"Latar Belakang : Infeksi COVID-19 telah diketahui masih dapat menyebabkan gejala sampai 90 hari dan bahkan lebih, meski infeksi akutnya telah berlalu. Hal ini disebabkan karena adanya fenomena sindroma pasca COVID-19. Mekanisme kejadian tersebut sampai saat ini masih belum diketahui pasti. Hal tersebut diduga kuat akibat adanya fibrosis di beberapa organ, terutama jantung dan paru. Sementara itu, beberapa studi telah menyebutkan bahwa sST2 merupakan penanda fibrosis jantung. Meskipun demikian, sampai saat ini belum ada penelitian yang mencoba mengetahui faktor-faktor apa saja yang memiliki hubungan dengan kejadian fibrosis pasca infeksi COVID-19. Kadar sST2 pada pasien komorbid kardiovaskular tanpa COVID-19 dan populasi orang sehat, khususnya di Indonesia juga belum diketahui.
Tujuan : Mengetahui perbandingan kadar sST2 pada pasien komorbid kardiovaskular 12 minggu pasca infeksi COVID-19 dengan pasien komorbid kardiovaskular tanpa COVID-19 dan populasi orang sehat, serta hubungannya dengan faktor-faktor admisi.
Metode : Penelitian ini merupakan studi observasional potong lintang. Kadar sST2 pada pasien 12 minggu pasca infeksi COVID-19 dibandingkan dengan komorbid kardiovaskular akan dibandingkan dengan kelompok kontrol, yaitu kontrol 1 yang merupakan pasien komorbid kardiovaskular tanpa COVID-19 dan kontrol 2 yang merupakan populasi orang sehat. Kelompok kontrol dipilih menggunakan metode matching. Hubungan faktor klinis dan laboratoris saat dengan kadar sST2 pada pasien 12 minggu pasca infeksi COVID-19 dianalisis menggunakan analisis multivariat.
Hasil : Terdapat 162 subjek yang menyelesaikan rangkaian penelitian yang terdiri atas 100 subjek dengan penyintas COVID-19 disertai komorbiditas kardiovaskular (kelompok kasus), 31 subjek dengan komorbiditas kardiovaskular tanpa COVID-19 (kelompok kontrol 1), dan 31 subjek sehat tanpa riwayat COVID-19 dan komorbiditas kardiovaskular (kelompok kontrol 2). Ketiga kelompok memiliki karakteristik yang sama. Terdapat perbedaan signifikan rerata nilai sST2 antara kelompok kasus dibandingkan kontrol 1 dan kontrol 2 (2786 ± 73 vs 2666 ± 162 pg/l, p <0.001 dan 2786 ± 73 vs 2517.15 ± 321 pg/l, p < 0.001), serta kontrol 1 dibandingkan kontrol 2 (2666 ± 162 pg/l vs 2517.15 ± 321 pg/l, p < 0.001). Analisis multivariat menunjukkan PaO2 (p < 0.001) dan nilai CT (p = 0.04) memiliki hubungan dengan kadar sST2 pada pasien 12 minggu pasca infeksi COVID-19.
Kesimpulan : Terdapat perbedaan signifikan antara kadar sST2 sebagai penanda fibrosis jantung pada ketiga kelompok subjek penelitian, dengan kadar sST2 lebih tinggi pada subjek dengan penyintas COVID-19 disertai komorbiditas kardiovaskular. Terdapat hubungan PaO2 dan nilai CT saat admisi dengan kadar sST2.

Background : Recent findings showed that symptoms associated with COVID-19 infection may persist up to 90 days even after the acute disease period has passed. This condition is now termed as post COVID-19 syndrome. Several pathophysiologic mechanisms of this event had been proposed, all of which still needed further elaboration. One of the proposed mechanisms involves fibrotic processes in several organs, especially heart and the lungs. SST2 has been suggested as a novel biomarker for cardiac fibrosis. However data are still needed to further elucidate the factors which are associated with the incidence of fibrosis post COVID-19 infection. Furthermore, data regarding sST2 levels in patients with cardiovascular comorbidities and in healthy subjects are still limited.
Objective : Knowing the differences on sST2 levels between subjects with cardiovascular comorbidities 12 weeks post COVID-19 infection, those without history of COVID-19 but with cardiovascular comorbidities, and healthy population, as well as knowing its relationship with admission factors.
Methods : This study is a cross-sectional observational study on patients 3 months after COVID-19 infection presented with cardiovascular comorbidities. Age and sex-matched control groups were used as comparison. The results were compared with a group without history of COVID-19 and healthy populations. Relationship between admission factors was assessed using multivariate analysis
Results : 162 subjects completed the study series, consisting of 100 subjects with COVID-19 survivors with cardiovascular comorbidities (case group), 31 subjects with cardiovascular comorbidities without COVID-19 (control group 1), and 31 healthy subjects without a history of COVID-19 and cardiovascular comorbidities (control group 2). All three groups had similar characteristics. There was a significant difference in the mean sST2 value between the case groups compared to control 1 and control 2 (2786 ± 73 vs 2666 ± 162 pg/l, p < 0.001 and 2786 ± 73 vs 2517.15 ± 321 pg/l, p < 0.001 respectively), and control 1 compared to control 2 (2666 ± 162 pg/l vs 2517.15 ± 321 pg/l, p < 0.001). Multivariate analysis revealed PaO2 (p < 0.001 and CT values (p = 0.04) as admission factor associated with increased sST2 3 months after initial COVID-19 infection.
Conclusion : SST2 levels were found to be significantly different between the three groups, with the highest level on the case group (subjects with history of COVID-19 and cardiovascular comorbidities). Factors upon admissions which include Arterial oxygen partial pressure (PaO2) (p < 0.001) and CT value (p = 0.04) were found to be associated with increased sST2 levels.

"Latar Belakang : Infeksi COVID-19 dewasa ini telah diketahui memiliki implikasi jangka panjang meski periode akut telah tertangani, suatu fenomena yang dinamakan long COVID syndrome atau sindrom pasca COVID-19. Patofisiologi dari kejadian ini masih belum diketahui dengan jelas. Studi melaporkan bahwa sindrom pasca COVID-19 melibatkan beberapa organ, diantaranya adalah sistem kardiovaskular. Pemeriksaan nilai LV GLS dan RV LS pada ekokardiografi dinilai akurat dalam mendeteksi disfungsi miokard dan fibrosis endomiokardial. Selain itu, hingga saat ini, data mengenai faktor-faktor saat admisi sebagai prediktor terhadap kejadian sindrom pasca COVID-19 masih terbatas.
Tujuan : Mengetahui nilai parameter ekokardiografi LV GLS dan RV LS sebagai penanda disfungsi miokard dan fibrosis jantung serta mengidentifikasi faktor-faktor saat admisi yang berpengaruh terhadap kejadian sindrom pasca COVID-19.
Metode : Penelitian ini adalah deskriptif-analisis menggunakan metode potong lintang. Pemilihan subjek dilakukan dengan metode consecutive sampling. Pemeriksaan ekokardiografi termasuk pemeriksaan global longitudinal strain (GLS) dilakukan oleh dua orang observer 4 bulan pasca perawatan rumah sakit. Selanjutnya, analisis multivariat berupa regresi linear dilakukan untuk mengetahui faktor admisi yang berpengaruh terhadap perbedaan nilai GLS pada kelompok penelitian.
Hasil : 100 subjek dengan komorbiditas kardiovaskular dan riwayat COVID-19 memenuhi kriteria dan syarat penelitian. Ditemukan nilai penurunan nilai LV-GLS pada kelompok ini. Subjek dengan komorbiditas kardiovaskular tanpa riwayat COVID-19 (n=31, kontrol 1) yang telah melalui proses matching berdasarkan usia, gender, dan faktor resiko, serta subjek sehat (n-31, kontrol 2) sebagai pembanding validitas GLS. Terdapat perbedaan signifikan rerata nilai LV GLS antar 3 kelompok (p<0.05, rerata ±SB -16.17 ± 3.379, -19.48 ± 1.141, -21.48 ± 1.777 berturut-turut untuk kelompok kasus, kontrol 1, kontrol 2), dengan nilai paling rendah pada kelompok kasus. Faktor saat admisi yaitu status CAD memiliki hubungan yang signifikan (p 0.038) dengan penurunan LV GLS pada pasien post covid-19 dengan komorbid kardiovaskular.
Kesimpulan : Terdapat penurunan nilai LV GLS yang signifikan pada sindrom pasca COVID-19 disertai komorbiditas kardiovaskular. CAD merupakan prediktor penurunan fungsi maupun fibrosis jantung sebagai manifestasi sindrom pasca COVID-19.

Background : Recently, COVID-19 infection has been known to have a longer implication, even after the initial acute phase has been managed, a phenomenon termed as long COVID syndrome or “sindroma pasca COVID-19”. The exact pathophysiological mechanism of this event is still unknown. Previous studies reported that long COVID syndrome involves multiple organs, one of which is the cardiovascular system. Measurement of echocardiography LV GLS and RV LS values are reported to be accurate to detect myocardial dysfunction and endomyocardial fibrosis. Moreover, up until now, data regarding admission factors as predictors for long COVID syndrome incidences are still limited.
Objective : Assessing echocardiography LV GLS and RV LS values as a marker for myocardial dysfunction and heart fibrosis and identifying admission factors which may predict the incidence of long COVID syndrome
Methods : This is an observational study with a cross-sectional using a consecutive sampling method. Echocardiography including global longitudinal strain (GLS) measurement was done by two examiners 3 months after initial hospitalization. Multivariate analysis linear regression was subsequently used to investigate admission factors which are associated with differences in GLS measurement.
Results : Total of 100 subjects with cardiovascular comorbidities and prior COVID-19 infection were enrolled. Echocardiography examination showed lower GLS values in this group compared to the normal population. Age, sex and risk factors-matched subjects with cardiovascular comorbidity without a history of COVID-19 (n=31, Control 1) and healthy subjects (n-31, Control 2) were subsequently used as comparisons to validate GLS results. There were significant differences in LV-GLS levels between the three groups, with the lowest values measured in the case group (p<0.05, mean ±SD -16.17 ± 3.379, -19.48 ± 1.141, -21.48 ± 1.777 respectively for case, control 1, and control 2 groups). A history of coronary artery disease upon admission was found to be associated with decreased LV GLS values in recovered COVID-19 patients with cardiovascular comorbidity.
Conclusion : LV GLS values significantly decrease in long COVID syndrome with cardiovascular comorbidities. Having a previous history of CAD upon admission may serve as predictors of deteriorated functions or heart fibrosis as manifestations of long COVID syndrome.

"Latar Belakang: COVID-19 di Indonesia menyebabkan kematian hingga lebih dari 150.000 orang. Salah satu populasi yang mengalami dampak dengan risiko kematian yang tinggi adalah populasi penyakit kardiovaskular. Severitas COVID-19 sering dikaitkan dengan rendahnya rasio PaO2/FiO2 dan tingginya kadar D-dimer. COVID-19 varian Omicron diketahui memiliki angka penyebaran yang lebih tinggi dengan severitas infeksi yang lebih rendah dibandingkan varian sebelumnya. Namun dampak jangka panjang pada pasien COVID-19 varian Omicron, khususnya pada populasi pasien dengan penyakit kardiovaskular masih menjadi pertanyaan. Penelitian ini ingin mengetahui dampak pasca COVID-19 varian Omicron dengan melihat kadar ST2 terlarut dan adanya gangguan paru yang dinilai dengan pemeriksaan spirometri.
Tujuan: Penelitian ini dilakukan untuk mengetahui hubungan Rasio PaO2/FiO2 dan Kadar D-dimer pada saat admisi terhadap kadar ST2 terlarut dan gambaran spirometri pada pasien pasca COVID-19 varian Omicron dengan penyakit kardiovaskular. Metode: Penelitian berupa studi potong lintang terhadap pasien COVID-19 varian Omicron dengan riwayat komorbid penyakit kardiovaskular yang dirawat di Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita. Diagnosis COVID-19 varian Omicron dilakukan dengan menggunakan metode WGS/SGTF. Pasien dengan kriteria inklusi menjalani pemeriksaan spirometri dan pengukuran kadar ST2 terlarut pada 6 bulan pasca perawatan.
Hasil dan Pembahasan: Penelitian ini menunjukkan rasio PaO2/FiO2 dengan median 454 dan kadar D-dimer 790ng/mL. Mayoritas pasien menunjukkan gambaran gangguan resktriktif. Kadar ST2 terlarut pasca perawatan memiliki median 2716,8pg/mL. Tidak ditemukan adanya hubungan yang signifikan antara rasio PaO2/FiO2 dan kadar D-Dimer terhadap kadar ST2 terlarut maupun gambaran spirometri pada 6 bulan pasca COVID-19. Hal ini dapat dikaitkan dengan severitas COVID-19 yang lebih rendah sehingga tidak terdapat hubungan bermakna terhadap parameter admisi serta hubungan pengukuran 6 bulan pasca COVID-19 dengan kemungkinan adanya perbaikan fibrosis.
Kesimpulan: Tidak ada hubungan yang signifikan antara rasio PaO2/FiO2 dan kadar D- Dimer terhadap kadar ST2 terlarut ataupun gambaran spirometri pada 6 bulan pasca COVID-19 varian Omicron.

Introduction: COVID-19 in Indonesia has caused more than 150,000 deaths. One of the affected populations with a high risk of death is the cardiovascular disease population. The severity of COVID-19 is associated with a low of PaO2/FiO2 ratio and the increased levels of D-dimer. Omicron variant is known to have higher transmission with less severe infection than the previous variant. However, research related to long term effect post COVID-19 with Omicron variant in cardiovascular population is not yet known.
Aim: This study was conducted to determine the relationship of PaO2/FiO2 ratio and D- dimer levels at admission to sST2 levels and spirometry profile in post COVID-19 variant Omicron patient with cardiovascular disease.
Method: Research in the form of a cross-sectional study was conducted on Omicron variant COVID-19 patients with a history of comorbid cardiovascular disease who were treated at the Harapan Kita Heart and Blood Vessel Hospital (RSJPDHK). The diagnosis of COVID-19 is carried out using the WGS/SGTF method. Patients undergo spirometry examination and measurement of sST2 levels at 6 month after hospitalization.
Results and Discussion: This study shows a PaO2/FiO2 ratio with a median of 454 with D-dimer levels 790 ng/mL. The majority of patients have a restrictive patterns. The median sST2 value in Omicron variant COVID-19 patients at 2716.8 pg/mL. There was no significant relationship between the ratio of PaO2/FiO2 and D-Dimer levels to sST2 levels and spirometry profile at 6 months after COVID-19 infection. This can be associated with lower COVID-19 severity so that there is no significant association with inflammatory parameters such as PaO2/FiO2 ratio and D-dimer levels, as well as the relationship between measurements 6 months post COVID-19 and the possibility of fibrosis improvement.
Conclusion: There was no significant relationship between the ratio of PaO2/FiO2 and D-Dimer levels to sST2 levels and spirometry abnormality at 6 months post COVID-19 variant Omicron.

"Latar Belakang: Pada pasien dengan penyakit jantung, gangguan pada fungsi paru dapat memperburuk kondisi jantung dengan menyebabkan terjadinya dekompensasi. Infeksi virus SARS-CoV-2 berpotensi mengakibatkan terjadinya fibrosis paru dan gangguan fungsi paru lainnya yang dapat dinilai dengan pemeriksaan spirometri dan kadar ST2 terlarut dalam darah. Infeksi COVID-19 varian Omicron mengakibatkan luaran yang relatif lebih baik dibandingkan varian Delta namun jumlah kasusnya lebih banyak dan tetap dapat mengakibatkan komplikasi tersebut. Oleh sebab itu, penelitian ini akan membandingkan kedua parameter tersebut pada pasien pasca infeksi COVID-19 varian Omicron dan Delta.
Tujuan: Mengetahui hubungan kadar ST2 terlarut dan luaran spirometri pada pasien pasca COVID-19 varian Omicron dibandingkan varian Delta pada pasien dengan penyakit kardiovaskular serta perbedaan pada kedua populasi.
Metode: Studi observasional potong lintang pada 76 pasien dengan penyakit jantung yang terinfeksi COVID-19 varian Omicron dan Delta dilakukan pemeriksaan spirometri dan kadar ST terlarut 12 minggu pasca konfirmasi negatif. Analisis statistik dilakukan untuk mengetahui hubungan antara kadar ST2 terlarut pada varian Omicron dibandingkan dengan Delta terhadap gangguan fungsi paru menggunakan spirometri dan perbedaan luaran kedua varian.
Hasil: Dari 76 subjek penelitian, proporsi pasien dengan varian Omicron lebih banyak dibandingkan Delta (53 orang berbanding 23 orang), dengan perbedaan pada proporsi pasien dengan gagal jantung, penyakit jantung koroner, penerima vaksin dan distres pernafasan akut pada admisi. Kadar ST2 terlarut (p=0.026, OR 1.01 (95% CI 1.00-1.01)) dan kondisi gagal jantung (p=0.019, OR 6.07 (95% CI 1.34-27.47)) memiliki hubungan terhadap kejadian luaran gambaran spirometri abnormal khususnya gambaran restriksi terutama pada varian Omicron, namun hubungan ini lemah dan kemaknaan klinis tidak signifikan.
Hasil studi ini mengindikasikan bahwa kadar ST2 terlarut, meskipun lemah, mungkin memiliki asosiasi terhadap kelainan spirometri pada penyintas COVID-19, namun temuan ini terbatasi oleh perbedaan pada masing-masing populasi dari studi ini.

Background: In patients with heart disease, impaired lung function can exacerbate cardiac conditions due to decompensation. Infection by the SARS-CoV-2 virus has the potential to cause pulmonary fibrosis and other lung impairment which could be assessed by spirometry and measurement of soluble ST2 levels. Omicron infection resulted in a relatively better outcome than the Delta variant but higher number of cases and it could still cause these complications. Therefore, this study aims to compare these two parameters in survivors of COVID-19 infected by Omicron and Delta variant.
Objective: To determine the relationship between soluble ST2 levels and spirometry outcomes in patients with cardiovascular disease after COVID-19 infection by Omicron variant as compared to the Delta variant and the differences in the two populations.
Methods: A cross-sectional observational study on 76 patients with heart disease who were infected by Omicron and Delta variant of COVID-19 underwent spirometry and blood sampling for soluble ST levels at the Outpatient clinic in 12 weeks after confirmed negative. Statistical analysis will be performed to find out the association between soluble ST2 levels in the Omicron variant compared to Delta and impairment of lung function using spirometry and the difference in the outcomes of the two variants.

 

Results: From 76 study subjects, the proportion of survivor of Omicron variant was higher than Delta (53 versus 23), with differences in the proportion of patients with heart failure, coronary artery disease, recipients of vaccine and acute respiratory distress syndrome on admission. Soluble ST2 levels (p=0.026, OR 1.01 (95% CI 1.00-1.01)) and heart failure (p=0.019, OR 6.07 (95% CI 1.34-27.47)) have a significant association to abnormal spirometry pattern, especially the restrictive pattern in the Omicron variant, however this relationship may be weak and the clinical implication might be insignificant.

Conclusion: The results of this study indicate that soluble ST2 levels, although weak, may have an association with spirometry abnormalities in COVID-19 survivors, but these findings may be limited by differences in each population and study design.

"ABSTRAK
Latar belakang: disglikemia adalah keadaan intoleransi glukosa berupa peningkatan kadar gula darah yang berhubungan dengan risiko penyakit kardiovaskular. Seiring dengan waktu, pada akhirnya diabetes akan menimbulkan kerusakan pada target organ, salah satu yang penting adalah pada sistem organ kardiovaskular, dapat berupa penyakit jantung koroner, kardiomiopati diabetes, penyakit serebrovaskuler, dan penyakit arteri perifer. Diabetes juga meningkatkan risiko terjadinya gagal jantung. Pada kardiomiopati diabetes, proses fibrosis yang masih reversibel sudah mulai terjadi bahkan ketika penderita masih asimtomatik. Pemeriksaan baku emas untuk mendeteksi terjadinya fibrosis miokard secara dini adalah pemeriksaan histopatologi jaringan miokardium melalui biopsi. Akan tetapi pemeriksaan ini sangat invasif dan tidak nyaman bagi subjek. Pemeriksaan yang kemudian berkembang adalah pencitraan menggunakan Cardiac Magnetic Resonance Imaging (CMRI). Akan tetapi pemeriksaan ini cukup mahal, dan tidak tersedia pada semua fasilitas kesehatan. Sementara itu, ST2 adalah penanda enzim jantung yang menggambarkan derajat proses fibrosis yang sedang terjadi pada miokard, terutama pada keadaan gagal jantung. Pemeriksaan menggunakan penanda enzim dapat menjadi alternatif dengan keuntungan lebih murah, dapat terjangkau luas dan mudah tersedia. Tujuan: Mengetahui hubungan antara kadar ST2 serum dengan fibrosis miokard interstisial pada penderita disglikemia. Metode: Pasien disglikemia yang lolos kriteria eksklusi berupa komorbid kardiovaskular akan menjalani pemeriksaan kadar ST2 serum dan T1 relaxation time menggunakan Cardiac MRI. Selanjutnya dilakukan analisis hubungan antara kadar ST2 serum dan T1 relaxation time. Hasil penelitian: Sebanyak 34 pasien diikutsertakan ke dalam penelitian ini. Didapatkan kisaran nilai kadar ST2 serum antara 12.40-53.22 ng/dL (median 19.95 ng/dL). Rerata nilai T1 relaxation time didapatkan sebesar 443.39 ± 113.35 ms. Terdapat korelasi bermakna antara kadar ST2 serum dengan fibrosis diffuse miokardium (Spearman correlation r = -0,547, p < 0.01). Pada analisa multivariat hubungan antara kadar ST2 serum dan T1 relaxation time tidak dipengaruhi oleh faktor perancu yang telah ditetapkan (r = -0,44, p = 0,033). Kesimpulan: Hasil penelitian ini menunjukkan kadar ST2 serum berkolerasi dengan fibrosis diffuse miokardium pada populasi disglikemia.ABSTRACT
Background: disglycemia is a state of glucose intolerance include increased blood sugar levels associated with risk of cardiovascular disease. Over time, eventually diabetes will cause damage to the target organ, especially the cardiovascular system, which include coronary heart disease, diabetic cardiomyopathy, diabetes, cerebrovascular disease, and peripheral arterial disease. Diabetes also increases the risk of heart failure. The clinical appearance of the disease is wide ranging from asymptomatic to symptomatic heart failure. Gold standard examination to detect the occurrence of early myocardial fibrosis is histopathological examination of myocardial tissue via biopsy. However, these tests are very invasive and uncomfortable for the subject. Examination which later evolved is imaging using cardiac magnetic resonance imaging (CMRI). However, these tests are quite expensive, and not available at all health facilities. Meanwhile, ST2 is a cardiac enzyme marker that describes the degree of fibrosis process in the myocardium, especially in the state of heart failure. Examination using enzyme markers can be a cheaper alternative, widely accessible and readily available. Aim: Knowing the relationship between serum levels of ST2 with myocardial interstitial fibrosis in disglycemic patients. Methods: Disglycemic patients who passed from the exclusion criteria (cardiovascular comorbid), will undergo a serum ST2 levels and T1 relaxation time using cardiac MRI. Then we analyzed the relationship between serum levels of ST2 and T1 relaxation time. Results: A total of 34 patients were included in this study. The range values of serum ST2 levels were between 12.40-53.22 ng/dL (median 19.95 ng/dL). The mean value of T1 relaxation time were 443.39 ± 113.35 ms. There is a significant correlation between serum levels of ST2 with diffuse myocardial fibrosis (Spearman correlation r = -0.547, p <0:01). Multivariate analysis showed the relationship between serum levels of ST2 and T1 relaxation time is not influenced by confounding factors (r = -0.44, p = 0.033). Conclusion: ST2 serum levels correlates with diffuse myocardial fibrosis on disglycemic population.;Background: disglycemia is a state of glucose intolerance include increased blood sugar levels associated with risk of cardiovascular disease. Over time, eventually diabetes will cause damage to the target organ, especially the cardiovascular system, which include coronary heart disease, diabetic cardiomyopathy, diabetes, cerebrovascular disease, and peripheral arterial disease. Diabetes also increases the risk of heart failure. The clinical appearance of the disease is wide ranging from asymptomatic to symptomatic heart failure. Gold standard examination to detect the occurrence of early myocardial fibrosis is histopathological examination of myocardial tissue via biopsy. However, these tests are very invasive and uncomfortable for the subject. Examination which later evolved is imaging using cardiac magnetic resonance imaging (CMRI). However, these tests are quite expensive, and not available at all health facilities. Meanwhile, ST2 is a cardiac enzyme marker that describes the degree of fibrosis process in the myocardium, especially in the state of heart failure. Examination using enzyme markers can be a cheaper alternative, widely accessible and readily available. Aim: Knowing the relationship between serum levels of ST2 with myocardial interstitial fibrosis in disglycemic patients. Methods: Disglycemic patients who passed from the exclusion criteria (cardiovascular comorbid), will undergo a serum ST2 levels and T1 relaxation time using cardiac MRI. "Latar Belakang: Telah diketahui infeksi COVID-19 dapat menetap menjadi sindroma pasca COVID-19. Magnetic Resonance Imaging (MRI) kardiak memiliki nilai diagnostik tinggi untuk menilai karakteristik jaringan miokard. Belum diketahui secara pasti efek jangka panjang COVID-19 terhadap jaringan miokardium serta faktor-faktor admisi yang memiliki pengaruh terhadap prevalensi sindroma pasca-COVID-19 pada populasi dengan penyakit kardiovaskular. 
Tujuan: Mengevaluasi prevalensi fibrosis miokardium dengan MRI kardiak pada pasien dengan penyakit kardiovaskular dan riwayat COVID-19 1 tahun pascaperawatan, serta mengidentifikasi faktor admisi yang berpengaruh terhadap fibrosis miokardium. 
Metode: Kohort prospektif dengan menilai parameter MRI kardiak pada pasien terkonfirmasi COVID-19 dengan penyakit kardiovaskular 1 tahun pascaperawatan. Selanjutnya, dilakukan analisa temuan MRI kardiak terhadap kelompok kontrol tanpa riwayat COVID-19 yang telah di-matching berdasarkan umur, jenis kelamin, dan faktor resiko. Analisa multivariat dilakukan untuk mengetahui faktor admisi yang memiliki hubungan dengan kejadian fibrosis miokardium. 
Hasil: Total 32 subjek dengan penyakit KV dan riwayat COVID-19 1 tahun pascaperawatan dengan 49 subjek kontrol disertakan dalam studi ini. Terdapat peningkatan yang signifikan pada parameter MRI kardiak yaitu proporsi abnormal T1 relaxation time (65,5% vs 36,7%; p-value=0,011) serta late gadolinum enhancement (LGE) skar noniskemik 62,5% vs 29,8%;p-value=<0,001) pada kelompok dengan riwayat COVID-19 dibanding kontrol. Tidak ditemukan faktor admisi yang berpengaruh terhadap peningkatan LGE noniskemik/T1 abnormal.
Kesimpulan: Terdapat peningkatan prevalensi fibrosis miokardium pada pasien dengan penyakit KV dan riwayat COVID-19 1 tahun pascaperawatan dibanding kontrol dinilai melalui MRI kardiak. Tidak terdapat hubungan antara faktor admisi dengan fibrosis miokardium 1 tahun pascaperawatan COVID-19.

Background: Presently, it has been acknowledged that COVID-19 infection may persist longer as Post-acute COVID-19 Syndrome. Cardiac Magnetic Resonance Imaging (cMRI) possesses a high diagnostic value to evaluate myocardial tissue characteristics. Data are still limited regarding longer implications of COVID-19 infection towards myocardial tissues and predictive admission factors in patients with Cardiovascular Diseases (CVD).
Aim(s): To evaluate the prevalences of myocardial fibrosis using cMRI in patients with CVD and one year post-COVID-19 hospitalization and identifying admission factors in correlation with myocardial fibrosis. 
Method(s): Prospective cohort to assess cMRI parameters in patients with CVD and history of COVID-19 one year post-hospitalization. The results were then compared with the age-, sex-, risk factors-matched control group without prior of COVID-19 infection. Lastly, multivariat analysis was done to identify relations between admission factors and myocardial fibrosis. 
Result(s): A total of 32 subjects with CVD one year post-COVID-19 hospitalization and 49 controls were included in this study. Significant increases of cMRI parameters, namely abnormal T1 relaxation time (65.5% vs 36.7%; p-value=0.011) and non-ischemic late gadolinum enhancement (LGE) (62.5% vs 29.8%;p-value=<0.001) were observed in the population with prior COVID-19 infection compared to control. No admission factors were found to be related with the increases in nonischemic LGE/abnormal T1.
Conclusion: There is a significant increase of myocardial fibrosis prevalence in patients with CVD one year post-COVID-19 hospitalization compared to control assessed through cMRI parameters. No relationships were found between admission factors and myocardial fibrosis.

Tuberkulosis masih menjadi epidemi global dengan tingkat morbiditas dan mortalitas yang signifikan. Meskipun keberhasilan pengobatan tuberkulosis telah meningkat, banyak pasien yang sembuh mengalami sequelae post-tuberkulosis, termasuk fibrosis paru, yang menyebabkan disabilitas dan menurunkan kualitas hidup. Sequelae ini berkontribusi besar terhadap beban kesehatan, dengan fibrosis menjadi komponen utama dalam perubahan jaringan paru post-tuberkulosis. Penelitian lebih lanjut diperlukan untuk memahami faktor-faktor yang mempengaruhi progresi fibrosis, termasuk peran status gizi. Metode
Metode penelitian adalah retrospektif dengan data sekunder berupa rekam medis, diambil pada bulan Januari 2024 sampai bulan Agustus 2024 di RSUP Persahabatan. Sampel berjumlah 62 subjek yang telah menyelesaikan pengobatan TBC paru di RSUP Persahabatan. Data yang diambil meliputi status gizi pasien, derajat keparahan fibrosis paru berdasarkan hasil radiologi, dan pola spirometri pasca infeksi tuberkulosis.
Hasil
Hasil penelitian menunjukkan usia rata-rata pasien dengan fibrosis minimal-ringan, sedang, dan berat masing-masing adalah 39,54 ± 15,23 tahun, 47,27 ± 20,09 tahun, dan 50,90 ± 12,95 tahun, dengan korelasi positif lemah antara usia dan keparahan fibrosis paru (r = 0,284, p = 0,025). Terdapat peningkatan signifikan dalam IMT sebelum dan sesudah pengobatan (p < 0,001), dengan kelompok minimal-ringan dan sedang memiliki IMT yang lebih tinggi dibandingkan kelompok berat. Hanya 18% subjek memiliki data spirometri, di mana semua pasien dengan fibrosis derajat sedang menunjukkan pola restriksi, sedangkan pasien dengan fibrosis minimal-ringan memiliki spirometri normal.
Kesimpulan
Penelitian ini menunjukkan bahwa terdapat peningkatan signifikan dalam Indeks Massa Tubuh (IMT) pada pasien tuberkulosis yang menjalani terapi Obat Anti-Tuberkulosis (OAT), mengindikasikan perbaikan status gizi selama pengobatan. Meskipun demikian, tidak ada hubungan yang signifikan antara peningkatan status gizi dan derajat keparahan fibrosis paru. Hasil spirometri terbatas menunjukkan bahwa subjek dengan fibrosis minimal-ringan cenderung memiliki fungsi paru yang lebih baik dibandingkan dengan subjek dengan fibrosis sedang.

Tuberculosis remains a global health issue with high morbidity and mortality rates. Despite successful treatment, many recovered patients still experience sequelae such as pulmonary fibrosis, which can lead to disability and reduced quality of life. This study aims to evaluate the relationship between nutritional status and the severity of pulmonary fibrosis in patients post-tuberculosis infection. The method used was retrospective with secondary data from medical records of 62 patients who had completed tuberculosis treatment at Persahabatan General Hospital between January and August 2024. Results showed a significant increase in Body Mass Index (BMI) before and after treatment (p < 0.001), indicating an improvement in nutritional status. However, no significant association was found between improved nutritional status and the severity of pulmonary fibrosis. Limited spirometry data showed that patients with minimal-mild fibrosis tended to have better lung function compared to patients with moderate fibrosis. This study highlights the importance of monitoring nutritional status in tuberculosis patients, although its impact on the severity of pulmonary fibrosis requires further investigation."

"Latar belakang: Matrix metalloproteinases (MMPs) merupakan protein yang berperan dalam proses inflamasi dan remodeling yang disebabkan oleh infeksi, termasuk tuberkulosis paru (TB), terutama multidrug resistance. Penelitian ini bertujuan untuk mengkorelasikan hubungan antara kadar serum dan polimorfisme MMP-1 dan MMP-9 dengan karakteristik kavitas, seperti jumlah, diameter, ketebalan dinding, dan distribusi fibrosis pada Multidrug-Resistant (MDR) dan Drug-Sensitive (DS) pasien TB.
Metode: Penelitian ini menggunakan desain studi potong lintang komparatif. Subyek yang berasal dari pasien rawat jalan RS Abdoel Moelok Lampung Indonesia telah lulus uji etik. Subjek dibagi menjadi dua kelompok, 34 subjek pada kelompok MDR-TB dan 36 subjek pada kelompok DS-TB. Kadar protein serum MMP-1 dan MMP-9 dilakuakn dengan uji ELISA, dan genotipe MMP-1 dan MMP-9 dengan Sequencing metode Sanger. Kemudian kavitas dan fibrosis dievaluasi dengan menggunakan pemeriksaan High-Resolution Computerized Tomography (HRCT) toraks.
Hasil: Terdapat perbedaan bermakna jumlah kavitas dengan diameter lebih dari 6, 6 mm, dan tebal kavitas pada pasien TB-MDR dibandingkan dengan pasien TB-DS. Distribusi fibrosis pada segmen paru juga berbeda nyata pada MDR-TB dibandingkan dengan DS-TB. Walaupun kadar MMP-9 pada kelompok MDR-TB lebih tinggi dibandingkan dengan kelompok DS-TB, namun secara statistik tidak terdapat perbedaan yang signifikan dari penelitian yang menunjukkan bahwa terdapat hubungan antara MDR-TB dan DS-TB mengenai jumlah kavitas, diameter kavitas, ketebalan dinding kavitas, serta distribusi fibrosis di segmen paru-paru yang terkena yang dievaluasi dengan HRCT. Penelitian ini mendapatkan frekuensi alel G pada MMP-1 pada populasi Indonesia (Asia) dan adanya hubungan yang signifikan dengan tebal kavitas dengan alel G pada MMP-1 dan alel T pada MMP-9 alel
Kesimpulan: Tidak terdapat hubungan antara genotipe MMP-1 (-1607G) dan MMP-9 (C1562T) dengan kadar serum MMP-1 dan MMP-9, genotipe MMP 1 pada kedua kelompok penelitian berbeda secara bermakna dan merupakan faktor pencegahan dua kali lipat kejadian MDR-TB. Selain itu, terdapat perbedaan yang substansial dalam ketebalan dinding kavitas antara genotipe G/G MMP-1 1607 T/T MMP-9 pada kedua kelompok penelitian.

Background: Matrix metalloproteinases (MMPs) are proteins that play a role in the inflammatory and remodeling processes caused by infections, including pulmonary tuberculosis (TB), especially multidrug resistance. This study aims to correlate the relationship between serum levels and polymorphism of MMP-1 and MMP-9 with cavity characteristics, such as number, diameter, wall thickness, and distribution of fibrosis in Multidrug-Resistant (MDR)- and Drug-Sensitive (DS)-TB patients.
Method: This study used a comparative cross-sectional study design. The subjects came from outpatients at Abdoel Moelok Hospital, Lampung Indonesia had passed the ethical test. Subjects were divided into two groups, 34 subjects in the MDR-TB group and 36 subjects in the DS-TB group. The levels of MMP-1 and MMP-9 were carried out by ELISA test, and the genotipes MMP-1 and MMP-9 were determined using PCR-the Sequencing method. In addition, cavities and fibrosis were measured using thoracic High- Resolution Computerized Tomography (HRCT) imaging.
Results: There was a significant difference in the number of cavities with a diameter of more than 6.6 mm, and cavity thickness in MDR-TB patients compared to DS-TB patients. The distribution of fibrosis in the lung segments was also significantly different in MDR-TB compared to DS-TB. Although MMP-9 levels in the MDR-TB group were higher than in the DS-TB group, there was no statistically significant difference from the study, which showed a relationship between MDR-TB and DS-TB regarding the number of cavities, cavity diameter, walls thickness cavity, as well as the distribution of fibrosis in the affected lung segments evaluated by HRCT. This study found the frequency of the G allele in MMP-1 in the Indonesian population (Asia) and a significant relationship with cavity thickness between the G allele in MMP-1 and the T allele in MMP-9.
Conclusion: There is no relationship between the MMP-1 (-1607G) and MMP-9 (C1562T) genotypes with serum levels of MMP-1 and MMP-9, the MMP 1 genotype in the two study groups was significantly different and was a factor preventing twice the incidence MDR-TB. In addition, the two study groups showed substantial differences in cavity wall thickness between the G/G MMP-1 1607 T/T MMP-9 genotypes.

"Latar belakang: Atresia bilier merupakan penyebab paling umum fibrosis hati pada anak, dan menjadi indikasi terbanyak transplantasi hati. Fibrosis hati dapat dinilai dengan pemeriksaan histopatologis dan kuantifikasi skor Laennec. Pemeriksaan biomarker dari darah dan metode pencitraan radiologis merupakan upaya lain untuk menilai derajat fibrosis hati. Penelitian ini dimaksudkan untuk meneliti perbandingan biomarker penanda fibrosis transforming growth factor (TFG-β), matrix metalloproteinase (MMP)-7, dan ultrasonografi (USG) Acoustic Radiation Force Impulse (ARFI) dengan derajat fibrosis berdasarkan pemeriksaan histopatologi. Metode penelitian: Penelitian ini merupakan penelitian desain cross sectional pada pasien anak dengan kolestasis yang dicurigai akibat atresia bilier di ruang rawat anak RSUPN Cipto Mangunkusumo. Data penelitian ini adalah data primer dari anamnesis, pemeriksaan fisik, serta data sekunder dari rekam medis untuk melihat riwayat penyakit pasien. Dilakukan pemeriksan serum biomarker TGF-β dan MMP-7, dan USG ARFI oleh operator yang telah ditentukan. Dibandingkan hasil pemeriksaan TGF-β, MMP-7, USG ARFI dengan hasil skoring fibrosis biopsi hati.
Hasil penelitian: Dari total 15 pasien dengan AB, terdapat 6 pasien F2-F3, dan 9 pasien F4 berdasarkan derajat fibrosis biopsi hati. Terdapat peningkatan kadar TGF-β dengan adanya peningkatan derajat fibrosis biopsi hati, namun tidak terdapat hubungan bermakna antara derajat fibrosis hati dengan kadar TGF-β (uji Mann-Whitney, p=0.768). Pada penelitian ini, rerata kadar MMP-7 pada kelompok F2-3 dan F4 menunjukan peningkatan, namun tidak terdapat hubungan yang bermakna (Uji Mann-Whitney, p=0.409). Terdapat hubungan bermakna antara derajat fibrosis hati F2-F3 dengan derajat F4 yang diperoleh berdasarkan hasil USG ARFI dan hasil biopsi hati (p<0,01). Kesimpulan: TGF-β dan MMP-7 merupakan biomarker yang cukup efektif namun memiliki keterbatasan tidak spesifik untuk AB. USG ARFI merupakan pemeriksaan minimal invasive yang efektif untuk menentukan derajat fibrosis.

Background: Biliary atresia is the most common cause of liver fibrosis and the highest indication for liver transplantation in pediatrics. Liver fibrosis is quantified through Laennec score based on histopathology of liver biopsy. Blood serum biomarkers and radiological examinations are alternative methods that could determine liver fibrosis. This study aimed to compare the significance of serum biomarkers transforming growth factor (TFG-β), matrix metalloproteinase (MMP)-7, and Acoustic Radiation Force Impulse (ARFI) ultrasonography (USG) in determining the degree of liver fibrosis compared to histopathology score from liver biopsy. Method: This was a cross-sectional study, on patients in the pediatric ward at dr. Cipto Mangunkusumo National Hospital, that were admitted with cholestasis with biliary atresia as the suspected etiology. Primary data was obtained through anamnesis, and physical examination, and secondary data was obtained through patients’ medical records. Serum biomarkers TGF-β and MMP-7, and USG ARFI were examined by related experts and results were obtained through medical records. Results of TGF-β, MMP-7, and USG ARFI were compared with fibrosis scores based on liver biopsy.
Result: From a total of 15 patients with AB, there were 6 F2-F3 patients, and 9 F4 patients according to the biopsy results. TGF-β levels showed and increasing trend alongside increase in liver biopsy fibrosis score, however it was not statistically significant (Mann- Whitney test, p=0.768). In this study, there was an increase in MMP-7 levels in F2-3 group compared to f4 group, however there was no statically significant difference (Mann-Whitney test, p=0.409). The ARFI USG results showed significant difference between F2-F3 group and F4 group based on ARFI USG compared to liver biopsy (p<0.01). Conclusion: TGF-β and MMP-7 are effective serum biomarkers, however, lacked specificity to determine fibrosis levels in biliary atresia. A minimally invasive test that is effective in determining the degree of fibrosis can be done through ARFI USG.

"Pulmonary fibrosis due to COVID-19 is recognized as sequel of ARDS characterized by failed alveolar re-epithelization, fibroblast activation, excessive collagen deposition and other extracellular matrix components that disrupt the normal lung architecture. There are risk factor for pulmonary fibrosis namely advanced age, severe ARDS infection, mechanical ventilation due to ventilator-induced lung injury, smoking and chronic alcoholism. Diagnosis of post-COVID pulmonary fibrosis can be made by clinical symptoms and characteristic finding from lung CT scan. To date, there is no definitive treatment for post-inflammatory pulmonary fibrosis after COVID-19 infection, however some of antifibrotic therapies may be considered. Beside medical treatment, pulmonary rehabilitation program and long-term oxygen treatment should be included as part of comprehensive treatment for pulmonary fibrosis due to COVID"
