"Program spesialis keperawatan medikal bedah dengan kekhususan pada sistem muskuloskeletal ini bertujuan mengaplikasikan peran perawat sebagai pemberi asuhan, pengelola, pendidik, peneliti serta sebagai inovator. Peran sebagai pemberi asuhan dilakukan dengan mengelola sebanyak 30 pasien dengan masalah sistem muskuloskeletal dan satu sebagai kelolaan utama dengan kasus open fracture femur dengan pendekatan Need Theory Virginia Henderson. Peran perawat sebagai peneliti dilakukan dengan penerapan tindakan keperawatan yang berbasis bukti ilmiah atau Evidence-Based Nursing Practice yaitu dengan menerapkan skrining kinesiophobia (rasa takut akan gerakan / kembali cedera) pada pasien low back pain dengan the tampa scale for kinesiophobia. Peran sebagai pendidik sekaligus sebagai inovator dilakukan dengan mengembangkan program pendidikan dengan berbasis web yang bertujuan untuk meningkatkan pengetahuan dan kesiapan pulang pada pasien dengan rencana operasi total hip arthroplasty.

The purpose of this medical-surgical nursing specialist program with a special focus on the musculoskeletal system is to apply the role of nurses as caregivers, managers, educators, researchers and as innovators. The role as a care givers is applies by managing 30 patients with musculoskeletal system problems and one as the main focuses of management, is a case with open fracture of the femur using Virginia Henderson's Need Theory approach. The role of nurses as researchers is explain by implementing scientific evidence-based nursing practices or Evidence-Based Nursing Practice, by applying kinesiophobia screening (fear of movement / reinjury) in low back pain patients with the tampa scale for kinesiophobia. The role as an educator as well as an innovator is implementing by developing a web-based educational program that aims to increase knowledge and readiness to go home in patients with total hip arthroplasty surgery plans."

"Praktik klinik residensi keperawatan medikal bedah pada pasien osteomielitis bertujuan untuk mengembangkan dan mengimplementasikan peran perawat spesialis sebagai pemberi asuhan keperawatan profesional, edukator, peneliti, dan pembaharu inovator. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan Teori Self Care Orem pada pasien Infected union of left tibia post debridement and bone graft application from iliac wing dan 30 pasien lainnya dengan berbagai gangguan sistem muskuloskeletal. The clinical practice of medical-surgical nursing residency in osteomyelitis patients aims to develop and implement the role of specialist nurses as professional nursing care providers, educators, researchers, and reformers innovators. The role of nursing care provider is carried out using the Orem Self Care Theory approach in patients with infected union of left tibia post debridement and bone graft application from iliac wing and 30 other patients with various musculoskeletal system disorders. The role of educator and researcher is carried out by specialist nurses to patients and also colleagues in hospital service settings. The implementation of EBN (Evidence Based Nursing) is by applying diaphragmatic breathing exercise interventions in postoperative Total Knee Replacement patients to reduce pain and anxiety. Innovation projects are also carried out as a manifestation of the role as a reformer or innovator, namely through the implementation of the Autar scale DVT Risk Assessment in order to improve the quality of professional nursing care in the hospital."

"Karya Ilmiah ini merupakan hasil analisis pengalaman praktik residensi spesialis keperawatan medikal bedah kekhususan peminatan neurologi. Laporan ini bertujuan untuk memberikan gambaran pengalaman praktik residensi spesialis keperawatan medikal bedah dalam mengolola berbagai kasus dengan gangguan neurologis. Karya ilmiah ini terdiri asuhan keperawatan pada kasus kelolaan utama pada klien dengan tuberculosis of spine dengan menggunakan pendekatan model adaptasi Roy, penerapan Evidence Based Nursing (EBN) dengan intervensi mengunyah permen karet untuk pemulihan gastrointestinal pasca operasi tulang belakang, proyek inovasi nurse-led early mobilization, dan analisis pada 30 kasus resume pada klien dengan gangguan neurologis. Hasil analisis diagnosa pada 30 kasus resume ditemukam tiga diagnose keperawatan yang paling sering pada klien dengan gangguan neurologis yaitu penurunan kapasitas adaptif intrakranial, hambatan mobilitas fisik dan nyeri. Intervensi keperawatan yang paling sering dilakukan dalam menjalankan asuhan keperawatan diantaranya promosi perfusi serebral, pemantauan tekanan intrakranial, dan terapi fisik: kontrol otot.

This scientific work is the result of an analysis of the practical experience of a medical surgical nursing specialist residency with a specialization in neurology. This report aims to provide an overview of medical surgical nursing specialist residency experience in managing various cases with neurological disorders. This scientific work consists of nursing care in primary managed cases for clients with tuberculosis of the spine using the Roy Adaptation Model approach, application of Evidence Based Nursing (EBN) with chewing gum interventions for gastrointestinal recovery post operative spine surgery, nurse-led early mobilization innovation projects, and analysis on 30 case resumes on clients with neurological disorders. The results of the analysis of diagnoses in 30 resume cases found the three most common nursing diagnoses in clients with neurological disorders, namely decreased intracranial adaptive capacity, impaired physical mobility, and pain. The most frequently performed nursing interventions in carrying out nursing care include promoting cerebral perfusion, monitoring intracranial pressure, and physical therapy: muscle control.

"Analisis praktik residensi keperawatan medikal bedah ini merupakan karya akhir ners spesialis. Analisis praktik residensi ini bertujuan memberikan gambaran pelaksanaan praktik resiensi, terutama pengelolaan berbagai kasus neurologi. Analisis ini terdiri dari kasus kelolaan utama dan resume menggunakan pendekatan model adaptasi Roy, penerapan evidence based nursing (EBN) dan proyek inovasi. Kasus kelolaan utama pada laporan ini adalah lumbal spinal canal stenosis. Pada kasus kelolaan diagnosis keperawatan nyeri kronis, hambatan mobilitas fisik, risiko ketidakstabilan kadar glukosa darah, ansietas, hambatan ventilasi spontan, penurunan curah jantung, defisien pengetahuan dapat teratasi. Pada 30 kasus resume dengan penyakit gangguan neurologi terdapat 3 diagnosis keperawatan terbanyak yaitu risiko jatuh, nyeri akut dan hambatan komunikasi verbal. Penerapan EBN yang dilakukan pada praktik residensi ini adalah tentang therapeutic positions pada pasien stroke perawatan hari ketiga, dan terbukti dapat menurunkan tekanan darah, heart rate, respiration rate dan meningkatkan SPO2. Pada praktik ini juga residen menerapkan proyek inovasi dalam meningkatkan pengetahuan perawat tentang Nurse-Led Early Mobilization dalam melakukan asuhan keperawatan pada pasien dengan pembedahan spinal, hasil penerapan inovasi ini menunjukkan terjadinya peningkatan pengetahuan perawat terkait asuhan keperawatan perioperatif pada pasien pembedahan spinal. Banyak manfaat yang didapatkan dari praktik residensi yang telah dilakukan diantaranya latihan critical thinking dalam mengelola kasus sulit, melatih kemampuan kolaborasi dengan tim kesehatan lainnya, menerapkan asuhan keperawatan berbasis bukti. Hal ini sangat mendukung kepuasan pasien terhadap pelayanan keperawatan di rumah sakit.

This analysis of medical surgical nursing residency practice is the final work of specialist nurses. This residency practice analysis aims to provide an overview of the implementation of residency practices, especially the management of various neurological cases. This analysis consists of main managed cases and resumes using the Roy adaptation model approach, evidence-based nursing (EBN) applications, and innovation projects. The main case managed in this report is lumbar spinal canal stenosis. In cases of managed nursing diagnoses of chronic pain, physical mobility barriers, risk of instability of blood glucose levels, anxiety, spontaneous ventilation barriers, decreased cardiac output and knowledge deficiency can be overcome. In 30 cases of resumes with neurological disorders, there were 3 nursing diagnoses, namely the risk of falling, acute pain, and verbal communication barriers. The application of EBN in this residency practice is about therapeutic positions in stroke patients on the third day of treatment and is proven to reduce blood pressure, heart rate, and respiration rate and increase SPO2. In this practice, residents also implemented an innovation project to increase nurses' knowledge about Nurse-Led Early Mobilization in carrying out nursing care for patients with spinal surgery. The results of implementing this innovation indicated an increase in nurses' knowledge related to perioperative nursing care for spinal surgery patients. Many benefits have been obtained from the residency practice that has been carried out including critical thinking exercises in managing difficult cases, training the ability to collaborate with other health teams, and implementing evidence-based nursing care. This greatly supports patient satisfaction with nursing services at the hospital."

"Professionalime ilmu keperawatan adalah bagian disiplin ilmu keperawatan sampai dengan keperawatan sub spesialisasi dimana perawat spesialis sebagai seorang perawat yang dipersiapkan melampaui level seorang perawat generalis yang berwenang untuk praktek sebagai spesialis dengan keahlian dalam cabang bidang keperawatan. Ners spesialis keperawatan medikal bedah dengan peminatan muskuloskletal berperan dalam perawatan kekhususan masalah dan kasus kelolaan yang berkaitan dengan orthopedi atau muskuloskletal. Tujuan utama memberikan asuhan keperawatan secara komprehensif pada pasien kasus Spinal Cord Injury dengan trauma melalui pendekatan keperawatan model teori self-care Orem. Asuhan berupa pengelolaan 1 kasus kelolaan uatam dan 30 resume yang terdiri 3 bagian kasus besar yaitu kasus Spine, ekstremitas atas, dan ekstremitas atas dengan menggunakan teori model keperawatan Self-Care Orem yang menekankan tentang kebutuhan perawatan diri. Pencegahan terjadinya luka tekan dengan pemberian Aloe vera gel sebagai bagian dari tindakan mandiri keperawatan berdasarkan bukti penelitian ilmiah. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.

"Praktik residensi keperawatan medikal bedah sistem muskuloskeletal merupakan program lanjutan pendidikan magister keperawatan yang berfokus pada konsep dan prinsip dasar medikal bedah dengan menerapkan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa adanya gangguan struktural pada sistem muskuloskeletal. Salah satu model keperawatan yang dapat diterapkan untuk memenuhi kebutuhan pasien gangguan muskuloskeletal adalah pendekatan Teori Keperawatan adaptasi Roy. The practice of residency in medical surgical nursing of the musculosceletal system is an advanced program of nursing master education that focuses on the concepts and basic principles of medical surgery by applying nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders of the musculosceletal system. One of nursing model that can be applied to meet the need of patients with musculosceletal disorder is Roys Adaptation Theory approach. This approach is a model of nursing care that can help patient to adapt various problems caused by disorder of the musculosceletal system. Another activity of residency practice is implementing evidence based nursing (EBN) through ambulation program intervention in postoperative patients lower limb their mobility. In addition, activities through the application of the role of nurses in implementing of "enhanced recovery after surgery (ERAS)" program in hip surgery patients are innovative projects that can be implemented in the surgical ward to speed up the recovery process, reduce length of stay (LOS), prevent complication, also reduce post morbidity and mortality operation."

"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian. Teori Virginia Henderson dan Teori Sosial Kognitif Bandura di RSUP FatmawatiJakarta. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahanyang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori Virginia Henderson pada 1 kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunitis. Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan skrining Mual dan Muntah pasien HIV/AIDS menggunakan Index of Nausea, Vomiting and Retcing dan hasil dari evidence based nursing ini menunjukkan bahwa kuisioner INVR akurat dan reliable untuk menilai status mual dan muntah pada pasien HIV. Sedangkan sebagai innovator, telah dilakukan inovasi optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien dengan HIV / AIDS yang mana media teknologi digital berupa laman web yang dengan berbagai topik edukasi dengan web yang dapat di akses menggunakan barcode. Hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan pengetahuan dan kemudahan pasien dalam mengakases informasi penting tentang penyakit HIV sehingga dapat meningkatkan kepatuhan dalam minum obat ARV.

Nursing Specialist Practice is an important process of professional education in order to apply the role of specialist nurses that have an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on science and technology through various researches. Virginia Henderson's Theory and Bandura's Social Cognitive Theory at RSUP Fatmawati Jakarta. The three competencies that must be achieved in this residency practice are providing nursing care using a nursing theory approach, implementing evidence-based nursing practice, and carrying out nursing innovation projects that are useful for the land where the residency practice. As a nursing care provider, residency students have provided nursing care using Roy's adaptation theory approach in 1 major management case and 30 resume cases of HIV/AIDS patients with various opportunitistic infections. As researchers, residency students have applied the application of Nausea and Vomiting screening for HIV/AIDS patients using the Index of Nausea, Vomiting and Retcing and the results of this evidence-based nursing show that the INVR questionnaire is accurate and reliable to assess the status of nausea and vomiting in HIV patients. Meanwhile, as an innovator, digital technology optimization innovations have been carried out in improving adherence to taking ARV drugs in patients with HIV / AIDS where digital technology media is in the form of web pages with various educational topics with the web that can be accessed using barcodes. The results of the implementation of this innovation project show very significant benefits for increasing knowledge and ease of patients in accessing important information about HIV disease so as to improve adherence in taking ARV drugs."

Tujuan penulisanKarya Ilmiah Akhir ini adalah menganalisis kegiatan praktik Ners Spesialis Keperawatan Medikal Bedah berdasarkan standar kompetensi spesialis keperawatan medikal bedah kekhususan sistem muskuloskeletal. Ners Spesialis dalam melakukan asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy baik pada kasus kelolaan utama maupun resume. Penerapan Evidence Based Nursing (EBN) melalui intervensi latihan isometrik pada pasien pasca operasi fraktur ektremitas bawah, efektif menurunkan nyeri dan meningkatkan kekuatan otot. Proyek inovasi berupa pengembangan Enhanced Recovery after Surgery (ERAS) for orthopaedic untuk mempercepat proses pemulihan pasien operasi hip. Pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy dapat membantu pasien untuk beradaptasi terhadap penyakitnya. Intervensi latihan isometrik jika dikombinasikan dengan terapi non farmakologis yang lain seperti nafas dalam berpotensi bisa memberikan efek yang lebih baik terhadap penurunan nyeri.

ABSTRACTThe number of problems caused by musculoskeletal disorders requires the role of nursespecialist musculoskeletal nursing to provide professional and advanced nursing care based on patient needs. The purpose of writing this final scientific paper is to analyzethe practical activities of nurse specialist medical surgical nursing based on the competency standards of medical surgical nursing specialists, specifically the musculoskeletal system. Nurse specialists in conducting nursing care in patients withmusculoskeletal disorders use the Roy Adaptation Model theory approach both in thecase of main and resume managed. The application of Evidence Based Nursing (EBN)through isometric exercise intervention in patients with lower extremity fracture surgery, effectively reduces pain and increases muscle strength. The innovation project was in the form of developing Enhanced Recovery after Surgery (ERAS) for orthopedics to accelerate the recovery process for hip surgery patients. Giving nursing care to patients with musculoskeletal disorders using the Roy Adaptation Model theory approach can help patients adapt to the disease. An isometric exercise intervention if combined with other non-pharmacological therapies such as deep breathing has the potential to have a better effect on reducing pain."
