"Pengkajian resep pasien yang berasal dari Program Rujuk Balik di Apotek Kimia Farma 143 Margonda selama periode bulan Maret 2023 bertujuan untuk menganalisis pola penggunaan obat, kepatuhan pasien terhadap resep, dan efektivitas program rujuk balik dalam mendukung pelayanan farmasi yang optimal. Metode penelitian yang digunakan adalah observasi terhadap resep-resep pasien yang diambil dari program rujuk balik, analisis data resep terhadap kesesuaian pemberian pengobatan sesuai dengan jenis penyakit pasien. Hasil penelitian menunjukkan bahwa seluruh resep secara keseluruhan telah tepat dan memenuhi persyaratan dari hasil pengkajian resep, dan sesuai dalam pola penggunaan obat oleh pasien yang mendapatkan resep melalui program rujuk balik. Selain itu, ditemukan beberapa faktor yang memengaruhi peresepan obat kepada pasien, seperti informasi penunjang pemahaman terhadap petunjuk penggunaan obat, efek samping, dan ketersediaan obat. Dalam penulisan resep oleh dokter, masih banyak informasi yang kurang atau belum ditambahkan sehingga apoteker perlu mengkonfirmasi terkait informasi obat kepada dokter penulis resep agar tidak terjadi kesalahan dalam pemberian obat dan informasi obat kepada pasien. Namun, secara keseluruhan hasil pengkajian resep pada pasien dalam program rujuk balik mengindikasikan kesesuaian terapi dalam pelayanan kefarmasian. Temuan ini memberikan kontribusi penting dalam pengembangan strategi yang lebih efektif untuk meningkatkan pelayanan farmasi, meningkatkan kepatuhan pasien, dan memastikan penggunaan obat yang aman dan optimal. Implikasi praktis dari penelitian ini dapat membantu Apotek Kimia Farma 143 Margonda dalam mengoptimalkan program rujuk balik serta meningkatkan kerjasama antara apotek dan pemberi layanan kesehatan untuk mencapai hasil terbaik bagi pasien.

The study of patient prescriptions from the Referback Program at Kimia Farma 143 Margonda Pharmacy during the period March 2023 aims to analyze drug use patterns, patient compliance with prescriptions, and the effectiveness of the referral program in supporting optimal pharmaceutical services. The research method used is observation of patient prescriptions taken from the referral program, analysis of prescription data on the appropriateness of providing treatment according to the type of patient's disease. The results of the study showed that all prescriptions as a whole were appropriate and met the requirements of the prescription review results, and were in accordance with the pattern of drug use by patients who received prescriptions through the referral program. In addition, several factors were found that influence drug prescribing to patients, such as information supporting understanding of drug use instructions, side effects, and drug availability. When writing prescriptions by doctors, there is still a lot of information that is missing or has not been added, so pharmacists need to confirm the drug information with the prescribing doctor so that there are no errors in administering drugs and drug information to patients. However, overall results of the review of prescriptions in patients in the referral program indicate the suitability of therapy in pharmaceutical services. These findings provide an important contribution to the development of more effective strategies to improve pharmaceutical care, increase patient compliance, and ensure safe and optimal medication use. The practical implications of this research can help Kimia Farma 143 Margonda Pharmacy optimize the referral program and increase collaboration between pharmacies and health service providers to achieve the best results for patients."

"Penyakit Paru Obstruksi Kronik (PPOK) merupakan penyakit yang ditandai dengan kelainan pada alveolar yang disebabkan oleh paparan signifikan terhadap partikel atau gas berbahaya sehingga mengakibatkan penyumbatan pada paru-paru dan keterbatasan aliran udara. Dalam upaya pencegahan dan pengobatan PPOK, apoteker berperan peting dalam pelayanan farmasi klinik yang berkaitan dengan sediaan farmasi untuk meningkatkan kualitas hidup pasien. Pengkajian resep merupakan salah satu pelayanan farmasi klinik yang dapat dilakukan oleh apoteker untuk menganalisa masalah terkait obat serta mencegah terjadinya kesalahan pemberian obat. Penelitian ini bertujuan untuk mengkaji aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis, serta kerasionalan 5 resep pasien paru di Apotek Kimia Farma Raya Bogor periode April 2023. Penelitian ini dilakukan dengan pengumpulan dan pengkajian resep pasien paru pada bulan april 2023. Hasil penelitian menunjukkan kelima resep pasien paru di Apotek Kimia Farma Raya Bogor telah memenuhi aspek administratif, aspek kesesuaian farmasetik, aspek pertimbangan klinis, dan resep rasional.

Chronic Obstructive Pulmonary Disease (COPD) is a disease characterized by alveolar abnormalities caused by significant exposure to harmful particles or gases, resulting in blockage of the lungs and limited air flow. In efforts to prevent and treat COPD, pharmacists play an important role in clinical pharmacy services related to pharmaceutical preparations to improve the quality of life of patients. Prescription review is one of the clinical pharmacy services that can be carried out by pharmacists to analyze drug-related problems and prevent medication administration errors. This research aims to examine administrative aspects, pharmaceutical suitability and clinical considerations, as well as the rationale for 5 prescriptions for pulmonary patients at Kimia Farma Raya Bogor for the period April 2023. This research was conducted by collecting and reviewing pulmonary patient prescriptions in April 2023. The research results showed that the five prescriptions for pulmonary patients at Kimia Farma Raya Bogor Pharmacy had fulfilled administrative aspects, pharmaceutical suitability aspects, clinical consideration aspects, and rational prescriptions."

Apotek adalah sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Standar Pelayanan Kefarmasian di Apotek meliputi standar pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi klinik. Salah satu pelayanan farmasi klinik di apotek adalah pengkajian resep. Pengkajian resep meliputi kajian administratif, farmasetik, dan klinis. Hal-hal yang termasuk dalam kajian klinis diantaranya yaitu ketepatan indikasi dan dosis obat, aturan, cara dan lama penggunaan obat, duplikasi dan atau polifarmasi, reaksi obat yang tidak diinginkan, kontra indikasi, dan interaksi. kajian klinis ini perlu dilakukan agar pengobatan untuk pasien tepat sehingga target terapi pasien dapat tercapai. Oleh karena itu, penelitian ini dilakukan untuk melakukan pengkajian klinis resep di Apotek Kimia Farma Siliwangi Cirebon bulan September 2022, diantaranya yaitu resep geriatri, pediatri, narkotika, psikotropika, dan polifarmasi. Melalui tugas khusus ini diketahui pada beberapa resep geriatri, pediatri, narkotika, dan psikotropika secara klinis untuk indikasi, dosis, waktu dan cara pemberian sudah sesuai, namun terdapat beberapa interaksi obat sehingga perlu dilakukan monitoring dan penyesuaian dosis, dan jika terdapat antibiotik pada resep disarankan tidak diracik bersama dengan obat lainnya. Untuk resep polifarmasi sebaiknya diperhatikan kembali karena polifarmasi dapat meningkatkan risiko terjadinya interaksi obat dan mungkin juga terdapat duplikasi obat.

One of the clinical pharmacy services in a pharmacy is prescription review. Prescription reviews include administrative, pharmaceutical, and clinical reviews. Things included in clinical studies such as the accuracy of drug indications and dosages, rules, method and duration of drug use, duplication and/or polypharmacy, unwanted drug reactions, contraindications, and interactions. This clinical study needs to be carried out so that the treatment for the patient is accurate so that the patient's therapeutic target can be achieved. Therefore, this research was conducted to obtain a clinical review of prescriptions at the Kimia Farma Siliwangi Pharmacy Cirebon in September 2022, including geriatric, pediatric, narcotics, psychotropic, and polypharmacy prescriptions. Through this special assignment it is known that several geriatric, pediatric, narcotics and psychotropic prescriptions are clinically appropriate for indications, dosage, time and method of administration, but there are several drug interactions so it is necessary to do monitoring and adjust doses, and if there is an antibiotic in the prescription it is recommended not mixed with other drugs. For polypharmacy prescriptions, must be reconsidered because polypharmacy can increase the risk of drug interactions and there may also be drug duplication.

"Pelayanan resep merupakan salah satu kegiatan pelayanan kefarmasian yang berupa kegiatan menyiapkan permintaan tertulis (resep) dari dokter, dokter gigi dan dokter hewan kepada apoteker untuk menyediakan dan menyerahkan obat yang tepat kepada pasien dan sesuai dengan peraturan yang berlaku. Diabetes melitus merupakan penyakit gangguan metabolisme yang menyumbangkan angka kematian tertinggi ketiga di Indonesia. Menurut analisis yang dilakukan terhadap beberapa resep pasien program rujuk balik penderita diabetes, dapat disimpulkan bahwa bentuk ketidakrasionalan resep PRB pada pasien diabetes melitus di Apotek Kimia Farma Pos Pengumben berupa duplikasi obat, terjadinya interaksi obat, dan kurang spesifiknya cara pemberian obat pada resep.

Data yang diambil adalah sebanyak lima resep pasien penyakit kronis RS Bhakti Yudha Depok selama bulan Oktober 2023 di Apotek Kimia Farma 389 yang diambil secara acak.Pada resep pasien ditemukan beberapa masalah terkait obat pada aspek administratif seperti paraf/tanda tangan dokter, berat badan pasien, dan alamat pasien. Masalah terkait obat padapertimbangan klinis seperti dosis obat yang kurang dan interaksi antara obat yang perlu diperhatikan. Masalah tersebut dapat diatasi dengan mengatur jadwal konsumsi obat pasien dan mengkonsultasikan kembali kepada dokter terkait pemberian obat yang kurang dosis.Perlu dilakukan pengkajian resep secara rutin untuk menjamin rasionalitas dankesesuaian dosis obat yang akan diterima pasien dan meminimalisir terjadinya kesalahan medikasi (medication error) dan masalah terkait obat.

Reviewing patient prescriptions is important in maintaining the quality of pharmaceutical services and preventing medication errors. Data was obtained by classifying prescriptions received at Kimia Farma 389 Pharmacy based on the date theprescription was received and the patient's hospital origin. The data taken were five prescriptions from patients withchronic diseases at Bhakti Yudha Hospital, Depok during October 2023 at Kimia Farma 389 Pharmacy which weretaken randomly. In the patient prescriptions, several drug-related problems were found in administrative aspects such as the doctor's initials/signature, the patient's weight, and patient address. Drug-related problems in clinical considerations such as insufficient drug dosage and interactions between drugs that need to be considered. This problem can be overcome by arranging the patient's medication consumption schedule and consulting the doctor again regarding administering insufficient doses of medication. It is necessary to review prescriptions regularly to ensure the rationality and suitability of the dosage of medication that the patient will receive and minimize the occurrence of medication errors and problems. drug related."

"Pengkajian resep adalah hal yang perlu dilakukan untuk melihat apakah pengobatan yang dilakukan bersifat rasional. Hal ini perlu diperhatikan khususnya pada pasien jantung koroner dan hipertensi karena pasien umumnya mengkonsumsi jumlah obat yang cukup banyak. Pengkajian resep dilakukan di Apotek Kimia Farma 348 Dramaga pada periode bulan Januari – Maret 2023. Telaah dilakukan dengan memperhatikan tiga aspek, yaitu telaah administratif, farmasetik, dan klinis pada 20 resep. Berdasarkan hasil telaah yang dilakukan, didapatkan hasil, yaitu pada aspek telaah administratif, terdapat beberapa syarat yang belum terpenuhi, seperti kurangnya informasi mengenai jenis kelamin, berat badan, dan usia pasien yang dapat diatasi dengan melakukan konfirmasi kembali pada rumah sakit dan dokter atau pasien yang bersangkutan; pada aspek telaah farmasetik memenuhi seluruh syarat; dan pada aspek telaah klinis terdapat beberapa syarat yang belum terpenuhi, seperti adanya polifarmasi dan kemungkinan interaksi obat. Permasalahan ini dapat diatasi dengan memberikan edukasi pada pasien untuk memberikan jeda konsumsi obat serta memberikan informasi mengenai pentingnya ketepatan waktu konsumsi obat untuk mendapatkan efek terapi yang maksimal.

A prescription review was carried out at Kimia Farma 348 Dramaga Pharmacy in the period January - March 2023. The review was carried out by paying attention to three aspects, namely administrative, pharmaceutical and clinical reviews on 20 recipes. Based on the results of the review carried out, the results were obtained, namely the aspect of administrative review, there were several conditions that had not been fulfilled, such as a lack of information regarding the patient's gender, weight, and age which could be overcome by confirming to the hospital, doctor, or patient; pharmaceutical review aspect meets all requirements; and in clinical aspect there are several requirements that have not been fulfilled, such as the existence of polypharmacy and the possibility of drug interactions. This problem can be overcome by educating patients to pause in taking drugs and providing information about the importance of taking medication on time to get the maximum therapeutic effect."

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.

Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Perencanaan merupakan tahapan yang penting karena faktor perencanaan obat yang baik dapat mencegah terjadinya kekurangan dan kelebihan stok obat, serta pemborosan anggaran. Perencanaan merupakan kegiatan penentuan penyusunan daftar kebutuhan obat (jenis dan jumlah) sesuai dengan kebutuhan dan anggaran, sebelum dilakukannya proses pengadaan. Terdapat tiga metode yang dapat digunakan untuk perhitungan kebutuhan pada perencanaan, diantaranya Metode Konsumsi, Metode Morbiditas dan Metode Proxy Consumption. Pada tahap perencanaan juga terdapat analisis atau evaluasi rencana kebutuhan sediaan farmasi yang bertujuan untuk menjamin ketersediaan obat dan efesiensi anggaran. Analisis perencanaan pengadaaan obat tersebut dapat dilakukan dengan berbagai metode, yaitu Analisis ABC, Analisis VEN, dan Analisis Kombinasi. Diantara ketiga metode tersebut, metode analisis ABC pareto merupakan metode evaluasi perencanaan perbekalan farmasi di apotek yang sering digunakan. Analisis ABC Pareto digunakan untuk mengetahui prioritas item yang digunakan di apotek dengan melihat persentase kumulatif dari jumlah pemakaian (nilai pakai), persentase kumulatif dari jumlah investasi (nilai investasi), serta nilai indeks kritis (melalui skor total dari nilai pakai dan nilai investasi). Analisis ABC Pareto dapat membantu Apoteker untuk merencanakan obat- obat atau barang dengan nilai ekonomi paling menguntungkan, sehingga dapat memperoleh keuntungan lebih banyak dan lebih cepat dengan penjualan obat- obat tersebut.

Procurement is the activity of determining the preparation of a list of drug requirements (type and quantity) in accordance with needs and budget, before the procurement process is carried out. There are three methods that can be used to calculate needs in procurement, including the Consumption Method, Morbidity Method and Proxy Consumption Method. At the planning stage there is also an analysis or evaluation of the planned need for pharmaceutical supplies which aims to ensure drug availability and budget efficiency. Analysis of drug procurement planning can be carried out using various methods, namely ABC Analysis, VEN Analysis and Combination Analysis. Among these three methods, the ABC Pareto analysis method is a method for evaluating pharmaceutical supply planning in pharmacies that is often used. ABC Pareto analysis is used to determine the priority of items used in pharmacies by looking at the cumulative percentage of the amount of use (use value), the cumulative percentage of the investment amount (investment value), as well as the critical index value (through the total score of use value and investment value). ABC Pareto analysis can help pharmacists to plan medicines or goods with the most profitable economic value, so that they can gain more and faster profits from selling these medicines."
