"his book provides a solution to “rare event” problems without using the classical theory of reliability and theory of probability. This solution is based on the methodology of risk assessment as “measure of danger” (in keeping with the ICS RAS) and an expert approach to determining systems’ safety indications using Fuzzy Sets methods. Further, the book puts forward a new concept: “Reliability, Risks, and Safety” (RRS).

"ABSTRAK
Tesis ini membahas mengenai evaluasi yang dilakukan mahasiswa terhadap
sistem manajemen mutu proyek EPC pada fase perancangan produk detail
engineering yang dikerjakan PT XYZ dengan berbasis manajemen risiko. Tujuan
dari penelitian ini adalah untuk mengetahui faktor-faktor dominan yang
mempengaruhi sasaran mutu fase engineering produk detail engineering.
Penelitian ini dilakukan dengan meneliti aktifitas-aktifitas yang dilakukan pada
proses bisnis dari PT XYZ. Faktor-faktor risiko yang mempengaruhi kinerja mutu
kemudian dianalisis dengan metode SPSS serta risk management.
Hasil dari penelitian ini menunjukkan bahwa terdapat 16 faktor-faktor risiko
dominan (dengan kategori risiko sedang dan tinggi) yang dapat mempengaruhi
kinerja mutu produk proyek EPC pada PT XYZ (studi kasus proyek boiler tank),
dengan risiko tertinggi yaitu X13 dan X37.

ABSTRACT
This thesis discuss about the student?s evaluation of quality management system
of detail engineering at EPC project at PT XYZ in the design phase ? based on
risk management. The purpose of this research is to knowing dominant factor
which can influence the quality performance of PT XYZ. This research conducts
by examine each activities in the business processes of PT XYZ. Based on those
activities, student has found the risk factors which influence the quality
performance of detail engineering?s product that has been produced by PT XYZ.
The outcome of this research shown there is 16 factors which can influence the
quality performance (medium and high category) of EPC project?s products by PT
XYZ (case study boiler tank project), and the highest risks are X13 and X37."

"House of Risk (HOR) merupakan integrasi antara dua model penelitian yaitu metode Failure Mode and Effect Analysis (FMEA) dan House of Quality (HOQ) yang berfokus pada penentuan tindakan pencegahan terhadap sumber risiko yang telah tereliminasi. Metode House of Risk (HOR) banyak digunakan untuk melakukan penanganan terhadap permasalahan yang terjadi di berbagai industri. Penanganan risiko sendiri terdiri dari proses identifikasi kejadian, lalu menganalisis kejadian tersebut apakah berpotensi menyebabkan munculnya risiko, dan terakhir menentukan tindakan pencegahan yang tepat dalam menangani risiko tersebut. Pada penelitian ini, House of Risk digunakan sebagai alat untuk menangani risiko terkait pemindahan workshop PT Indopelita Aircraft Service dari Rancaekek ke Pondok Cabe. Penelitian ini diawali dengan mengidentifikasi kejadian risiko yang bisa memberikan pengaruh buruk terhadap proses pemindahan workshop serta menganalisis kejadian untuk mendapatkan informasi tentang sumber risiko. Proses identifikasi dan analisis tersebut dilakukan bersama dengan para ahli yang berpengalaman di bidangnya. Setelah melakukan pengolahan data dengan House of Risk (HOR) tahap 1 maka diperoleh 21 kejadian risiko dan 60 penyebab risiko. Berdasarkan perhitungan Pareto, terdapat 15 agen risiko yang mencakup 80% dari total Aggregate Risk Potential (ARP) dan dipilih sebagai prioritas mitigasi. Penyebab risiko dengan nilai Aggregate Risk Potential (ARP) terbesar adalah pekerja melakukan pekerjaan diluar standar operasional prosedur yang telah ditetapkan dengan nilai sebesar 1872. Lalu pada House of Risk (HOR) tahap 2 diperoleh nilai efektivitas tertinggi yaitu sebesar 15387 yaitu mengumpulkan data-data sebelumnya baik dari buku manual ataupun dari hasil dokumentasi untuk menghindari risiko kehilangan.

House of Risk (HOR) is an integration between two research models, namely the Failure Mode and Effect Analysis (FMEA) method and the House of Quality (HOQ) which focus on determining preventive measures for eliminated risk sources. The House of Risk (HOR) method is widely used to handle problems that occur in various industries. Risk management itself consists of the process of identifying events, then analyzing these events whether they have the potential to cause risks, and finally determining appropriate preventive measures in dealing with these risks. In this study, the House of Risk is used as a tool to deal with risks related to the transfer of the PT Indopelita Aircraft Service workshop from Rancaekek to Pondok Cabe. This research begins by identifying risk events that could adversely affect the process of moving the workshop and analyzing events to obtain information about risk sources. The process of identification and analysis is carried out together with experts who are experienced in their fields. After processing the data with the House of Risk (HOR) stage 1, 21 risk events and 60 risk causes were obtained. Based on Pareto calculations, there are 15 risk agents covering 80% of the total Aggregate Risk Potential (ARP) and are selected as mitigation priorities. The cause of the risk with the largest Aggregate Risk Potential (ARP) value is workers doing work outside the standard operating procedures that have been set with a value of 1872. Then in the House of Risk (HOR) stage 2, the highest effectiveness value was obtained, namely 15387, namely collecting previous data either from manuals or from documentation results to avoid the risk of loss."

"Penelitian ini merupakan pemaparan hasil penelitian kualitatif mengenai penerapan sistem manajemen keselamatan dan kesehatan kerja pada PT SURVEYOR INDONESIA. Penelitian ini bertujuan untuk menganalisis penerapan SMK3 di PTSI. Penelitian ini bersifat deskriptif dan bertujuan untuk mengidentifikasi komitmen, tujuan dan hambatan yang ada di perusahaan.
Hasil dari penelitian ini menunjukkan bahwa secara umum pelaksanaan SMK3 pada PTSI berjalan sesuai ketentuan yang bersumber dari UU dan PP mengenai SMK3 dan dari peraturan yang dibuat oleh perusahaan. Namun, masih terdapat beberapa hal yang perlu diperbaiki seperti dari segi sosialisasi dan peningkatan kesadaran individu akan pentingnya K3.

This research is a presentation of qualitative research on the application of occupational health and safety management systems on PT. SURVEYOR INDONESIA. This study aims to analyze the application of SMK3 in PTSI. This research is a descriptive analysis method and have an aims to identify the commitments, goals and barriers that exist in the company.
The results of this study indicate that in general of the implementation went according PTSI SMK3 on provisions sourced from Law and Regulation of the President of the SMK3 and regulations made by the company. However, there are still some things that need to be fixed such as in terms of socialization and individual awareness of K3.

"Manajemen risiko merupakan proses mengelola risiko agar organisasi dapat mencapai tujuan. Dibutuhkan pondasi yang kuat tentang konsep manajemen risiko sebelum menerapkannya. Penelitian ini akan menganalisis manajemen risiko keselamatan pertambangan di PT HPU site PDU, DMI, KMO, dan MGA berdasarkan Sistem Manajemen Keselamatan Pertambangan Mineral dan Batubara. Penelitian dilakukan dengan wawancara mendalam dan telaah dokumen. Wawancara mendalam dilakukan dengan triangulasi sumber yaitu, sumber data dari pengawas tingkat Project Manager, Superintendent, dan Foreman.
Hasil wawancara dilakukan analisis konten dan dibandingkan dengan dokumen PT HPU berdasarkan SMKP Minerba dilengkapi referensi lain tentang standar manjemen risiko (ISO 31000:2009, AS/NZS 4360:2004, dan ISO 45001:2018). Berdasarkan analisis konten, didapati bahwa interpretasi pengawas di PT HPU tentang manajemen risiko belum sepenuhnya sesuai dengan standar manajemen risiko karena prosedur perusahaan belum mengakomodir seluruh proses manajemen risiko. Oleh karena itu perlu adanya penyusunan prosedur tentang manajemen risiko yang terintegrasi dengan sistem manajemen keselamatan pertambangan perusahaan dan dipahami oleh setiap lini manajemen.

Risk management is a process of managing risk so the organization can achieve its goals. A strong fundamental is necessary for understanding the concept of risk management before it is implemented. This study will analyze mining safety risk management at PT HPU site PDU, DMI, KMO, and MGA based on the Mineral and Coal Mine Safety Management System (SMKP Minerba). The study was conducted by in-depth interviews and document review. In-depth interviews were conducted with data source triangulation, namely, the supervisors from three level: Project Manager, Superintendent, and Foreman.
The results of the interviews were analyzed by its content and compared to PT HPU documents based on Mineral and Coal Mining Safety Management System (SMKP Minerba) and also other risk management standards (ISO 31000: 2009, AS / NZS 4360: 2004, and ISO 45001: 2018) for additional reference. Based on content analysis, it was found that the supervisor's interpretation of PT HPU regarding risk management was not fully in accordance with risk management standards since the company's procedures had not accommodated the entire risk management process. Therefore, it is necessary to formulate procedures for risk management that are integrated with the company's mining safety management system and understood by each line of management.

"Salah satu parameter penting dalam meningkatkan laju produktivitas dan nilai profitabilitas dari sebuah aset adalah dengan mengaplikasikan faktor keselamatan kedalam kendali proses. Selain berfungsi untuk mengurangi potensi terjadinya kecelakaan kerja, meminimalisasi terjadinya potensi shutdown, dan mengurangi tingkat kebocoran zat berbahaya atau terlepasnya material hidrokarbon ke lingkungan, faktor tersebut memiliki peran yang sangat kritikal dalam pengalokasian perangkat keselamatan yang ditujukan secara khusus untuk mereduksi kriteria risiko pada tingkatan yang dapat ditoleransi.
Sebuah industri strategis harus berperan secara aktif dalam menjalankan fungsi pengelolaan dan pengawasan terhadap semua aspek yang berkaitan dengan perencanaan dan pemeliharaan fasilitas produksi atau aset, sesuai dengan regulasi dan pola kebijakan yang mengatur tentang keselamatan dan kesehatan kerja, serta langkah-langkah didalam meminimalisasi potensi risiko, baik secara kualitatif maupun kuantitatif.
Karena penilaian risiko yang terasosiasi kedalam faktor keselamatan dan biaya akan sangat tergantung dari manajemen perencanaan dan pemeliharaan fasilitas industri atau aset yang dilakukan, maka secara obyektif penelitian ini akan mendeskripsikan alternatif pemilihan perangkat Safety Critical Element (SCE) melalui Safety Instrumented System (SIS), yang dikaji melalui penerapan metode Layer of Protection Analysis (LOPA) pada Liquefied Petroleum Gas (LPG) Plant, guna menentukan nilai Safety Integrity Level (SIL), dan melihat korelasi antara Cost Effectiveness dan System/Operational Effectiveness dari perangkat tersebut.

The most important parameters that are used to increase the rate of productivity and profitability of an asset, is to applied safety factor in the process controlling. In addition to reduce the potential of accidents, minimize the potential shutdown, and to reduce the rate of leakage of hazardous substances or hydrocarbons materials release into the environment, these factors have a very critical role in the allocation of safety devices intended specifically to reduce the levels of risk criteria that can be tolerated.
A strategic industry should play an active role in carrying out the functions of management and oversight of all aspects related to the planning and maintenance of production facilities or assets, in accordance with the regulations and policies governing the pattern of occupational safety and health, as well as measures in minimizing potential risks, both qualitatively and quantitatively.
Because the risk assessment that associated with the safety factor and cost will depend on the management of planning and maintenance that practically implemented by the industrial facilities or assets, then the objective of this study will describe the alternative selection of the Safety Critical Element (SCE) through the Safety Instrumented System (SIS), which assessed through the application of the Layer of Protection Analysis (LOPA) methods in the Liquefied Petroleum Gas (LPG) plant, in order to determine the values of Safety Integrity Level (SIL), and the correlation between the Cost Effectiveness and System/Operational Effectiveness of the device.
