"ABSTRAKKesiapan meningkatkan proses melahirkan merupakan suatu hal yang harus diperhatikan pada periode kehamilan. Hal tersebut dapat dilakukan dengan memberikan edukasi kepada ibu hamil dan pasangannya mengenai fisiologi persalinan, edukasi tanda dan gejala persalinan, edukasi pentingnya keterlibatan pasangan atau keluarga saat persalinan, dan edukasi manajemen nyeri persalinan. Materi edukasi manajemen nyeri persalinan yang dapat perawat berikan antara lain cara untuk menghadapi nyeri persalinan. Kesiapan meningkatkan proses melahirkan menjadi masalah keperawatan utama pada Ny. M berkaitan dengan persiapan ibu menghadapi nyeri persalinan. Implementasi yang dilakukan untuk masalah tersebut adalah dengan melakukan manajemen nyeri nonfarmakologis dengan teknik counterpressure. Evaluasi dari tindakan tersebut adalah skala nyeri klien berkurang dan klien mampu beradaptasi terhadap nyeri dengan menerapkan teknik tersebut. Hasil karya ilmiah ini merekomendasikan pelayanan kesehatan dapat membuka kelas edukasi bagi ibu hamil dan pasangannya, yang menekankan pada adaptasi proses melahirkan.

ABSTRACTReadiness for enhanced childbearing process is a matter of concern for the period of pregnancy. This can be done by providing education to pregnant women and their partners about the physiology of labor, signs and symptoms of labor, the importance of the involvement of spouses or families during childbirth, and management of labor pain. Maternity management education materials that nurses can be provided is ways to deal with labor pain. Readiness for enhanced childbearing process becomes a major nursing problem in Mrs. M which is related to the preparation of the mother to deal with labor pain. Implementation carried out for the problem is to perform nonpharmacologic pain management with counterpressure techniques. Evaluation of the intervension is reduce client pain scale and able to adapt the pain by applying the technique. The results of this scientific work recommends that health care can open an education class for pregnant women and their partners, emphasizing the adaptation of the childbearing process."

"ABSTRAKChronic Kidney Disease (CKD) merupakan penyakit gagal ginjal kronik, terjadi penurunan fungsi ginjal secara progresif dan ireversible. Hipertensi dan diabetes mellitus adalah faktor resiko utama penyebab CKD. Pada klien CKD terjadi ketidakmampuan ginjal untuk memfiltrasi cairan tubuh dan penumpukan zat sisa hasil metabolisme yang dapat berpengaruh terhadap metabolik tubuh. Berbagai komplikasi dapat terjadi pada klien dengan CKD, seperti overload cairan menjadi masalah utama yang terjadi. Sehingga klien membutuhkan terapi pengganti ginjal, salah satunya berupa Hemodialisis (HD) dan kontrol faktor resiko sangat penting dilakukan oleh perawat dalam asuhan keperawatan. Untuk itu, penting penatalaksanaan pembatasan cairan, kontrol faktor resiko yaitu kontrol tekanan darah, kontrol glikemik dan edukasi guna pencegahan komplikasi dan overload cairan. Hasil analisa menunjukan terdapat penurunan overload cairan, kadar ureum dan kreatinin setelah dilakukan Hemodialisis, intervensi kontrol teknanan darah, kontrol glikemik dan edukasi diharapkan mampu mengurangi komplikasi.

ABSTRACTChronic Kidney Dieseas (CKD) is a chronic kidney failure disease, there is a progressive and such decrease in renal function. Hypertension and Diabetes Mellitus are the major risk factors for CKD. In CKD client there is inability of the kidney to filter body fluid and accumulation of residual substance of metabolism which can affect to metabolic body. Various complications can occur in clients with CKD, such as fluid overload being the main problem that occurs. So the client needs renal replacement therapy, one of which is Hemodialysis (HD) and risk factor control is very important done by nurses in nursing care. Therefore, it is important to manage fluid restrictions, control of risk factors such as blood pressure control, glycemic control and education for the prevention of complications and fluid overload. The results showed that there was a decrease in fluid overload, urea and creatinine levels before and after Hemodialysis, blood pressure control, glycemic control and intervention education were expected to reduce complications."
