"Kegiatan Praktik Kerja Profesi Apoteker (PKPA) bertujuan agar mahasiswa profesi apoteker dapat melihat langsung aktivitas yang berlangsung dalam suatu industry farmasi dan juga sarana pelayanan kefarmasian (Apotek dan Puskesmas), memperoleh pengetahuan dan wawasan tentang penerapan CPOB di Industri farmasi dan praktek kefarmasian berdasarkan standar pelayanan kefarmasian, serta dapat memiliki pemahaman yang mendalam mengenai peran dan tugas apoteker baik di bidang industri maupun pelayanan. Tugas Khusus yang diberikan pada PKPA di Industri berjudul “Kualifikasi Konkuren Excel Spreadsheet Grup 7”. Penyusunan tugas khusus ini bertujuan untuk mengetahui proses validasi dan kualifikasi di PT. SOHO Industri Pharmasi. Selanjutnya Tugas khusus yang diberikan pada PKPA Apotek berjudul “Pengkajian Resep Secara Administrasi, Farmasetis dan Klinis Pada Resep Program Rujuk Balik di Apotek Kimia Farma Burlian”. Penyusunan tugas khusus ini bertujuan untuk mengetahui salah satu peran apoteker dalam pelayanan klinis di apotek yaitu pengkajian resep. Pada PKPA di Puskesmas tugas khusus yang diberikan berjudul “Monitoring Tingkat Kepatuhan Sarana Kefarmasian (Apotek) di Kecamatan Cipayung Terhadap Standar Pelayanan Kefarmasian” yang bertujuan untuk memahami salah satu peran apoteker dalam pengawasan, pengendalian dan pembinaan saranan pelayanan kefarmasian dalam penerapan standar pelayanan kefarmasian.

The Pharmacist Professional Work Practice (PKPA) activity aims to provide pharmacist students with firsthand experience of the activities that occur in the pharmaceutical industry as well as pharmaceutical service facilities (pharmacies and health centers), as well as knowledge and insight into the application of GMP in the pharmaceutical industry and pharmaceutical practice based on service standards, and have a thorough understanding of pharmacists' roles and responsibilities in both industry and service sector. In Industry, PKPA was given a special assignment titled "Concurrent Qualification of Excel Spreadsheet Group 7." This special task is being prepared in order to determine the validation and qualification process at PT. SOHO Pharmaceutical Industry. Furthermore, PKPA at Pharmacy has been assigned the special task of "Administrative, Pharmaceutical, and Clinical Prescription Assessment in the Referral Referral Prescription Program at Kimia Farma Burlian Pharmacy." This special task's preparation aims to determine one of pharmacists' roles in clinical services in pharmacies, namely prescription assessment. A special task entitled "Monitoring the Compliance Level of Pharmaceutical Facilities (Apotek) in Cipayung District Against Pharmaceutical Service Standards" was assigned at the PKPA at Puskesmas, with the goal of understanding one of the roles of pharmacists in monitoring, controlling, and fostering pharmaceutical service advice in the application of pharmaceutical service standards."

"Praktik Kerja Profesi Apoteker adalah sarana yang memberikan kesempatan bagi para calon apoteker untuk mengalami dan mempelajari tugas-tugas apoteker di setiap macam sektor bisnis yang dipilih secara langsung. Pada setiap tempat PKPA yaitu di PT Kimia Farma Trading & Distribution Jakarta 3, Apotek Jacoebson, dan PT SOHO Industri Pharmasi, calon apoteker melakukan observasi dan kemudian menganalasis kesesuaian keadaan di lapangan dengan literatur-literatur yang bersangkutan dengan topik bahasan yang telah ditentukan. Telah ditemukan bahwa terdapat beberapa ketidaksesuaian kondisi di lepangan dengan literatur, mau dengan justifikasi ataupun tidak. Oleh sebab itu, calon apoteker telah memberi beberapa saran atau solusi yang dapat dilakukan untuk permasalahan-permasalahan yang ditemukan.

The Pharmacist Professional internship is a medium that provides opportunities for prospective pharmacists to directly experience and learn the duties of pharmacists in every type of business sector chosen. At each internship location, namely at PT Kimia Farma Trading & Distribution Jakarta 3, Apotek Jacoebson, and PT SOHO Industri Pharmasi, prospective pharmacists made observations and then analyzed the suitability of the situation in the field with the relevant literature with a predetermined topic of discussion. It has been found that there were some discrepancies between the conditions in the field and the literature, whether justified or not. Therefore, prospective pharmacists have given several suggestions or solutions that can be done for the problems found."

"Profesi apoteker memainkan peran yang krusial dalam menyediakan pelayanan kesehatan yang aman dan efektif. Apoteker berkolaborasi dengan tenaga medis dan pasien untuk memastikan penggunaan obat yang tepat, memberikan informasi farmasi yang akurat, serta mengelola proses distribusi dan pengawasan obat. Praktek kerja profesi apoteker merupakan bagian penting dari pendidikan dan pelatihan untuk menjadi seorang apoteker yang berkualitas. Selama praktek kerja, mahasiswa atau calon apoteker berkesempatan untuk menerapkan pengetahuan yang telah dipelajari dalam lingkungan kerja yang nyata. Tujuan dari praktek kerja ini adalah untuk mendapatkan pemahaman praktis tentang peran dan tanggung jawab seorang apoteker dalam memberikan layanan farmasi kepada masyarakat. Praktik dilaksanakan dari bulan Maret hingga Oktober 2022. Praktik kerja dilakukan di PT Kimia Farma Trading & Distribution, Puskesmas Kecamatan Pancoran, RSPAD Gatot Soebroto, Apotek Roxy, dan PT SOHO Industri Pharmasi sebagai gambaran peran apoteker dibidang industri, distribusi maupun pelayanan. Selain belajar secara langsung dari mengamati, calon apoteker juga diberikan kesempatan untuk menganalisis masalah yang ada di masing-masing tempat kerja. Oleh karena itu, laporan praktik kerja ini akan membahas mengenai masalah beserta solusi yang diajukan di tiap tempat praktik kerja.

The pharmacist profession plays a crucial role in providing safe and effective health services. Pharmacists collaborate with medical personnel and patients to ensure proper drug use, provide accurate pharmaceutical information, and manage the drug distribution and control process. Pharmacist professional practice is an important part of education and training to become a qualified pharmacist. During internships, students or prospective pharmacists have the opportunity to apply the knowledge they have learned in a real work environment. The aim of this work practice is to gain a practical understanding of the roles and responsibilities of a pharmacist in providing pharmaceutical services to the public. The practice is held from March to October 2022. The practical work is carried out at PT Kimia Farma Trading & Distribution, Pancoran District Health Center, Gatot Soebroto Army Hospital, Roxy Pharmacy, and PT SOHO Industri Pharmasi as an illustration of the role of pharmacists in industry, distribution and service. Apart from learning directly from observing, prospective pharmacists are also given the opportunity to analyze the problems that exist in each workplace. Therefore, this internship report will discuss the problems and solutions proposed in each workplace.

"Praktik kerja profesi apoteker dilakukan di PT Dankos Farma, PT Anugerah Pharmindo Lestari, dan Apotek Kimia Farma 352 Margonda pada bulan Juli-Desember tahun 2021. Laporan Praktik Kerja Profesi Apoteker ini memberikan beberapa informasi terkait dengan peran apoteker dalam mengaplikasikan Annex 1 Good Manufacturing Practices (GMP) Guide tentang Manufacture of sterile drugs (GUI-0119) dan Cara Pembuatan Obat yang Baik (CPOB) 2018 Aneks 1 tentang Pembuatan Produk Steril, mengaplikasikan Corrective and Preventive Action (CAPA) dalam upaya pencegahan dan penyelesaian masalah, merancang aplikasi kualifikasi pelanggan untuk PBF Anugerah Pharmindo Lestari, dan melakukan proses praktik kefarmasian jarak jauh atau Telefarma. Laporan disusun menggunakan metode pengkajian atau penelusuran studi literatur dan wawancara dengan pasien atau narasumber yang dilakukan pada periode Juli – Desember tahun 2021. Wawancara dengan pasien atau narasumber dibimbing secara langsung oleh Pembimbing Lapangan (Pembimbing I) dan secara tidak langsung oleh Pembimbing II. Hasil dari beberapa kegiatan ini menunjukkan bahwa apoteker baik di industri farmasi, PBF, maupun apotek memiliki peran yang sangat penting dalam meningkatkan kualitas produk farmasi, pendistribusian produk farmasi, dan pelayanan kefarmasian. Selain itu, kegiatan ini penting dilaksanakan sebagai upaya untuk meningkatkan pengalaman dan pengetahuan seorang calon apoteker.

The professional practice of pharmacists was carried out at PT Dankos Farma, PT Anugerah Pharmindo Lestari, and Apotek Kimia Farma 352 Margonda from July-December 2021. This Pharmacist Professional Practice Report provides some information related to the role of pharmacists in applying Annex 1 Good Manufacturing Practices (GMP) Guide on Manufacturing of sterile drugs (GUI-0119) and Good Manufacturing Practices (GMP) 2018 Annex 1 on Manufacturing of Sterile Products, applying Corrective and Preventive Action (CAPA) in efforts to prevent and solving problems, designing customer qualification applications for PBF Anugerah Pharmindo Lestari, and carry out the process of remote pharmacy practice or Telefarma. The report was prepared using the study method or literature study search and interviews with patients or interviewees from July to December 2021. Interviews with patients or interviewees were guided directly by the Field Supervisor (Supervisor I) and indirectly by Supervisor II. The results of these activities indicate that pharmacists in the pharmaceutical industry, PBF, and pharmacies have a vital role in improving the quality of pharmaceutical products, distribution of pharmaceutical products, and pharmaceutical services. In addition, this activity is essential to increase the experience and knowledge of a prospective pharmacist."

"Dalam rantai pemasokan farmasi, obat-obatan yang dibuat dari produsen farmasi atau industri farmasi akan didistribusikan oleh distributor sampai di tangan konsumen melalui pelayanan kefarmasian di fasilitas kesehatan, termasuk Apotek. Penelitian ini memiliki beberapa tujuan, yaitu untuk untuk mengidentifikasi ketidakselarasan database vendor pada Quality Compliance dengan Approved Vendor List (AVL) di PT. Soho Industri Pharmasi selama periode Juli – Agustus 2021; mengkaji resep obat-obat pada pasien kardiovaskular di Apotek Roxy Cabang Biak periode September 2021, serta pembuatan pembatas antar batch berbeda dalam satu produk di PT Anugerah Phamrindo Lestari cabang Bogor. Pengambilan data diperoleh dari database vendor yang dimiliki oleh Quality Compliance dan Quality Assurance; pembuatan pembatas dilakukan dengan observasi langsung ke Gudang PT Pharmindo Lestari cabang Bogor; serta resep diperoleh dari pengunjung yang melakukan penebusan resep di Apotek Roxy Biak. Hasil penelitian diperoleh yakni kegiatan kefarmasian di PT. Soho Industri Pharmasi khususnya pada Quality Compliance bahwa terdapat gap pada database vendor di Quality Compliance dan AVL; distributor PT Anugerah Pharmindo Lestari memiliki pembatas untuk menghindari terjadinya kesalahan pengambilan produk dengan nomor batch yang berbeda; Apotek Roxy Biak melakukan pekerjaan kefarmasian yang telah sesuai dengan peraturan perundang-undangan yang berlaku, melalui pelaksanaan penyiapan dan pelayanan resep yang diawali dengan skrining resep secara administratif, farmasetik dan klinis, kemudian dilanjutkan dengan dispensing obat.

In the pharmaceutical supply chain, medicines made from pharmaceutical manufacturers or the pharmaceutical industry will be distributed by distributors to consumers through pharmaceutical services at health facilities, including pharmacy or drugstore. This study has several objectives, there is to identify the inconsistency of the vendor database on Quality Compliance with the Approved Vendor List (AVL) at PT. Soho Industri Pharmasi during the period July – August 2021; studied the prescription of drugs for cardiovascular patients at the Roxy Pharmacy, Biak Branch for the period of September 2021, as well as the manufacture of barriers between different batches in one product at PT Anugerah Phamrindo Lestari, Bogor branch. Data retrieval is obtained from vendor databases owned by Quality Compliance and Quality Assurance; making the barrier is done by direct observation to the warehouse of PT Pharmindo Lestari Bogor branch; and recipes obtained from visitors who redeem prescriptions at the Roxy Biak Pharmacy. The results obtained are pharmaceutical activities at PT. Soho Industri Pharmasi especially on Quality Compliance that there is a gap in the vendor database in Quality Compliance and AVL; the distributor of PT Anugerah Pharmindo Lestari has a barrier to avoid the occurrence of errors in taking products with different batch numbers; Roxy Biak Pharmacy carries out pharmaceutical work in accordance with applicable laws and regulations, through the preparation and service of prescriptions, beginning with administrative, pharmaceutical and clinical prescription screening, followed by drug dispensing."

"CPOB atau Cara Pembuatan Obat yang Baik bertujuan untuk menjamin obat dibuat secara konsisten, memenuhi persyaratan yang ditetapkan dan sesuai dengan tujuan penggunaannya. Dalam CPOB tercantum persyaratan untuk ruang karantina. PT Hexpharm Jaya Laboratories memiliki ruang Work In Process (WIP) sebagai ruang karantina produk yang merupakan area penting dalam proses. Area ini digunakan sebagai tempat untuk karantina produk antara dan/atau tablet yang siap untuk dilakukan proses primary packaging. Kegiatan dan produk yang ada di ruang WIP harus sesuai dengan pedoman Cara Pembuatan Obat yang Baik (CPOB), produk yang berada dalam ruang WIP harus jelas identitasnya, ruangan harus terjaga kebersihannya, wadah yang digunakan harus sesuai ketentuan, dan akses untuk memasuki ruang WIP harus terbatas hanya untuk orang yang memiliki wewenang, seperti supervisor dan PIC pada ruang produksi. Penumpukan dalam ruang WIP juga dapat menyebabkan mix up batch yang dapat terjadi apabila operator salah mengambil material atau saat operator menggeser wadah, label identitas dari produk yang menempel pada wadah terjatuh mengakibatkan kemungkinan label tertukar. Oleh karena itu, dalam tugas khusus ini penulis diminta untuk memanajemen ruang WIP agar lebih terstruktur dan sesuai dengan pedoman Cara Pembuatan Obat yang Baik dengan cara pembuatan mapping dan lay out pada ruang WIP. Hasil yang didapat, hampir semua aspek pada ruang WIP PT. Hexpharm Jaya Laboratorium sudah sesuai dengan pedoman CPOB, hanya aspek kerapihan saja yang masih perlu ditingkatkan. Aspek kerapihan dapat diatasi dengan memanajemen ruang WIP dengan pembuatan mapping, hasilnya cukup efektif untuk membuat ruang WIP lebih terstruktur sehingga dapat menghindari risiko mix up batch dan memudahkan operator dalam mencari MSC atau MSS yang akan diproses

CPOB or Good Manufacturing Practices of Drugs aims to ensure that drugs are made consistently, meet the stipulated requirements and are in accordance with their intended use. In the GMP, the requirements for quarantine rooms are listed. PT Hexpharm Jaya Laboratories has a Work In Process (WIP) room as a product quarantine room which is an important area in the process. This area is used as a place for quarantine of intermediate products and/or tablets that are ready for the primary packaging process. The activities and products in the WIP room must comply with the guidelines for Good Manufacturing Practices (GMP), the product in the WIP room must have a clear identity, the room must be kept clean, the containers used must comply with the provisions, and access to enter the WIP room must be limited to authorized persons, such as supervisors and PICs in the production room. Stacking in the WIP chamber can also cause a mix up batch which can occur if the operator picks up the wrong material or when the operator shifts the container, the identity label of the product attached to the container falls, resulting in the possibility of labels being confused. Therefore, in this special task, the author is asked to manage the WIP room so that it is more structured and in accordance with the guidelines for the Good Manufacturing Practice of Medicine by mapping and laying out the WIP room. The results obtained, almost all aspects of the PT. Hexpharm Jaya Laboratory has complied with GMP guidelines, only the tidiness aspect still needs to be improved. The neatness aspect can be overcome by managing the WIP space by making a mapping, the results are quite effective in making the WIP room more structured so as to avoid the risk of batch mix ups and make it easier for operators to find MSC or MSS to be processed."

"Pada pembuatan obat, pengendalian menyeluruh adalah sangat esensial untuk menjamin bahwa konsumen menerima obat yang bermutu tinggi. Pembuatan secara sembarangan tidak dibenarkan bagi produk yang digunakan untuk menyelamatkan jiwa, atau memeliharakan kesehatan. Apotek yang merupakan salah satu fasilitas pelayanan kesehatan, adalah suatu sarana memelihara dan meningkatkan kesehatan, mencegah dan menyembuhkan penyakit serta memulihkan kesehatan perorangan, keluarga, kelompok dan masyarakat yang diselenggarakan secara sendiri atau bersama-sama dalam suatu organisasi. PBF adalah perusahaan berbentuk badan hukum yang memiliki izin untuk pengadaan, penyimpanan, penyaluran sediaan farmasi dalam jumlah besar sesuai ketentuan peraturan perundang-undangan. Pedagang Besar Farmasi (PBF) sebagai merupakan salah satu unit terpenting dalam kegiatan penyaluran sediaan farmasi ke fasilitas pelayanan kesehatan seperti apotek, instalasi farmasi rumah sakit, puskesmas, klinik dan toko obat agar dapat sampai ke tangan masyarakat. Apoteker sebagai penanggung jawan di PBF harus mampu melakukan kegiatan pengelolaan sediaan farmasi di PBF dimulai dari pengadaan, penyimpanan hingga pendistribusia sediaan farmasi ke sarana pelayanan kesehatan.

Pharmacy which is one of the health service facilities, is a means of maintaining and improving health, preventing and curing disease and restoring the health of individuals, families, groups and communities which are held individually or together in an organization. Pharmaceutical wholesaler is a company in the form of a legal entity that has a license for the procurement, storage, distribution of pharmaceutical preparations in large quantities in accordance with the provisions of the legislation. Pharmacy Wholesalers is one of the most important units in the distribution of pharmaceutical preparations to health service facilities such as pharmacies, hospital pharmacy installations, health centers, clinics and drug stores so that they can reach the people. The pharmacist as the person in charge of the pharmaceutical wholesaler must be able to carry out the management of pharmaceutical preparations at the pharmaceutical wholesaler starting from procurement, storage to distribution of pharmaceutical preparations to health care facilities."

"Praktek Kerja Profesi Apoteker (PKPA) dilaksanakan di Apotek Roxy Pondok Labu dan PT. SOHO Industri Pharmasi. Kegiatan PKPA di apotek dilaksanakan pada tanggal 1 – 30 September 2021, dengan tujuan dapat memberikan gambaran secara langsung mengenai peran apoteker di apotek, salah satunya ialah dalam hal pengadaan dan pengendalian sediaan farmasi. Tugas khusus yang berjudul “Analisis Penggunaan Obat Antihipertensi di Apotek Roxy Pondok Labu Pada Periode 1 – 31 Agustus 2021”, bertujuan untuk menganalisis penggunaan obat antihipertensi terbanyak di apotek.Kegiatan PKPA di PT. SOHO Industri Pharmasi berlangsung selama dua bulan. Melalui kegiatan ini penulis berkesempatan untuk melihat aktivitas yang ada di industri farmasi, terutama penerapan CPOB dalam industri farmasi. Tugas khusus yang diberikan berjudul “Pemetaan Suhu Pada Climatic Chamber QACLI204, QACLI221 dan QACLI222” bertujuan untuk memberikan pemahaman mengenai kegiatan kualifikasi, prinsip kualifikasi dan pemetaan suhu pada instrumen chamber. Proses kualifikasi dilakukan sesuai dengan standar operating procedure (SOP) yang berlaku di industri bersangkutan. Penulis melakukan pemetaan suhu pada climatic chamber untuk memperoleh dokumentasi distribusi suhu pada beberapa titik dalam climatic chamber.

Apothecary Profession Internsip (PKPA) was carried out at Roxy Pondok Labu Pharmacy and PT. SOHO Industri Pharmasi. PKPA activities in pharmacy is held on September 1 – 30, 2021, with the aim to provide an overview of the role of pharmacists in pharmacies, one of which is procurement and product control. This assignment entitled "Antihypertensive Drug Use Analysis at Roxy Pondok Labu Pharmacy in the Period 1 - 31 August 2021", aims to analyze the most use of antihypertensive drugs in pharmacies.

PKPA activities at PT. SOHO Industri Pharmasi lasts for two months. Through this activity, the author had the opportunity to see activities in the pharmaceutical industry, especially the implementation of GMP in the pharmaceutical industry. The assignment entitled “Mapping Temperature for Climatic Chamber QACLI204, QACLI221 and QACLI222” aims to give an understanding of qualification activities, qualification principles and mapping temperature in chamber instruments. The qualification process is carried out in accordance with the industry’s standard operating procedure (SOP). The author carried out mapping temperature in climatic chamber to obtain documentation of temperature distribution at several points in the climatic chamber."

"Apoteker dalam prakteknya didasarkan pada pedoman yang diatur dalam undang-undang. Apoteker klinis berperan dalam meningkatkan pelayanan obat, sedangkan apoteker di industri berperan dalam menjamin mutu produksi obat. Diantara penyimpangan mutu yang terjadi adalah penyimpanan obat yang tidak tepat dan kejadian di luar spesifikasi ( Out Of Spesification /OOS ) selama produksi. Karya ilmiah ini bertujuan untuk mengevaluasi penyimpanan obat di apotek dan menginvestigasi penyimpangan terhadap produk “x” BN ss6 yang memiliki nilai viskositas di luar spesifikasi. Evalusi dilakukan dengan cara pengamatan kulitatif dan kuantitatif. Investigasi dilakukan untuk mencari akar masalah dengan metode Fish Bone. Hasil evalusi menunjukan penyimpanan obat yang sesuai ketentuan sebanyak 89,4%. Hasil investigasi menunjukan bahwa faktor perubahan sumber bahan baku dapat mempengaruhi produk akhir walaupun secara skala pilot sudah memenuhi syarat.

Pharmacists in practice are based on guidelines regulated by law. Clinical pharmacists play a role in improving drug services, while pharmacists in industry play a role in ensuring the quality of drug production. Among the quality deviations that occur are inappropriate drug storage and out of specification (OOS) events during production.

This scientific work aims to evaluate drug storage in pharmacies and investigate deviations from product “x” BN ss6 which has a viscosity value outside the specification. Evaluation is done by means of qualitative and quantitative observations. An investigation was conducted to find the root of the problem using the Fish Bone method. The results of the evaluation showed that the storage of drugs in accordance with the provisions was 89.4%.

The results of the investigation show that the change in the source of raw materials can affect the final product even though on a pilot scale it has met the requirements."

"Apoteker merupakan profesi yang menjalankan pekerjaan kefarmasian sesuai dengan standar kompetensi dan praktek kefarmasian. Apoteker dalam menjalankan perannya dalam bidang kefarmasian baik di industri maupun pelayanan kefarmasian di fasilitas kesehatan dituntut untuk memiliki kompetensi yang sesuai. Oleh karena itu dilakukan praktik kerja prodesi untuk calon apoteker dengan tujuan untuk memberikan gambaran, pelatihan dan pengalaman di lingkungan pekerjaan yang akan menjadi tempat apoteker melakukan pekerjaannya. Pelaksanaan praktik kerja profesi apoteker dilakukan pada bulan September – Desember 2021 di fasilitas kefarmasian yaitu Apotek Kimia Farma 193 Utan Kayu dan Puskesmas Kecamatan Ciracas, serta di Industri farmasi yaitu PT. Finusolprima Farma Internasional. Harapan dari dilakukannya kegiatan praktik kerja profesi apoteker yaitu menjadi sarana bagi calon apoteker dalam meningkatkan dan mengembangkan keterampilan serta kompetensi yang dibutuhkan untuk persiapan dalam memasuki dunia kerja dan menjalani praktik profesi apoteker.
