"Apoteker merupakan profesi dalam bidang kesehatan yang memiliki ruang lingkup pekerjaan yang sangat luas. Apoteker dapat bekerja di industri farmasi, pelayanan (apotek, rumah sakit), pemerintahan, dan juga Pedagang Besar Farmasi (PBF). Praktik Kerja memberikan gambaran dan pengalaman di dunia kerja. Pengamatan secara langsung mengenai kegiatan yang ada di setiap departemen (Produksi, Quality Control, dan Quality Assurance) dilakukan pada praktik kerja di industri farmasi, Apoteker juga dilatih untuk menganalisis berbagai masalah yang terjadi dan mengidentifikasi penyelesaiannya. Pada praktik kerja di PBF, apoteker diperlihatkan secara garis besar mengenai pekerjaan yang ada di setiap departemen dan ditugaskan untuk membuat proyek inovasi. Pada praktik kerja di apotek, apoteker melakukan pelayanan kefarmasian. Apoteker ditugaskan untuk melakukan skrining resep, penyiapan resep, dan disertai dengan konseling ke pasien. Skrining resep dilakukan untuk mencegah atau meminimalisir terjadinya medication error.

Pharmacist is a profession in the field of health that has a very wide scope of work. Pharmacist can work in the pharmaceutical industry, health services (pharmacies, hospitals), government and also pharmaceutical distributor. Internship provide an overview and experience in the world of work. Direct observations about the activities in each department (Production, Quality Control, dan Quality Assurance) are occurred during internship at the pharmaceutical industry. Pharmacist also learnt how to analyze many problems that happened and have to identified the closure. During internship at the pharmaceutical distributors, pharmacist was shown every department’s responsibility in general and had to make an innovation project. During internship at pharmacy, pharmacists did any pharmaceutical services. Pharmacist had to do prescription screening, prescription services, which be given with counselling. Prescription screening was done to prevent or minimize the occurrence of medication error."

"Profesi apoteker memiliki peranan penting dalam pekerjaan kefarmasian. Salah satu hal penting yang harus dilakukan untuk menjadi seorang apoteker professonal adalah berpartisipasi langsung dalam melakukan praktik kefarmasian. Apoteker merupakan profesional di bidang kesehatan yang berperan dalam memberikan informasi tentang obat-obatan yang akan dikonsumsi, memastikan bahwa segala aspek di industri farmasi sejalan dengan ketentuan pada CPOB dan menjadi penanggung jawab pada pelaksanaan distribusi obat. Melalui Praktik Kerja Profesi Apoteker, diharapkan calon apoteker dapat memahami apa yang menjadi tugas dan tanggung jawabnya. Praktik kerja dilaksanakan dari September hingga Desember 2021. Praktik kerja dilakukan di PT Takeda Indonesia periode September – Oktober, PT Anugerah Pharmindo Lestari periode November, dan Apotek Roxy Pondok Labu periode Desember, sebagai gambaran awal peran apoteker dibidang industri, distribusi, dan pelayanan. Selain belajar secara langsung dari mengamati, calon apoteker diharapkan mampu memperoleh wawasan, pengetahuan, keterampilan, dan pengalaman yang sesuai untuk melakukan pekerjaan kefarmasian.

The pharmacist profession has an important role in pharmacy work. One of the important things that must be done to become a professional pharmacist is to participate directly in the practice of pharmacy. Pharmacists are professionals in the health sector who play a role in providing information about the drugs to be consumed, ensuring that all aspects of the pharmaceutical industry are in line with the provisions of the GMP and are responsible for the implementation of drug distribution. Through the Pharmacist Professional Internship, it is expected that prospective pharmacists can understand what their duties and responsibilities are. The Internship are carried out from September to December 2021. Interships are carried out at PT Takeda Indonesia for the September – October period, PT Anugerah Pharmindo Lestari for the November period, and the Roxy Pondok Labu Pharmacy for the December period, as an initial overview of the pharmacist's role in industry, distribution, and pharmaceutical care. In addition to learning directly from observing, prospective pharmacists are expected to be able to gain insight, knowledge, skills, and experience that are suitable for carrying out pharmaceutical work."

"Pemastian mutu dalam industri farmasi dapat dilakukan melalui pengkajian mutu produk (Product Quality Review) setiap tahun atau secara berkala. Tujuan pelaksanaan tugas khusus di industri farmasi PT Mahakam Beta Farma adalah mengetahui tujuan pembuatan dan departemen terkait dalam penyusunan laporan PQR serta mampu menyusun laporan PQR. Dokumen PQR bersifat komprehensif dengan tujuan memverifikasi konsistensi proses suatu produk selama periode berjalan, kesesuaian spesifikasi bahan baku, bahan pengemas dan produk jadi, melihat dan mengevaluasi tren analisis dari tiap tahapan produksi, mengidentifikasi peluang tindakan perbaikan dan pencegahan yang diperlukan untuk produk dan proses, melakukan evaluasi terhadap rekomendasi serta meningkatkan mutu produk sediaan likuid pada produksi periode yang akan datang. Fasilitas pelayanan kesehatan yang berada di lingkungan masyarakat adalah Apotek. Tujuan pelaksanaan tugas khusus di Apotek Roxy Pondok Labu adalah menjadikan poster edukasi hipertensi sebagai media penunjang dalam pemberian edukasi tentang hipertensi kepada masyarakat. Pembuatan poster edukasi tentang hipertensi dapat menjadi media informasi di lingkungan Apotek Roxy Pondok Labu sehingga meningkatkan pemahaman terkait gejala, komplikasi, dan pencegahan hipertensi serta penyampaian informasi kepada pasien lebih efektif dan maksimal.

Making educational posters about hypertension can be a medium of information in the Roxy Pondok Labu Pharmacy environment understanding related to symptoms, complications, and prevention of hypertension as well as delivering information more effectively and maximally."

"Salah satu upaya yang dilakukan industri farmasi dalam rangka menjaga dan meningkatkan kualitas obat yang diproduksinya yaitu dengan menerapkan CPOB (Cara Pembuatan Obat yang Baik). Salah satu cara untuk mengevaluasi suatu produk yaitu dengan melakukan Product Quality Review (PQR) atau Pengkajian Mutu Produk (PMP). Pasien dapat membeli obat bebas, obat bebas terbatas, dan obat yang masuk ke dalam daftar obat wajib apotek secara langsung serta obat yang menggunakan resep dokter di apotek. Salah satu obat yang tinggi permintaannya di Apotek adalah obat golongan antihipertensi. Oleh karena itu, penelitian ini bertujuan untuk mengetahui gambaran penggunaan obat antihipertensi (golongan obat generik berlogo) di Apotek Roxy Pondok Labu periode Oktober 2021, sehingga gambaran tersebut dapat menjadi pertimbangan bagi apoteker atau tenaga teknis kefarmasian untuk melakukan perencanaan dan pengadaan obat di periode selanjutnya. Agar distribusi obat berjalan dengan baik dan sesuai dengan peraturan yang berlaku, PBF harus mengacu pada Cara Distribusi Obat yang Baik (CDOB). Salah satu aspek CDOB yaitu penyimpanan, kegiatan yang terkait dengan penyimpanan obat dan/atau bahan obat harus memastikan terpenuhinya kondisi penyimpanan yang dipersyaratkan dan memungkinkan penyimpanan secara teratur sesuai kategorinya. Oleh karena itu, laporan praktik kerja ini akan membahas mengenai masalah beserta solusi yang diajukan di PT Mahakam Beta Farma, Apotek Roxy Pondok Labu, dan PT Anugerah Pharmindo Lestari yang dilaksanakan pada periode bulan Juli – Nopember 2021.

One of the efforts made by the pharmaceutical industry in order to maintain and improve the quality of the drugs it produces is by applying GMP (Good Manufacturing Practice). One way to evaluate a product is to do a Product Quality Review (PQR). Patients can purchase over-the-counter medications, limited over-the-counter medications, and pharmacy's mandatory drugs list and prescription drugs at the pharmacy. One of the drugs that are in high demand in pharmacies is antihypertensive class drugs. Therefore, this study aims to find out an overview of the use of antihypertensive drugs (generic drug class with logos) at Roxy Pondok Labu Pharmacy for the period of October 2021, so it can be a consideration for pharmacists or pharmaceutical technical personnel to carry out drug planning and procurement in the next period. In order for drug distribution to run well and in accordance with applicable regulations, PBF or Pharmaceutical Distributor must refer to the Good Distribution Practice (GDP). One aspect of GDP is storage, activities related to the storage of drugs and/or medicinal materials must ensure the fulfillment of the required storage conditions and allow regular storage according to their categories. Therefore, this practice report will discuss the problems and solutions proposed at PT Mahakam Beta Farma, Apotek Roxy Pondok Labu, and PT Anugerah Pharmindo Lestari which was held in the period July – November 2021"

"Continued Process Verification dilakukan selama siklus hidup produk dan merupakan persyaratan wajib baru dan diterapkan terlepas dari pendekatan apapun yang dipilih pada proses validasi. Akan tetapi dalam penerapannya industri farmasi mengalami banyak kesulitan terutama industri yang sistem pendataanya masih manual. FDA mengharapkan industri farmasi untuk menjaga peralatan laboratorium dalam keadaan bersih dan tersanitasi untuk memberikan konfidensi pada hasil analisis. Salah satu cara untuk memberikan konfidensi ini adalah melalui program validasi yaitu validasi pembersihan. Namun, detail pada validasi pembersihan alat gelas laboratorium tidak harus sama dengan validasi pembersihan peralatan yang digunakan untuk produksi karena alat gelas laboratorium hanya digunakan untuk tujuan pengujian dan dinilai memiliki resiko yang rendah terhadap produk dan pasien. Sehingga perlu dilakukan kajian resiko untuk menentukan produk penanda, peralatan laboratorium penanda dan kriteria keberterimaan yang sesuai.

The FDA expects the pharmaceutical industry to keep laboratory equipment clean and sanitized to provide confidence in the results of the analysis. One way to provide this confidence is through a validation program, namely cleaning validation. However, the details on the cleaning validation of laboratory glassware do not have to be the same as the validation of cleaning equipment used for production because laboratory glassware is only used for testing purposes and is considered to have a low risk to the product and the patient. So it is necessary to carry out a risk assessment to determine the appropriate marker product, marker laboratory equipment and acceptance criteria."

Pharmacist Internship at Roxy Jatibaru Pharmacy aims to study the activities of pharmaceutical services in pharmacies, study the roles and functions of pharmacists in pharmacies, and specifically analyze deviations in drug storage based on pharmacological aspects at Roxy Jatibaru Pharmacy."

"Obat merupakan bahan atau campuran yang digunakan sebagai penetapan diagnosis, pencegahan, penyembuhan, pemulihan, serta peningkatan kesehatan manusia. Oleh sebab itu produk yang dihasilkan industri farmasi harus dilakukan product quality review (PQR) yang dilakukan secara berkala terhadap semua produk farmasi yang terdaftar termasuk ekspor, untuk menilai standar kualitas masing-masing produk obat dengan melihat konsistensi proses yang ada, kesesuaian dari spesifikasi bahan awal, bahan pengemas hingga produk jadi. Hiperkolesterolemia adalah salah satu gangguan metabolisme lipid yang ditandai dengan peningkatan kadar kolesterol di dalam darah melebihi batas normal yang memiliki risiko kematian penyakit jantung koroner. Banyaknya pasien berkunjung dan melakukan swamedikasi di Apotek Roxy Pondok Labu untuk meredakan gejala hiperkolesterolemia yang dikeluhkannya. Sehingga perlu diketahui gambaran konsumsi obat hiperkolesterolemia dalam mengatasi keluhan pasien sehingga dapat dijadikan sebagai dasar pertimbangan bagi farmasis dalam melakukan kegiatan pengelolaan sediaan farmasi mulai dari perencanaan, pengadaan hingga pengendalian stok obat di Apotek Roxy Pondok Labu. Penyusunan Product Quality Review, dan gambaran konsumsi obat hiperkolesterolemia dilakukan pada saat Praktik Kerja Profesi Apoteker di PT Mahakam Beta Farma, dan Apotek Roxy Pondok Labu. Pengerjaan dilakukan pada periode Juli – Agustus, dan Oktober 2021.

Drugs are substances or mixtures that are used as a diagnosis, prevention, cure, recovery, and improvement of human health. Therefore, for the products produced by the pharmaceutical industry, product quality review (PQR) must be carried out periodically on all registered pharmaceutical products including exports, to assess the quality standards of each drug product by looking at the consistency of the process used, the suitability of the specifications of starting materials, packaging materials to finished products. Hypercholesterolemia is a lipid metabolism disorder which is characterized by an increase in cholesterol levels in the blood that exceeds the normal limit which has a risk of death from coronary heart disease. Many patients visit and do self-medication at the Roxy Pondok Labu Pharmacy to relieve the symptoms of hypercholesterolemia that they complain about. So it is necessary to know the description of hypercholesterolemic drug consumption in overcoming patient complaints so that it can be used as a basis for consideration for pharmacists in carrying out pharmaceutical preparations management activities ranging from planning, procurement to drug stock control at Roxy Pondok Labu Pharmacy. Preparation of Product Quality Review, and description of hypercholesterolemia drug consumption was carried out during the Pharmacist Professional Work Practice at PT Mahakam Beta Farma, and Roxy Pondok Labu Pharmacy. Work is carried out in the period July – August, and October 2021."

"Rumah Sakit dituntut untuk memberikan pelayanan yang bermutu sesuaidengan standar yang ditetapkan dan dapat menjangkau seluruh lapisan masyarakat.Standar pelayanan minimal di rumah sakit pada pelayanan farmasi salah satunyaadalah penulisan resep sesuai formularium harus 100% dengan tujuan yaitutergambarnya efisiensi pelayanan obat kepada pasien. Formularium Rumah Sakitbermanfaat dalam kendali mutu dan kendali biaya obat yang akan memudahkanpemilihan obat yang rasional, mengurangi biaya pengobatan, dan mengoptimalkanpelayanan kepada pasien. Pemantauan dan evaluasi Formularium Rumah Sakitdilakukan terhadap kepatuhan penggunaan Fornas dan kepatuhan penggunaanFormularium Rumah Sakit.Penelitian ini dilakukan di RSUI pada tanggal 6 September 2021 – 31Oktober 2021 dan merupakan penelitian deksriptif menggunakan metodepengambilan sampel secara retrospektif dengan mengamati dan mengevaluasikesesuaian peresepan obat pasien depo farmasi rawat jalan RSUI periode Januari –September 2021 dengan Formularium Rumah Sakit RSUI berdasarkan kriteriainklusi yaitu resep pasien rawat RSUI dengan tanggal resep pada periode Januari-September 2021 dan kriteria eksklusi yaitu resep alat kesehatan dan bahan medis habis pakai, juga order location yang bukan berasal dari poliklinik rawat jalan RSUI. Selain itu juga dilakukan evaluasi kesesuaian obat lost sales di depo farmasi rawat jalan RSUI periode Januari – September 2021 dengan Formularium Rumah Sakit RSUI dengan kriteria inklusi seluruh resep pasien dari depo rawat jalan yang terdapat salah satu jenis item obat atau semua item yang obatnya disalin resep (copy resep) keseluruhan atas permintaan pasien atau tidak ditanggung oleh guarantor atau stok obat habis dan kriteria ekslusi yaitu Resep pasien dari depo rawat jalan yang obatnya tidak disalin resep dan disalin sebagian saja. Data diolah dan dihitung menggunakan Microsoft Excel 2013 kemudian data yang dihasilkan ditampilkan dalam bentuk persentase. Hasil penelitian di dapat bahwa peresepan obat pada depo farmasi rawat jalan RSUI periode Januari – September 2021 secara keseluruhan didapatkan 59,52% yang sesuai dengan Formularium Rumah Sakit dan yang tidak sesuai yaitu 40,47%. Dengan peresepan obat pada depo farmasi rawat jalan non COVID-19 RSUI periode Januari – September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 61,08% dan yang tidak sesuai yaitu 38,92%. Sedangkan peresepan obat pada depo farmasi rawat jalan khusus COVID-19 RSUI periode Januari – Praktik Kerja di RSUI, PT. Anugerah PharmindoLestari, dan Apotek Kimia Farma Periode Bulan September – Desember 2021ix September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 59,38% danyang tidak sesuai yaitu 40,63%. Untuk obat lost sales yang sesuai denganFormularium Rumah Sakit yaitu 61,12% dan yang tidak sesuai yaitu 38,88%.Kata Kunci: Rumah Sakit, Formularium Rumah Sakit, Lost Sales, Depo Farmasi Rawat Jalan.

Hospitals are required to provide quality services in accordance with

established standards and can reach all levels of society. Minimum service

standards in hospitals specifically in pharmaceutical services, one of which is

writing prescriptions according to the formulary must be 100% with the aim of

depicting the efficiency of drug services to patients. Hospital Formularies are useful

in quality control and drug cost control which will facilitate rational drug selection,

reduce treatment costs, and optimize services to patients. Monitoring and evaluation

of the Hospital Formulary is carried out on compliance with the use of National

Drug Formulary and compliance with the use of Hospital Formulary.

This study was conducted at RSUI on September 6, 2021 – October 31, 2021

and is a descriptive study using a retrospective sampling method by observing and

evaluating the suitability of drug prescriptions for outpatient pharmacy depots at

RSUI for the period January – September 2021 with the RSUI Hospital Formulary

based on inclusion criteria are prescriptions for RSUI inpatients with prescription

dates in the January – September 2021 period and exclusion criteria are

prescriptions for medical devices and consumable medical materials, as well as

order locations that are not from the RSUI outpatient polyclinic. In addition, an

evaluation of the suitability of lost sales drugs at the outpatient pharmacy depot of

RSUI for the period January – September 2021 with the RSUI Hospital Formulary

was also carried out with the inclusion criteria of all patient prescriptions from the

outpatient depot which contained one type of drug item or all items whose

medicines were copied by prescription (copy of prescription) in its entirety at the

request of the patient or not covered by the guarantor or the drug stock runs out and

the exclusion criteria are patient prescriptions from outpatient depots whose

medicines are not copied and only partially copied. The data is processed and

calculated using Microsoft Excel 2013 then the resulting data is displayed in

percentage form.

The results of the study showed that drug prescriptions at the outpatient

pharmacy depot of RSUI for the period January – September 2021 overall obtained

59.52% which were in accordance with the Hospital Formulary and those that were

not appropriate, 40.47%. By prescribing drugs at non-COVID-19 outpatient

pharmacy depots at RSUI for the period January – September 2021, which are in

accordance with the Hospital Formulary, 61.08% and those that are not appropriate,

38.92%. Meanwhile, drug prescriptions at the outpatient pharmacy depot

specifically for COVID-19 RSUI for the period January – September 2021 which

Tugas khusus praktik kerja di Rumah Sakit Universitas Indonesia yang diberikan berjudul Evaluasi Penggunaan Obat dengan Metode ABC di Depo Farmasi Rawat Jalan di Rumah Sakit Universitas Indonesia Periode Januari – September 2021. Tujuannya untuk mengetahui obat-obatan yang tergolong fast moving, slow moving dan dead stock. Obat yang tergolong slow moving perlu dieleminasi agar tidak menjadi dead stock. Tugas khusus praktik kerja di PT Anugerah Pharmindo lestari adalah Digitalisasi Proses Quality Assurance: Pengembangan Sistem eQMS Menggunakan Aplikasi Smartphone bertujuan untuk mengatasi masalah yang dirasakan BRQA agar mempermudah pekerjaan, mempersingkat waktu dan meminimalisir terjadinya kesalahan. Tugas khusus praktik kerja di Apotek Kimia Farma adalah Evaluasi Kerasionalan Peresepan Obat Pada Pasien Diabetes Melitus di Apotek Kimia Farma 382 Depok. Tujuannya untuk mengetahui apakah obat yang diberikan sudah rasional atau belum agar dilakukan pengkajian ulang agar tidak terjadi kesalahan berulang.
