"Pelayanan obat atas resep dokter merupakan salah satu dari pelayanan kefarmasian di Apotek. Resep yang sesuai harus memuat cukup informasi yang memungkinkan tenaga kefarmasian yang bersangkutan mengerti obat yang akan diberikan kepada pasiennya. Resep juga harus ditulis dengan jelas agar terhindar dari salah persepsi antara dokter dengan tenaga kefarmasian yang bersangkutan. Namun pada kenyataanya, masih ada beberapa permasalahan yang ditemui di dalam peresepan. Permasalahan dalam peresepan obat tersebut disebut juga Medication error. Medication error merupakan kejadian yang dapat merugikan pasien akibat pemakaian obat yang seharusnya dapat dicegah dalam pengobatan. Upaya dalam pencegahan terjadinya kesalahan pemberian obat (Medication error) adalah pengkajian resep pada resep yang diterima dari pasien. Metode penelitian yang digunakan dalam penyusunan laporan tugas khusus ini yaitu pengambilan data secara retrospektif dimana dilakukannya pengamatan dan mencatat terkait dengan aspek administrasi, aspek farmasetik, dan aspek klinis dari resep tersebut dan di ceklis pada form yang telah dibuat. Berdasarkan hasil Praktik Kerja Profesi Apoteker di Apotek Kimia Farma 352 Margonda yang telah dilakukan oleh peneliti dapat disimpulkan bahwa Ketepatan resep pada pasien gastritis berdasarkan aspek administrasi masih terdapat beberapa kekurangan sehingga diperlukan kelengkapan dalam resep pada aspek administrasi, Ketepatan resep pada pasien gastritis berdasarkan aspek farmasetik telah memenuhi persyaratan standar pelayanan kefarmasian, namun terdapat beberapa resep yang masih belum memenuhi persyaratan sehingga kelengkapannya perlu diperhatikan kembali, dan Ketepatan resep pada pasien gastritis berdasarkan aspek klinis telah memenuho persyaratan standar pelayanan kefarmasian seluruhnya sehingga harus dipertahankan seterusnya seperti ini.

Drug service on a doctor's prescription is one of the pharmaceutical services at the pharmacy. An appropriate prescription must contain sufficient information to enable the pharmacist concerned to understand the drug to be given to the patient. Prescriptions must also be written clearly to avoid misunderstandings between the doctor and the pharmacist concerned. But in fact, there are still some problems encountered in the prescription. Problems in prescribing these drugs are also called Medication errors. Medication error is an event that can harm the patient due to the use of drugs that should be prevented in treatment. Efforts to prevent medication errors from occurring are reviewing prescriptions on prescriptions received from patients. The research method used in the preparation of this special assignment report was retrospective data collection where observations and notes were made related to the administrative, pharmaceutical, and clinical aspects of the prescription and checked on the form that had been made. Based on the results of the Pharmacist Professional Work Practice at Kimia Farma 352 Margonda Pharmacy which has been carried out by researchers, it can be concluded that the accuracy of prescriptions for gastritis patients based on administrative aspects still has some deficiencies, so completeness of prescriptions is needed on administrative aspects. Accuracy of prescriptions for gastritis patients based on pharmaceutical aspects has been meet the standard requirements for pharmaceutical services, but there are several prescriptions that still do not meet the requirements so that their completeness needs to be considered again, and the accuracy of prescriptions for gastritis patients based on clinical aspects has fulfilled the standard requirements for pharmaceutical services in its entirety so it must be maintained onwards like this."

Profesi apoteker merupakan salah satu profesi dalam tenaga kesehatan yang penting untuk melakukan layanan kefarmasian. Pelayanan kefarmasian adalah suatu proses yang mendukung terciptanya layanan kesehatan yang paripurna. Maka dari itu, calon apoteker dituntut untuk melakukan praktik kerja profesi sebagai bekal dan pengalaman untuk memahami peran apoteker, serta meningkatkan kompetensi yang sesuai dengan standar kompetensi apoteker dalam melakukan pelayanan kefarmasian. Praktik Kerja Profesi Apoteker dilakukan di Rumah Sakit Universitas Indonesia Periode Maret - April 2022, dan di Apotek Kimia Farma 352 Margonda Periode Mei 2022. Melalui praktik kerja profesi apoteker di Rumah Sakit dan Apotek sebagai fasilitas layanan kesehatan, mahasiswa apoteker diharapkan mampu memperoleh wawasan, pengetahuan, keterampilan, dan pengalaman yang sesuai untuk melakukan pekerjaan kefarmasian.

Pharmacist Professional Practice in Kimia Farma (KF) Pharmacy No. 352 Depok aims to evaluate the general activities of pharmaceutical cares in KF Pharmacy No. 352 Depok; and to know the role and function of the pharmacist in the professional aspects and managerial aspects in Pharmacy in general, and in KF Pharmacy No. 352 Depok in particular. Pharmacists have three roles in the Pharmacy, which are a professional, seller, and manager. As a professional, the pharmacist must ensure the quality and the patient received treatment rationality. As a seller, the pharmacist should be able to provide service to customers in order to increase the turnover of Pharmacy. As a manager, the pharmacist should be able to manage the pharmacy with good managerial system for continuity of his/her pharmacy. "Standar pelayanan kefarmasian yang diatur dalam PMK No. 73 Tahun 2016 merupakan pedoman yang harus diimplementasikan di apotek untuk mengoptimalkan kesehatan di komunitas. Laporan ini mendeskripsikan implementasi pelayanan kefarmasian dan pengkajian resep pasien diabetes melitus tipe 2 dan hipertensi di apotek kimia farma 002 periode november 2022. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP di Apotek Kimia Farma 002 sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016. Pengkajian yang dilakukan pada satu resep belum memenuhi aspek kelengkapan resep, yaitu aspek administratif dan pertimbangan klinis. Petugas apotek perlu menghubungi pihak dokter atau rumah sakit untuk mengkonfirmasi resep. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pelayanan swamedikasi di Apotek Kimia Farma 002 sudah cukup baik. Penggalian informasi pasien yang dilakukan sudah sesuai dengan literatur. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan PIO sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016, namun tidak didokumentasikan dengan baik.

The standard of pharmaceutical services regulated in Ministerial Decree No. 73 of 2016 is a guideline that must be implemented in pharmacies to optimize community health. This report describes the implementation of pharmaceutical services and the assessment of prescriptions for patients with type 2 diabetes mellitus and hypertension at Chemist Pharmacy 002 during the period of November 2022. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of the management of pharmaceutical preparations, medical devices, and health supplies at Chemist Pharmacy 002 is already quite good and in accordance with Ministerial Decree No. 73 of 2016. The assessment conducted on a prescription did not meet the completeness aspects of a prescription, namely administrative aspects and clinical considerations. Pharmacy staff need to contact doctors or hospitals to confirm the prescription. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of self-medication services at Chemist Pharmacy 002 is already quite good. Patient information gathering is in accordance with the literature. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of Pharmaceutical Care Program activities is already quite good and in accordance with Ministerial Decree No. 73 of 2016, but it is not well documented."
