"Perawat sebagai bagian dari tenaga kesehatan memiliki peran untuk memberikan asuhan keperawatan yang komprehensif untuk memantau dan memonitor kondisi ibu. Salah satu peran perawat maternitas adalah memberikan asuhan keperawatan pada pasien dengan penyakit trofoblas dengan menerapkan teori keperawatan. Dari lima kasus kelolaan, dua kasus berumur 40 tahun. Semua kasus mengalami peradarahan. Dua kasus mengalami penyakit trofoblas ganas low risk. Dua kasus dikuretase, satu kasus histerektomi dan dua kasus kemoterapi. Semua kasus mengalami anemia baik anemia ringan sampai berat. Pasien mengalami masalah kekurangan volume cairan, kecemasan, defisit perawatan diri dan kesiapan peningkatan pengetahuan. Teori ini sesuai diterapkan pada pasien dengan penyakit trofoblas untuk memperoleh rasa nyaman selama proses tindakan dan dapat melewati proses kuretase dan kemoterapi dengan risiko minimal. Penanganan sedini mungkin akan meningkatkan peluang hidup ibu cukup besar dan peluang memiliki anak kembali cukup besar.

Nurse as a part from health worker has role for giving comprhensive nursing care to see and observe the mother condition. One of the maternity nurse role is to give a nursing care to the patient with mola hidatidosa and applied the nursing concept. Five case management, the age of two cases are under 20 years old, one case is 27 years old, and two cases over the 40 years old. All of the cases have the bleeding. Two cases is low risk trofoblast gestational disease. Curettage is done for the two cases, hysterectomy is done for one case, and chemotherapy is done for the two cases. All of the cases get anemia, start from high until low anemia. Patients experience problems of fluid volume deficit, anxiety, self care deficit, and readness to increase knowledge. Both theory are able to be applied for the patient with Mola Hidatidosa to get the comfort feeling along the surgical prosess and can pass the curettage and chemotherapy process with the minimal risk. By early managing will increase the probability of the life and have the next child again."

"ABSTRAKKesehatan ibu dan bayi memegang peranan penting pada pepmbangunan suatu bangsa. Angka kematian ibu (AKI) di Indoneesia pada tahun 2018 sebanyak 305/100.000 kelahiran hidup dan Angka kematian Bayi (AKB) pada tahun 2018 sebanyak 15/1000 kelahiran hidup. Persalinan prematur atau preterm adalah kelahiran hidup dengan usia gestasi kurang dari 37 minggu. Penyebab terjadinya persalinan preterm diantaranya kehamilan gemelli. Asuhan keperawatan yang diberikan pada ibu primipara diharapkan perawat membantu proses pencapaian peran ibu serta mengajarkan kemandirian dalam merawat diri dan bayinya. Metode yang digunakan adalah studi kasus dengan fokus penerapan teori self care Orem dan maternal role attainment Mercer. Aplikasi teori Orem dan Mercer efektif diterapkan pada kelima kasus yang dikelola, perawat melakukan observasi dan wawancara pada perilaku pasien serta kebutuhan yang diharapkan dari perawat. Intervensi keperawatan yang sesuai dengan kebutuhan pasien membantu dalam pencapaian kemandirian pasien dalam merawat diri dan pencapaian peran menjadi ibu lebih optimal. ABSTRACTMaternal and infant health plays an important role in the development of a nation. The maternal mortality rate (MMR) in Indonesia in 2018 was 305 / 100,000 live births and the Infant Mortality Rate (IMR) in 2018 was 15/1000 live births. Preterm or preterm labor is a live birth with a gestational age of less than 37 weeks. Cause labor. Nursing care given to primiparous mothers is expected to help nurses process the mother s role and ask for independence in caring for themselves and their babies. The method used is a case study with a focus on applying Orem's self-care theory and Mercer s achievement of the mother s role. The application of the theory of Orem and Mercer effectively applied to managed cases, nurses make observations and interviews with patients with the expected needs of nurses. Nursing interventions in accordance with patient needs help in achieving patient independence in caring for themselves and supporting the role of being a mother more optimally. "Wanita yang menderita kanker serviks stadium lanjut mengalami malodor yang akan mengganggu kualitas hidupnya. Perawat memiliki kewajiban untuk membantu pasien dengan menggunakan pendekatan teori Unpleasant Symptom dan Self Care. Dengan menggunakan teori Unpleasant Symptom pasien dibimbing untuk mengenali keluhan dan dampak yang mungkin terjadi pada dirinya, sedangkan dengan teori Self Care pasien dibimbing untuk mengenali potensi dirinya dalam mengatasi keluhannya secara mandiri sesuai dengan kemampuan diri dan support systemnya. Dengan berdasar kepada dua konsep yang dikolaborasikan, dilakukan implementasi evidence based practice nursing (EBPN) menggunakan intervensi perawatan luka kanker dengan menggunakan formula topikal standar dengan melibatkan pasien dan keluarga. Hasil menunjukan bahwa perawatan luka kanker dengan formula topikal standar dapat mengurangi instensitas malodor dan meningkatkan gambaran diri pasien.

Woman with advance cervical cancer will experience malodour which would influence their quality of life. Nurses has a responsibility to help patient by using the Unpleasant Symptom and Self Care theory approaches. Using the Unpleasant Symptom theory, the patient is guided to recognize symptom and the effect that may occur to herself, whereas with the self care theory the patient is hint to know her own potential and manage her symptom independently according to her abilities and support system. Based on two colaborative concepts, evidence based practice nursing is implemented. Intervention given by nurse involved patients and families in giving a cancer wound care using standard topical formulas. The result show that wound care with standard topical formula effective to reduce malodour and improving patient self image."

"Diabetes mellitus merupakan penyakit kronik pada system endokrin yang paling banyak dijumpai dipelayanan kesehatan. Perawat dalam memberikan asuhan keperawatan dapat menggunakan berbagai metode pendekatan, salah satunya dengan model pendekatan Self Care Orem yang menggunakan pendekatan untuk memandirikan pasien diabetes melalui manajemen diabetes secara mandiri.Residensi keperawatan medikal bedah peminatan keperawatan endokrin bertujuan melakukan analisis terhadap penerapan model pendekatan Self Care Orem dalam melakukan kegiatan pemberian asuhan keperawatan. Diabetes mellitus is a chronic disease in the endocrine system of the most prevalent in health care system. Nurses in providing nursing care can use various methods for approach , one of them with models of Orem Self Care approach.

That uses approach for diabetic patients through independent diabetes management. Nursing specialist of Medical-Surgical practice for endocrine conduct an analysis of the application models of Orem Self Care approach in the provision of nursing care activities. It also made the application of evidencebased practice with an assessment of the diabetic foot in 3 minutes in outpatient

"Persalinan seksio sesaria merupakan salah satu alternatif persalinan yang mengalami peningkatan dari tahun ke tahun. Masalah fisik dan psikologis lebih sering muncul pada ibu post partum dengan persalinan seksio sesaria. Keluhan fisik dan psikologis post seksio sesaria tersebut dirasakan lebih berat oleh ibu-ibu usia remaja. Dukungan perawat maternitas penting untuk membantu ibu post seksio cesarea usia remaja dalam memenuhi kebutuhan dasarnya dan dapat beradaptasi dengan peran baru sebagai ibu. Teori self care berfokus pada kemampuan individu untuk memenuhi kebutuhan perawatan dirinya secara mandiri sedangkan teori becoming a mother bertujuan memenuhi kebutuhan dasar ibu post seksio usia remaja dan membantu ibu dalam pencapaian peran menjadi ibu. Penggabungan penerapan teori self care dan becoming a mother cocok digunakan perawat untuk membantu ibu post seksio cesarean usia remaja untuk merawat dirinya mencapai peran sebagai orangtua. Perawat dapat menggunakan kerangka kerja Orem dan Mercer sebagai panduan pengkajian dan intervensi keperawatan untuk memfasilitasi pencapaian peran maternal.

Physical and psychological problems are more common in post partum mothers with sesarian sectional delivery. "Periode postpartum merupakan periode kritis pada kesehatan maternal. Peningkatan prevalensi persalinan gemelli menjadi tantangan tersendiri bagi perawat maternitas untuk memberikan asuhan yang paripurna. Studi kasus dilakukan dengan fokus penerapan teori Self Care dan Unpleasant Symptoms (TOUS) pada asuhan keperawatan ibu postpartum dengan gemelli. Aplikasi teori difokuskan pada kelima kasus yang berfokus pada pengkajian gejala ketidaknyamanan dan kemampuan perawatan diri ibu postpartum dengan gemelli. Intervensi keperawatan yang optimal dan sesuai dapat membantu klien mencapai kemandirian dalam perawatan diri sendiri dan pencapaian kenyamanan pada periode postpartum. Ibu postpartum dengan gemelli harus memiliki self dan time management yang baik agar perawatan postpartum menjadi optimal.

Postpartum period is a critical period of maternal health. Inreasing prevalence of twin delivery is a challenge for maternity nurse to serve holistic care. Case study is applied with focus on the application of Self Care theory and Theory of Unpleasant Symptoms (TOUS) on nursing care in postpartum women with twin. The application of Self Care and TOUS effectively applied to five cases that focused on identification unpleasant symptoms and self care ability of postpartum women with twin. Optimum and appropriate interventions could help client to receive self care independently and feel comfort during postpartum period. Postpartum mother with twin should have appropriate self and time management so that optimum postpartum self care will be reached."
