"Latar Belakang. Kemoterapi masih menjadi pilihan pengobatan untuk pasien dengan kanker termasuk pada pasien usia lanjut. Instrumen untuk memprediksi toksisitas berat akibat kemoterapi pada pasien usia lanjut diperlukan untuk menurunkan mortalitas dan meningkatkan kualitas hidup. Penelitian ini bertujuan menilai performa diskriminasi dan kalibrasi instrumen CARG untuk memprediksi toksisitas berat akibat kemoterapi pada pasien usia lanjut dengan kanker.
Metode. Penelitian ini menggunakan desain kohort retrospektif menggunakan data primer penelitian sebelumnya yang menggunakan CGA sebagai prediktor toksisitas kemoterapi. Subyek merupakan pasien usia lanjut yang menjalani kemoterapi di KSM Penyakit Dalam RSUPN Cipto Mangunkusumo Jakarta dari Oktober 2019-Januari 2021 dan dilakukan pengambilan sampel dengan metode
total sampling. Skor CARG yang sudah dihitung dibagi menjadi risiko rendah, risiko sedang dan risiko tinggi. Performa diskriminasi dinyatakan dengan c statistic (AUC) dan performa kalibrasi dinyatakan dengan uji Hosmer Lemeshow. Luaran dari penelitian ini adalah prediksi toksisitas berat (CINV, anemia, febrile neutropenia, dan kematian) akibat kemoterapi pada pasien usia lanjut di KSM Penyakit Dalam RSUPN Cipto Mangunkusumo.Hasil. Dalam penelitian ini terdapat 193 pasien. Kanker terbanyak adalah hematologi (33,2%), kepala dan leher (24,9%) serta gastrointestinal (14%). Toksisitas berat terbanyak pada kanker gastrointestinal (29,6%). Skor CARG memberikan performa diskriminasi yang baik dengan c statistic (AUC) 0,756 (IK95% 0,681-0,830; p<0,001) dan performa kalibrasi yang baik dengan Uji Hosmer Lemeshow p 0,282. NPV pada penelitian ini 79,27%.Kesimpulan. Instrumen CARG memiliki performa diskriminasi dan kalibrasi yang baik untuk memprediksi toksisitas berat pasca kemoterapi pada pasien usia lanjut.
Background. Chemotherapy is still treatment of choice for patients with cancer, including the elderly. Intruments to predict chemotherapy-induced severe toxicity in elderly patients are needed to reduce mortality, improve quality of life. This study aims to assess the discrimination and calibration performance of the CARG instrument to predict severe chemotherapy-induced toxicity in elderly patients with cancer.Methods. This study used a retrospective cohort design using primary data from previous study that used CGA as a predictor of chemotherapy toxicity. The subjects were elderly patients who underwent chemotherapy at Internal Medicine Department CIpto Mangunkusumo National General Hospital from October 2019-January 2021 and sampled was carried using the total sampling method. The calculated CARG score is divided into low risk, medium risk and high risk. Discrimination performance is expressed by c statistic (AUC) and calibration performance is expressed by Hosmer Lemeshow test. The outcome of this study is prediction of severe toxicity (CINV, anemia, febrile neutropenia, and death) due to chemotherapy in elderly patients at Internal Medicine Department Cipto Mangunkusumo National General Hospital.Results. In this study there were 193 patients. The most common cancer were hematology (33.2%), head and neck (24.9%) and gastrointestine (14%). The most severe toxicity was gastrointestin cancer (29.6%). The CARG score provides a good discrimination performance with c statistic (AUC) 0.756 (CI 95% 0.681-0.830; p <0.001) and good calibration performance with Hosmer Lemeshow test p 0.282. The NPV in this study was 79.27%.Conclusion. The CARG instrument has a good performance for predicting severe in post-chemotherapy toxicity in elderly patients."
