberculosis dan terjadinya resistensi terhadap isoniazid menjadi salah satu kendala pemberantasan TB di Indonesia. Tuberkulosis ekstrapulmonal merupakan suatu komplikasi dari Mycobacterium tuberculosis di luar dari paru. Tuberkulosis ekstrapulmonal memiliki angka insidensi yang cukup tinggi dalam kejadian tuberkulosis. Diagnosis tuberkulosis ekstrapulmonal ditegakkan dengan pemeriksaan BTA dari isolat ekstrapulmonal seperti pus dan cairan sendi.
Penelitian ini bertujuan menentukan pola resistensi M. tuberculosis terhadap isoniazid serta mengetahui perbandingan angka kejadian resistensi pada spesimen pus dan spesimen sputum. Penelitian ini dilakukan dengan menganalisis data sekunder sebanyak 676 sampel dengan kultur positif dari Departemen Mikrobiologi FKUI pada September 2005 sampai Desember 2007 dan telah menjalani pemeriksaan resistensi sesuai dengan panduan WHO/IUATLD. Dari hasil analisis didapatkan bahwa pola resistensi terhadap isoniazid sebanyak 13.5%. dan terdapat perbedaan angka resistensi sebesar 19.49 %, di mana spesimen pus lebih tinggi.

Extrapulmonary tuberculosis is one of complication from Mycobacterium tuberculosis in everywhere instead of lung. Extrapulmonary tuberculosis has quite high incidence rate in tuberculosis case. Diagnosis of extrapulmonary tuberculosis is decided by BTA test from extrapulmonal isolate such as pus and synovial liquid.
This research aimed to determine the resistance of isoniazid and also determine the ratio of resistant in pus specimen and sputum specimen. This research was done by collecting and analyzing 676 secondary samples which culture results are positive from Microbiology Department Medical Faculty University of Indonesia in September 2005 until December 2007 and had undergone resistance tests based on WHO/IUATLD guidelines. The results of the analysis was obtained that resistance of isoniazid was 13.5%. and there are 19.49 % difference of resistance, where pus specimen is higher."

"[ABSTRAK
Latar Belakang. Pasien TB-MDR sedang menjalankan pengobatan akan memengaruhi kondisi kejiwaan yang dapat disebabkan dari obat-obatan TB-MDR dan atau stres psikososial. Tujuan penelitian adalah mendapatkan gambaran gangguan psikiatri pada pasien TB-MDR dan stres psikososial yang memengaruhi.
Metode. Penelitian ini menggunakan desain potong lintang dengan subjek penelitian berjumlah 50 orang. Pengambilan sampel pada subjek menggunakan metode konsekutif. Instrumen yang digunakan pada penelitian ini adalah MINI ICD-10 dan Life Experiences Survey (LES) dari Irwin G. Sarason yang terdiri dari 60 item yang dinilai dengan skala likert -3 sampai 3. Pada subjek dinilai dampak positif dan negatif stresor menggunakan instrumen LES. Data demografi meliputi usia, jenis kelamin, status pernikahan, jumlah anak, agama, suku, agama, pendapatan, tingkat pendidikan, obat-obatan yang digunakan dan jangka waktu pengobatan. Data dianalisis dengan menggunakan program SPSS untuk windows versi 20. Tingkat kemaknaan yang digunakan untuk uji statistik adalah p < 0,05.
Hasil. Proporsi gangguan psikiatri pada subyek TB-MDR adalah 62%. Proporsi gangguan psikiatri pada subjek TB-MDR terbanyak pada gangguan depresi (32%) diikuti dengan risiko bunuh diri (26%), gangguan panik (24%), gangguan anxietas menyeluruh (20%), gangguan depresi berulang (12%), gangguan psikotik (12%), gangguan agorafobia (8%), gangguan obsesif kompulsif (8%), agorafobia dengan gangguan panik (4%), anorexia nervosa (2%) dan gangguan berkaitan dengan zat psikoaktif (2%). Sebagian besar subjek mendapatkan regimen standar pengobatan TB-MDR mengalami gangguan psikiatri sebesar 58,1%. Terdapat hubungan yang bermakna antara usia subjek dengan gangguan psikiatri sebesar <0,001, antara obat TB-MDR yang didapatkan dengan risiko bunuh diri (p<0,005) dan antara stresor psikososial dengan gangguan psikiatri.
Kesimpulan. Terdapat gangguan psikiatri pada subjek TB-MDR selama menjalani pengobatan. Kelompok subjek TB-MDR dengan gangguan psikiatri cenderung memiliki skor stres negatif yang lebih tinggi (lebih banyak yang mengalami stresor negatif) dibandingan dengan subjek tanpa gangguan psikiatri.ABSTRACT Background. Data were analyzed using SPSS for Windows version 20. The level of significance used for the statistical tests was p <0.05.
Results. Proportion of psychiatric disorders in subjects with MDR-TB is 62%. Proportion of psychiatric disorders in subjects with MDR-TB are depressive disorders (32%) followed by risk of suicide (26%), panic disorder (24%), anxiety disorder (20%), recurrent depressive disorder (12%), psychotic disorder (12%), agoraphobia disorders (8%), obsessive compulsive disorder (8%), agoraphobia with panic disorder (4%), anorexia nervosa (2%) and psychoactive substances associated disorders (2%). Proportion of psychiatric disorders in subjects getting standard treatment regimen for MDR-TB are 58.1%. A significant relationship is made statistically between age of subjects with psychiatric disorders, MDR-TB drugs with suicide risk and psychosocial stressors with psychiatric disorders.

"[ABSTRAK
Multi Drug Resitance Tuberculosis (MDR TB) merupakan masalah terbesar dalam
pencegahan dan pemberantasan TB dunia. Indonesia berada di peringkat 8 dari 27 negara
dengan MDR TB terbanyak di dunia. WHO Global Report 2013, memperkirakan pasien
MDR TB di Indonesia berjumlah 6.900 pasien. MDR TB adalah penyakit yang
disebabkan oleh Mycobacterium Tuberculosis yang resisten minimal terhadap rifampisin
dan isoniazid. Penelitian ini berfokus pada bagaimana pengalaman pasien menjalani
pengobatan Multi Drugs Resistance Tuberculosis dan seperti apa pasien memaknai
pengalamannya tersebut. Desain penelitian kualitatif fenomenologi dipilih untuk
mendapatkan informasi yang sifatnya individual secara mendalam sesuai pengalaman dan
kondisi penyakit yang dialami. Partisipan ditentukan dengan tehnik purposive sampling,
berjumlah tujuh orang. Tehnik wawancara mendalam dilakukan menggunakan alat
perekam dan panduan wawancara semiterstruktur, serta catatan lapangan. Analisa data
menggunakan qualitative content analysis dengan pendekatan Collaizi. Tema yang
ditemukan sebagai hasil penelitian yaitu : Pemahaman penyakit MDR TB;
ketidaknyamanan saat menjalani pengobatan Multi Drugs Resistance Tuberculosis;
hambatan penderita untuk sembuh, menerima dukungan, dan harapan pengobatan.
Diperlukan studi lebih lanjut untuk mengkaji secara mendalam tema yang telah

purposive sampling technique to seven people. In-depth interview conducted using a
recorder and semi-structured approach. Themes found as a result of the research : the
understanding of MDR TB; the experience discomfort while undergoing treatment of
Multi Drugs Resistance Tuberculosis; difficulties experienced during treatment; the
support received during treatment; and client expectations. Further study is needed to
profoundly examine the themes that have been identified on more numerous and varied
participants, Multi Drug Resistant (MDR - TB ) is the biggest problem of TB prevention and

"ABSTRAK

Gen Multi Drug Resistance 1 (MDR1) merupakan gen yang mengkode P-glikoprotein (P-gp). P-gp merupakan pompa ATP-dependent yang berperan penting dalam eliminasi senyawa seperti obat dan xenobiotik. Gen MDR1 dikenal sebagai gen yang polimorfik. Diantara varian-varian gen MDR1, suatu polimorfisme yang dinamakan C3435T, dapat memodulasi metabolisme dan respon substrat senyawa xenobiotik atau obat pada P-gp seperti steroid, antrasiklin dan alkaloid vinka yang penting terhadap terapi LLA. Beberapa studi menunjukkan bahwa polimorfisme ini juga digunakan untuk mengevaluasi konstribusi kandidat gen yang berpengaruh terhadap risiko insiden leukemia limfoblastik akut (LLA) pada anak-anak. Tujuan dari penelitian ini adalah untuk melihat profil polimorfisme C3435T pada 18 pasien anak-anak berumur kurang dari 3 tahun di RSCM yang menderita LLA menggunakan analisis PCR-RFLP dengan enzim restriksi MboI. Sebanyak 3 (16.67%) sampel LLA, dapat terdigesti dan 15 (83,33%) sampel LLA tidak dapat terdigesti. Alel C yang ditandai dengan DNA yang terdigesti mengindikasikan ekspresi p-gp yang tinggi (overekspresi) yang dapat mengeliminasi obat anti leukemia yang mengakibatkan prognosis buruk. Sedangkan, DNA yang tidak terdigesti (alel T) ditemukan banyak pada populasi pasien anak berumur kurang dari 3 tahun yang menderita LLA dibandingkan alel C sehingga diasosiasikan dengan insiden LLA. Kesimpulannya, persentase alel T pada pasien LLA berumur kurang dari 3 tahun di Departemen Ilmu Kesehatan Anak RSCM lebih besar (83,33%) dibandingkan alel C (16,67%).


ABSTRACT

The Multiple Drug Resistance 1 (MDR1) gene codes for P-glycoprotein (P-gp) is an ATP-dependent efflux pump which has great importance in elimination of several important drugs and xenobiotics. It has been shown to be polymorphic. Among the multiple variants of the MDR1 gene, a silent polymorphism, namely C3435T, has been shown to modulate the metabolization and thus the response to the drug or xenobiotic substrates for P-gp such as steroid, antracycline and vinca alkaloid which associated with drug resistance mechanism. In many studies this polymorphism also can be used to evaluate the contribution of candidate gene to the understanding genetic susceptibility to childhood acute lymphoblastic leukemia (ALL). Our study was an attempt to analyze the MDR1 gene polymorphism profile. We screened for the C3435T polymorphism 18 children under 3 years old with ALL, using PCR-RFLP assay with MboI as the restriction enzyme. We found 3 (16,67%) ALL samples, were digestable and 15 (84,33%) ALL samples were undigestable. C allele which is associated with the digestable DNAs, indicated with high expression p-gp might be eliminating antileukemic drug resulting in high risk diagnosis. On the other hand, the undigestable DNAs (T allele) were found abundant in patients and might to be associated with the incidence of ALL. In conclusion, the T allele percentage on ALL patients under 3 years old in Cipto Mangunkusumo Hospital were found higher (83,33%) compared to the C allele (16,67%).

"Indonesia masuk kedalam Negara dengan tiga beban TB tertinggi, salah satunya adalah TB-MDR. Persentase kematian pada pasien TB-MDR selama masa pengobatan di Indonesia melebihi batasan target WHO yaitu 10. Penelitian ini bertujuan untuk mendapatkan faktor-faktor yang berhubungan dengan kematian pada pasien Tuberkulosis Multi Drug Resistance TB-MDR selama masa pengobatan di Indonesia tahun 2010-2014. Desain penelitian yang digunakan adalah studi cross sectional menggunakan data sekunder registrasi kohort e-TB Manager Surveilans TB Resistan Obat 2010-2014.
Variabel independen pada penelitian ini meliputi faktor kerentanan individu usia, jenis kelamin, komorbid diabetes mellitus, jumlah resistansi OAT, hasil pemeriksaan sputum di awal pengobatan, faktor kerentanan sistem kesehatan riwayat pengobatan TB sebelumnya dan interval inisiasi pengobatan, dan faktor kerentanan sosial wilayah tempat tinggal. Variabel dependen pada penelitian ini adalah hasil akhir kematian pada pasien TB-MDR. Hasil penelitian didapatkan bahwa terdapat hubungan bermakna antara usia dengan kematian pada pasien TB-MDR selama masa pengobatan.

Indonesia is one of the countries in three high burden country list, partially MDR TB. The presentation of mortality among MDR TB patients during treatment in Indonesia is above WHO target which is 10. This study aimed to describe the epidemiological and factors associated with mortality among MDR TB patients during treatment in Indonesia from 2010 through 2014. The study was conducted with cross sectional using secondary data cohort registration e TB Manager Surveillance of TB Drugs Resistance 2010 2014.
Independent variables of this study were individual vulnerability age, sex, diabetes mellitus comorbidities, number of drugs resistance, initial sputum test, programmatic or institutional vulnerability previous history of TB treatment and interval of treatment, and social vulnerability living status. Dependent variable of this study was the end of treatment result for mortality among MDR TB patients. The results indicated that age associated with mortality among MDR TB patients during treatment.

"Tuberkulosis merupakan penyakit yang masih menjadi masalah utama kesehatan bagi masyarakat di Indonesia. Berdasarkan Global TB Report 2021, Asia Tenggara merupakan wilayah dengan beban Tb paling tinggi dan Indonesia menyumbang sekitar 8% dari keseluruhan beban Tb didunia. Namun pasien Tb yang berhasil ditemukan, diobati dan dilaporkan kedalam sistem informasi nasional hanya sekitar 48%. Tb yang resisten terhadap obat terus menjadi ancaman kesehatan manusia. Resistensi terhadap obat anti tuberkulosis bisa terjadi akibat infeksi primer dengan bakteri Tb resisten atau bisa disebabkan oleh pengobatan yang tidak adekuat, sehingga muncul strain yang resisten akibat adanya perubahan atau mutasi pada gen-gen tertentu dalam genom Mycobacterium tuberculosis. Saat ini sudah banyak teknologi dan metode yang digunakan untuk mendeteksi resistensi terhadap obat anti tuberculosis. Salah satunya adalah Next Generation Sequencing. Next Generation Sequencing merupakan teknologi sekuensing akurat, hemat biaya dan throughput tinggi yang memungkinkan penyelidikan genom termasuk Whole Genome Sequencing untuk studi epidemiologi dan untuk mendeteksi penanda resistensi obat dan keragaman strain. Analisis Whole Genome Sequencing dapat digunakan untuk mendeteksi resistensi terhadap obat anti tuberkulosis lini pertama y a n g sangat menjanjikan sebagai pengganti uji kepekaan obat konvensional. Hasil dari analisis Whole Genome Sequencing pada Mycobacterium tuberculosis yang resisten rifampisin menunjukkan adanya mutasi-mutasi yang ada pada wilayah operon Rpo terutama pada gen RpoB dan RpoC. Mutasi yang paling dominan pada gen RpoB adalah perubahan S450L (36,45%) dan pada gen RpoC adalah G594E (30,95%). Dan analisis whole genome sequencing juga menunjukkan adanya mutasi baru yang berbeda dengan mutasi-mutasi yang ada berdasarkan penelitian sebelumnya.

Tuberculosis is a disease that is still a major health problem for people in Indonesia. Based on the Global TB Report 2021, Southeast Asia is the region with the highest burden of TB and Indonesia produces around 8% of the total burden of TB in the world. However, only about 48% of TB patients who have been found, treated and reported to the national information system. Drugresistant TB continues to be a threat to human health. Resistance to anti-tuberculosis drugs can occur as a result of primary infection with resistant TB bacteria or can be caused by inadequate treatment, resulting in the emergence of resistant strains due to the presence or mutations in certain genes in the genome of Mycobacterium tuberculosis. Currently there are many technologies and methods used to detect resistance to anti-tuberculosis drugs. One of them is Next Generation Sequencing. Next Generation Sequencing is an accurate, cost-effective and highthroughput sequencing technology that enables genomic investigations including Whole Genome Sequencing to study epidemiology and to detect markers of drug resistance and strain diversity. Whole Genome Sequencing analysis can be used to detect resistance to first-line anti-tuberculosis drugs which are very promising as a substitute for conventional drug sensitivity tests. The results of Whole Genome Sequencing analysis on rifampicin-resistant Mycobacterium tuberculosis showed that there were mutations in the Rpo operon region, especially in the RpoB and RpoC genes. The most dominant mutation in the RpoB gene was the change in S450L (36.45%) and in the RpoC gene was G594E (30.95%)."

Laporan residensi ini menjelaskan tentang pelaksanaan praktik residensi maternitas, peran dan fungsi perawat maternitas, pencapaian kompetensi praktik dan upaya untuk memodifikasi hambatan yang dihadapi. Laporan residensi ini fokus pada penerapan teori need for help Wiedenbach dan self care Orem pada ibu hamil dengan tuberculosis multi drug resistance. Perawat maternitas perlu meningkatkan pengetahuan dan keterampilan klinik secara berkelanjutan sehingga mampu meningkatkan kualitas asuhan keperawatan maternitas.

Maternity nursing practice residency organized to produce nurse of maternity who master the application of nursing theory and have special skills to do a good maternity care in hospitals and in the community. This residency final report describes the implementation of the practice residency maternity, roles and functions of nurses maternity, the attainment of practice and attempts to modify the barriers. Focus of this final report is application of theory need for help Wiedenbach and self care Orem in pregnant women with tuberculosis multi drug resistance. Nurse maternity needs to improve knowledge and clinical skills on an ongoing basis to improve the quality of maternity nursing care.
