"Nitric oxide(NO) is a biologic mediator of various physiologic functions. Recent evidence suggests the clinical utility of fractional exhaled NO (FeNO) as a biomarker for assessing asthma and other respiratory diseases. FeNO methodologies have been recently standardized by international research groups subsequently validated in several Korean population studies. Normal ranges for FeNO have been reported for various ethnic groups and and the clinical utility has been widely evaluated in asthma and various respiratory diseases. Based on current evidence including most of Korean population data, this position paper aims to introduce the and provide guidance for the FeNO measurements Korean populations."

"Latar belakang penelitian : Gejala klinis dan fungsi paru pada asma tidak sensitif dalam mencerminkan inflamasi saluran napas yang mendasarinya dan monitoring proses inflamasi pada asma yang terbaru telah tersedia saat ini. Kadar NO pada udara ekspirasi saat ini dikenali sebagai tanda peradangan eosinofil, merupakan pemeriksaan non invasif dan sangat mudah untuk dikerjakan akan tetapi masih sangat mahal.
Metode penelitian : Asma dibagi menjadi 2 kategori yaitu terkontrol dan tidak terkontrol. Sembilan puluh enam subyek penelitian yang memenuhi kriteria inklusi dan ekslusi dinilai kontrol asmanya dengan ACT kemudian dilakukan pengukuran kadar NO dan spirometri dengan menggunakan metode penelitian cross-sectional comparative.
Hasil penelitian : Sembilan puluh enam subyek penelitian berhasil dikumpulkan. Lima puluh orang subyek merupakan kelompok asma terkontrol (41 orang asma terkontrol sebagian dan 8 orang asma terkontrol penuh) dan 47 orang merupakan kelompok asma tidak terkontrol. Semua pasien mendapatkan terapi asma sesuai dengan GINA 2011. Berdasarkan nilai spirometri VEP1/KVP untuk menilai derajat obstruksi 26 (53,3%) kelompok asma tidak terkontrol memiliki nilai normal, 14 (29,8%) dengan obstruksi ringan dan 7 (14,9%) dengan derajat obstruksi sedang. Sementara itu 25 (51%) kelompok asma terkontrol memiliki nilai normal, 21 (42,9%) dengan derajat obstruksi ringan dan 3 (6,1%) dengan derajat obstruksi sedang. Tidak ditemukan derajat obstruksi berat pada kedua kelompok asma. Nilai median NO pada kelompok asma terkontrol adalah 27 part per billion (ppb) xx (6;10), sedangkan pada kelompok asma tidak terkontrol 40 ppb (5;142) dengan nilai p 0,002.
Kesimpulan : Kelompok asma tidak terkontrol memiliki nilai NO lebih tinggi dibandingkan dengan kelompok asma terkontrol. Lebih dari 50% subyek penelitian ditemukan tidak memiliki obstruksi berdasarkan nilai VEP1/KVP.

Background: Clinical findings and lung function test results of asthmatic patient are happen to be less sensitive in reflecting the underlying airway inflammation. Such required monitoring of this process has only recently become available. Exhaled nitric oxide is recognized as reliable surrogate marker of eosinophilic airway inflammation and offers the advantage of being completely non-invasive, easy procedure.
Methods: This cross-sectional comparative study involves 96 asthmatic subjects whom fulfilled the inclusion and exclusion criteria. Subjects are then classified into two main categories of asthma which are controlled asthma and uncontrolled asthma based on ACT questionnaire. Nitric oxide level measurement and spirometry examination are then performed in both of controlled asthma and uncontrolled asthma.
Results: Ninety six subjects were included in this study. Fifty subjects had controlled asthma (41 partially controlled, 8 fully controlled) and 47 had uncontrolled asthma. All patients had been using asthma medication on regular basis. Based on FEV1/FVC 26 (55,3%) uncontrolled asthma patients had normal results, 14 (29,8%) had mild obstruction and 7 (14,9%) had moderate obstruction. Meanwhile, 25 (51%) controlled asthma patients had normal results, 21 (42,9%) had mild obstruction, and 3 (6,1%) had moderate obstruction. No patients had severe obstruction. Median of NO in controlled asthma patients was 27 part per billion (ppb), (6;110) while in uncontrolled asthma was 40 ppb (5;142) with pvalue 0,002.
Conclusion: Uncontrolled asthma patients had higher measured level of exhaled NO compared to controlled asthma patients. More than 50% subjects had no obstruction based on FEV1/FVC.

"Pendahuluan: Asma merupakan suatu penyakit yang bersifat heterogen dan ditandai oleh inflamasi jalan napas kronik dengan gejala mengi, sesak napas, dada terasa berat, dan batuk yang bersifat bervariasi dalam intensitas dan waktu serta disertai dengan keterbatasan aliran udara ekspirasi yang bervariasi. Obstructive sleep apnea (OSA) ditandai dengan kolaps berulang jalan napas atas saat tidur yang menyebabkan berkurang atau berhentinya aliran udara walaupun sedang dalam keadaan bernapas yang dapat mengakibatkan gangguan pertukaran gas secara intermiten dan tidur yang terganggu. Berbagai studi sebelumnya telah mendapatkan hubungan antara asma dan OSA akibat keparahan asma yang diderita.
Tujuan: Mengetahui perbedaan keadaan inflamasi dengan penilaian kadar fractional- exhaled nitric oxide (FeNO) di antara kelompok derajat obstructive sleep apnea (OSA) pada penderita asma persisten rawat jalan.
Metode: Penelitian ini telah melibatkan sebanyak 13 orang penderita asma persisten sedang yang berobat di poli asma yang menjalani skrining kuesioner STOP-BANG dan spirometri, kemudian dilanjutkan dengan pemeriksaan polysomnography dan pengukuran FeNO.
Hasil: Pasien asma persisten sedang didominasi oleh pasien perempuan dengan rerata usia 48,38+12,494 tahun. Sebanyak 53,85% pasien memiliki berat badan obese dengan rerata 28,67+7,385 kg/m2. Median skor ACT 17 (7-23) dengan 61,46% pasien memiliki skor ACT yang tidak terkontrol. Rerata nilai VEP1 prediksi 70,38+20,230% dengan 61,54% pasien menunjukkan obstruksi sedang. Rerata rasio VEP1/KVP 72,85+12,681% dengan 53,85% pasien menunjukkan rasio VEP1/KVP obstruksi. Median skor STOP-BANG 4 (3-
6) dengan 53,85% pasien menunjukkan risiko sedang. Rerata kadar FeNO 29,62+9,152 ppb dengan 76,92% pasien memiliki kadar FeNO 25-50 ppb. Rerata AHI 11,39+18,222 kejadian/jam dengan 76,92% pasien menunjukkan AHI derajat ringan. Pasien asma persisten sedang dominan menunjukkan kadar FeNO 25-50 ppb dengan skor ACT tidak terkontrol berdasarkan derajat AHI dan risiko STOP-BANG. Pasien asma persisten sedang dominan menunjukkan nilai VEP1 prediksi obstruksi sedang dan rasio VEP1/KVP obstruksi berdasarkan risiko STOP-BANG. Pasien asma persisten sedang dominan menunjukkan nilai VEP1 prediksi obstruksi sedang dan rasio VEP1/KVP obstruksi berdasarkan derajat AHI.
Kesimpulan: Pasien asma persisten sedang memiliki skor STOP-BANG risiko sedang dan OSA derajat ringan dengan kadar FeNO 25-50 ppb, skor ACT tidak terkontrol, nilai VEP1 prediksi obstruksi sedang dan rasio VEP1/KVP obstruksi.

Introductions: Asthma is a heterogeneous disease manifested by airway inflammation and wheezing, dyspnea, breathlessness and cough which are varied in intensity and time accompanied with variably restricted expiratory airflow. Obstructive sleep apnea (OSA) is manifested by repeated airway collapse or deceased airflow despite breathing causing the impaired gas exchange intermittently and sleep disorder. Some previous studies have correlated between asthma and OSA led by the severity of asthma.
Aims: To reveal the difference of inflammation condition by assessing fractional-exhaled nitric oxide (FeNO) level among graded obstructive sleep apnea (OSA) of persistent asthmatic outpatients.
Methods: This study involved 13 persistent asthmatic patients from asthma policlinic who had been screened using asthma control test (ACT) as well as STOP-BANG questionnaire and spirometry assessment of forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC), then the elective polysomnography and fractional- exhaled nitric oxide (FeNO) measurement.
Results: The moderate persistent asthmatic patients were dominated by female aged 48.38+12.494 years old. There were 53.85% patients presented with obesity whose mean body mass index was 28.67+7.385 kg/m2. The median ACT score was 17 (7-23) of whom 61.46% patients had uncontrolled ACT score. The mean of predicted FEV1 was 70.38+20.230% of whom 61.54% patients showed moderate obstruction. The mean of FEV1/FVC ratio was 72.85+12.681% of whom 53.85% patients showed obstruction of FEV1/FVC ratio. The median STOP-BANG score was 4 (3-6) of whom 53.85% patients showed moderate risk. The mean of FeNO level was 29.62+9.152 ppb of whom 76.92% patients had FeNO level ranging 25-50 ppb. The mean of apnea hypopnea index (AHI) was 11.39+18.222 events/hour of whom 76.92% patients showed the mild grade of AHI. The moderate persistent asthmatic patients mostly showed FeNO level ranging 25-50 ppb whose uncontrolled ACT score based on both grade of AHI and risk of STOP- BANG. The moderate persistent asthmatic patients showed moderate obstruction of predicted FEV based on risk of STOP-BANG. The moderate persistent asthmatic patients mostly showed moderate obstruction of predicted FEV1 and obstruction of FEV1/FVC ratio based on grade of AHI.
Conclusions: The moderate persistent asthmatic patients showed the moderate risk of STOP-BANG score and mild grade of AHI as well with FeNO 25-50 ppb, uncontrolled ACT, moderate obstruction of predicted FEV1 and obstructive FEV1/FVC ratio.

Latar Belakang: Pasien asma dengan tingkat kontrol yang buruk dan adanya
komorbiditas seperti gangguan depresi dan stres psikososial akan memengaruhi
kualitas hidup pasien asma serta meningkatkan beban dan biaya ekonomi yang
harus ditanggung oleh pasien dan keluarganya. Untuk itu perlu diketahui
hubungan antara gangguan depresi dengan kualitas hidup, stresor psikososial, dan
tingkat kontrol asma pada pasien asma.
Metode: Penelitian cross-sectional deskriptif-analitik pada 37 pasien asma yang
memiliki gangguan depresi dan 37 pasien asma yang tidak memiliki gangguan
depresi di Poliklinik Alergi dan Imunologi RSUPN Dr. Cipto Mangunkusumo
Jakarta menggunakan Structured Clinical Interview for DSM IV Disorder(SCID)1,
instrumen World Health Organization Quality Of Life (WHOQOL)-BREF,
instrumen stresor psikososialHolmes & Rahe, dan kuesioner Ashtma Control Test
(ACT).
Hasil: Terdapat hubungan antara ada tidaknya gangguan depresi pada pasien asma
dengan skor kualitas hidup berdasarkan kesehatan fisik (p < 0,001), skor kualitas
hidup berdasarkan kesehatan psikologis (p < 0,001), skor kualitas hidup
berdasarkan relasi sosial (p = 0,023), skor kualitas hidup berdasarkan lingkungan
(p = 0,022), stresor psikososial (OR 3,85; p = 0,005), dan tingkat kontrol asma (p
= 0,001).
Simpulan: Pasien asma yang memiliki gangguan depresi cenderung memiliki
skor kualitas hidup yang lebih rendah pada domain kesehatan fisik, kesehatan
psikologis, relasi sosial, dan lingkungan dibandingkan pasien asma yang tidak
memiliki gangguan depresi. Pasien asma yang mengalami stresor psikososial yang
tinggi berisiko 3,8 kali untuk memiliki gangguan depresi. Pasien asma yang
memiliki gangguan depresi cenderung memiliki skor tingkat kontrol asma yang lebih rendah dibandingkan pasien asma yang tidak memiliki gangguan depresi. ABSTRACT Background: Asthmatic patients with poor control level and the presence of

"ABSTRAK
Pendahuluan: Faktor lingkungan seperti alergen dan polusi udara dapat memicu ataupun memperberat gejala asma akut serta menyebabkan persitens gejala asma terutama asma alergi. Telah banyak usaha dilakukan untuk mengurangi kadar alergen dalam udara. Salah satu caranya adalah dengan mempertahankan keseimbangan anion-kation sehingga terjadi denaturasi protein tungau debu rumah yang merupakan alergen utama. Kondisi keseimbangan anion-kation dalam udara ini serupa dengan kondisi alamiah hutan. Lingkungan yang serupa dapat dicapai menggunakan filter udara dengan ioniser sehingga tercapai keseimbangan anion-kation dalam udara. Apakah kondisi ini akan mempengaruhi kondisi inflamasi saluran napas dan fungsi paru pasien asma alergi belum pernah dibuktikan sebelumnya.
Metode penelitian: Penelitian ini dilakukan dengan uji klinis silang (cross-over), terbuka, pada pasien-pasien asma alergi. Data subjek diambil secara mandiri oleh pasien dan pemeriksaan berkala di Poli Asma. PPOK RS Persahabatan sejak Desember 2011 sampai September 2012. Pemeriksaan FeNO menggunakan NIOX mino, uji fungsi paru (spirometri), serta penilaian ACT dilakukan setiap bulan di RS Persahabatan.
Hasil Penelitian: Terdapat 36 pasien yang berhasil mengikuti penelitian sampai selesai, selebihnya mengundurkan diri. Terdapat enam subjek laki-laki dan 30 perempuan, rerata usia 42,72 tahun (18-63). Klasifikasi terbanyak adalah asma persisten ringan (19) diikuti dengan asma persisten sedang dan berat (10 dan 7). Perbandingan selisih nilai ACT akhir dengan nilai awal antara kelompok kontrol dengan perlakuan berbeda bermakna secara statistik (p=0,008) maupun secara klinis (rerata kenaikan 3,31 poin). Tidak didapatkan perbedaan yang bermakna secara statistik dalam nilai FeNO dan uji fungsi paru antara kedua kelompok pengamatan.
Kesimpulan: Pada penelitian ini didapatkan peningkatan nilai asthma control test setelah penggunaan filter udara dengan ioniser namun tidak didapatkan perbedaan inflamasi saluran napas dan nilai faal paru. Penggunaan filter udara anion-kation seimbang dapat direkomendasikan pada pasien asma alergi.

ABSTRACT
Introduction: Many attempts have been tried to reduce concentration of allergens which may precipitate acute asthma or cause persistence of symptoms especially in allergic asthma patients. One of the techniques used is air filter and ionizer which creates a balance anion-cation ambiance. Studies have showed that its use can reduce airborne allergen concentration. Indoor ionised air has been proven to cause protein denaturation of house dust mite allergen, one of the most prominent indoor allergen. Ionised air has been proved to cause protein denaturation of mite allergens. This condition is similar to the natural condition existing in uncontaminated natural forests. This condition may be achieved by using a commercially available air purifier and ionizer. Whether this condition affects airway inflammation and lung function test in allergic asthma patients is yet to be proven.
Methods: This is a cross-over, unblinded, clinical trial, conducted in allergic asthma patients. Serial spirometry and FeNO measurements are performed monthly. Subjects are also asked to fill ACT for assement of asthma control. Subjects is observed for two months without using air filter in their bedrooms and two months using air filter in their bedrooms with a two weeks interval in between observation.
Results: There were 50 patients enrolled in the beginning of this study but 14 dropped out while 36 completed the study. There were six male subjects and 30 female, averaging 42,72 (min 18, max 63). Most patients were mild persistent (n=30), followed by moderate ande severe persistent asthma (10 and 7, respectively). The difference between baseline and end of two months observation in control and treatment group was statistically and clinically significant (paired t-test, p=0,008, 3 points ACT increase). Although there was a trend of decreased FeNO and increased FEV1/predicted ratio, time series and multivariate analysis in both was not statistically significant.
Conclusion: There was an increase of ACT score after air filter with ionizer usage but the change in FeNO and lung function test was not statistically significant. Air purifier can be recommended in allergic asthma patients to increase asthma control."

"Menurut Global Initiative for Asthma, (2020), lebih dari 70-80% pasien asma tidak menggunakan obat inhalasi dengan benar. Teknik penggunaan obat inhalasi yang tidak tepat dikaitkan dengan tingkat kontrol asma yang buruk dan risiko tinggi terjadinya eksaserbasi. Edukasi teknik penggunaan obat inhalasi yang tepat dibutuhkan untuk mencapai asma yang terkontrol sepenuhnya. Edukasi berbasis aplikasi seluler memudahkan akses pasien asma dalam memperoleh informasi terkait teknik penggunaan obat inhalasi yang tepat. Penelitian ini bertujuan untuk mengetahui pengaruh edukasi berbasis aplikasi seluler yang dikembangkan oleh tim peneliti terhadap tingkat kontrol asma yang dinilai dengan skor Asthma Control Test (ACT, rentang skor 5-25) serta ketepatan penggunaan obat inhalasi. Studi prospektif dengan desain one group pretest-posttest (pre-eksperimental) ini dilakukan di Poliklinik Paru Rumah Sakit Grha Permata Ibu. Sebanyak 40 pasien asma yang memenuhi kriteria inklusi dan bersedia mengikuti penelitian, menerima akses untuk memperoleh edukasi teknik penggunaan obat inhalasi melalui aplikasi seluler. Tingkat kontrol asma dan teknik penggunaan obat inhalasi dinilai sebelum intervensi dan dinilai kembali setelah 1 bulan intervensi. Hasil analisis menggunakan uji Wilcoxon menunjukan adanya peningkatan yang bermakna secara statistik dan secara klinik pada median skor ACT [18 vs. 22, (p<0,05)]. Peningkatan ketepatan teknik penggunaan obat inhalasi yang bermakna (p<0,05) juga ditemukan pada pengguna obat inhalasi jenis turbuhaler, diskus, dan pMDI. Berdasarkan hasil penelitian dapat disimpulkan bahwa pemberian edukasi berbasis aplikasi seluler yang dikembangkan oleh tim peneliti mengenai teknik penggunaan obat inhalasi memengaruhi tingkat kontrol asma dan ketepatan penggunaan obat inhalasi.

According to the Global Initiative for Asthma (2020), more than 70-80% of asthma patients in the world do not use inhaler properly. Inappropriate inhaler technique is associated with poor asthma control and high risk of exacerbations. Education on the proper use of inhaler technique is needed to achieve fully controlled asthma. Mobile application-based education eases asthma patient to access information related to appropriate technique of using inhalers. This study aims to examine the effect of mobile application-based education developed by the research team on the level of asthma control assessed by Asthma Control Test (ACT, range 5-25) score and accuracy of inhaler technique. A prospective, one-group pre- and post-test research design (pre-experiment) was conducted at the Pulmonary Clinic of Grha Permata Ibu Hospital. A total of 40 asthma patients who met the inclusion criteria and agreed to participate in the study, received access to mobile application-based education. Asthma control and inhaler technique were assessed before the intervention and reassessed after 1 month of intervention. The results of the analysis using the Wilcoxon test showed a statistically and clinically significant increase in the ACT score [18 vs. 22, (p<0,05)]. A significant increase in the accuracy of inhaler technique (p<0.05) was also found in users of turbuhaler, diskus, and pMDI inhalers. Based on the result of this study, it can be concluded that mobile application-based education developed by the research team about proper inhaler technique affects the level of asthma control and accuracy of inhaler technique."
