"ABSTRAK
Maxillary permanent canines are impacted in about 2% of orthodontic patients, second only in frequency of impaction to third molars. Bringing an impacted canine into the arch creates a set of problems primarily because it is usually a long way from the line of occlusion. Four cases of impacted maxillary canines are presented to show some of the problems encountered during orthodontic traction of these teeth. The cases were treated using a preadjusted edgewise appliances. Careful judgement are needed to treat impacted maxillary canines successfully and to minimize discouraging side effects that often occurs during the procedure."

"Latar Belakang: Maloklusi adalah ketidakteraturan kesejajaran gigi dan/atau hubungan lengkung gigi dengan gigi yang tidak normal yang diakibatkan oleh berbagai faktor dan dapat menyebabkan ketidakpuasan estetika sampai masalah pada segi fungsional. Pasien dengan maloklusi memerlukan perawatan ortodonti salah satunya untuk memperbaiki maloklusi. Inklinasi dan angulasi gigi menjadi salah satu faktor yang mempengaruhi keberhasilan perawatan ortodonti yang stabil dan optimal. Tujuan: Mengetahui gambaran sudut inklinasi dan angulasi gigi anterior pada pasien maloklusi skeletal kelas I pasca perawatan ortodonti cekat di klinik spesialis ortodonti RSKGM FKG UI. Metode: Penelitian yang dilakukan merupakan penelitian deskriptif dengan desain potong lintang (cross-sectional) menggunakan sampel berupa data sekunder rekam medik. Hasil: Dari 96 rekam medik pasien maloklusi kelas I yang telah selesai mendapatkan perawatan ortodonti cekat di Klinik Spesialis Ortodonti RSKGM FKG UI, didapatkan rerata sudut U1-SN sebesar 105,60° ± 5,80°, rerata sudut U1-PP sebesar 114,55° ± 6,21°, rerata sudut L1-MP sebesar 93,63° ± 7,94°, dan rerata sudut IMPA adalah sebesar 96,40° ± 7,96°. Rerata angulasi gigi 11 sebesar 89,03° ± 3,26°, rerata angulasi gigi 21 sebesar 90,35° ± 3,07°, rerata angulasi gigi 31 sebesar 89,28° ± 4,33°, dan rerata angulasi gigi 41 sebesar 90,61° ± 5,04°. Kesimpulan: Berdasarkan penelitian tentang Gambaran Inklinasi dan Angulasi Gigi Anterior pada Pasien Maloklusi Kelas I Pasca Perawatan Ortodonti Cekat di Klinik Spesialis Ortodonti RSKGM FKG UI, rerata sudut inklinasi gigi anterior pasien termasuk dalam kisaran nilai normal, kecuali pada sudut IMPA. Rerata sudut angulasi gigi anterior pasien relatif tegak dan paralel.

Background: Malocclusion is the irregularity of teeth and is considered as oral health problem resulting from various etiological factors causing esthetic dissatisfaction to functional impartment. Patients with malocclusion require orthodontic treatment to correct the malocclusion. Inclination and angulation of teeth are one of the factors that influence the success of stable and optimal orthodontic treatment. Objective: This study aims to describe the inclination and angulation of anterior teeth on class I malocclusion patients after fixed orthodontic treatment at the Orthodontic Specialist Clinic of RSKGM FKG UI. Methods: Cross-sectional descriptive study is done using the secondary data found in the patient’s medical record. Results: From 96 medical records of class I malocclusion patients who have completed fixed orthodontic treatment at the Orthodontic Specialist Clinic of RSKGM FKG UI, the mean U1-SN angle is 105.60° ± 5.80°, the mean U1-PP angle is 114.55°. ± 6.21°, the mean angle of L1-MP is 93.63° ± 7.94°, and the mean angle of IMPA is 96.40° ± 7.96°. The mean angulation of tooth 11 is 89.03° ± 3.26°, mean angulation of tooth 21 is 90.35° ± 3.07°, mean angulation of tooth 31 is 89.28° ± 4.33°, and mean angulation of tooth 41 is of 90.61° ± 5.04°. Conclusion: Based on research on the Inclination and Angulation of Anterior Teeth on Class I Malocclusion Patients after Fixed Orthodontic Treatment at the Orthodontic Specialist Clinic of RSKGM FKG UI, the inclination of anterior teeth is within the normal range, except at the IMPA angle. The angulation of anterior teeth is relatively upright and parallel."

"Ekstraksi premolar dalam perawatan ortodonti membantu proses uprighting gigi molar 3 impaksi sehingga dapat erupsi dengan baik.
Tujuan: mengukur perubahan angulasi gigi molar 3 rahang bawah yang impaksi mesioangular sebelum dan sesudah perawatan ortodonti.
Metode: penelitian ini menggunakan 25 radiograf panoramik berusia 10-21 tahun sebelum dan sesudah perawatan ortodonti.
Hasil: uji Wilcoxon dan uji T berpasangan (p<0,05) menunjukkan tidak ada perubahan angulasi molar 3 yang bermakna pada kedua sisi (p>0,05) dan cenderung mengalami peningkatan angulasi dengan meskipun secara statistik perbandingan perubahan keduanya tidak berbeda bermakna (p>0,05). Peningkatan angulasi paling banyak terjadi pada kelompok usia dewasa (17-21 tahun).
Kesimpulan: ekstraksi premolar dalam perawatan ortodonti tidak memengaruhi angulasi gigi molar 3 impaksi secara bermakna.

Premolar extraction in orthodontic treatment helps uprighting process of impacted third molars so that they could erupt well.
Aim: to measure mesioangular impacted lower third molars angulation change during orthodontic treatment.
Methods: this study used 25 panoramic radiograph aged 10-21 years old before and after orthodontic treatment.
Result: Wilcoxon test and paired Ttest (p<0,05) showed there were no significant change in lower third molars angulation on both sides (p>0,05) and tended to experience the increase in angulation though statistically comparison between them were not significant (p>0,05). These increase happen the most in the adult group (17-21 years old).
Conclusion: premolars extraction in orthodontic treatment does not affect impacted third molars angulation significantly.

"Orthodontic medical treatment is performed to move a tooth to the optimal position to obtain optimal occlusion. Orthodontic treatment is accompanied by mechanical stress due to orthodontic force and by psychological stress that is experienced as pain or displeasure. The purpose of this study was to identify stress marker proteins during orthodontic treatment. Levels of receptor activator of NFκB (RANKL) and heat shock protein 70 (HSP70) in the gingival crevicular fluid (GCF) were analyzed as markers of mechanical stress, and levels of chromogranin A (CgA) and amylase in whole saliva were analyzed as markers of psychological stress. GCF was collected from control and experimental teeth at initiation of treatment and 24 h after treatment. Whole saliva was collected before treatment, at initiation of treatment and 24 h after treatment. RANKL was expressed at 24 h after treatment in the experimental GCF, but not in the control GCF. HSP70 appeared to be constitutively expressed in GCF, and its levels showed no major change between the control and experimental groups from initiation of treatment to 24 h after treatment. Amylase activity in whole saliva was enhanced at 24 h after treatment compared to control, but CgA levels showed little change between the groups. These results indicated that RANKL and amylase may be the candidate markers for mechanical and psychological stress, respectively, during orthodontic treatment, even though the total protein concentration and amylase activity displayed a large standard deviation among subjects. Further studies are therefore required to establish these markers for clinical use."

"The purpose of this study was to investigate the patterns of third molar agenesis and incidence of agenesis of other permanent teeth in a Japanese orthodontic patient group. A total of 417 Japanese subjects (134 males and 283 females) with agenesis of one or more third molars were divided into four groups according to the agenesis pattern, and 874 other Japanese subjects (302 males and 572 females) without third molar agenesis were assigned to a control group. Panoramic radiographs and medical and dental records were used to examine for tooth agenesis. The Chi-square test and odds ratio were used to make statistical comparisons. The prevalence of third molar agenesis worked out at 32.3 % with no statistically significant gender difference. A high prevalence rate of agenesis of third molars, unilateral or bilateral, could be considered characteristic of the Japanese orthodontic population. Significant increases in occurrence of oligodontia, and unilateral or bilateral agenesis of other teeth, including maxillary lateral incisors and maxillary and mandibular second premolars, were observed in all or almost all of the third molar agenesis groups, compared with the controls. A significantly increased prevalence rate of mandibular lateral incisor agenesis was observed in almost all of the third molar agenesis groups. The Japanese patients with third molar agenesis had a significantly increased occurrence of oligodontia, and unilateral or bilateral agenesis of maxillary and mandibular lateral incisors and second premolars, except for bilateral agenesis of mandibular lateral incisors."

"Perawatan endodontik setelah perwatan bedah reposisi gigi insisif permanen intrusi pascatrauma. Laporan ini menggambarkan sebuah kasus trauma gigi berat pada seorang anak perempuan berusia 10 tahun yang mengakibatkan intrusi insisif sentral dan lateral rahang atas, yang terjadi 6 hari sebelum ditangani di Klinik Gigi Rumah Sakit Siloam Lippo Village (SLVH). Reposisi gigi secara bedah dan penggunaan splin dilakukan untuk merawat gigi intrusi. Perawatan endodontik dilakukan 3 minggu setelah reposisi. Saluran akar diirigasi dengan larutan NaOCl 2,5% dan ditumpat sementara menggunakan kalsium hidroksida. Splin dilepas setelah 3 bulan. Perawatan saluran akar definitif dilakukan 6 bulan kemudian. Evaluasi perawatan endodontik 9 bulan setelah trauma tidak menunjukkan komplikasi, namun terjadi diskolorasi gigi 2 bulan berikutnya. Kontrol lanjutan dilakukan setiap tahun selama 5 tahun, karena biasanya trauma luksasi belum menunjukkan masalah pada di tahun-tahun pertama. Kombinasi reposisi bedah dengan perawatan endodontik merupakan perawatan yang diperlukan untuk menangai kasus intrusi gigi pascatrauma pada gigi yang sudah tumbuh sempurna.

This report presents the case of a 10-year-old girl who was referred to the Siloam Hospital Lippo Village (SLVH) Dental Clinic 6 days after sustaining a severe traumatism that led to intrusion of the maxillary central and lateral incisors. The intruded teeth were repositioned by using surgical reposition and splinted. Endodontic treatment was performed 3 weeks after repositioning. The root canals were rinsed with 2.5% NaOCl, then calcium hydroxide dressing was placed. Splint was removed after 3 months. Definitive root canal fillings were accomplished 6 months later. The result implied that endodontic treatment until 9 months showed no periodontal complication, but 2 months later the teeth were slightly discolored. Further follow up should be made yearly for at least 5 years, because luxation injuries frequently do not show problems initially. In this case, surgical repositioning combined with endodontic treatment is a viable alternative treatment for severe traumatic intrusion in mature permanent teeth."
