"Praktek Kerja Profesi Apoteker di Apotek Kimia Farma No.254 Pos Pengumben dilaksanakan selama 4 minggu dari 1-31 Mei 2016. Praktek Kerja Profesi ini bertujuan agar mahasiswa memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktek pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika yang berlaku. Secara keseluruhan kegiatan pelayanan kefarmasian di Apotek Kimia Farma No.256 yang meliputi Pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai telah sesuai dengan Permenkes No. 35 Tahun 2015 tentang Standar Pelayanan Kefarmasian di Apotek tahun 2014. Tetapi untuk kegiatan Pelayanan Farmasi Klinis masih perlu ditingkatkan. Tujuan khusus dalam Praktek Kerja Profesi Apoteker di Kimia Farma No.254 Pos Pengumben agar mahasiswa dapat menganalisis kerasionalan resep dari pasien Rheumatoid Arthritis atau Rematik.

Pharmacist Professional Practice in Kimia Farma 254 Pos Pengumben was held 4 weeks from May 1st-31st, 2016. The Profession practice aims for students to be able to understand the duties and responsibilities of pharmacists in Pharmacy (Apotek) management, and to practice pharmacy services in accordance with the provisions of law and pharmaceutical ethics. Overall, the activity in the pharmaceutical service Kimia Farma 256 which includes the Management of Pharmaceutical Products, Medical Devices, Medical Materials and Consumables accordance with Permenkes No. 35, 2015 on Standards of Pharmaceutical Services in Pharmacy , 2014. But for Clinical Pharmacy Services activities need to be improved. Special assigment given intended students to analyze the rationality of recipes from patients with rheumatoid arthritis."

"ABSTRAKKegiatan praktek kerja profesi di apotek dilakukan agar calon apoteker mampumemahami peranan, tugas, dan tanggung jawab apoteker di puskesmas sesuaidengan ketentuan standar pelayanan farmasi di apotek. Selain itu juga, diharapkan agar calon apoteker mempelajari mengenai struktur apotek dan memahami proses pelayanan yang tersedia di apotek. Dalam melakukan pelayanan resep, dilakukan pengkajian dan analisis terhadap rasionalitas resep yang diterima. Untuk itu, pada tugas khusus dilakukan pengkajian dan analisis terhadap resep pasien rheumatoid arthritis.

"Seorang apoteker memegang peranan penting di rumah sakit, puskesmas, dan apotek. Apoteker harus memenuhi standar kompetensi sebagai persyaratan untuk memasuki dunia kerja dan menjalani praktik profesi. Standar kompetensi profesi apoteker Indonesia terdiri dari sepuluh standar kompetensi sebagai kemampuan yang diharapkan oleh apoteker saat lulus dan masuk ke tempat praktik kerja profesi.Sebagai bekal dan pengalaman calon apoteker untuk dapat memahami peran apoteker dan meningkatkan kompetensi, maka dilaksanakan Praktik Kerja Profesi Apoteker di Apotek Kimia Farma No. 375, Puskesmas Kecamatan Kebayoran Lama, dan Rumah Sakit Umum Pusat Fatmawati periode bulan Juli-Oktober 2018. Selain PKPA, diharapkan calon apoteker dapat memperluas wawasan, pemahaman, dan pengalaman untuk melakukan pekerjaan kefarmasian di tempat praktik kerja profesi.

A pharmacist plays an important role in hospital, health center, and pharmacy. Pharmacists must have competency standards as a requirement for entering the workforce and undergoing professional practice. Indonesian pharmacist professional competency standards consist of ten competency standards as the capabilities expected by pharmacists when they graduate and enter professionals work practices. As a preparation and experiences of prospective pharmacists to be able to understand the role of pharmacists and improve competence, the Pharmacists Professional Work Practice are implemented at Apotek Kimia Farma No. 375, Puskesmas Kecamatan Kebayoran Lama, and Rumah Sakit Umum Pusat Fatmawati period July-October 2018. In addition to PKPA, it is expected that pharmacist candidates can broaden their horizons, understanding and experiences to do pharmacy work in professional workplaces."

"Tugas khusus ini memberikan gambaran terkait praktik kerja profesi apoteker di beberapa institusi kesehatan selama periode Maret hingga Oktober 2022, antara lain Puskesmas Kecamatan Palmerah, Kimia Farma Trading & Distribution (KFTD) Cabang Bogor, Apotek Kimia Farma 352 Margonda, PT. "Standar pelayanan kefarmasian yang diatur dalam PMK No. 73 Tahun 2016 merupakan pedoman yang harus diimplementasikan di apotek untuk mengoptimalkan kesehatan di komunitas. Laporan ini mendeskripsikan implementasi pelayanan kefarmasian dan pengkajian resep pasien diabetes melitus tipe 2 dan hipertensi di apotek kimia farma 002 periode november 2022. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP di Apotek Kimia Farma 002 sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016. Pengkajian yang dilakukan pada satu resep belum memenuhi aspek kelengkapan resep, yaitu aspek administratif dan pertimbangan klinis. Petugas apotek perlu menghubungi pihak dokter atau rumah sakit untuk mengkonfirmasi resep. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pelayanan swamedikasi di Apotek Kimia Farma 002 sudah cukup baik. Penggalian informasi pasien yang dilakukan sudah sesuai dengan literatur. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan PIO sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016, namun tidak didokumentasikan dengan baik.

The standard of pharmaceutical services regulated in Ministerial Decree No. 73 of 2016 is a guideline that must be implemented in pharmacies to optimize community health. This report describes the implementation of pharmaceutical services and the assessment of prescriptions for patients with type 2 diabetes mellitus and hypertension at Chemist Pharmacy 002 during the period of November 2022. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of the management of pharmaceutical preparations, medical devices, and health supplies at Chemist Pharmacy 002 is already quite good and in accordance with Ministerial Decree No. 73 of 2016. The assessment conducted on a prescription did not meet the completeness aspects of a prescription, namely administrative aspects and clinical considerations. Pharmacy staff need to contact doctors or hospitals to confirm the prescription. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of self-medication services at Chemist Pharmacy 002 is already quite good. Patient information gathering is in accordance with the literature. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of Pharmaceutical Care Program activities is already quite good and in accordance with Ministerial Decree No. 73 of 2016, but it is not well documented."
