"Apexification is a procedure well recognized and accepted by clinicians and researchers alike. Apexification is an endodontic treatment that intends to stimulate of apex formation. One material in dentistry is calcium hydroxide that is most commonly used to induce an apical hard tissue barrier. Because of the capacity to promote enviromental condition for tissue repair, calcium hydroxide can be used in the management of most endodontic problem, such as pulp capping, pulpotomy, root canal treatment, apexification and various endodontic situations. Due to its high alkalinity is has an antibacterial effect, promotes mineralization, induces healing, and initiates hard tissue formation. Although there have been many reports on the subject, the mechanisms of action are still unknown."

"Endodontic instruments such as file or reamer has potential risk to breakage during cleaning and shaping procedure especially in curved canals. In this case the instrument was fractured because it was forced into the dentin walls to gain deeper penetration, and its removal result in breakage. In this case report removal of the instrument fragment in root canal with conventional technique was failed. So it was decided to continue the treatment, with the consideration that there is no complain within one month observation."

"The primary tooth with caries process could be abscess and became non-vital inflammation of the pulp tissue extents into the root canals. An effort to maintain the non-vital primary teeth with fistula is
pulpectomy. The primary tooth pulpectomy is a common clinical procedure. The choice of filling material and antibacterial properties against organism to inhabit infected primary root canal is important. The choice of filling material is zinc oxide eugenol and antibacterial medicament is formocresol. One appointment formocresol pulpectomy procedure is indicated when inflammation of the pulp tissue
extend into the root canals, and possibly involves the periapical and/or bifurcation area. ln this case, after treatment with one-appointment formocresol pulpectomy in non-vital incisive lateral primary tooth with
fistula is successful in one and eight months. This procedure provides an alternative to extraction and space maintenance when the pediatric dentist encounters a non-vital primary tooth."

The aim of the present study was to assess the response of rat dental pulp to direct pulp capping with ethanol extracts of propolis (EEP). An occlusal cavity was prepared on the right maxillary first molar of Spraque-Dawley rats. The dental pulp was exposed and then lined with a zinc oxide-based filler as a control (group I), or with EEP (group II). Then, each cavity was filled with glass ionomer cement. The animals were sacrificed at week l, 2, or 4. Biopsy samples were obtained, and these were stained and viewed by a light microscopic. The histological examination was based on the presence of polymorph nuclear leukocytes and macrophages, and also the dentinal bridge formation. Data were statistically analyzed by using Kruskall-Wallis and Mann-Whitney tests. The results showed that the inflammatory response of EEP group was relatively milder compared to the control group, but there was no significant difference (p>0.05) between the two groups. There was no dentinal bridge formation seen in both groups. Therefore, the present results suggest that direct pulp capping with EEP in rats may delay dental pulp inflammatory process and fail to stimulate reparative dentin."

"Penelitian ini bertujuan untuk mengetahui frekuensi dan letak ramifikasi saluran akar yang ditemukan pada gigi molar. Penelitian ini menggunakan 56 gigi molar satu dan molar dua yang telah dicabut. Gigi-gigi ini terdiri dari 14 gigi molar satu rahang atas, 3 gigi molar dua rahang atas, 21 gigi molar satu rahang bawah dan 18 gigi molar dua rahang bawah.
Metode : gigi direndam dalam larutan saline sampai saat percobaan. Dilakukan pembukaan akses dan preparasi dengan k-file sampai no. 15 kemudian saluran akar diirigasi dengan larutan NaOCl 2,5%. Setelah dikeringkan, gigi didekalsifikasi. Agar gigi terlihat bening gigi direndam dalam metil salisilat. Untuk mengidentifikasi ramifikasi, tinta cina diinjeksikan ke dalam sistem saluran akar. Masingmasing gigi diperiksa jumlah, tipe dan letak ramifikasi di bawah stereomikroskop.
Hasil : Dari 56 gigi molar satu dan molar dua, 60,7% memiliki ramifikasi (46,4% saluran lateral; 10,7% apical ramifications dan 10,7% isthmus saluran akar). Sebanyak 50% ramifikasi terletak di 1/3 apikal dan 19,6% terletak di 1/3 tengah.
Kesimpulan: Frekuensi ramifikasi saluran akar pada gigi molar satu dan molar dua cukup tinggi dan paling banyak terletak pada daerah 1/3 apikal."

"Latar Belakang: Karies gigi merupakan masalah kesehatan yang dialami setengah populasi penduduk dunia (3,58 milyar jiwa) dan penyakit gigi dengan prevalensi terbesar di Indonesia. Insidensi karies mencapai pulpa juga selalu meningkat setiap tahunnya. Perawatan saluran akar merupakan tindakan kuratif yang dilakukan untuk mengatasi hal tersebut. Menurut studi di berbagai negara, tingkat kegagalan PSA dapat mencapai 30% dengan melibatkan banyak faktor. Saat terjadi kegagalan, tindakan yang paling diutamakan untuk dilakukan adalah perawatan saluran akar ulang untuk mempertahankan gigi asli dari pasien. Oleh karena itu, perlu dilakukan penelitian mengenai prevalensi PSA ulang di RSKGM FKG UI dengan mengidentifikasi dan mengevaluasi penyebab kegagalan PSA dan faktor-faktor yang mempengaruhi hal tersebut untuk mencegah hal tersebut terjadi lagi di masa yang akan datang. Tujuan: Mengetahui prevalensi perawatan saluran akar ulang di Rumah Sakit Khusus Gigi dan Mulut Fakultas Kedokteran Gigi Universitas Indonesia periode 2019-2021. Metode: Penelitian merupakan penelitian deskriptif dan analitik komparatif yang bersifat retrospektif menggunakan data sekunder rekam medis pasien konservasi di RSKGM FKG UI. Hasil: Dari 3503 pasien PSA di RSKGM FKG UI periode Januari 2019-Juli 2021, 181 pasien dengan kegagalan PSA memilih untuk PSA ulang dan 20 pasien lainnya dilakukan ekstraksi. Melalui analisis komparatif, terdapat perbedaan secara statistik antara etiologi kegagalan PSA dengan status penyakit periapeks pada pasien, tidak ditemukan perbedaan antara sosiodemografi, elemen gigi dan diagnosis periapeks pasien pada perawatan PSA ulang dan ekstraksi, dan terdapat perbedaan secara statistik antara etiologi kegagalan PSA dengan perawatan yang dipilih (PSA ulang dan ekstraksi). Kesimpulan: Prevalensi PSA ulang di RSKGM FKG UI adalah 5,1%. Penyebab kegagalan PSA yang paling banyak ditemukan adalah pengisian saluran akar yang kurang. Diagnosis penyakit periapeks pasca PSA, paling banyak ditemukan adalah abses periapikal. Berdasarkan sosiodemografis, pasien paling banyak didominasi oleh jenis kelamin perempuan dan kelompok usia yang paling banyak ditemukan adalah kelompok usia 50-59 tahun. PSA ulang paling banyak terjadi pada gigi molar mandibula. PSA yang inadekuat memiliki pengaruh yang signifikan terhadap penyakit periapeks, proporsi tertinggi etiologi kegagalan PSA pada tindakan PSA ulang adalah PSA inadekuat dan proporsi tertinggi etiologi kegagalan PSA pada tindakan ekstraksi adalah restorasi inadekuat
Background: Dental caries is a serious health problem experienced by half of the world’s population (3.58 billion people) and an oral disease with the highest prevalence in Indonesia. The incidence of pulpitis is also increasing every year. Root canal treatment is taken to cure the disease. According to studies in various countries, endodontic treatment failure rate can reach to 30% involving many factors. When endodontic treatment failure occurs, the most applied action to be taken is endodontic retreatment to preserve patient’s teeth. Therefore, it is necessary to conduct a research on the prevalence of endodontic retreatment at RSKGM FKG UI by discovering the causes of the failure and other factors that contributed to the failure to prevent it from happening in the future. Objectives: This study aims to determine the prevalence of endodontic retreatment at RSKGM FKG UI for the period of 2019-2021. Methods: Retrospective descriptive and comparative analytical study is done using secondary data found in patient’s medical record. Results: There were 3503 endodontic patients at RSKGM FKG UI for the period of January 2019-July 2021, 181 patients with endodontic failure chose to be treated with endodontic retreatment and another 20 patients underwent extraction. Through comparative analysis, there were statistical differences between the etiology of endodontic failure and periapical disease. No differences found between the sociodemographic and the tooth, periapical diagnoses of patients with the choices of treatment between endodontic retreatment and extraction, and there were statistical differences between the etiology of endodontic failure and the choice of treatment. Conclusion: The prevalence of endodontic retreatment at RSKGM FKG UI is 5.1%. The most common etiology of endodontic failure is underobturation. Periapical abscess is the most found diagnosis of post endodontic treatment. Based on sociodemographics, most patients are female and the age group that commonly found was 50-59 years old age group. Endodontic retreatment mostly treated on mandibular molars. the biggest proportion of etiology of failure on endodontic retreatment treatment choice is an inadequate endodontic treatment while the highest proportion of etiology of failure on extraction is inadequate restoration"

"Penelitian in vitro ini bertujuan untuk mengetahui variasi morfologi gigi insisif, caninus, premolar dan molar pada penampang melintang 1/3 servikal, 1/3 tengah dan 1/3 apikal akar.
Metode: Penelitian dengan desain observasi deskriptif ini menggunakan 128 gigi yang telah dicabut. Sampel ditandai dengan spidol permanen pada 1/3 servikal (garis batas CEJ), 1/3 tengah dan 1/3 apikal akar. Selanjutnya dilakukan pemotongan secara melintang menggunakan diamond disc dengan low speed straight hand piece pada 1/3 servikal. Saluran akar dipreparasi dengan file kemudian diirigasi dengan larutan NaOCl 2,5%. Setelah saluran akar bersih, dilanjutkan dengan pemotongan secara melintang pada 1/3 tengah dan 1/3 apikal akar. Penampang melintang yang mempunyai 2 saluran akar dipisahkan untuk kemudian dideteksi adanya isthmus dengan pengaplikasian methylene blue. Pengamatan dilakukan dengan stereomicroscope perbesaran 10x dengan digital camera.
Hasil: 100% gigi anterior hanya memiliki 1 saluran akar dengan variasi bentuk oval, long oval dan bulat pada penampang melintangnya. 80% P1 atas memilki 2 saluran akar dan 100% gigi premolar bawah memiliki 1 saluran akar dengan dominasi bentuk oval, long oval dan bulat pada penampang melintangnya. Gigi molar mayoritas memiliki 3 saluran akar dan bentuk saluran akar bervariasi mulai dari oval, long oval, bulat dan flat/pipih. Isthmus ditemukan pada gigi P1 atas, P bawah, akar mesiobukal M1 atas, dan akar mesial molar bawah dengan berbagai tipe. Pada gigi molar 2 bawah ditemukan saluran akar berbentuk C (c-shaped) walaupun sangat sulit membedakan c-shaped dengan menyatunya 2 atau 3 saluran akar akibat berfusinya akar.
Kesimpulan: Berbagai variasi morfologi saluran akar yang ditemukan pada potongan melintang 1/3 servikal, 1/3 tengah dan 1/3 apikal akar harus menjadi pertimbangan dalam melakukan perawatan saluran akar.

The purpose of this study was to assess the root canal morphological variations of insisive, caninus, premolar and molar using the cross section method.
Methods: 128 extracted teeth were collected and stored in NaCl solution. The teeth were equally divided into a coronal, middle, and apical third. To divide it, the exact lengths of the root canals were determined. The root were resected using diamond disc with low speed straight hand piece. The resected root surface were polished, rinse and dried. The cross-sectional root surfaces that have two canals stained with methylene blue to investigate root canal isthmus classification. All the cross-sectional root surface examine using a stereomicroscope with digital camera.
Results: 100% of the anterior teeth demonstrated a single canal, 80% of maxillary first premolar demonstrated two canals and 100% of mandibular molar had single canal. The shaped of canal was varied from oval, long oval or round in the coronal, middle and apical third. The molar teeth showed a high incidence of three root canals and the shaped more varied from oval, long oval, round or flat in the coronal, middle, and apical third. Isthmus frequently presence between two root canals within one root. It can observed in the maxillary first premolar, mandibular premolar, mesiobuccal roots of maxillary first molar and mesial roots of mandibular molar with different type. C-shaped mostly found in mandibular second molar. It may be difficult to distinguish between a C-shaped canal and a mandibular second molar with single or three canals joining apically.
Conclussion: These root canal morphological variations should be considered during root canal treatment."

"This case study reviewed conventional root canal treatment of a maxillary first premolar which unexpectedly had a single canal with two foramens. this tooth was recognized as the least frequent tooth appeared neither with additional canal nor unusual root anatomy. Retreatment for this casewas successfull through widening of the access cavity and the root canal. While trying the master cone, it revealed that the canal splitted and had two foramens. A thorough knowledge of the root canal anatomy as well as careful radiograph interpretations were essential in enhancing the root-cleaning procedure."
