"Apotek Roxy Poltangan merupakan perusahaan ritel farmasi yang bertujuan untuk memberikan pelayanan obat yang berkualitas kepada pasien atau masyarakat. Pelayanan farmasi klinis merupakan pelayanan langsung yang diberikan Apoteker kepada pasien untuk meningkatkan outcome terapi dan meminimalkan risiko efek samping karena obat. Pelayanan farmasi klinis dapat dilakukan dengan kegiatan konseling, PIO, Home Pharmacy Care, atau PTO. Sistem Informasi Monitoring dan Pembinaan Fasilitas Pelayanan Kefarmasian (SIMONA) merupakan sistem yang telah dikembangkan oleh Kementerian Kesehatan Republik Indonesia yang dilakukan untuk monitoring dan pembinaan fasilitas pelayanan kefarmasian di fasilitas kesehatan. Tujuan pembuatan laporan tugas khusus ini adalah untuk mengetahui manfaat dari laporan bulanan pelayanan kefarmasian yang dilakukan oleh Apotek Roxy Poltangan melalui SIMONA. Pembuatan laporan tugas khusus ini dilakukan dengan cara mengisi formulir Pelayanan Informasi Obat (PIO) dan konseling, sesuai dengan resep yang diterima pada bulan November. Resep dikaji dan dilakukan penelusuran literatur untuk pengisian formulir PIO dan konseling. Formulir PIO dan konseling pada bulan November kemudian dilakukan pelaporan melalui laman SIMONA. Pada bulan November, jumlah pelayanan informasi obat (PIO) yang didokumentasikan Apotek Roxy Poltangan adalah sebanyak 42 resep. Sedangkan, untuk total konseling yang berhasil didokumentasikan oleh Apotek Roxy Poltangan adalah sebanyak 15 konseling. Berdasarkan hasil tersebut, Apotek Roxy Poltangan telah melakukan pelaporan pelayanan farmasi klinik melalui laman SIMONA, dimana hal tersebut dilakukan untuk penjaminan mutu pelayanan kefarmasian sesuai dengan peraturan yang berlaku.

Roxy Poltangan Pharmacy is a pharmaceutical retail company that aims to provide quality drug services to patients or the public. Clinical pharmacy services are direct services provided by pharmacists to patients to improve therapeutic outcomes and minimize the risk of side effects due to drugs. Clinical pharmacy services can be provided through counseling, PIO, Home Pharmacy Care, or PTO activities. The Monitoring and Development Information System for Pharmaceutical Service Facilities (SIMONA) is a system that has been developed by the Ministry of Health of the Republic of Indonesia and is carried out for monitoring and developing pharmaceutical service facilities in health facilities. The purpose of making this special assignment report is to find out the benefits of the monthly report on pharmaceutical services carried out by Roxy Poltangan Pharmacy through SIMONA. This special assignment report is made by filling out the Drug Information Service (PIO) and counseling form, according to the prescription received in November. Prescriptions are reviewed, and literature searches are carried out for filling out PIO and counseling forms. PIO and counseling forms in November are then reported via the SIMONA page. In November, the number of drug information services (PIO) documented by Roxy Poltangan Pharmacy was 42 prescriptions. Meanwhile, the total counseling that was successfully documented by Roxy Poltangan Pharmacy was 15 counseling sessions. Based on these results, Roxy Poltangan Pharmacy has reported clinical pharmacy services via the SIMONA page, which is done to guarantee the quality of pharmaceutical services in accordance with applicable regulations."

"ABSTRAKPraktek Kerja Profesi di Apotek Kimia Farma Cibubur Periode Bulan April Tahun 2016 bertujuan untuk memiliki wawasan, pengetahuan, dan pengalaman praktis dalam melakukan pekerjaan kefarmasian di apotek, memiliki pengetahuan dan pemahaman mengenai standar pelayanan kefarmasian yang dilakukan di Apotek Kimia Farma Cibubur, memiliki gambaran secara nyata mengenai peran, fungsi, dan tanggung jawab sebagai apoteker dalam bekerja di apotek, dan mengetahui permasalahan yang dapat terjadi dalam melakukan pekerjaan di apotek dan memahami strategi penyelesaiannya. Selama melakukan praktek kerja, mahasiswa melakukan berbagai kegiatan yang termasuk dalam standar pelayanan kefarmasian di apotek yaitu pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, serta melakukan pelayanan farmasi klinik kepada pasien. Praktek kerja profesi dilakukan selama empat minggu dengan tugas khusus yaitu Profil Peresepan di Apotek Kimia Farma Cibubur pada Bulan Maret 2016.

ABSTRAKProfession internship at Kimia Farma Cibubur Pharmacy on April 2016 was intended to have insight, knowledge, and practical experience in doing pharmaceutical practice in pharmacy, have knowledge and understanding about standard of pharmaceutical service performed in Kimia Farma Cibubur Pharmacy, have description about roles, functions, and responsibilities as clinical apothecary in pharmacy, and understand to solve problems happened during working in pharmacy. During practice, student was performed several activity related to pharmaceutical services are management of pharmaceutical dosage, medical devices, medical disposable material, and doing pharmaceutical service to patients. Practice was performed for four weeks with the specific assignment is Profile Prescribing in Kimia Farma Cibubur in March 2016."
