"Latar belakang: Operasi jantung terbuka dengan menggunakan mesin bypass kardiopulmoner (CPB) berpotensi menimbulkan respons inflamasi yang signifikan. Salah satu penyebab respons inflamasi ini adalah kontak darah dengan sirkuit ekstrakorporeal dan shear stress non-fisiologis selama operasi pompa CPB. Penelitian ini bertujuan untuk menentukan teknik mana yang menghasilkan respons sindrom inflamasi sistemik (SIRS) yang lebih ringan.
Metode: Penelitian kohort retrospektif. Pengambilan data dilakukan di Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita (RSJPDHK) sejak Juli 2018 hingga Februari 2023. Subjek yang direkrut secara konsekutif adalah pasien dengan diagnosis penyakit jantung koroner yang menjalani operasi bedah pintas arteri koroner, baik dengan menggunakan teknik CCABG atau dengan teknik OPCAB. Setelahnya subjek dilakukan pemeriksaan IL-6 pada 6 jam pasca operasi, CRP dan PCT pada 6, 24, dan 48 jam pascaoperasi. Setelahnya, pasien dinilai keluaran SIRS dan mortalitasnya.
Hasil: Total subjek penelitian ada 70 subjek, dengan perbandingan laki-laki yang menjalani OPCAB (82,9%) dan CCABG (91,4%), sisanya berjenis kelamin perempuan. Terdapat perbedaan bermakna antara SIRS dengan jenis operasi (p = 0,048). Kadar IL-6 pada 6 jam pascaoperasi menunjukkan hasil berbeda bermakna dengan jenis operasi (0,014). Pada 24 jam pascaoperasi, penanda inflamasi menunjukkan hasil berbeda bermakna pada CRP (p = 0,013) dan PCT (0,001). Sedangkan pada 48 jam pascaoperasi juga menunjukkan hasil berbeda bermakna pada CRP (p = 0,002) dan PCT (p = 0,022). Peningkatan angka kejadian aritmia pada CCABG menunjukkan perbedaan bermakna juga dengan nilai p <0,001 (IK95% 6,14(1,63-23,16)).

Background: Open heart surgery using a cardiopulmonary bypass (CPB) machine has the potential to induce a significant inflammatory response. One of the causes of this inflammatory response is blood contact with the extracorporeal circuit and non-physiological shear stress during CPB pump operation. This study aims to determine which technique yields a milder systemic inflammatory response syndrome (SIRS) outcome.
Method: This is a retrospective cohort study. Data collection was conducted at Harapan Kita Heart and Blood Vessel Hospital (RSJPDHK) from July 2018 to February 2023. Consecutively recruited subjects were patients diagnosed with coronary heart disease who underwent coronary artery bypass grafting (CABG) surgery, either using the conventional technique (CCABG) or off-pump technique (OPCAB). Subsequently, IL-6 levels were examined at 6 hours post-surgery, while CRP and PCT levels were measured at 6, 24, and 48 hours post-surgery. Following these assessments, patients were evaluated for the occurrence of systemic inflammatory response syndrome (SIRS) and mortality.
Result: There were a total of 70 subjects, with a comparison of males who underwent OPCAB (82.9%) and CCABG (91.4%), the rest were female. There was a significant difference between SIRS and the type of surgery (p = 0.048). IL-6 levels at 6 hours postoperatively showed significantly different results with the type of surgery (0.014). At 24 hours postoperatively, inflammatory markers showed significantly different results for CRP (p = 0.013) and PCT (0.001). Whereas at 48 hours postoperatively it also showed significantly different results on CRP (p = 0.002) and PCT (p = 0.022). The increase in the incidence of arrhythmias in CCABG also showed a significant difference with a value of p <0.001 (95%CI 6.14 (1.63-23.16)).

"Latar belakang: Mesin cardiopulmonary bypass (CPB) yang digunakan untuk operasi conventional coronary artery bypass graft (CCABG) meningkatkan risiko terjadinya gangguan fungsi ginjal pascaoperasi. Teknik operasi off-pump coronary artery bypass (OPCAB) tidak menggunakan mesin CPB, sehingga diharapkan dapat menurunkan kejadian acute kidney injury (AKI) pascaoperasi. Gangguan fungsi ginjal pascaoperasi dapat berkomplikasi menjadi penyakit ginjal kronik dan bahkan meningkatkan mortalitas.
Tujuan: Membandingkan gangguan fungsi ginjal pascaoperasi OPCAB dan CCABG dengan menilai peningkatan kreatinin serum, derajat AKI, dan kebutuhan hemodialisis pascaoperasi.
Metode: Penelitian ini menggunakan metode penelitian kohort retrospektif yang dilakukan dengan menganalisis data rekam medis pasien di Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita tahun 2019 – 2021. Data kreatinin serum pascaoperasi diambil pada waktu 24 jam dan 48 jam pascaoperasi, kemudian hasilnya dibandingkan di antara kedua kelompok. Derajat AKI pascaoperasi dan kebutuhan hemodialisis pascaoperasi yang terjadi di antara kedua kelompok juga dibandingkan.
Hasil: . Kelompok pasien OPCAB (n = 277) dan CCABG (n = 770) memiliki data demografis yang tidak berbeda bermakna. Kelompok OPCAB memiliki nilai median kreatinin serum pascaoperasi yang lebih rendah pada waktu 24 jam (1,04 mg/dL vs 1,20 mg/dL; p <0,05) dan 48 jam pascaoperasi (1,12 mg/dL vs 1,21 mg/dL; p<0,05). Kejadian AKI pascaoperasi pada semua stadium dan kebutuhan hemodialisis pascaoperasi juga lebih rendah secara bermakna pada kelompok OPCAB.
Kesimpulan: Teknik operasi OPCAB menghasilkan kreatinin serum dan derajat AKI lebih rendah serta kebutuhan hemodialisis pascaoperasi yang lebih sedikit dibandingkan dengan CCABG.

Background: Cardiopulmonary bypass (CPB) machine, that is used in conventional coronary artery bypass graft (CCABG), increases the risk of postoperative renal dysfunction. Off-pump coronary artery bypass technique does not utilize the CPB machine, therefore is expected to decrease postoperative acute kidney injury (AKI) incidents. Postoperative renal dysfunction can complicate into chronic kidney disease and even increases mortality risk.
Purpose: To compare the postoperative renal dysfunction after OPCAB and CCABG by evaluating the increase of creatinine serum, AKI, and postoperative hemodialysis.
Methods: For this retrospective cohort study, we analyzed the data from patient’s medical record in National Cardiovascular Center Harapan Kita from 2019 to 2021. The patients in OPCAB group (n=277) and CCABG group (n=770) had similar demographic characteristics. Postoperative creatinine serum was measured at 24 hours and 48 hours postoperative, then the results were compared between the two groups. Postoperative AKI and hemodialysis were also compared.
Results: The OPCAB group had lower median value of postoperative creatinine serum at 24 hours (1.04 mg/dL vs 1.20 mg/dL; p <0.05) and 48 hours postoperation. (1.12 mg/dL vs 1.21 mg/dL; p<0.05). All stages of postoperative AKI and hemodialysis were also lower significantly in the OPCAB group.
Conclusion: OPCAB technique resulted in lower postoperative creatinine serum, AKI rates, and less hemodialysis neeeds compared with CCABG technique .

"Latar Belakang: Derajat kompleksitas lesi koroner yang berat merupakan prediktor mortalitas dan Major Adverse Cardiovascular Event (MACE) serta penentuan revaskularisasi pada penyakit jantung koroner (PJK). Fragmented QRS (fQRS) dinilai sebagai penanda iskemia atau cedera miokardium PJK. Hubungan fQRS dan derajat kompleksitas lesi koroner perlu diteliti lebih lanjut pada pasien PJK di Indonesia.
Tujuan: Mengetahui hubungan fQRS dan derajat kompleksitas lesi koroner pada pasien penyakit jantung koroner.
Metode: Penelitian potong lintang di Rumah Sakit Cipto Mangunkusumo, mengambil data sekunder pada 172 pasien jantung koroner yang menjalani percutaneous coronary intervention (PCI) di Cath Lab pada bulan Januari-Juni 2018 secara total sampling. Pasien dibagi berdasarkan adanya tidaknya fQRS. Data demografi, klinis, dan deajat kompleksitas (skor Gensini) diteliti. Hubungan antara adanya fQRS dan derajat kompleksitas lesi koroner dianalisis dengan uji kesesuaian.
Hasil: Sembilan puluh empat (54,6%) subjek terdapat gambaran fQRS. Pada analisis didapatkan hubungan antara fQRS dengan kategori skor Gensini ringan-sedang dan ringan-berat dengan kesesuaian baik (kappa 0,721 dan 0,820; p <0,001). Hubungan dengan kesesuaian yang baik juga didapatkan antara fQRS dan PJK signifikan (kappa 0,670; p <0,001) serta fQRS dan PJK multivessel (kappa 0,787; p <0,001).
Simpulan: Terdapat hubungan fragmented QRS complexes dan derajat kompleksitas lesi koroner pada pasien penyakit jantung koroner.

Background. The severity of coronary artery lesion is used as a predictor of mortality, major adverse cardiovascular event, and revascularization in coronary artery disease (CAD). Fragmented QRS complex (fQRS) as a novel marker of myocardial ischemia/scar in patients with coronary artery disease. The relationship between the two in Indonesia should be studied further.
Purpose. To determine the relationship between fQRS and the severity of coronary lesion in coronary artery disease.
Methods. A cross sectional study was conducted at Cipto Mangunkusumo Hospital. Secondary data were taken from 172 patients with CAD who underwent percutaneous coronary intervention (PCI) from January-June 2018 with total sampling. Patients were divided based on the existence of fQRS. Demographic, clinical, and severity of coronary artery lesion (Gensini score) characteristics were studied. Data were analysed using Cohens kappa agreement test.
Results. fQRS was present in 94 subjects (54.6%). Bivariate analysis showed a significant difference between fQRS with mild-moderate Gensini score as well as mild-severe Gensini score (kappa 0,721 and 0,820; p<0,001), fQRS with significant CAD (kappa 0.670; p<0,001), and fQRS with multivessel CAD (kappa 0.787; p<0,001).
Conclusion. There is a significant relationship between fQRS and the degree of severity of coronary lesion in coronary artery disease patients.

"CABG (coronary artery bypass graft) merupakan prosedur revaskularisasi utama bagi penderita penyakit jantung koroner yang bertujuan untuk meningkatkan aliran darah ke arteri koroner. Kualitas hidup pasien yang menjalani operasi CABG masih menjadi bahan pertanyaan dan diskusi karena dipengaruhi oleh banyak faktor salah satunya adalah kualitas tidur. Kurang tidur mempengaruhi kualitas hidup pasien dan kesejahteraan pasien. Gangguan tidur adalah masalah yang sering terjadi pada pasien pasca bedah dibandingkan pada pasien medical (nonbedah).
Studi ini dilakukan untuk mengetahui hubungan antara kualitas tidur dengan fatigue pada pasien pasca CABG dengan menggunakan metode cross sectional yang melibatkan 80 orang responden. Hasil pemodelan yang didapat dari uji regresi logistik, didapatkan bahwa penyakit penyerta, nyeri, dan kualitas tidur yang buruk berkontribusi sebesar 53,7% untuk meningkatkan fatigue pada pasien post CABG. Studi ini memberikan arahan pada perawat untuk lebih memperhatikan pengkajian kualitas tidur pada pasien pasca coronary artery bypass graft (CABG) dengan cara menyiapkan format pengkajian kualitas tidur. 

CABG (coronary artery bypass graft) is the main revascularization procedure for patients with coronary heart disease which aims to increase blood flow to the coronary arteries. The quality of life of patients undergoing CABG surgery is still a question and discussion material because it is influenced by many factors, one of which is the quality of sleep. Sleep deprivation affects the quality of life and the well-being of patients. Sleep disturbance is a problem that often occurs in postoperative patients compared to medical (non-surgical) patients.
This study was conducted to determine the relationship between the quality of sleep and fatigue in patients after CABG using a cross sectional method involving 80 respondents. The modeling results obtained from logistic regression tests, found that comorbidities, pain, and poor sleep quality contributed 53.7% to improve fatigue in post CABG patients. This study provides direction for nurses to pay more attention to the assessment of sleep quality in post coronary artery bypass graft (CABG) patients.

"Coronary Artery Bypass Graft (CABG) merupakan tindakan revaskularisasi pada pembuluh darah koroner yang tersumbat, operasi mayor, pemulihan yang lebih lama dan resiko komplikasi tinggi. Prevalensi kualitas tidur buruk cukup tinggi pada pasien paska operasi CABG. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan kualitas tidur pada pasien paska operasi CABG. Metode penelitian ini menggunakan cross sectional, consecutive sampling dengan jumlah 100 responden. Kuesioner yang digunakan pada penelitian ini adalah Pittsburgh Sleep Quality Index (PSQI), International Physical Activity Questionnaire (IPAQ), dan Depression Anxiety Stress Scale (DASS). Hasil penelitian menunjukkan prevalensi kualitas tidur buruk (51%) cukup tinggi dibandingkan kualitas tidur baik (49%). Fraksi ejeksi (p 0,031; OR 4,718) dan usia (p 0,039; OR 3,309)) memiliki hubungan yang signifikan dengan kualitas tidur paska operasi CABG pada hasil analisis bivariat. Hasil regresi logistik: fraksi ejeksi, usia, beta bloker dan diabetes melitus memiliki hubungan dengan kualitas tidur paska operasi CABG, fraksi ejeksi (p 0,017 OR 5,520) sebagai prediktor kualitas tidur. Implikasi: perawat sebaiknya melakukan pengkajian dan indentifikasi kualitas tidur dengan memperhatikan variabel tersebut, sehingga dapat menentukan intervensi yang tepat untuk meningkatkan kualitas tidur pasien paska operasi CABG.

Prevalence of poor sleep quality is quite high in patient post-CABG surgery. This study aims to identify factors associated with sleep quality in patient post-CABG surgery. This research method used cross sectional, consecutive sampling with 100 respondents. The questionnaires used ini this study were Pittsburgh Sleep Quality Index (PSQI), International Physical Activity Questionnaire (IPAQ), dan Depression Anxiety Stress Scale (DASS). The results showed that the prevalence of poor sleep quality (51%) was quite high compared to good sleep quality (49%). Ejection fraction (p 0,031 OR 4,718) and age (p 0,039; OR 3,309) and had a significant relationship with post-CABG sleep quality on the results of bivariate analysis. Results of logistic regression: ejection fraction, age, beta blockers and diabetes mellitus had a relationship with post-CABG sleep quality, ejection fraction (p 0,017; OR 5,520) was a dominant factor of sleep quality. Implication: nurses should conduct assessment and identification of sleep quality and be considered of those variables, so that nurse can determine the appropriate intervention to improve sleep quality in patient post-CABG surgery. "

"Coronary artery disease (CAD) is an important cause of death in end-stage renal disease (ESRD) patients on regular hemodialysis. The high risk of CAD occurrence in ESRD patients is partially due to a high prevalence of established atherosclerotic risk factors, which are hypertension, diabetes and dyslipidemia. However, unique renal-related risk factors are also likely to contribute to this high risk of CAD. The high prevalence of hyperhomocysteinemia in ESRD patients is of interest because of the probable cardiovascular risk associated with the increase of total plasma homocysteine concentration. The aim of this study was to evaluate the role of homocysteine as a risk factor for CAD in non-diabetic ESRD patients on regular hemodialysis.
Total fasting plasma homocysteine, total cholesterol, LDL cholesterol, HDL cholesterol, triglyceride, hypertension and smoking habit were documented from 80 non-diabetic ESRD patients on regular hemodialysis (48 men, 32 women; mean age 54.5±6.5 years). Twenty-two (27.5%) among these patients suffered from CAD according to ECG and echocardiographic criteria. The risk of CAD was analyzed using a stepwise multiple logistic regression. Total fasting plasma homocysteine concentration and other risk factors for CAD were also determined in 80 age- and sex-matched normal controls.
Total fasting plasma homocysteine concentration was significantly higher in non-diabetic ESRD patients than in normal controls (26.0±1.5 versus 14.6±1.3 fimol/L; p<0.01). Hyperhomocysteinemia was observed in 92.5% ESRD patients. Homocysteine concentration was significantly higher in ESRD patients with CAD than without CAD (33.8±1.4 versus 23.5+1.5 umol/L; p<0.01). High total plasma homocysteine concentration and hypertension were independently associated with CAD in non-diabetic ESRD patients on regular hemodialeine concentration in the upper tertile (>30.6 jxmol/L) had an adjusted odds ratio of 2.95 (CI, 1.02 to 8.53; p<0.05). In ESRD patients, the intake of folic acid is the only factor associated with total plasma homocysteine concentration. The increase of total plasma homocysteine concentration in normal controls was associated with increased age and smoking habit.
This study concludes that a high total plasma homocysteine concentration is an independent risk factor for coronary artery disease in non-diabetic ESRD patients on regular hemodialysis."

"Current guidelines for the management of patients with acute coronary syndromes (ACSs) focus on the electrocardiogram to divide patients into ST-elevation acute myocardial infarction (STEMI) or non-ST-elevation acute myocardial infarction (NSTEMI)/unstable angina (UA). Patients with STEMI in the earliest time will receive reperfusion therapy to destruct occlusive thrombus. An ST segment elevation is the sine qua non for diagnosing acute total coronary occlusion causing transmural myocardial infarction. Left circumflex coronary artery (LCx) occlusion is often categorized as NSTEMI because of the absence of significant ST-elevation on the 12 lead standard electrocardiogram. An ST segment elevation is presented in fewer than 50% of patients with LCx total occlusion, such that the reperfusion therapy is delayed. We reported a 77 years old woman whom being diagnosed with NSTEMI because a 12 lead electrocardiogram showed ST segment depression in lead V2-V5. On coronary angiography, we found a total occlusion in the LCx artery as the culprit lession."
