"Atresia ani merupakan malformasi kongenital pada anorektum. Klien dengan atresia ani biasanya dilakukan kolostomi. Kolostomi seringkali mengalami iritasi pada kulit peristomal. Penulisan Karya Ilmiah Akhir Ners ini bertujuan untuk memberikan gambaran asuhan keperawatan pada klien dengan atresia ani dengan kolostomi yang memiliki kondisi iritasi pada kulit peristomal. Salah satu intervensi untuk menghindari komplikasi lebih lanjut yaitu dengan perawatan stoma. Perawatan stoma dilakukan dengan cara menilai iritasi kulit dengan ostomy skin tools, melakukan intervensi, dan pemantauan kondisi kulit. Setelah dilakukan perawatan stoma selama 3 hari ditemukan hasil berkurangnya kemerahan pada kulit dan hilangnya iritasi dari tiga lokasi menjadi hanya satu lokasi iritasi. Hasil karya ilmiah ini merekomendasikan pihak rumah sakit untuk mengembangkan asuhan keperawatan khususnya pada klien dengan kerusakan integritas kulit untuk meningkatkan kualitas pelayanan.

Atresia ani was a congenital malformation in anorectum. Clients with atresia ani usually had colostomy procedure. Colostomy often experienced for peristomal skin irritation. The purpose of this study was provided an overview of nursing care to clients with atresia ani who had peristomal skin irritation. One intervention to avoid further complications was by stoma care. Stoma care was done by assessing skin irritation with ostomy skin tools, interventions, and monitoring the condition of the skin. After stoma care for 3 days found resulted in redness less and reduce irritation location from three locations into one location. From the result of this study recommend the hospital need to develop a nursing care on clients with skin integrity damage to improve the quality of service."

"[ABSTRAKKesehatan reproduksi merupakan masalah yang perlu diperhatikan dalam meningkatkan kesehatan perempuan. Ners spesialis keperawatan maternitas mempunyai peranan untuk mencapai upaya tersebut. laporan ini bertujuan menggambarkan praktik residensi ners spesialis keperawatan maternitas meliputi asuhan keperawatan pada masa prenatal, intranatal, dan post natal serta masalah reproduksi perempuan. Residen juga melakukan asuhan keperawatan dengan fokus penerapan model konsep need for help Wiedenbach dan Lost and grief Kubler Ross pada ibu bersalin dengan IUFD. masalah keperawatan yang mungkin muncul diakitkan dengan tahapan persalinan yang dibutuhkan klien. perbedaan yang terjadi pada klien berkaitan dengan psikologisnya yaitu dapat beradaptasi dengan cepat dan mengalami tahap penerimaan lebih cepat, dan ada yang proses penerimaannya membutuhkan waktu yang lama dan mengalami tahap tawar menawar serta penolakan yang lebih lama. selama praktik residensi ini kompetensi yang tercapai menjadikan pengalaman yang berarti dalam peningkatan keterampilan residen maternitas.

"Tuberkulosis adalah penyakit yang sering muncul pada masyarakat miskin perkotaan. Spondilitis Tuberkulosis merupakan komplikasi dari TB yang dapat menyebabkan perubahan struktur dan kekuatan tulang belakang dengan resiko kelumpuhan dan cedera tulang belakang dan biasanya ditandai dengan gejala awal nyeri. Nyeri tersebut diakibatkan pertumbuhan abnormal kuman Tuberkulosis yang menjadi massa pada tulang belakang dan menyebabkan tertekannya jaringan saraf spinal. Treatment Operasi debridemen akan mengakibatkan nyeri pasca operasi dilakukan. Peran perawat dalam mengatasi masalah nyeri tersebut adalah dengan edukasi manfaat pemasangan spinal brace, sehingga klien tidak takut akan terjadi nyeri saat melakukan mobilisasi. Setelah dilakukan intervensi, pengetahuan klien bertambah, nyeri klien berkurang dan dapat melakukan mobilisasi.

The role of nurse in to overcome the problem of pain was to education benefits the installation of spinal brace, that clients not afraid will happen pain on do mobilization. After conducted intervention, knowledge client?s increase, pain client?s reduced and can do mobilization."

"Kanker payudara merupakan salah satu masalah kesehatan utama di perkotaan. Mastektomi sebagai tindakan pembedahan kanker payudara, berpotensi menyebabkan penurunan rentang gerak, kekakuan bahu, dan limfedema pada lengan sisi payudara yang mengalami pembedahan, terutama jenis modified radical mastectomy (MRM). Karya ilmiah akhir ini bertujuan untuk menggambarkan asuhan keperawatan pasien kanker payudara pasca mastektomi. Latihan rentang gerak pada lengan yang terkena bertujuan untuk meningkatkan rentang gerak dan mencegah kekakuan bahu serta limfedema. Hasil penerapan latihan rentang gerak menunjukkan bahwa terjadi peningkatan rentang gerak dan tidak terjadi kekakuan bahu serta limfedema. Perawat perlu mengajarkan latihan rentang gerak lengan pada pasien pasca mastektomi untuk meningkatkan kualitas hidup pasien.

Breast cancer is one of the mayor health problems in urban area. Mastectomy as a treatment of breast cancer potentially cause lymphoedema, decrease range of motion, and shoulder stiffness of the arm was had mastectomy, especially the type of modified radical mastectomy (MRM). This paper was aimed to explore nursing process of patient post mastectomy. Range of motion exercise was aimed to increase range of motion and prevent shoulder stiffness and lymphoedema. Patient showed the increasing of range of motion and shoulder stiffness and lymphoedema did not occur after doing exercise. Based on finding, Nurses have to teach range of motion exercise to patient post mastectomy to increase their quality of life."

"Perubahan yang terjadi pada sendi dan rasa nyeri merupakan faktor internal yang dapat menyebabkan hambatan mobilitas fisik pada lansia. Karya ilmiah ini bertujuan untuk memaparkan hasil asuhan keperawatan pada klien selama lima minggu. Evaluasi akhir menunjukkan gerakan fleksi-ekstensi sendi bahu sebesar 1100 (meningkat 200 dari pemeriksaan awal), edema tidak terlihat pada ekstremitas bawah, kekuatan otot meningkat pada ekstremitas atas, dan klien mengatakan bahwa badan terasa enak setelah melakukan gerakan. Hasil pemberian asuhan keperawatan menyarankan kepada petugas kesehatan untuk memberikan latihan rentang pergerakan sendi setiap hari kepada lansia yang mengalami hambatan mobilitas fisik untuk mencegah komplikasi dari imobilisasi dan meningkatkan taraf kesehatan lansia.

Changes that occur in the joints and pain became internal factors that may cause impaired physical mobility in elderly. This paper aimed to present the result of the nursing care in five weeks. Final evaluation showed the flexion-extension motion in shoulder joint about 1100 (increased 200 from the initial examination), edema wasn't seen in lower extremity, increased of muscle strength in the upper extremities, and the client said that her body felt good after do the motion. Nursing care result has suggested to healthcare practitioners in providing "row of motion" exercise every day to the elderly who had impaired physical mobility to prevent complications of immobilization and improve the elderly health."

"[ABSTRAK Tuberkulosis paru merupakan salah satu masalah kesehatan masyarakat perkotaandan penyebab kematian tertinggi di Indonesia. Penulisan karya ilmiah bertujuanuntuk menganalisis asuhan keperawatan pada pasien TB Paru dengan kontekskeperawatan kesehatan masyarakat perkotaan. Asuhan keperawatan diberikansesuai dengan proses keperawatan. Masalah keperawatan utama yang munculadalah bersihan jalan nafas tidak efektif, intoleransi aktifitas dan kurangpengetahuan tentang TB dan pengobatannya. Intervensi yang diberikan adalahmemberikan posisi semipowler, latihan ROM secara aktif, dan intervensi khususpemberian pendidikan kesehatan tentang kepatuhan minum Obat AntiTuberkulosis. Hasil evaluasi dari asuhan keperawatan menunjukkan jalan nafasadekuat, aktivitas mandiri, dan peningkatan pengetahuan klien tentang Obat AntiTuberkulosis.
