"Rumah sakit merupakan salah satu dari sarana kesehatan dan rujukan pelayanan kesehatan dengan fungsi utama menyelenggarakan upaya kesehatan yang bersifat penyembuhan dan pemulihan bagi pasien. Pelayanan farmasi rumah sakit menjadi salah satu kegiatan di rumah sakit yang menunjang pelayanan kesehatan yang bermutu. Apoteker di rumah sakit memiliki peran dalam manajemen pengelolaan perbekalan farmasi dan farmasi klinis. Dalam menjalankan peran tersebut, apoteker tidak hanya memerlukan ilmu pengetahuan farmasi namun juga keterampilan dan kemampuan komunikasi yang baik. Praktek Kerja Profesi Apoteker (PKPA) dilakukan pada tanggal 03 Juli - 30 Agustus 2013 di Rumah Sakit Umum Pusat Nasional (RSUPN) Dr. Cipto Mangunkusumo guna memberikan pengetahuan untuk memahami tugas pokok seorang apoteker di rumah sakit, yaitu peran manajerial dan pelayanan farmasi klinis. Sedangkan tujuan dari tugas khusus adalah untuk membantu tenaga medis dalam menentukan penggunaan obat berdasarkan kriteria STOPP dan START pada pasien geriatri sebagai bentuk pelayanan informasi obat secara aktif.

The hospital is one of the health facilities and referral health services with the main function organized health efforts that are healing and recovery for the patient. Hospital pharmacy services to be one of the activities that support hospital quality health services. Pharmacists in hospitals have a role in the management of pharmaceuticals and clinical pharmacy. In carrying out this role, the pharmacist not only requires pharmaceutical science but also skills and good communication skills. Pharmacists Internship Program (PIP) held on July 3rd to August 30th, 2013 in the General Hospital National Center Dr. Cipto Mangunkusumo to provide the knowledge to understand the fundamental duty of a pharmacist in a hospital, namely the role of managerial and clinical pharmacy services. While the purpose of the special task is to help medical staff by determining drug use for geriatric based on STOPP and START criteria as an active drug information service."

"Praktik kerja profesi di RSUPN Dr. Cipto Mangunkusumo Periode Bulan Oktober-November Tahun 2018 bertujuan untuk  memahami tugas dan tanggung jawab apoteker di Rumah Sakit sesuai dengan ketentuan dan etika yang berlaku, memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktik kefarmasian di Rumah Sakit, memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktik kefarmasian. Pelaksanaan praktik kerja profesi ini berlangsung selama dua bulan dengan tugas khusus yaitu Peran Apoteker Dalam Mencegah Medication Error Akibat LASA di RSUPN Dr. Cipto Mangunkusumo Periode Bulan Mei-September Tahun 2018. Tujuan tugas khusus ini adalah mengetahui persentase medication error akibat LASA,  persentase fase dan tipe medication error akibat LASA, dan persentase peran apoteker dalam penyelesaian medication error akibat LASA di RSUPN Dr. Cipto Mangunkusumo periode Mei-September 2018.

The aims of internship held in RSUPN Dr. Cipto Mangunkusumo in October-November 2018  are to understand the duties and responsibilities of pharmacists at the hospital in accordance with applicable laws and ethics, have the insight, knowledge, skills and practical experience to undertake pharmaceutical practices in hospital, have the insight of pharmaceutical practice issues and learn strategies and activities that can be undertaken in the course of pharmaceutical practice development.

This internship was conducted for two month with a special assignment The Role of Pharmacists in Preventing Medication Errors due to LASA in RSUPN Dr. Cipto Mangunkusumo Period May-September 2018. The purpose of this special assignment is to know the percentage of medication error due to LASA, the percentage of phase and type of medication errors due to LASA, and the percentage of pharmacist role in solving medication errors due to LASA at RSUPN Dr. Cipto Mangunkusumo May-September 2018."

"Tujuan pelaksanaan kegiatan Praktek Kerja Profesi Apoteker (PKPA) ini adalah untuk memberikan pemahaman serta pengalaman kepada mahasiswa apoteker tentang tugas pokok seorang apoteker di rumah sakit dalam peran manajemen perbekalan farmasi dan pelayanan farmasi klinik di RSUPN Dr. Cipto Mangunkusumo. Praktik Kerja Profesi Apoteker (PKPA) di Apotek JacoebSon bertujuan untuk Memahami peranan, tugas, dan tanggung jawab apoteker di Apotek sesuai dengan ketentuan dan etika pelayanan farmasi khususnya dan pelayanan kesehatan umumnya. Tugas Khusus yang diberikan berupa analisis prioritas pengendaliaan obat khususnya obat antibiotik di apotek JacoebSon Pharmacy dengan menggunakan metode pareto (ABC) periode Oktober 2018-September 2019.

ABSTRACT

The Pharmacist Professional Work Practice (PKPA) at the Farmawati Central General Hospital aims to understand the roles, duties, and responsibilities of pharmacists in the hospital in accordance with the provisions and ethics of pharmacy services in particular and health services in general. A special task is given in the form of monitoring drug therapy in DM type 2 patients with acute on CKD and hypertension stage II to evaluate the accuracy, effectiveness, safety and rationality of drug use therapy in patients. The Pharmacist Professional Work Practice (PKPA) at the Puskesmas Kecamatan Pesanggrahan aims to understand the roles, duties, and responsibilities of pharmacists in the Puskesmas in accordance with the provisions and ethics of pharmacy services in particular and health services in general. The special task given is in the form of providing health information to the public about how to prevent hypertension and how to control blood pressure so that hypertension can be controlled properly. The Pharmacist Professional Work Practice (PKPA) at the JacoebSon Pharmacy aims to understand the roles, duties, and responsibilities of pharmacists in the Pharmacy in accordance with the provisions and ethics of pharmacy services in particular and health services in general. The special task given is in the form of a priority analysis of drug control, especially antibiotic drugs at the JacoebSon Pharmacy using the Pareto (ABC) method for the period October 2018-September 2019.

Tugas khusus ini bertujuan untuk mengetahui jenis obat kemoterapi oral yang digunakan di RSCM berdasarkan Formularium RSCM 2013 serta menyusun buku panduan bagi pasien kanker mengenai cara penanganan obat kemoterapi oral di rumah, cara penanganan terhadap efek samping obat kemoterapi oral dan cara meningkatkan kepatuhan pasien minum obat kemoterapi oral.

Tugas khusus ini bertujuan untuk mengetahui tingkat kepuasan pelanggan terhadap pelayanan kefarmasian yang diberikan satelit – satelit farmasi di RSUPN Dr.Cipto Mangunkusumo.

This particular task aims to know level of customer satisfaction with the services provided satellite pharmacy in RSUPN Dr.Cipto Mangunkusumo."
