"PURPOSE: Rectal gastrointestinal stromal tumors (GISTs) are rare. Accordingly, their clinical features are not well-documented and optimal treatment has not been established. The objective of this study is to clarify the rates and patterns of recurrence after surgical resection of rectal GISTs, with a focus on outcomes and therapeutic modalities.
METHODS: The registry was designed to collect data on rectal GISTs recorded between January, 2003 and December, 2007 at 40 participating institutions of the Kinki GIST Study Group. The principal variables were the rates and patterns of recurrence of rectal GISTs. Other study variables were age, sex, tumor size, mitotic count, distance from the anal verge, tumor location, surgical procedures, surgical margins, and recurrence-free survival.
RESULTS: Twenty-four cases were registered, 11 (45.8%) of which were classified as high-risk by the modified NIH criteria. Locoregional recurrence (7/23, 30.4%) was the predominant recurrence pattern after curative resection, with rates that did not differ after local excision (33.3%; 3/9) vs. extended resection (28.6%; 4/14). The recurrence rates were high (25.0%) even for patients with low-risk disease. There was only one case of recurrence among patients who received perioperative treatment with imatinib.
CONCLUSIONS: Rectal GISTs showed high rates of local recurrence regardless of the surgical procedure. Perioperative treatment with imatinib may improve outcomes."

"Latar belakang: Karsinoma sel hati (KSH) merupakan jenis keganasan primer hati
tersering dengan gambaran histologik menunjukkan diferensiasi sel hepatoselular. Selain
insiden yang tinggi, beban yang berat dari keganasan ini adalah prognosis yang sangat
buruk dengan angka rekurensi yang tinggi. Terdapat banyak faktor resiko secara
klinikopatologik yang telah diketahui mempengaruhi prognosis KSH, seperti kadar alfa
fetoprotein, derajat diferensiasi, dan invasi mikrovaskular. Secara molekular, mutasi p53
dan β-catenin merupakan dua mutasi tersering dalam KSH. β-catenin merupakan protein
multifungsi yang dikode oleh gen CTNNB1 yang dapat ditemukan pada 3 kompartemen
sel, yaitu di membran sel, sitoplasma dan inti. Jalur Wnt/β-catenin meregulasi proses
seluler yang terkait inisiasi, pertumbuhan, survival, migrasi, diferensiasi, dan apoptosis.
Meski sudah banyak diketahui beberapa jalur patofisiologi molekular
hepatokarsinogenesis, hubungan dengan aplikasi klinik membutuhkan pemahaman lebih
mengenai hubungan sifat molekuler dan sifat fenotip tumor, terutama dalam penentuan
faktor prognosis dan pengembangan terapi target. Penelitian ini bertujuan untuk menilai
ekspresi β-catenin pada KSH dan hubungannya dengan berbagai faktor prognosis yaitu
AFP, derajat diferensiasi dan invasi mikrovaskular.
Bahan dan cara: Penelitian ini menggunakan desain potong lintang. Sampel terdiri atas
35 kasus KSH yang sudah ditegakkan diagnosisnya berdasarkan pemeriksaan
histopatologik dan/atau imunohistokimia di RSCM dari Januari 2013 sampai September
2019. Dilakukan pulasan β-catenin dan analisis statistik dengan uji komparatif terhadap
berbagai karakteristik klinikopatologik dan faktor resiko berupa AFP, derajat diferensiasi
dan invasi mikrovaskular.
Hasil: Terdapat perbedaan bermakna ekspresi β-catenin terhadap AFP (p=0,037) dan
derajat diferensiasi (p=0,043) pada KSH. Ekspresi β-catenin pada inti dengan/tanpa
sitoplasma lebih sering ditemukan pada kasus KSH dengan kadar AFP rendah dan derajat
diferensiasi baik-sedang. Tidak ditemukan perbedaan bermakna ekspresi β-catenin
terhadap invasi mikrovaskular pada KSH (p=1,000).
Kesimpulan: Terdapat perbedaan bermakna ekspresi β-catenin terhadap AFP dan derajat
diferensiasi pada KSH.

Background: Hepatocellular carcinoma (HCC) is the most common primary liver
cancer, displaying histologically hepatocellular differentiation. In addition to its high
incidence, the disease burden of HCC is due to its poor prognosis with high recurrence
rate. Some of the previously known clinicopathologic prognostic factors of HCC include
alpha-fetoprotein (AFP) level, tumor grade and microvascular invasion. At molecular
level, p53 and β-catenin are the two most common driver mutations in HCC that are
mutually exclusive. β-catenin is a multifunction protein that is encoded by CTNNB1 gen.
It is found in 3 compartments of cells, which are membrane cell, cytoplasm and nucleus.
Wnt/ β-catenin pathway regulates cellular process which is related to initiation, growth,
survival, migration, differentiation and apoptosis. Although molecular pathogenesis
pathways of hepatocarcinogenesis are known, clinical application warrants more
understanding in terms of molecular characteristic and tumor phenotype, especially in
determining prognosis and target therapy development. This current study aims to analyze
the expression of β-catenin and its association with prognostic factors, such as AFP,
tumor grade and microvascular invasion.
Material and method: A cross-sectional study was conducted comprising 35 samples of
surgically resected HCCs between January 2013 to September 2019 in Cipto
Mangunkusumo General Hospital. The cases were diagnosed based on histopathological
and immunohistochemical findings and was then performed β-catenin staining. β-catenin
expression was analyzed with statistical tests to determine expression difference between
AFP level, tumor grade and microvascular invasion.
Result: There were statistically significant difference of β-catenin expression in AFP
level and tumor grade (p=0.037 and 0.043, respectively). Nuclear with/without
cytoplasmic expression of β-catenin was more frequently found in HCC with low AFP
level and well-to-moderately differentiated tumors. No significant difference was
observed in β-catenin expression between HCC with and without microvascular invasion
(p=1.000).
Conclusion: β-catenin expression was significantly different in AFP level and tumor
grade."

Perawat spesialis neurosains berperan dalam praktik keperawatan berupa pemberi asuhan keperawatan lanjut, melakukan pembuktian ilmiah dan agen pembaharu. Asuhan keperawatan dilakukan pada kasus pasien dengan Tumor Otak dan 30 pasien gangguan neurologis dengan pendekatan Model adaptasi Roy. Perilaku maladaptif paling banyak terganggu pada mode fisologis dengan diagnosa resiko ketidakefektifan perfusi jaringan serebral. Evidence Based Nursing dilakukan dengan menerapkan thermal tactile stimulation pada 3 pasien stroke yang mengalami disfagia dengan hasil yang signifikan p value 0,038 . Program inovasi menerapkan enam screening tools yaitu Insomnia Severity Index ISI , National Institute Health Stroke Scale NIHSS , 3 Incontinence Question 3 IQ , Berg Balance Scale BBS , Frenchay Aphasia Screening Test FAST dan Adult Non Verbal Pain Scale ANVPS pada pasien dengan gangguan neurologi yang terbukti memudahkan perawat dalam menegakkan diagnosa keperawatan yang tepat. Model Adaptasi Roy telah berpengaruh besar terhadap profesi keperawatan. Model ini adalah salah satu model yang paling banyak digunakan dalam memandu penelitian, pendidikan dan praktik keperawatan ABSTRACT Neuroscience nurse specialist play role in nursing practice as advanced nursing care providers, conduct scientific evidence and innovator. Roy adaptation model approach was used in the nursing care of the tumor brain patients and 30 patients of neurological disorders cases. "ABSTRACT
Sel tumor adalah sel yang terbentuk akibat kegagalan beberapa protein dalam mengatur siklus sel. Protein TP53 berperan penting dalam mengatur siklus sel, khususnya dalam menekan perkembangan sel tumor. Perubahan pada gen TP53 ditemukan dalam lebih dari setengah kasus tumor pada manusia. Protein lain yang berhubungan dengan protein TP53 juga ditemukan terlibat dalam proses pembentukan kanker. Analisis interaksi protein TP53 dengan melakukan clustering jaringan interaksi protein (PPI) TP53 adalah hal penting dalam membantu mengatasi sel tumor. Jaringan PPI dinyatakan sebagai graf dengan protein dan interaksinya masing-masing sebagai simpul dan busur pada graf. Spectral clustering adalah metode graph clustering yang menggunakan eigenvector dari matriks Laplacian.

ABSTRACT
Fuzzy random walk adalah metode fuzzy clustering yang menggunakan probabilitas transisi dari random walk pada data. Dua metode tersebut akan digabungkan dan diimplementasikan pada penelitian ini. Menggunakan data PPI protein TP53 dari STRING database, didapat gabungan kedua metode tersebut mampu menghasilkan cluster yang fuzzy dan robust di mana setiap cluster dapat menjelaskan bagian tertentu dari fungsi protein TP53. Tumor cell is formed as a result of malfunctioning of some proteins that regulates the cell cycle. TP53 protein plays an important role in managing cell cycle, especially in tumor cell suppression. An alteration of TP53 gene is found in more than half cases of human tumor. Moreover, TP53-related proteins are also found involved in the carcinogenesis process. Therefore, it is important to analyze the interactions of TP53 protein by clustering protein-protein interactions (PPI) network of TP53. PPI networks are usually represented as a graph network with proteins and interactions as vertices and edges respectively. Spectral Clustering is a graph clustering algorithm based on eigenvector of the graph Laplacian. Fuzzy Random Walk is a fuzzy clustering method based on transition probability from a random walk on a dataset. In this paper, we combine both Spectral Clustering and Fuzzy Random Walk. Using PPI datasets of TP53 obtained from the STRING database, we found the combined algorithm is proven to produce both robust and fuzzy clusters with each cluster explains one of TP53 proteins functionality."

Salah satu tindakan yang aman dan direkomendasikan untuk mengeluarkan sputum yang menyumbat saluran napas pada pasien dengan tumor paru adalah menggunakan teknik Active Cycle of Breathing Technique ACBT . Teknik ini dilakukan terhadap salah satu pasien anak berusia 12 tahun yang mengalami tumor mediastinum dengan masalah ketidakefektifan bersihan jalan napas akibat penumpukan sputum di ruang rawat anak lantai 1 gedung A RSUPN Cipto Mangunkusumo Jakarta. Setelah dilakukan tindakan selama 11 hari didapatkan hasil, pasien dapat mengeluarkan sputum yang menyumbat sehingga produksi sputum berkurang. Berdasarkan hasil tersebut, teknik ACBT terbukti efektif membantu pasien untuk mengeluarkan sputum yang menyumbat pada pasien dengan tumor paru.Kata kunci: ACBT, Penumpukan Sputum, Tumor Mediastinum.

One of the safest and recommended actions to remove sputum that obstructs the airway in patients with lung tumors is using Active Cycle of Breathing Technique ACBT technique. This technique was performed on the 12 year old pediatric patients who had a mediastinal tumor with inefective airway clearence problems due to sputum accumulation in first floor child unit Gedung A of Cipto Mangunkusumo Jakarta Hospital. After an 8 day action, the result showed patient can remove the clogged sputum so that sputum production is reduced. Based on these results, the ACBT technique proved to be effective in helping patients to remove clogged sputum in patients with lung tumors.Keywords ACBT, Sputum Accumulation, Mediastinal Tumor."

"Penelitian survival secara historical cohort pada enam puluh dua penderita tumor ovarium borderline. Terdapat 9 penderita stadium FIGO IA, 9 stadium IC, 3 stadium IIIA, 2 stadium IIIB, 4 stadium IIIC, 1 stadium IV dan 34 stadium inadekuat. Dua puluh satu penderita dilakukan pembedahan radikal, 10 penderita hanya dilakukan histerektomi total dan salfingo-ooforektomi bilateral, 6 penderita dilakukan pembedahan konservatif, 24 penderita hanya dilakukan unilateral salfingo-ooforektomi atau kistektomi dan 1 penderita hanya biopsi saja. Enam belas penderita mendapat kemoterapi adjuvan kombinasi dengan platinum base, yaitu 8 penderita stadium inadekuat, 7 stadium III dan 1 stadium IV. Lama pengamatan lanjut antara 0,002 sampai 10,48 tahun dengan median 3,5 tahun. Lima puluh sembilan penderita tetap hidup. Tiga penderita meninggal karena penyakitnya. Residif terjadi pada 4 penderita. Ketahanan hidup penderita 2 tahun 96% dan 10 tahun 94%. Pada test ?log rank? didapatkan residu dan tipe histologi merupakan faktor prognostik yang bermakna mempengaruhi survival. (Med J Indones 2002; 11: 222-9)

Sixty-two patients with borderline tumors of ovary were historical cohort analyzed for survival characteristics. There were 9 patients with FIGO stage IA, 9 with stage IC, 3 with stage IIIA, 2 with stage IIIB, 4 with stage IIIC, 1 with stage IV and 34 with inadequate stage tumors. Twenty one patients had surgical staging with radical surgery, 10 patient had at least a total abdominal hysterectomy and bilateral salpingo-oophorectomy, 6 patient had surgical staging with conservative surgery, 24 patient had at least a unilateral salphingo-oophorectomy or ovarian cystectomy and 1 patient had biopsy. Sixteen patients received cisplatin-based combination chemotherapy, that were 8 with inadequate stage tumors, 7 with stage III tumors and 1 with stage IV tumor. Follow-up range from 0.02 to 10.48 years, with a median of 3.5 years. Fifty nine patient were alive. Three patients died, all of disease. Recurrence were found in 4 patients. The overall 2-years survival rate was 96% and 10-years survival rate was 94%. In log rank test, residual disease and histology type were significant predictor of survival. Latar Belakang. Kejang merupakan gejala yang sering ditemukan pada tumor primer intrakranial dan penyebab utama morbiditas terhadap pasien.Pemeriksaan EEG diperlukan untuk menentukan kesesuaian antara fokus kejang dengan lokasi tumor pada MRI yang akan menentukan prognosis kejang serta banyak faktor-faktor yang mempengaruhi kesesuaian antara fokus kejang dengan lokasi tumor. Belum adanya data mengenai kejang pada tumor primer intrakranial serta kesesuaian berdasarkan gambaran EEG dan MRI menjadi dasar dilakukannya penelitian ini. Tujuan.Mengetahui kesesuaian antara aktivitas epileptiform pada EEG dan lesi tumor dengan MRI pada pasien tumor primer intrakranial dengan klinis kejang. Metode.Desain penelitian berupa studi potong lintang (cross sectional).Subyek penelitian adalah semua pasien dengan tumor primer intrakranial yang ada di ruang rawat inap dan rawat jalan neurologi, bedah saraf, radiologi RS Cipto Mangunkusumo yang sudah dilakukan EEG dan MRI.Ditentukan aktivitas epileptiform dan dianalisa kesesuaiannya dengan lokasi tumor berdasarkan MRI. Hasil.Dari 33 subyek dengan tumor primer intrakranial dengan klinis kejang , didapatkan hanya 17 subyek yang menunjukkan aktivitas epileptiform (51,5%), perempuan lebih banyak dari lelaki, dengan rerata usia adalah 34 tahun. Sebagian besar mengalami kejang parsial dan secondary generalized seizure(SGS) merupakan tipe kejang parsial terbanyak (16 dari 17 subyek). Kejang sering ditemukan pada tumor di frontal (11 dari 17 subyek) dan pada jenis tumor primer Low grade(8 dari 17 subyek). Kesesuaian aktivitas epileptiform dengan lokasi tumor didapatkan pada 8 dari 17 subyek dengan lebih banyak yang sesuai pada lobus temporal. Kesimpulan.Dari seluruh pasien tersangka tumor primer intrakranial dengan klinis kejang hanya didapatkan 8 dari 17 subyek yang sesuai antara aktivitas epileptiform pada EEG dengan lesi tumor pada MRI.Gambaran aktivitas epileptiform pada EEG tidak dipengaruhi oleh usia, bentuk bangkitan kejang, jenis tumor, lokasi berdasarkan lobus, lokasi berdasarkan parenkim otak, durasi sakit, dan ukuran tumor.

ABSTRAT
Background.Seizures are a common symptom in primary intracranial tumors and a major cause of morbidity to the patient. EEG examination is necessary to determine the suitability of the seizure focus to the location of the tumor on MRI that will determine the prognosis of seizures as well as a lot of factors that affect compatibility between focal seizures with tumor location. The absence of data on seizures in primary intracranial tumors and suitability based on EEG and MRI picture is the basis of this study. Purpose.Knowing the correspondence between epileptiform activity on EEG and MRI tumor lesions in patients with primary intracranial tumors with clinical seizures. Method.Design research is a cross-sectional study (cross-sectional). Subjects were all patients with primary intracranial tumors that exist in the inpatient and outpatient neurology, neurosurgery, radiology Cipto Mangunkusumo already done EEG and MRI. Epileptiform activity determined and analyzed for compliance with the location of the tumor by MRI. Result. From 33 subjects with primary brain tumors with clinical seizures, obtained only 17 subjects demonstrated epileptiform activity (51.5%), more women than men, with a mean age was 34 years. Most had partial seizures and secondary generalized seizures (SGS) is a type of partial seizure majority (16 of 17 subjects). Seizures are often found in tumors in the frontal (11 of 17 subjects) and the type of primary tumor Low grade (8 of 17 subjects). Suitability of epileptiform activity by tumor location obtained in 8 of 17 subjects with more appropriate in the temporal lobe. Conclusion.From all patients suspected of primary brain tumors with clinical seizures obtained only 8 of the 17 subjects that fit between epileptiform activity on EEG with tumor lesions on MRI. Picture of epileptiform activity on EEG was not influenced by age, shape seizures, tumor type, location based lobes, based on the location of the brain parenchyma, duration of illness, and tumor size."

"Latar balakang. Sekitar 75% kanker leher rahim datang ke rumah sakit dalam stadium lanjut. Terapi utama kanker serviks stadium lanjut adalah radioterapi. Kombinasi terapi radiasi dengan kemoterapi ternyata tidak menghasilkan respons terapi yang lebih baik, tetapi bahkan menimbulkan efek samping yang lebih berat dibandingkan terapi radiasi saja. Perubahan pola makan saat ini, diduga berpengaruh pada respons terapi. Rasio asupan Polyunsaturated Fatty Acids (PUFA) n-6 : n-3 sekitar 1-2 : 1 dapat melawan pertumbuhan kanker. Beberapa publikasi melaporkan bahwa PUFA n-3 (AL n-3) mempunyai efek pro-apoptosis, anti-inflamasi, anti-proliferatif dan anti-angiogenik. Penelitian ini bertujuan untuk mengetahui apakah suplementasi nutrisi tinggi PUFA bersamaan dengan terapi radiasi dapat meningkatkan respons tumor pada kanker serviks stadium lanjut.
Metode. Penelitian ini merupakan Randomized Clinical Trial, double blind pada pasien kanker serviks stadium lanjut, jenis karsinoma sel skuama yang hanya mendapat terapi radiasi di Departemen Radioterapi, Rumah Sakit Cipto Mangunkusumo (RSCM), Jakarta. Subjek penelitian diambil secara konsekutif, melalui randomisasi dibagi dua kelompok, yaitu kelompok perlakuan dan kontrol. Selama radiasi subjek penelitian mendapat suplementasi nutrisi mengandung isokalori dan isoprotein dengan rasio AL n-6 : n-3 = 1,27 : 1 dan kontrol. Selama perlakuan dilakukan evaluasi setiap lima kali radiasi, meliputi BB dan efek samping radiasi akut. Parameter yang dinilai adalah respons klinis, respons histopatologis serta perubahan kadar PGE2 dan VEGF serum.
Hasil. Sebanyak 31 subjek dari 45 subjek dapat menyelesaikan penelitian. Sebelum perlakuan, status sosio-demografi, pola asupan AL n-6 dan n-3, status gizi dan klinis tidak didapatkan perbedaan bermakna. Setelah perlakuan, didapatkan penurunan kadar PGE2 serum pada kelompok perlakuan, sedangkan kelompok kontrol mengalami kenaikan (p = 0,127). Penurunan kadar VEGF kelompok perlakuan lebih besar dibanding kontrol (p = 0,626). Respons klinis dan respons histopatologis kelompok perlakuan lebih baik dibanding kontrol, dengan kemaknaan masing-masing p = 0,172 dan p = 0,169. Secara statistik belum didapatkan perbedaan yang bermakna pada penurunan kadar PGE2 dan VEGF serum, respons klinis dan respons histopatologis, tetapi didapat kemaknaan klinis yang nyata pada kelompok perlakuan dibanding kontrol. Analisis statistik gabungan dari respons klinis, respons histopatologis, kadar PGE2 dan VEGF serum didapatkan respons tumor total yang bermakna (p = 0,048).
Kesimpulan. Suplementasi nutrisi tinggi PUFA dengan rasio asam lemak n-6 : n- 3 = 1,27 : 1 pada kanker serviks stadium lanjut bersama terapi radiasi memberikan respons tumor total yang lebih baik.

Background. Approximately 75% of uterine cervical cancer came to the hospital in an advanced stage. Primary therapy of advanced cervical cancer is radiotherapy. The combination of radiotherapy with chemotherapy did not give better outcomes, but even cause more severe side effects than radiotherapy. Changes in dietary intake at this time, are supposed to influence the response to therapy. The ratio of n-6 to n-3 polyunsaturated fatty acids (PUFA) about 1-2: 1 can fight cancer growth. Several publications stated that n-3 PUFA has pro- apoptotic effect, anti-inflammatory, anti-proliferative and anti-angiogenic. This study aimed to examine whether radiotherapy combined with daily nutritional supplements enriched PUFA enhance tumor response in advanced cervical cancer.
Method. This study was a double-blind Randomized Clinical Trial (RCT) in patients with advanced squamous cell cervical cancer, received radiation only as control group, and those who received nutritional supplementation containing isocaloric and isoprotein with ratio of n-6 : n-3 PUFA = 1.27: 1 as treatment group. This study was performed in the Department of Radiotherapy Dr. Cipto Mangunkusumo General Hospital (RSUPNCM), Jakarta. Evaluation was done every five times radiation, including Karnofsky Perfomance Scale, body weight and side effects of acute radiation. Parameters assessed were clinical and hematological response, histopathological response and changes in the levels of PGE2 and VEGF serum.
Results. A total of 31 subjects from 45 subjects participated the study completely. Prior to treatment, socio-demographic status, patterns of n-6 and n-3 PUFA, clinical and nutritional status were analyzed, no significant differences were noted. After treatment, a decrease in serum PGE2 levels in the treatment group was found, while in the control group increased (P = 0.127). VEGF in the treatment group declined higher than controls (P = 0.626). Treatment group showed significant better clinical response and histopathological responses compared to the control group (p = 0.172 and p = 0.169). No significant differences were found in the reduction of PGE2 and VEGF serum levels, clinical response and histopathological response, although from clinical standpoint there is a better significant response in the treatment group than the control. The combined statistical analysis of clinical response, histopathological response, PGE2 and VEGF serum levels obtained total tumor response (P = 0.048).
Conclusion. Nutritional supplementation enriched PUFA (ratio of n-6: n-3 PUFA = 1.27 : 1) in advanced cervical cancer receiving radiotherapy gives better total tumor response.
