"Penanganan tuberkulosis (TB) di Indonesia masih dihadapkan pada keterlambatan pendeteksian terapi yang tidak adekuat. Secara klinis, kenaikan berat badan dianggap sebagai salah satu indikasi perbaikan klinis penderita TB yang praktis. Namun, apakah kenaikan berat badan benar berhubungan dengan kesembuhan klinis pasien TB masih perlu dibuktikan. Oleh karena itu, dalam upaya menilai potensi berat badan sebagai indikator klinis terapi TB, dilakukan riset yang bertujuan untuk melihat asosiasi antara penambahan berat badan dengan konversi sputum pada akhir fase inisial pengobatan anti-TB dengan kategori 1. Studi ini menggunakan desain kohort retrospektif dengan mengumpulkan data sekunder dari rekam medis pasien di RS Persahabatan pada tahun 2009 (n=102).
Hasil menunjukkan bahwa 75,49% (n=77) pasien TB mengalami konversi sputum pada akhir fase inisial. Penambahan berat badan ditemukan pada sekitar setengah sampel dari grup dengan konversi sputum (51,95%) dan juga pada hampir setengah dari grup tanpa konversi sputum (dua belas dari 25). Studi ini menunjukkan bahwa penambahan berat badan tidak memiliki asosiasi yang signifikan dengan konversi sputum pada akhir fase inisial pengobatan anti-TB dengan kategori 1 selama dua bulan di RS Persahabatan (p= 0.732), namun studi lebih lanjut disarankan untuk meneliti asosiasi tersebut pada periode terapi yang lebih lama dan juga dengan mempertimbangkan IMT pasien.

Tuberculosis (TB) management in Indonesia was facing delayed detection of inadequate therapies. Clinically, weight gain was considered as one of the simple indicators pointing towards clinical improvement of TB patients. However, whether weight gain was really associated with clinical recovery of TB patients was yet to be proven. Therefore, as an effort to assess the possibility of observing weight gain to evaluate anti-TB therapy, a research was conducted aiming to assess the association between weight gain and sputum conversion at the end of initial phase category 1 anti-TB therapy. This study used a retrospective cohort design by collecting secondary data from the medical records of TB patients in Persahabatan hospital in 2009 (n=102).
Results showed that 75.49% (n=77) of TB patients underwent sputum conversion at the end of second month of therapy. Regardless of sputum conversion, weight gain was observed in approximately half of both groups with (51.95%) and without (twelve out of 25) sputum conversion. This study revealed that weight gain was not significantly associated with sputum conversion at the end of two months initial phase category 1 anti-TB therapy in Persahabatan hospital (p= 0.732), however future studies were encouraged to explore the association in longer therapy period and with considering patients BMI.

Salah satu indikator yang digunakan untuk memantau dan menilai pengobatan adalah dengan menentukan konversi sputum. Status gizi yang baik akan dapat mempengaruhi perubahan konversi sputum Tuberkulosis Paru dan keberhasilan terapi. Pada penelitian ini dilakukan analisis mengenai hubungan perubahan berat badan dengan konversi sputum pasien Tuberkulosis Paru di RS Persahabatan tahun 2013 - 2015. Desain studi penelitian ini adalah potong lintang dengan sampel penelitian sebanyak 100. Sampel penelitian diambil dari data rekam medis dan dianalisis dengan uji Chi-Square.

"Latar belakang pendidikan dan pengetahuan mengenai Tuberkulosis (TB) merupakan salah satu faktor yang dapat mempengaruhi konversi sputum TB. Studi ini bertujuan untuk mengetahui hubungan antara level edukasi pasien dan tingkat pengetahuan tentang TB dengan konversi sputum pada dua bulan.Studi potong lintang ini di lakukan di Rumah Sakit Persahabatan dengan menganalisa latar pendidikan dan pengetahuan mengenai TB. Dari 106 pasien (63 laki-laki, 43 perempuan) dengan rentang umur 20-65 tahun dilakukan interview langsung dan pengisian kuesioner untuk mengetahui tingkat pendidikan dan pengetahuan akan TB. Uji sampel chi-square digunakan untuk menilai signifikansi statistik pada penelitian ini. Terdapat hubungan yang bermakna antara latar pendidikan dan pengetahuan mengenai TB (p<0.05); pendidikan dan sputum konversi (p<0.05). Tidak terdapat nilai statistic yang signifikan antara pengetahuan TB dan sputum konversi (p>0.05). Hasil penemuan studi ini menunjukan bahwa latar pendidikan yang lebih tinggi berhubungan dengan tingkat pengetahuan TB dan sputum konversi yang lebih baik. Akan tetapi, tingkat pengetahuan TB yg lebih baik tidak menunjukan bahwa pasien memiliki konversi sputum yang positif pada dua bulan.

Educational backgrounds and level of knowledge are factors that might affect the sputum conversion of the Tuberculosis (TB) patients. This study focused to investigate the association between educational background and level of knowledge of the TB patient and the sputum conversion at two months. This cross-sectional study was done in Persahabatan hospital among 106 patients (63 male, 43 female) with the age ranging from 20-65 years old. The educational background and knowledge level of TB were assessed using a questionnaire and direct interview. A chi- square test was conducted to assess the association between knowledge level of TB and education level, education level and sputum conversion, and knowledge level of TB and sputum conversion. There were a statistically significance association between education level and knowledge about Tuberculosis (p<0.05); education level and sputum conversion (p<0.05); however, knowledge level of TB and sputum conversion were not statistically significant (p > 0.05). These findings suggest that the higher the education, the higher the patient's knowledge level of TB and sputum conversion rate. However, higher knowledge level of TB does not necessary mean that the patient will have a positive sputum conversion at two months."

"Tuberkulosis multidrug resistant (TB MDR) merupakan penyakit infeksi yang terus mengalami peningkatan jumlah kasus setiap tahunnya. Indonesia menempati peringkat ke-delapan dari 27 negara dengan kasus TB MDR paling banyak di dunia (WHO, 2013). Pengobatan yang lebih kompleks dengan durasi yang lebih lama, menjadikan pasien TB MDR seringkali mengalami kegagalan konversi sputum. Kegagalan konversi sputum ini dipengaruhi oleh banyak faktor (multifaktorial). Penelitian ini bertujuan untuk mengetahui hubungan antara faktor demografi (usia dan jenis kelamin), riwayat merokok serta penyakit komorbid (diabetes melitus dan HIV/AIDS) terhadap kejadian gagal konversi sputum pasien TB MDR di RSUP Persahabatan Jakarta tahun 2014-2016. Penelitian ini tergolong penelitian potong lintang dengan data sekunder yang diperoleh dari 51 rekam medis di Poli TB MDR RSUP Persahabatan.
Hasil penelitian ini menunjukkan prevalensi pasien TB MDR dengan gagal konversi sputum sebesar 5.6%. Hasil analisis univariat menunjukkan pasien TB MDR dengan gagal konversi sputum didominasi oleh laki-laki (62.7%); usia dewasa (80.4%); memiliki kebiasaan merokok (58.8%); tidak memiliki riwayat diabetes melitus (82.4%); dan tidak memiliki riwayat HIV/AIDS (100%). Hasil analisis bivariat menunjukkan hubungan yang tidak bermakna antara usia (p=0.084); jenis kelamin (p=0.421); kebiasaan merokok (p=0.550); riwayat diabetes melitus (p=0.799) dengan kegagalan konversi sputum pasien TB MDR. Dari hasil tersebut, dapat disimpulkan bahwa usia, jenis kelamin, riwayat merokok, diabetes melitus, dan HIV/AIDS tidak memiliki hubungan yang bermakna dengan kejadian gagal konversi sputum pasien TB MDR di RSUP Persahabatan Jakarta tahun 2014-2016.

Multidrug resistant tuberculosis (MDR TB) is an infectious disease that continues to increase in the number of cases every year. Indonesia is on 8th rank among 27 countries with the most cases of MDR TB in the world (WHO, 2013). More complex treatment with longer duration, makes MDR TB patients often have sputum conversion failure. This sputum conversion failure is influenced by many factors (multifactorial). The aim of this study is to determine the relationship between demographic factors (age and gender), smoking habit, comorbid diseases (diabetes mellitus and HIV/AIDS) with sputum conversion failure of MDR TB patients at RSUP Persahabatan Jakarta in 2014-2016. The design of this study is a cross-sectional study with secondary data obtained from 51 medical records in MDR TB Polyclinic at Persahabatan Hospital.
The results of this study showed the prevalence of MDR TB patients with sputum conversion failure is 5.6%. The results of univariate analysis showed that MDR TB patients with sputum conversion failure were dominated by men (62.7%); adult age (80.4%); have a smoking habit (58.8%); have no history of diabetes mellitus (82.4%); and have no history of HIV/AIDS (100%). The results of bivariate analysis showed an insignificant relationship between age (p=0.084); gender (p=0.421); smoking habits (p=0.550); history of diabetes mellitus (p=0.799) with sputum conversion failure of MDR TB patients. From these results, it can be concluded that age, gender, smoking habit, diabetes mellitus, and HIV/AIDS do not have significant relationships with sputum conversion failure of MDR TB patients at RSUP Persahabatan Jakarta in 2014-2016.

"ABSTRAK
Indonesia merupakan negara dengan beban ganda pada penyakit menular dan tidak menular. Tuberkulosis merupakan salah satu penyakit infeksi yang memiliki prevalensi cukup tinggi dengan Indonesia berperingkat dua dari seluruh negara dalam hal jumlah pasien tuberculosis, diabetes mellitus merupakan salah satu penyakit tidak menular yang memiliki prevalensi cukup tinggi juga. Konvergensi keduanya menimbulkan implikasi serius terhadap morbiditas dan mortalitas dari masing-masing penyakit tersebut, pada tuberkulosis dengan adanya komorbiditas ini meningkatkan resiko munculnya kavitas; kavitas inilah yang memiliki implikasi untuk memperlambat konversi. Dengan menggunakan metode cross-sectional serta analisisi chi-square pada pasien tuberkulosis dengan komorbiditas diabetes mellitus diteliti. Hasilnya tidak ditemukan adanya hubungan yang jelas pada subjek yang memiliki lesi kavitas dan tidak terutama pada konversi pasca 2 bulan pengobatan. Sehingga terdapat perbedaan antara pasien dengan komorbid diabetes mellitus dan tuberkulosis pada umumnya. Perlu dilakukan studi lebih lanjut dengan desain kohort prospektif dengan menghitung kontrol gula darah pada diabetesnya

ABSTRACT
Indonesia is a country with a double burden of both transmissible and intransmissible disease. Tuberculosis is one of the transmissible disease that is quite prevalent in Indonesia and currently Indonesia is second in number of tuberculosis patient, meanwhile one of the leading chronic disease in number in Indonesia is diabetes Mellitus. The convergence of both disease could lead to serious implication in both the morbidity and mortality of each, from tuberculosis standpoint it could lead to delays in elongated duration in treatment; also cavitary lesion is shown to be more common in diabetes mellitus patient, and in general tuberculosis patient it can result in delay of conversion. So, using chi-square analysis the relationship between cavitary lesion and conversion in patient with tuberculosis comorbidity diabetes mellitus is studied. The result is inconclusive (p=0.906) with the realiton between cavitary lesion and conversion in tuberculosis patient with diabetes mellitus after 2 month intensive treatment. Thus this study should be reassessed by using prospective study design and with the control of glucose level to be respected

"ABSTRAK
Latar belakang: Tuberkulosis TB menempati peringkat kedua penyebab kematian akibat infeksi setelah human immunodeficiency virus HIV di dunia. Tanpa pengobatan, angka kematian TB tinggi. Selama pengobatan TB, dapat terjadi hepatitis imbas obat HIO . Kejadian ini dapat menyebabkan pasien mendapat perubahan paduan obat antituberkulosis OAT . Perubahan paduan obat mungkin akan berakibat pada angka konversi.Metode: Lima puluh dari 72 sampel dengan TB paru bakteriologis kasus baru dengan HIO yang tercatat di dalam rekam medik diambil datanya secara retrospektif. Data usia, jenis kelamin, status gizi, hasil pemeriksaan batang tahan asam BTA , waktu timbulnya HIO, faktor komorbid HIV dan DM , riwayat merokok, alkohol, OAT yang dihentikan, jenis OAT yang digunakan saat HIO dan parameter hematologi dicatat untuk kemudian dianalisis.Hasil penelitian: Angka konversi TB paru kasus baru yang mendapat perubahan paduan OAT akibat HIO adalah 70 . Kami dapatkan 26 pasien dengan usia > 50 tahun, 60 status gizi kurang dan 26 dengan DM. Tidak didapatkan hubungan bermakna antara usia, jenis kelamin, status gizi, komorbid DM dan HIV serta jenis OAT yang digunakan saat HIO terhadap terjadinya konversi namun didapatkan responden HIO dengan status gizi kurang sebesar 60 mengalami konversi yang rendah 67 . Obat anti tuberkulosis yang digunakan saat HIO terbanyak adalah kombinasi RHES 76 dengan angka konversi 65,7 .Kesimpulan: Angka konversi TB paru kasus baru yang mendapat perubahan paduan OAT akibat HIO adalah 70 . Pasien TB paru dengan usia tua, status gizi kurang dan DM perlu mendapat pemantauan selama pengobatan. Perlu penelitian lebih lanjut dengan jumlah sampel yang lebih besar serta diikuti secara prospektif untuk mendapatkan data yang lebih detail sehingga faktor lain yang berpengaruh terhadap angka konversi dapat diketahui.

ABSTRACT
Background Tuberculosis TB ranks as the second leading cause of death from an infectious disease worldwide after the human immunodeficiency virus HIV . Without treatment, the mortality rates of TB are high. Drug induced hepatotoxicity can occure during TB treatment which is leading to non standard antituberculosis drugs use. Modification of therapy might influence the conversion rate.Method Data collected from medical records retrospectively, 50 0f 72 samples with newly diagnosed pulmonary tuberculosis and drug induced hepatitis who received modified regimen included in this study. Age, gender, nutritional status, sputum smear, time to occurance of hepatotoxicity, comorbid, smoking history, antituberculosis drug used after hepatotoxicity and hematology parameter are written for analysed.Results Conversion rate in newly diagnosed pulmonary TB patients with drug induced hepatitis who received modified regimen was 70 . We found 32 patients with age 50 years old, 60 poor nutritional status and 26 with DM. No significant assosiation found between age, gender, nutritional status, comorbid DM, HIV and antituberculosis drug used after hepatotoxicity to conversion. Subjects with poor nutritional status are 60 with less sputum conversion 67 . Combination of RHES were more frequence used of antituberculosis drugs 76 with conversion rate 65,7 .Conclution Conversion rate in newly diagnosed pulmonary TB patients with drug induced hepatitis who received modified regimen was 70 . Pulmonary tuberculosis patients with older age, poor nutritional status and DM need evaluation during treatment. Further research with large samples and prospective design are needed for getting more information and find other factors that influence sputum conversion."

"[Munculnya berbagai tantangan baru dalam pengendalian TB, salah satunya multidrug resistant tuberculosis (TB MDR). TB MDR adalah salah satu jenis resistensi TB yang disebabkan oleh bakteri Mycobacterium tuberculosis yang tidak merespon (resisten), setidaknya, isoniazid dan rifampicin yang merupakan dua jenis obat yang paling efektif pada lini pertama obat anti TB (OAT). Penelitian bertujuan untuk mengetahui faktor-faktor yang mempengaruhinya
konversi kultur sputum pada pasien TB Paru MDR. Penelitian dilakukan di dilakukan di RSUD Labuang Baji Kota Makassar dimulai dari bulan April 2015-Juni 2015. Desain penelitian adalah kohort retrospektif. Jumlah sampel dalam penelitian ini yakni 183 pasien, 139 pasien (76,0%) yang mengalami konversi kultur sputum, 4 pasien (2,2%) yang tidak mengalami konversi kultur sputum, dan 40 pasien (21,8%) yang loss to follow up. Dari penelitian ini diketahui bahwa probabilitas konversi kultur sputum pasien TB paru MDR sebesar 95,52%. Hasil
analisis multivariat menunjukkan bahwa interupsi pengobatan (HR:0,45; 95%CI: 0,26-0,79), status diabetes melitus (DM) sebelum 33 hari (HR:0,75; 95%CI: 0,29- 1,95) dan setelah 33 hari yakni (HR:1,95; 95%CI: 0,90-7,60), serta riwayat pengobatan yang pernah mendapatkan OAT lini I (HR:0,32; 95%CI: 0,12-0,90) serta yang pernah mendapatkan OAT lini II (HR:0,27; 95%CI: 0,10-0,77). Diperlukan penanganan secara intensif dan lengkap pada pasien TB paru MDR di Poli TB MDR dengan memperhatikan interupsi pengobatan, status DM, dan riwayat pengobatan sebelumnya;One of the new emerging challenges in TB controlling is multidrug resistant tuberculosis (MDR TB). MDR TB is a type of TB resistant caused by the unresponsiveness (resistancy) of Mycobacterium tuberculosis to at least isoniazid and rifampicin in which both are the most effective anti-TB drugs in first line. "Tuberkulosis merupakan salah satu masalah kesehatan global. Terdapat banyak pasien tuberkulosis memiliki status gizi kurang saat awal diagnosis yang berdampak pada penurunan daya tahan tubuh pasien tersebut, sehingga meningkatkan risiko terjadinya kegagala dapn konversi. Penelitian ini bertujuan untuk mengetahui hubungan antara status gizi pasien tuberkulosis pada awal diagnosis dengan keberhasilan konversi sputum.
Penelitian ini menggunakan desain kohort retrospektif dengan menggunakan data sekunder yang didapat dari Kartu Pasien TB.01 di UPT Puskesmas Sukmajaya, UPF Puskesmas Villa Pertiwi dan UPF Puskesmas Abadi Jaya n=131. Pada penelitian ini didapatkan 93,2 pasien dengan status gizi kurang BMI0,05 antara status gizi pasien tuberkulosis saat awal diagnosis dengan keberhasilan konversi sputum setelah pengobatan fase intensif dilakukan RR 1,016 ,95 CI,0,932-1,108.

Tuberculosis is one of global health problem. There is many tuberculosis patients who have low nutritional status in the initial of diagnosis that can lower the immune system of the patients and increase the risk of conversion failure. The aim of this study is to evaluate the correlation between the nutritional status of tuberculosis patient in the initial of diagnosis and the success of sputum conversion after an intensive phase of treatment been performed.
This study used a retrospective cohort design using secondary data which obtained from Kartu Pasien TB.01 in UPT Puskesmas Sukmajaya, UPF Puskemas Villa Pertiwi and UPF Puskesmas Abadi Jaya n 131. In this study, 93,2 patients with low nutritional status BMI 0,05 between the nutritional status of tuberculosis patients in the initial of diagnosis and the success of sputum conversion after an intensive phase of treatment been performed RR 1.016, 95 CI, 0.932 to 1.108.

"Tingkat kejadian TB Paru di Indonesia masuk dalam kelompok urutan ketiga setelah India dan China berdasarkan laporan WHO tahun 2009 (Riskesdas, 2010). Di Indonesia TB Paru merupakan pembunuh nomor satu diantara penyakit menular dan merupakan penyebab kematian nomor tiga setelah jantung dan penyakit pernapasan akut pada seluruh kalangan usia (Soedarsono, 2006). TB Paru merupakan penyakit menular yang menyebar melalui batuk dan dahak. Penyakit TB ini dipengaruhi oleh faktor usia, pekerjaan, status pernikahan dan riwayat penyakit.
Desain yang digunakan pada penelitian ini adalah deskriptif, sampel yang digunakan yaitu pasien TB di poliklinik paru RS Persahabatan sejumlah 99 responden. Pengambilan sampel menggunakan simple non random sampling dengan teknik accidental sampling.
Hasil penelitian dianalisis menggunakan analisis univariat ditemukan 89,1% responden memiliki harga diri tinggi (normal). Pada penelitian ini berarti tidak adanya gangguan pada harga diri pasien TB di poliklinik paru RS Persahabatan. Rekomendasi untuk pihak RS Persahabatan untuk memberikan pendidikan kesehatan kepada pasien TB tentang pengobatan teratur sehingga tidak terjadi kasus TB berulang.

Pulmonary TB incidence rate in Indonesia in the group third after India and China by 2009 WHO report (Riskesdas, 2010). In Indonesian Tuberculosis is the number one killer among infectious diseases and is the third cause of death after heart disease and acute respiratory in all the ages (Sudarsono, 2006). Tuberculosis is an infectious disease that spreads through coughing and phlegm. TB disease is influenced by age, occupation, marital status and history of disease.
Design used in this study is descriptive, ie the sample used in the clinic of pulmonary TB patients Friendship Hospital a number of 99 respondents. Sampling using non simple random sampling with accidental sampling technique.
The results were analyzed using univariate analysis found 89.1% of respondents have a high selfesteem (normal). In this study means no compromise on dignity pulmonary TB patients in the clinic Persahabatan Hospital. Recommendations for the Persahabatan Hospital to provide health education to patients on TB treatment so it does not happen regularly recurrent TB cases.
