Optimalisasi Pelayanan Resep Berbasis Digital melalui Aplikasi Kimia Farma Mobile di Apotek Kimia Farma 07 Juanda Bogor = Optimalization of Digital-Based Prescription Services through Kimia Farma Mobile Application at Kimia Farma Pharmacy 7 Juanda Bogor
"Revolusi industri 4.0 pada era digital mendorong perkembangan teknologi dengan pesat. Salah satu bidang yang terus berkembang dan mengadopsi teknologi adalah e-commerce. E-commerce sendiri tidak hanya bergerak di bidang bisnis namun juga sekarang telah merambah dunia kesehatan. Salah satunya adalah Kimia Farma dengan aplikasi Kimia Farma Mobile. Kimia Farma Mobile memiliki beberapa fitur yaitu layanan klinik, layanan laboratorium dan layanan apotek. Apotek kimia farma 07 Juanda Bogor melayani resep dengan pembayaran tunai dan resep dengan pembayaran kredit (BPJS dan inhealth). Dalam fitur tebus resep pada Kimia Farma Mobile, resep yang dilayani berupa resep pembayaran tunai. Pelayanan resep kredit khususnya BPJS masih belum dapat dilakukan melalui aplikasi Kimia Farma Mobile. Banyaknya antrian, proses verifikasi dokumen pada saat pasien datang, penyiapan obat dan penulisan etiket secara manual, merupakan hal- hal yang mempengaruhi waktu tunggu pasien. Selain itu, pada fitur tebus resep, untuk resep tunai, pilihan yang tersedia adalah obat yang diresepkan akan diantarkan ke alamat dan tidak ada pilihan untuk pengambilan obat di Apotek apabila pasien ingin ambil obat langsung ke Apotek dengan kondisi tertentu. Hal tersebut dapat diatasi dengan dibuatnya sistem yang terintegrasi antara pelayanan resep BPJS dengan Kimia Farma Mobile sehingga mengoptimalkan pelayanan resep di Apotek Kimia Farma 07 Bogor dan penambahan pilihan pada fitur tebus resep, obat diantar ke alamat atau pasien ambil di Apotek. Perbaikan sistem secara berkala perlu dilakukan pada aplikasi Kimia Farma Mobile khususnya pada fitur tebus resep saat proses verifikasi diri.
The industrial revolution 4.0 in the digital era has driven technological developments rapidly. One area that continues to develop and adopt technology is e-commerce. E-commerce itself is not only engaged in business but has also now penetrated the world of health. One of them is Kimia Farma with the Kimia Farma Mobile application. Kimia Farma Mobile has several features, namely clinic services, laboratory services, and pharmacy services. Kimia Farma Pharmacy 07 Juanda Bogor serves prescriptions with cash payments and prescriptions with credit payments (BPJS and in health). In the recipe redemption feature on Kimia Farma Mobile, recipes served are cash payment recipes. Credit prescription services, especially BPJS, still cannot be done through the Kimia Farma Mobile application. The number of queues, the process of verifying documents when patients arrive, preparing medicines, and writing labels manually, are things that affect patient waiting time. In addition, in the redeem prescription feature, for cash prescriptions, the available options are that the prescribed drug will be delivered to the address and there is no option to pick up the drug at the Pharmacy if the patient wants to take the drug directly to the Pharmacy under certain conditions. This can be overcome by creating an integrated system between BPJS prescription services and Kimia Farma Mobile to optimize prescription services at the Kimia Farma 07 Bogor Pharmacy and add options to the redeem prescription feature, drugs delivered to the address, or patients picked up at the Pharmacy. Periodic system improvements need to be made to the Kimia Farma Mobile application, especially the recipe redemption feature during the self-verification process.

"Pelayanan kefarmasian di apotek merupakan pelayanan yang bersentuhan langsung dengan masyarakat, sehingga dituntut untuk mampu menjamin ketersediaan obat yang aman, bermutu, dan berkhasiat. Diabetes mellitus (DM) merupakan gangguan metabolisme kronis yang ditandai dengan terjadinya hiperglikemia persisten. Terapi pasien DM tipe dua bertujuan untuk mempertahankan tingkat konsentrasi glukosa dalam darah dalam batas normal, dan untuk mencegah berkembangnya komplikasi jangka panjang dari kondisi diabetes. Penulisan Tugas Khusus ini dilakukan agar penulis lebih memahami secara mendalam terkait analisis dan pengkajian resep di apotek, khususnya resep pasien diabetes. Metode pelaksanaan analisis resep dilakukan dengan pendekatan pengkajian, pelayanan, dan screening resep terhadap resep pasien diabetes di KFA Matraman sesuai dengan yang tertera pada buku Petunjuk Teknis Standar Pelayanan Kefarmasian di Apotek. Kesimpulan yang didapat dari analisis resep pasien diabetes kali ini yaitu secara umum seringkali terdapat kombinasi pengunaan obat pada pasien (polifarmasi), sehingga potensi terjadi interaksi obat pada pasien dapat meningkat. Menyikapi hal ini, apoteker perlu memastikan setiap kombinasi terapi yang diterima pasien merupakan kombinasi terapi yang rasional, sehingga efektif untuk menunjang kesembuhan pasien.
Community pharmaceutical services in pharmacies are services that are in direct contact with the community, so they must be able to guarantee the availability of safe, quality, and efficacious drugs. Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia. Therapy for type 2 DM patients aims to maintain blood glucose concentration levels within normal limits, and to prevent the development of long-term complications from diabetes. This Special Assignment was written so that the author could get better understanding in the analysis and assessment of prescriptions in pharmacies, especially prescriptions for diabetes patients. The analysis of diabetes prescription in this assignment was carried out using the approach of assessment, service, and screening of prescriptions for diabetes patients at KFA Matraman in accordance with that stated in the Technical Instructions for Standard Pharmaceutical Services in Pharmacies. The conclusion obtained from the analysis of diabetes patient prescriptions this time is that in general there is often a combination of drug use in patients (polypharmacy), so that the potential for drug interactions in patients can increase. In response to this, pharmacists need to ensure that every combination of therapy received by patients is a rational combination of therapy, so that it could be effective in supporting patient recovery."

"Stabilitas obat dapat dipengaruhi dengan lamanya waktu penyimpanan. Beyond Use Date (BUD) dan Expired Date (ED) merupakan istilah yang sering digunakan sebagai batasan waktu terkait stabilitas obat. Beyond Use Date merupakan batas waktu penggunaan suatu sediaan obat steril maupun non-steril yang terhitung setelah obat diracik atau setelah kemasan primer sediaan obat dibuka atau dirusak. Pembuatan materi terkait Beyond Use Date dalam bentuk leaflet bertujuan untuk memberikan informasi bagi Masyarakat yang membaca, sedangkan materi dalam bentuk poster dibuat dengan tujuan memudahkan tenaga kefarmasian untuk mendapatkan informasi terkait BUD suatu sediaan guna disampaikan kepada pasien. Laporan ini dibuat dengan melakukan studi literatur dari beberapa referensi. Data dan materi yang dikumpulkan berdasarkan hasil studi literatur divisualisasikan dalam bentuk desain leaflet  dan poster. BUD merupakan salah satu parameter untuk menjamin efikasi dan keamanan obat. Edukasi mengenai BUD penting dilakukan untuk meningkatkan pengetahuan masyarakat agar lebih memperhatikan batas watu pemakaian sediaan farmasi yang sedang digunakan.
Drug stability can be affected by the length of storage time. Beyond Use Date (BUD) and Expired Date (ED) are terms that are often used as time limits related to drug stability. Beyond Use Date is the time limit for the use of a sterile or non-sterile drug preparation counted after the drug is compounded or after the primary packaging of the drug preparation is opened or damaged. The making of leaflet related to the Beyond Use Date aims to provide information for the community, while the poster was made to help pharmacists to obtain information related to the BUD of a preparation to be delivered to patients. This report was written by conducting a literature study from several references. The data and materials collected based on the results of the literature study were visualized in the form of leaflets and poster designs. BUD is one of the parameters to ensure drug efficacy and safety. Education about BUD is important to increase public knowledge to pay more attention to the time limit for the use of pharmaceutical preparations that are being used."

"Pelayanan kefarmasian pada awalnya hanya berfokus kepada pengelolaan obat (drug oriented) kemudian berkembang menjadi pelayanan komprehensif meliputi pelayanan obat dan pelayanan kepada pasien (patient oriented) yang bertujuan kepada pelayanan kefarmasian (pharmaceutical care). Era digital seperti sekarang ini, menjadikan media sosial sebagai trend dalam komunikasi pemasaran. Media sosial adalah sebuah media online, yang penggunanya bisa dengan mudah berpartisipasi, berbagi, dan menciptakan isi meliputi video, blog, jejaring sosial, wiki, forum dan dunia virtual.Promosi melalui media sosial memiliki pengaruh yang positif dan juga signifikan pada keputusan pembelian konsumen. PT. Kimia Farma merupakan salah satu perusahaan yang menggunakan media sebagai metode untuk meningkatkan eksistensi sekaligus membantu memudahkan pelanggan dalam pelaksanaan kegiatan yang biasanya dilakukan di apotek. Metode yang digunakan ialah dengan membuat aplikasi Kimia Farma Mobile. Aplikasi ini tergolong baru keberadaan nya, maka dari itu saya tertarik untuk mengetahui respon pelanggan terutama yang ada di Apotek Kimia Farma KS Tubun terhadap eksistensi aplikasi Kimia Farma Mobile.

In today's digital era, social media has become a trend in marketing communications. Social media is an online media, where users can easily participate, share and create content including videos, blogs, social networks, wikis, forums and virtual worlds. Promotion through social media has a positive and significant influence on consumer purchasing decisions. PT. Kimia Farma is one of the companies that uses media as a method to increase its existence as well as help make it easier for customers to carry out activities that are usually carried out in pharmacies. The method used is to create a Kimia Farma Mobile application. This application is relatively new in existence, therefore I am interested in knowing the response of customers, especially those at Kimia Farma KS Tubun Pharmacy, to the existence of the Kimia Farma Mobile application.

"Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian oleh Apoteker. Pengkajian resep merupakan salah satu standar pelayanan kefarmasian yang dilakukan di Apotek. Pengkajian resep dilakukan untuk melakukan analisa adanya permasalahan terkait obat dan apabila ditemukan masalah terkait obat harus segera dikonsultasikan kepada dokter penulis resep. Tujuan dilakukannya pengkajian resep untuk mencegah permasalahan terkait obat sebelum obat diserahkan kepada pasien sehingga tidak terjadi medication error yang berdampak pada kegagalan terapi dan efek obat yang tidak diharapkan yang merugikan pasien. Penelitian ini bertujuan untuk mengetahui kelengkapan administrasi, kesesuaian farmasetik dan kesesuaian klinis pada peresepan di Apotek Kimia Farma Menteng Huis. Dalam penelitian ini, dilakukan pengkajian resep Pasien Rujuk Balik (PRB) pasien asma dengan mengkaji secara administratif, farmasetik dan klinis. Pada persyaratan administratif, didapatkan ketidaklengkapan berat badan pasien dan paraf dokter. Pada pengkajian secara farmasetik, resep yang tertulis sudah sesuai bentuk dan kekuatan sediaannya. Pada pengkajian secara klinis, terdapat duplikasi dan interaksi pada beberapa obat. Dapat disimpulkan bahwa masih banyak ketidaksesuaian dalam penulisan resep yang diamati sehingga harus dikonsultasikan kepada dokter penulis resep.
Pharmacy is a pharmaceutical service facility where pharmacists practice pharmacy. Prescription review is one of the standard pharmaceutical services carried out at pharmacies. Prescription review is carried out to analyze drug-related problems and if a drug-related problem is found, a doctor who prescribes it must be consulted immediately. The purpose of reviewing prescriptions is to prevent drug-related problems before the drugs are dispensed to patients so that medication errors do not occur which result in therapy failure and unexpected drug effects that harm the patient. This study aims to determine the completeness of administration, pharmaceutical suitability and clinical suitability for prescriptions at the Kimia Farma Pharmacy, Menteng Huis. In this study, the prescription review of Back-Referral Program (PRB) was carried out in asthma patient by examining administrative, pharmaceutical and clinical aspects. In administrative requirements, incomplete patient weight and doctor's initials were obtained. In the pharmaceutical study, the written prescription have appropriate dosage forms and dosage strengths. In clinical studies, there are duplications and interactions with several drugs. It can be concluded that there are still many observed discrepancies in prescription writing, so a doctor who prescribes it must be consulted."

Pengkajian Resep Pasien Dengan Osteoarthritis Di Apotek Kimia Farma 049 = Prescription Review Of Patients With Diagnosis Of Osteoarthritis At Kimia Farma Pharmacies 049.
"Peraturan Menteri Kesehatan RI Nomor 73 tahun 2016 menetapkan standar pelayanan kefarmasian di Apotek, yang bertujuan untuk meningkatkan mutu kehidupan pasien. Pelayanan kefarmasian telah berkembang dari orientasi produk menjadi berorientasi pada pasien. Pengkajian resep adalah aspek operasional penting dalam pelayanan kefarmasian, yang bertujuan untuk mengidentifikasi dan menyelesaikan permasalahan terkait obat sebelum disiapkan. Dalam pengkajian resep, terdapat tiga aspek yang diperhatikan: administratif, farmasetik, dan klinis. Tujuan penelitian ini adalah melakukan pengkajian pada resep pasien dengan diagnosis osteoarthritis di Apotek Kimia Farma 049 untuk mengidentifikasi kemungkinan kesalahan pengobatan dan memberikan saran. Ditemukan bahwa resep pasien osteoarthritis memiliki beberapa permasalahan administratif, seperti ketidaklengkapannya, tetapi dari segi farmasetik dan klinis sudah sesuai. Kesimpulan dari pengkajian adalah bahwa meskipun ada permasalahan administratif pada resep, obat yang diresepkan sudah sesuai dengan tatalaksana osteoarthritis. Saran yang diberikan adalah untuk memperhatikan aspek administratif yang kurang lengkap pada resep dan melakukan konfirmasi kepada pasien atau dokter jika diperlukan.
Minister of Health Regulation No. 73 of 2016 sets the standard for pharmaceutical services in pharmacies, aimed at improving the quality of patient life. Pharmaceutical services have evolved from product-oriented to patient-oriented. Prescription assessment is a crucial operational aspect in pharmaceutical services, aiming to identify and resolve medication-related issues before preparation. In prescription assessment, three aspects are considered: administrative, pharmaceutical, and clinical. The aim of this study is to assess prescriptions for patients diagnosed with osteoarthritis at Kimia Farma Pharmacy 049 to identify potential medication errors and provide recommendations. It was found that prescriptions for osteoarthritis patients had some administrative issues, such as incompleteness, but were adequate in terms of pharmaceutical and clinical aspects. The conclusion of the assessment is that despite administrative issues in prescriptions, the prescribed medications are suitable for osteoarthritis management. Recommendations include paying attention to incomplete administrative aspects in prescriptions and confirming with patients or doctors if necessary."

Pengkajian Resep Pasien Paru di Apotek Kimia Farma Raya Bogor Periode April 2023 = Assessment of Lung Patient Prescriptions at Kimia Farma Raya Bogor Pharmacy Period April 2023
"Penyakit Paru Obstruksi Kronik (PPOK) merupakan penyakit yang ditandai dengan kelainan pada alveolar yang disebabkan oleh paparan signifikan terhadap partikel atau gas berbahaya sehingga mengakibatkan penyumbatan pada paru-paru dan keterbatasan aliran udara. Dalam upaya pencegahan dan pengobatan PPOK, apoteker berperan peting dalam pelayanan farmasi klinik yang berkaitan dengan sediaan farmasi untuk meningkatkan kualitas hidup pasien. Pengkajian resep merupakan salah satu pelayanan farmasi klinik yang dapat dilakukan oleh apoteker untuk menganalisa masalah terkait obat serta mencegah terjadinya kesalahan pemberian obat. Penelitian ini bertujuan untuk mengkaji aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis, serta kerasionalan 5 resep pasien paru di Apotek Kimia Farma Raya Bogor periode April 2023. Penelitian ini dilakukan dengan pengumpulan dan pengkajian resep pasien paru pada bulan april 2023. Hasil penelitian menunjukkan kelima resep pasien paru di Apotek Kimia Farma Raya Bogor telah memenuhi aspek administratif, aspek kesesuaian farmasetik, aspek pertimbangan klinis, dan resep rasional.
Chronic Obstructive Pulmonary Disease (COPD) is a disease characterized by alveolar abnormalities caused by significant exposure to harmful particles or gases, resulting in blockage of the lungs and limited air flow. In efforts to prevent and treat COPD, pharmacists play an important role in clinical pharmacy services related to pharmaceutical preparations to improve the quality of life of patients. Prescription review is one of the clinical pharmacy services that can be carried out by pharmacists to analyze drug-related problems and prevent medication administration errors. This research aims to examine administrative aspects, pharmaceutical suitability and clinical considerations, as well as the rationale for 5 prescriptions for pulmonary patients at Kimia Farma Raya Bogor for the period April 2023. This research was conducted by collecting and reviewing pulmonary patient prescriptions in April 2023. The research results showed that the five prescriptions for pulmonary patients at Kimia Farma Raya Bogor Pharmacy had fulfilled administrative aspects, pharmaceutical suitability aspects, clinical consideration aspects, and rational prescriptions."

"Apotek termasuk salah satu sarana pelayanan kefarmasian sebagai tempat dilakukan praktik kefarmasian oleh Apoteker. Apoteker dituntut untuk meningkatkan pengetahun serta keterampilannya dalam memberikan informasi obat kepada pasien serta memberikan konseling kepada pasien yang membutuhkan. Apoteker juga harus menyadari kemungkinan terjadinya kesalahan penyerahan obat dalam proses pelayanan sehingga perlu diidentifikasi untuk mencegah terjadinya kesalahan penyerahan obat. Salah satu upaya untuk menjamin kesehatan masyarakat adalah melakukan penyerahan obat dengan tepat yaitu dilakukan pengkajian resep terlebih dahulu sebelum melakukan pelayanan resep obat. Hal ini dilakukan untuk meningkatkan efektifitas dalam pemberian informasi obat kepada pasien sehingga meminimalkan kesalahan dalam pemberian obat serta dapat meningkatkan kualitas hidup pasien dengan optimal. Pengkajian resep dapat dilakukan dengan mempertimbangkan beberapa aspek yang terdiri dari aspek administratif, kesesuaian farmasetik, dan pertimbangan klinis. Dari hasil analisis, kelima resep dinyatakan tidak lengkap secara administratif karena tidak mencantumkan secara lengkap mengenai umur pasien, berat badan pasien, alamat dokter, dan nomor telepon dokter. Pada aspek farmasetik, resep 2 dan 5 telah memenuhi kesesuaian farmasetik sedangkan resep 1, 3, dan 4 tidak memenuhi syarat karena tidak lengkap mencantumkan bentuk dan kekuatan sediaan. Pada aspek kesesuaian klinis, hanya 1 resep yang tidak terjadi interaksi obat yaitu resep 4.
Pharmacy is one of the pharmaceutical service facilities as a place for pharmacists to practice pharmacy. Pharmacists are required to improve their knowledge and skills in providing drug information to patients and providing counseling to patients in need. Pharmacists must also be aware of the possibility of drug administration errors in the service process so that they need to be identified in advance. One of the efforts to ensure public health is to deliver drugs appropriately, namely by assessing the prescription first before carrying out prescription drug services. This is done to increase the effectiveness of providing drug information to patients so as to minimize errors in drug administration and can improve the quality of life of patients optimally. Prescription review can be done by considering several aspects consisting of administrative aspects, pharmaceutical suitability, and clinical considerations. From the analysis, the five prescriptions were declared administratively incomplete because they did not include the patient's age, weight, doctor's address, and doctor's telephone number. In the pharmaceutical aspect, prescriptions 2 and 5 have met the pharmaceutical suitability while prescriptions 1, 3, and 4 did not meet the requirements because they did not include the dosage form and strength. In terms of clinical suitability, only 1 prescription did not have drug interactions, namely prescription 4."

"Pelayanan kefarsmasian adalah suatu pelayanan langsung dan bertanggung jawab seorang apoteker kepada pasien yang berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien. Pengkajian resep merupakan suatu proses pemeriksaan resep. Skrining resep dilakukan bertujuan untuk menganalisis adanya masalah terkait obat dan apabila terdapat kesalahan atau kejanggalan dapat dikonsultasikan dengan dokter penulis resep sehingga pasien dapat terhindar dari risiko medication error. Hipertensi adalah suatu keadaan dimana tekanan darah meningkat melebihi batas normal. Faktor resiko yang menyebabkan hipertensi yaitu umur, jenis kelamin, keturunan, obesitas, merokok, kurangi aktivitas fisik, konsumsi garam berlebihan, dyslipidemia. Terapi pada pasien hipertensi bertujuan untuk mencapai dan mempertahankan target tekanan darah. Adanya peningkatan tekanan darah yang terjadi pada pasien hipertensi dapat menurunkan aliran darah ke ginjal. Aliran darah ginjal yang rendah akan menstimulasi reabsorpsi asam urat. Tekanan darah yang semakin tinggi memiliki kemungkinan untuk memperbesar risiko penyakit mikrovaskuler yang dapat memicu iskemia jaringan. Penggunaan obat yang tepat untuk penderita hipertensi diperlukan agar pengobatan pasien menjadi efektif. Penggunaan obat yang tidak efektif dapat mengakibatkan kegagalan terapi.
Pharmaceutical services are direct and responsible services provided by a pharmacist to patients related to pharmaceutical preparations with the aim of achieving definite results to improve the patient's quality of life. Prescription review is a process of examining prescriptions. Prescription screening is carried out with the aim of analyzing drug-related problems and if there are errors or irregularities, it can be consulted with the prescribing doctor so that patients can avoid the risk of medication errors. Hypertension is a condition where blood pressure increases beyond normal limits. Risk factors that cause hypertension are age, gender, heredity, obesity, smoking, reduced physical activity, excessive salt consumption, dyslipidemia. Therapy for hypertensive patients aims to achieve and maintain target blood pressure. Increased blood pressure that occurs in hypertensive patients can reduce blood flow to the kidneys. Low renal blood flow will stimulate uric acid reabsorption. Increasing blood pressure has the potential to increase the risk of microvascular disease that can trigger tissue ischemia. The use of appropriate drugs for hypertensive patients is needed so that patient treatment is effective. Ineffective use of drugs can result in therapy failure."

Pengkajian Resep Penyakit Diabetes dan Hipertensi di Apotek Kimia Farma 348 = Prescription Screening in Patients with Type 2 Diabetes Mellitus and Hypertension at Apotek Kimia Farma 348 Dramaga.
"Sebagai seorang apoteker, salah satu cara untuk memastikan pasien telah mendapatkan pengobatan yang rasional adalah melalui pengkajian resep, yang meliputi ketepatan penilaian kondisi pasien, tepat diagnosis, tepat indikasi, tepat jenis obat, tepat dosis, tepat cara dan lama pemberian, tepat informasi, dengan memperhatikan keterjangkauan harga, kepatuhan pasien, dan waspada efek samping. Pengkajian resep dilakukan berdasarkan aspek administratif, farmasetik dan klinis. Pengkajian resep dilakukan pada pasien yang menebus obat di Apotek Kimia Farma Dramaga 348 yang memiliki diagnosis diabetes tipe 2, hipertensi, dan diabetes tipe 2 yang disertai dengan hipertensi. Resep-resep yang akan dikaji umumnya berisi obat-obatan rutin pasien yang diberikan selama 1 bulan. Dari hasil kaji resep, Aspek administratif belum memenuhi persyaratan karena kurangnya informasi mengenai berat badan dan alamat pasien. Namun, kekurangan tersebut dapat diatasi dengan mengkonfirmasi kembali ke pasien dan dokter yang bersangkutan. Aspek farmasetik sudah memenuhi persyaratan, sementara aspek klinis belum memenuhi persyaratan karena ada beberapa obat yang saling berinteraksi dan merupakan polifarmasi. Namun, hal tersebut dapat diintervensi dengan mengedukasi pasien untuk menjeda konsumsi pemberian obat dan memotivasi pasien pentingnya untuk meminum obat secara rutin untuk mendapatkan hasil terapi yang optimal.

Prescription reviews are carried out based on administrative, pharmaceutical and clinical aspects. Prescription reviews were carried out on patients who redeemed medicines at Kimia Farma Dramaga 348 Pharmacy who had a diagnosis of type 2 diabetes, hypertension, and type 2 diabetes along with hypertension. The prescriptions that will be studied generally contain the patient's routine medicines that are given for 1 month. From the results of the prescription review, the administrative aspects did not meet the requirements due to lack of information regarding the patient's weight and address. However, this deficiency can be overcome by confirming it again with the patient and the doctor concerned. The pharmaceutical aspect meets the requirements, while the clinical aspect does not meet the requirements because there are several drugs that interact with each other and polypharmacy. However, this can be intervened by educating patients to separate the consume between interacted medications and motivating patients to the importance of taking medication regularly to get optimal therapeutic results."
