"Studi penentuan genotip (pulsotip) terhadap isolat-isolat Salmonella typhi (S. typhi) telah dilakukan menggunakan elektroforesis medan listrik berpulsasi (PFGE = Pulse-Field Gel Electrophoresis). Penelitian ini bertujuan untuk mempelajari diversitas genetik dan hubungan antara karakter genetik dengan manifestasi kliniknya. Sebanyak 66 isolat S. typhi yang berasal dari kasus demam tifoid yang dirawat di rumah sakit telah dianalisis. Empat isolat ditemukan identik dan hasil konstruksi dendogram menunjukkan terdapatnya 33 pulsotip dimana 13 di antaranya dapat dipisahkan dalam 30 subtip. Keragaman genetik di antara mereka relatif tinggi yang ditunjukkan dengan koefisien Dice 0,486-1,000. Pada derajat similaritas 65%, analisis sidik gerombol menunjukkan adanya 2 sidik gerombol utama, sehingga timbul dugaan bahwa S. typhi yang beredar bukan berasal dari klon tunggal. Pada derajat similaritas 90%, dari 9 sidik gerombol yang beranggotakan > 3 isolat, didapatkan manifestasi klinik yang sangat bervariasi dari ringan sampai berat tersebar diantara 9 sidik gerombol tersebut. Walaupun data rekam medis yang didapat kurang lengkap, 2 dari 4 pasien demam tifoid dengan S. typhi yang berasal dari sidik gerombol 1 memperlihatkan kenaikan total bilirubin yang tidak ditemukan pada 19 pasien yang berasal dari 8 sidik gerombol yang lain. Dengan adanya temuan ini, diduga adanya kemungkinan suatu trofisme pada system hepatobilier dari kuman S. typhi pulsotip I1dan I2 yang berasal dari sidik gerombol 1. (Med J Indones 2003; 12: 13-20)

A study of genotyping (pulsotyping) of Salmonella typhi (S. typhi) isolates using pulse-field gel electrophoresis (PFGE) methods was performed to examine their genetic diversity, and relationship between genetic characteristics and clinical outcomes. Sixty-six S. typhi isolates obtained from sporadic hospitalized typhoid fever cases were used in this study. Four isolates were found identical and the dendogram constructed showed 33 pulsotypes in which 13 of them can be divided into 30 subtypes. Diversity among them were high as shown by the Dice coefficients that ranged from 0.486 to 1.000. Cluster analysis showed 2 main clusters with 65% degree of similarity, suggested that they were not originated from one clone. Further, at 90% degree of similarity, 9 clusters containing at least 3 isolates were determined to explore any possible existence of relationship between genetic profile and particular clinical outcomes. Clinical manifestations ranged from mild to severe were in fact distributed diversely among these clusters. Although the clinical data obtained were incomplete, 2 out of 4 patients infected by the S. typhi belonged to cluster 1 showed an elevation of total bilirubin, whereas it was not found in 19 other patients distributed in other 8 clusters. Even though specific clinical manifestations were apparently not found to relate with particular clusters of genotypes, S. typhi isolates grouped in cluster 1 seemed to show trophism to hepatobiliary system. "Indonesia memiliki insiden demam tifoid yang cukup tinggi khususnya kelompok usia anak. Namun terdapat keterbatasan penelitian yang mempelajari distribusi demam tifoid pada anak, sehingga perlu dilakukan penelitian karakteristik pasien demam tifoid yang terbukti dengan temuan isolat S.typhi, penanganannya, pola resistensi antibiotik dan serotipe S.typhi yang bersirkulasi di Indonesia.
Tujuan: Mengetahui karakteristik pasien demam tifoid, diagnosis dan penanganannya, pola resistensi antibiotik dan serotipe S.typhi pada anak di beberapa daerah di Jakarta.
Metodologi: Studi potong lintang dengan total 142 subjek. Evaluasi karakteristik dan uji resistensi antimikroba terhadap 22 subjek demam tifoid yang telah dikonfirmasi dengan kultur darah dan pemeriksaan penunjang Tubex ®TF ≥4, serta flagellin based-PCR pada 22 isolat S.typhi untuk analisis serotipe dengan gen fli-B, fli-C dan aro-C.
Hasil: Subjek lelaki lebih banyak daripada perempuan, kelompok usia tertinggi 5-9 tahun dan lama demam sekitar 6-8 hari. Manifestasi klinis demam tifoid yang diamati bervariasi, paling sering diare, mual dan muntah, namun tidak berbeda bermakna dengan penyebab demam yang lain yang memiliki klinis gangguan digestif. Sebagian besar subjek penelitian telah mendapatkan pengobatan sebelumnya (61,3%) namun riwayat pemberian antibiotik tidak diketahui pasti, dengan kecenderungan pemberian terapi golongan sefalosporin lebih banyak daripada antibiotik lini pertama.
Pemeriksaan penunjang dengan Tubex ®TF mendeteksi positif 67 subjek (47,2%) dengan sensitivitas 95,2%, spesifisitas 56,1%. Berdasar konfirmasi temuan isolat S.typhi pada kultur darah, hanya 22 subjek (15,5%) yang benar menderita demam tifoid dengan karakteristik pasien secara umum tidak berbeda bermakna dengan pasien bukan demam tifoid. Berdasarkan pemeriksaan Tubex ®TF dan kultur darah, demam tifoid lebih banyak didiagnosis dengan tepat di rumah sakit bila dibandingkan dengan puskesmas. Ditemukan 22 isolat positif gen aro-C, 20 isolat positif gen fli-C H:d, 2 fli-C H:j dan 2 isolat yang sama positif dengan gen fli-B. Serotipe S.typhi yang bersirkulasi, namun tidak ditemukan perbedaan manifestasi klinis, dan semua isolat masih sensitif terhadap kloramfenikol, ampisilin dan kotrimoksazol.
Simpulan: Angka kejadian demam tifoid yang didiagnosis pasti dengan konfirmasi kultur darah jauh lebih rendah daripada diagnosis klinis. Penelitian ini menunjukkan bahwa kloramfenikol masih dapat diberikan karena serotipe S.typhi yang ditemukan masih sensitif terhadap semua antimikroba.

Indonesia is known with high incidence of typhoid fever, however there are limited studies available to observe this disease burden in children. Thus a study to observe not only characteristic of patients, its clinical manifestation, diagnostic approach and management, but also thorough evaluation needed to evaluate its drug resistance pattern and Salmonella typhi serotypes circulating in Indonesia.
Objective: To evaluate characteristics of children with typhoid fever, its diagnostics and management, antimicrobial resistance pattern and serotype S.typhi circulating amongst children in Jakarta.
Methods: Descriptive study with 142 subjects clinically diagnosed with typhoid fever. Characterization of 22 subjects with confirmed case typhoid fever based on S.typhi isolate finding in blood culture and confirmation with Tubex®TF ≥4. Evaluation of antimicrobial resistance pattern and Flagellin based-PCR using fli-B, fli-C and aro-C genes to further analyze serotype S.typhi in children.
Results: There were more male than female participants in the study. The highest rate age of group observed were 5to 9 years-old. Most subjects had fever between 6-8 days with clinical manifestations varied but mostly related to digestive system such as diarrhea, nausea and vomit, however there was not difference between typhoid and non typhoid in clinical manifestation. Majority of subjects had received antibiotics prior to diagnose (61,3%), however many were oblivious to the type of treatment received, yet many were prescribed Cephalosporine instead of firstline treatments.
Tubex ®TF detected 67 cases of typhoid fever (47.2%), sensitivity 95.2%, specificity 56.1%. Only 22 subjects were confirmed case of typhoid fever (15.5%), however all patients yielded similar characteristics to patients in probable case. Based on laboratory assessment tools, doctors in tertiary hospital showed better accuracy in diagnosing patients with suspected typhoid if compared to primary health care one. Meanwhile all isolates showed positive results with aro-C gene controls, 20 showed positive with fli-C H:d genes whereas only 2 with H: j alleles, and 2 yielded positive results for fli-B. No differences were found in clinical manifestation and all serotypes were sensitive to all antimicrobials tested.
Conclusions: Confirmed case of typhoid fever is lower in prevalence if compared to probable case in society. Chloramphenicol is still recommended as first drug of choice for typhoid fever since S.typhi isolated in this research did not reveal any resistancy towards first line antibiotics."

Penelitian menganalisis impelementasi Clinical Pathway (CP) Typhoid fever melalui deskripsi utilisasi pelayanan serta tagihannya pada periode sebelum dan sesudah implemenatsi CP. Studi dilakukan di RS PMI Bogor bertujuan untuk mengeksplor siklus pembuatan CP serta utilisasi pelayanan kesehatan yang diberikan sehingga menimbulkan tagihan. Metode kualitatif digunakan untuk menjelaskan tahapan dalam pembuatan CP dan metode kuantitatif digunakan untuk mengeksplor utilisasi layanan dan tagihan yang ditimbulkan serta melihat signifikansi implementasi CP terhadap utilisasi pelayanan dan billing. Simulasi INA-CBG dilakukan akibat temuan dalam penelitian. Data berasal dari sistem informasi rumah sakit, billing dan rekam medis. Hasil penelitian menunjukan bahwa tidak ada signifikansi/perubahan pada utilisasi pelayanan secara statistik p-value >0.05 antara kelompok pada periode sebelum dan sesudah implementasi CP melalui Uji T dan Uji non parametrik Mann-Whitney U dengan tingkat kepercayaan 95%. Namun secara substansi terjadi perubahan tagihan pasca implementasi clinical pathway Typhoid fever dari Rp. 4,269,051 meningkat menjadi Rp. 5,225,384. Setelah dilakukan penyesuaian obat yang berfungsi terapeutik dan simtomatik terhadap Typhoid fever, maka total tagihan menjadi Rp. 4,771,016 dan meningkat menjadi Rp. 5,959,796. Proses pencatatan diagnosis di dalam rekam medis menjadi isu di RS PMI Bogor. Adanya potensi undercode yang mempengaruhi severity level kasus INA-CBGs (A-4-14), rumah sakit berpotensi kehilangan sebesar Rp. 485,200 hingga Rp. 1,450,400.


This research elaborated Typhoid fever Clinical Pathway (CP) implementation which were described using service utilization and the incurred billing before and after the implementation of CP. Study was conducted in PMI hospital Bogor and aimed to explore CP development cycle and the later service utilization delivered and hence, the incurred billing from each period (before and after CP implementation). Qualitative method was used to explore stages in CP development and quantitative method was used to explore the significance of CP implementation to service utilization and the billing. INA-CBGs grouping simulation was conducted due to a research finding. Data were derived from hospital information system, billing, and medical records. Study resulted in no significance of service utilization before and after CP implementation and it was predicted using T-test and Mann-Whitney U test showing p-value >0.05. However, changes in billing substantially changed from IDR 4,269,000 to IDR. 5,225,384. Adjustment was done by excluding drugs other than for therapeutic and symptomatic pursposes resulting in the increment of billings (e.g. IDR. 4,771,016 before and IDR. 5,959,796 after CP implementation). Simulation through INA-CBGs grouping showed that there were potential undercoding from higher severity level of Typhoid fever case (A-4-14). Hospital might subsequently lose IDR 485,200 up to IDR.1,450,400 each case reimbursed.

"Demam tifoid adalah penyakit infeksi umum akut yang disebabkan oleh Salmonella typhi. Penyakit yang tersebar hampir di seluruh dunia ini merupakan penyakit tropik sistemik, bersifat endemis dan masih merupakan problem kesehatan masyarakat di dunia, terutama di negara-negara berkembang, termasuk Indonesia. Uji Widal merupakan salah satu uji serologis yang sampai saat ini masih digunakan secara luas, khususnya di negara berkembang termasuk Indonesia. Uji serologi Widal memiliki sensitivitas dan spesifisitas yang rendah serta sering memberikan hasil positif palsu maupun negatif palsu. Oleh karena itu dilakukan deteksi molekuler real time PCR terhadap gen penyandi secretion system ATPase type III ssaN Salmonella enterica subsp.enterica dari spesimen darah pasien demam tifoid.
Uji spesifisitas teknik real time PCR didapatkan bahwa primer dan probe yang digunakan tidak bereaksi silang terhadap mikroorganisme lain yang diuji pada penelitian ini. Pada uji sensitivitas teknik didapatkan kemampuan deteksi minimal adalah 10 cfu/ml pada spesimen darah. Pada penerapan uji terhadap spesimen darah, didapatkan real time PCR dapat mendeteksi 19 38 sampel positif Salmonella enterica subsp.enterica dari 50 spesimen darah pasien yang diduga terinfeksi demam tifoid. Sebelas sampel dengan serologi Widal negatif memberikan hasil positif pada real time PCR. Dengan demikian, uji real time PCR terhadap target gen ssaN yang digunakan dalam penelitian ini dapat meningkatkan tingkat pengujian positif sebesar 22 dibandingkan uji Widal.

Typhoid fever is an acute infectious disease caused by Salmonella typhi. Diseases spread almost all over the world is a tropical disease systemic, endemic and remains a public health problem in the world, especially in developing countries, including Indonesia. In areas where typhoid fever occur, the clinical diagnosis of typhoid fever is inadequate, because the symptoms are not specific and overlapping with other febrile illnesses. Diagnosis of typhoid fever is often enforced only based on clinical symptoms and serological tests alone. Widal test is a serological test which is still widely used, particularly in developing countries, including Indonesia. Widal serological test has a very low sensitivity and specificity and often give false positives or false negatives result. Therefore, were performed detection of gene encoding secretion system ATPase type III ssaN in Salmonella enterica subsp.enterica from blood specimen of typhoid fever patients.
Specificity test of real time PCR technique showed that the primers and probes used are not cross react against other microorganisms tested in this study. On the sensitivity test techniques obtained minimal detection is at least 10 cfu ml of blood specimen. On the application of test in blood clinical specimens, real time PCR could detect 19 38 Salmonella enterica subsp.enterica positive samples of 50 blood specimen from suspected typhoid fever patients. Eleven samples with negative Widal serology gives positive results in real time PCR. Thus, real time PCR test with the ssaN gene target used in this study could increase rate of positive testing about 22 compared with Widal test.

Latar Belakang: Peningkatan jumlah bakteri Salmonella Typhi dengan resistensi terhadap antibiotik lini pertama menjadi beban ganda bagi negara berkembang di Asia termasuk Indonesia sehingga diperlukan antibiotik alternatif untuk menghadapinya. Daun Carica papaya memiliki komponen-komponen antibiotik sehingga berpotensi menjadi antibiotik alternatif.

Metode: Pada penelitian ini dilakukan uji eksperimental dengan ekstraksi daun C.papaya dengan pengenceran menjadi 4 konsentrasi; 100 mg/ml, 75 mg/ml, 50 mg/ml, dan 25 mg/ml. Tiap konsentrasi diuji aktivitas antibakteri terhadap Salmonella Typhi dengan metode difusi cakram dan uji konfirmasi dengan metode broth dilution. Diameter zona inhibisi pertumbuhan bakteri dengan metode difusi cakram diukur kemudian dianalisis

Hasil: Hasil dari penelitian difusi cakram tidak membuktikan adanya aktivitas antibiotik dari ekstrak daun C.papaya terhadap Salmonella Typhi sementara uji konfirmasi dengan metode broth dilution membuktikan adanya aktivitas antibakteri oleh keempat konsentrasi ekstrak terhadap Salmonella Typhi.

Kesimpulan: Kesimpulan dari penelitian ini adalah ekstrak daun C.papaya memiliki aktivitas antibakteri terhadap Salmonella Typhi


Background: Increasing incidence of resistant Salmonella Typhi strain towards first-line antibiotics has become a high burden in Asia’s developing countries including Indonesia impacting the need of an alternate antibiotic. Carica papaya leaves extract contain antibiotic components making it a potential alternative antibiotic.

Methods: This experimental research uses extraction of the C.papaya leaves with dilution into 4 concentrations; 100 mg/ml, 75 mg/ml, 50 mg/ml, and 25 mg/ml. Each concentration undergo antimicrobial susceptibility testing towards Salmonella Typhi with disc diffusion method and confirmatory test with broth dilution method. Diameter of inhibition zone of Salmonella Typhi bacterial growth in disc diffusion method will be measured and analyzed.

Results: Results show that the disc diffusion method was not able to prove the antibacterial activity of C.papaya leaves extract against Salmonella Typhi while the confirmatory test with broth dilution method has successfully proven antibacterial activity of the four extract concentrations towards Salmonella Typhi.

Conclusion: This research concluded that Carica papaya leaves extract has antibacterial activity towards Salmonella Typhi.

"Objective: There are many factors that govern growth and resistant of Salmonella typhi. A study had reported that the use of sodium benzoate caused antibiotic resistant. However, no study has directly evaluated the effect of sodium benzoate exposure on S. typhi sensitivity to chloramphenicol. The aim of this study was to evaluate the resistance or sensitivity of S. typhi to chloramphenicol after sodium benzoate exposure. Methods: The study was conducted in seven groups: three treatment groups (sodium benzoate insensitive S. typhi+8 μg/mL, 16 μg/mL, and 32 μg/mL of chloramphenicol), three positive control groups (sodium benzoate sensitive S. typhi+8 μg/mL, 16 μg/mL, and 32 μg/mL of chloramphenicol), and one negative control groups (sodium benzoate sensitive S. typhi+0 μg/mL of chloramphenicol). The effect of sodium benzoate exposure to S. typhi sensitivity to chloramphenicol was measured after 24 hours. Spearman test was used to analyzed this association. Results: In this study, we found that the average S. typhi growth in the treatment groups (A, B, C) was 445 CFU/mL, 385 CFU/mL, and 171 CFU/mL, respectively. While in the positive control group (D, E, F) was not obtained any S. typhi growth. Average S. typhi growth in the negative control group was 430 CFU/mL. We found that sodium benzoate exposure inhibited S. typhi growth and affected S. typhi sensitivity to chloramphenicol (p<0.05). In addition, we found that 32 μg/mL chloramphenicol had the highest mean difference value, so this showed that the dose 32 μg/mL of chloramphenicol had the best effectiveness of various treatment groups (p<0.05). Conclusions: Sodium benzoate exposure can inhibit S. typhi growth and cause S. typhi resistant to chloramphenicol.;"

"Masyarakat perkotaan banyak mengalami masalah kesehatan baik fisik maupun psikososial. Perubahan gaya hidup, polusi, dan sanitasi lingkungan yang buruk menimbulkan masalah kesehatan salah satunya penyakit typhoid. Typhoid merupakan penyakit sistemik yang disebabkan oleh Salmonella typhi dan sering terjadi kambuh sehingga merupakan penyakit kronis. Salah satu respon psikososial yang muncul akibat penyakit kronis adalah ketidakberdayaan. Karya ilmiah ini menggambarkan asuhan keperawatan ketidakberdayaan yang diberikan pada Ny. S dengan masalah fisik Typhoid. Karya ilmiah ini mengindikasikan pentingnya penerapan asuhan keperawatan psikososial untuk menunjang kesembuhan dan kesehatan fisik pasien.

Many urban communities experiencing health problems both physical and psychosocial. Lifestyle changes, pollution, and poor environmental sanitation cause health problems one of which typhoid disease. Typhoid is a systemic disease caused by Salmonella typhi and frequent recurrence that is a chronic disease. One of the emerging psychosocial response was helplessness. This paper describes the nursing care given to Mrs.S with powerlessness and typhoid as physical problems. This scientific work indicates the importance of the application of psychosocial nursing care to support healing and physical health of the patient.

"Demam tifoid, atau yang lebih dikenal sebagai penyakit tipes merupakan penyakit endemik di Indonesia. Diperkirakan hampir 100.000 penduduk Indonesia terjangkit demam tifoid tiap tahunnya. Salah satu langkah pencegahan penyebaran demam tifoid adalah melalui vaksinasi. Terdapat 3 jenis vaksin tifoid, yaitu TCV, vaksin polisakarida non konjugat suntik, dan vaksin oral. Sayangnya, ketiga vaksin memiliki rata-rata efektivitas < 75%. Maka dari itu, model penyebaran demam tifoid dengan intervensi vaksin dikonstruksi pada penelitian ini. Kajian analitik pada model dilakukan untuk melihat kepositifan, eksistensi dan kestabilan titik ekulibrium, dan juga bilangan reproduksi dasar dari model. Tak hanya itu, simulasi numerik dan interpretasinya dilakukan untuk melihat elastisitas dan sensitivitas R0 yang berkorelasi dengan paremeter efektivitas vaksin. Terakhir, dilakukan juga simulasi autonomous untuk melihat bagaimana perubahan setiap kelompok dalam populasi terhadap perubahan nilai parameter.

Typhoid fever is one of the endemic diseases in Indonesia. Around 100.000 Indonesians are estimated to get this disease every year. Typhoid vaccination is one of the effective strategies for preventing and controlling typhoid fever. There are 3 types of typhoid vaccines, the Typhoid Conjugate Vaccine (TCV), the unconjugated Vi Polysaccharide (ViPS), and the oral vaccine (Ty21a). The effectiveness of the three vaccines is no more than 75%. For that reason, a mathematical model of typhoid fever infection was constructed in this paper. An analytical study of the model was done to see the positivity of solutions, and the existence and stability of equilibrium points. Additionally, numerical simulation and interpretation were done to see the elasticity and sensitivity of the basic reproduction number."
