"Pembangunan di Indonesia semakin meningkat setiap tahunnya untuk mendukung peningkatan perekonomian masyarakat dan menunjang distribusi barang dan jasa, hal ini memicu pertumbuhan yang pesat juga terhadap Perusahaan Jasa Konstruksi. Namun dalam pelaksanaannya, ditemukan banyaknya kegagalan konstruksi yang dilakukan perusahaan jasa konstruksi yang menyebabkan adanya penambahan biaya dan waktu. Salah satu penyebab utama proyek konstruksi yang bermasalah dalam pelaksanaan pekerjaan sehingga menimbulkan kegagalan konstruksi diakibatkan oleh kurangnya kesadaran terhadap budaya mutu yaitu distribusi informasiDalam Penelitian ini bertujuan untuk mendapatkan variabel - variabel dalam distribusi informasi yang mempengaruhi budaya mutu dan kegagalan konstruksi serta untuk merancang strategi peningkatan distribusi informasi dalam sistem komunikasi terkait budaya mutu proyek untuk menurunkan tingkat kegagalan konstruksi. Hasil dari studi literatur didapatkan 11 variabel distribusi informasi. Variabel kemudian divalidasi awal oleh 8 pakar. Pengolahan data dilakukan menggunakan software SPSS dan SmartPLS yang mengolah 108 responden. Berdasarkan hasil analisis terdapat 2 variabel yang berpengaruh signifikan terhadap kegagalan konstruksi yaitu Metode Distribusi Informasi (X5) dan Dokumen Akhir Hasil Pekerjaan yang Berisi Seluruh Proses Pelaksanaan Pekerjaan (X11) dengan nilai T-Statistics (level of significance 5%) berturut-turut adalah 2,229 dan 2,285. Berdasarkan dari hasil temuan, maka perlu dibuat strategi peningkatan distribusi informasi dalam upaya untuk menurunkan tingkat kegagalan konstruksi di Indonesia pada perusahaan jasa konstruksi swasta nasional.

Development in Indonesia is increasing every year to support the improvement of the community's economy and support the distribution of goods and services, this triggers rapid growth also for Construction Service Companies. However, in practice, it was found that many construction failures were carried out by construction service companies which caused additional costs and time. One of the main causes of construction projects that have problems in carrying out work, causing failure due to lack of awareness of culture is information distribution. projects to reduce construction failure rates. The results of the literature study obtained 11 information distribution variables. The variables were then validated by 8 experts. Data processing was carried out using SPSS and SmartPLS software which processed 108 respondents. Based on the analysis, there are 2 variables that have a significant effect on construction failure, namely the Information Distribution Method (X5) and the Final Document of Work Results containing the entire Work Implementation Process (X11) with the T-Statistics value (level of significant 5%) respectively 2.229 and 2,285. Based on the result, it is necessary to improve a strategy to increase the information distribution to reduce construction failure levels at national private construction service companies."

"Untuk membantu kegiatan/aktivitas operasional pengeboran minyak antara lain digunakan komponen yang bernama centralizer. Centralizer adalah spring steel guides yang diikatkan pada casing untuk membantu menjaga posisi agar terpusat di dalam sumur (well) sehingga dengan demikian memberikan uniform cement sheath disekitar casing pipe. Pada centralizer tersebut telah terjadi failure, dimana pada beberapa bagian lengannya mengalami fracture. Untuk mengetahui sumber penyebab (root of cause) failure pada centralizer ini dilakukan tahapan-tahapan failure analysis, antara lain: (1) initial observations, (2) background data, (3) laboratory studies, dan (4) synthesis of failure.
Serangkaian pengamatan dan pengujian dilakukan untuk mengumpulkan data fakta dan bukti yang mengarah kepada penyebab terjadinya kegagalan material centralizer, diantaranya yaitu: pengamatan visual, pengukuran ketebalan plat, pengujian komposisi kimia, pengamatan fraktografi dengan foto makro, pengamatan metalografi dengan foto mikro, pengamatan Scanning lectron Microscope (SEM), pengujian Energy Dispershe X-ray (EDAX) dan pengujian X­ray Diffaction (XRD).
Berdasarkan hasil pengujian dan pengamatan, diperoleh kesimpulan bahwa penyebab failure adalah corrosion fatigure, yaitu fracture akibat beban siklik (fllactuating stress) pada material serta lingkungan korosif yang menghasilkan surface-origin fatigue cracking.
