"Latar Belakang
COVID-19 adalah sebuah infeksi virus yang memiliki angka mortalitas yang sangat tinggi dalam periode waktu yang sangat sempit. Infeksi COVID-19 akan menyebabkan inflamasi di jaringan paru tubuh yang dapat diukur dengan serum D-dimer. Keparahan gejala dari infeksi COVID-19 dapat dilihat dalam gejala klinis dan keterlibatan segmen paru pada gambaran CT-Scan. Penelitian ini bertujuan untuk mengetahui hubungan antara level D-dimer dengan keparahan klinis pasien COVID-19 serta dengan keterlibatan lobus paru pada gambaran CT-Scan pasien COVID-19
Metode
Penelitian ini menggunakan metode potong lintang retrospektif dengan uji korelasi gamma untuk melihat hubungan antara level d-dimer dengan keparahan klinis dan keparahan gambaran CT-Scan pada pasien COVID-19. Penelitian ini menggunakan data sekunder dari penelitian “Implementasi Pencitraan Diagnostik Foto Toraks dan CT- Scan Toraks Coronavirus Disease 2019 (COVID-19) Menggunakan Artificial Intelligence” oleh Kamelia T, dkk. di tiga rumah sakit (Rumah Sakit Bunda Menteng, Rumah Sakit Bunda Depok, dan Rumah Sakit dr. Cipto Mangunkusumo) dalam periode 2020-2024, dimana terdapat 50 subjek yang melakukan pemeriksaan laboratorium (D- dimer) dan pemeriksaan CT-Scan.
Hasil
Didapatkan 50 subjek dimana terdapat 25 (50%) subjek yang mengalami peningkatan level D-dimer dan 25 (50%) subjek dengan level D-dimer normal. Uji korelasi antara level D-dimer dengan keparahan klinis memiliki nilai P <0,001 dengan koefiesien korelasi sebesar 0,806. Uji korelasi antara level D-dimer dengan keparahan CT-Scan memiliki nilai P 0,012 dengan koefisien korelasi sebesar 0,528. Koefisien korelasi yang dianggap bermakna pada penelitian ini adalah 0,4.
Kesimpulan
Level D-dimer memiliki korelasi sangat kuat dengan keparahan klinis serta korelasi sedang dengan keparahan CT-Scan, sehingga dapat menjadi biomarker potensial untuk menentukan tingkat keparahan gejala klinis pasien dan keparahan gambaran CT-Scan thoraks pasien COVID-19.

Introduction
COVID-19 is a viral infection associated with a high mortality rate within a relatively short period. The infection triggers inflammation in lung tissues, which can be measured using serum D-dimer levels. The severity of COVID-19 symptoms can be assessed through clinical manifestations and the extent of pulmonary segment involvement observed on CT-Scan imaging. This study aims to determine the correlation between D- dimer levels and the clinical severity of COVID-19, as well as the involvement of pulmonary lobes on CT-Scan images of COVID-19 patients.
Method
This study employed a retrospective cross-sectional method with a gamma correlation test to examine the relationship between D-dimer levels, clinical severity, and CT-Scan severity in COVID-19 patients. This study utilized secondary data from the research “Implementation of Diagnostic Imaging of Chest X-rays and CT-Scans for Coronavirus Disease 2019 (COVID-19) Using Artificial Intelligence” by Kamelia T, et al., conducted at three hospitals (Bunda Menteng Hospital, Bunda Depok Hospital, and Dr. Cipto Mangunkusumo Hospital) from 2020 to 2024, involving 50 subjects who underwent laboratory (D-dimer) and CT-Scan examinations.
Results
A total of 50 subjects were included, with 25 (50%) showing elevated D-dimer levels and 25 (50%) with normal D-dimer levels. The correlation test between D-dimer levels and clinical severity showed a p-value <0.001 with a correlation coefficient of 0.806. The correlation test between D-dimer levels and CT-Scan severity yielded a p-value of 0.012 with a correlation coefficient of 0.528. A correlation coefficient of 0.4 or higher was considered significant in this study.
Conclusion
D-dimer levels can serve as a potential biomarker for determining the severity of clinical symptoms and the severity of thoracic CT-Scan findings in COVID-19 patients.

Pandemi COVID-19, yang disebabkan oleh SARS-CoV-2, berdampak global, termasuk di Indonesia, dengan komplikasi utama seperti trombosis terkait peningkatan kadar D- dimer, biomarker pembekuan darah. Kadar D-dimer yang tinggi dikaitkan dengan prognosis buruk, seperti risiko trombosis, lama rawat inap yang lebih lama, dan kematian. Penelitian ini bertujuan mengeksplorasi hubungan antara kadar D-dimer, tingkat keparahan penyakit, dan lama rawat pasien COVID-19 di RSUP Persahabatan.
Metode
Penelitian ini bersifat observasional-analitis dengan desain potong lintang pada pasien COVID-19 derajat sedang, berat, dan kritis di RSUP Persahabatan, Jakarta, selama Januari–Desember 2021. Data dikumpulkan dari rekam medis dan dianalisis dengan metode consecutive sampling, melibatkan 57 sampel. Penelitian mencakup pengukuran kadar D-dimer, tingkat keparahan penyakit, dan lama rawat inap. Analisis data dilakukan menggunakan SPSS 25.0 dengan uji univariat dan bivariat, termasuk uji Chi-square atau Fisher’s exact untuk memeriksa hubungan antarvariabel.
Hasil
Hasil penelitian ini menunjukkan bahwa tidak ada hubungan signifikan antara kadar D- dimer awal dengan tingkat keparahan COVID-19 (p=0,364), meskipun subjek dengan kadar D-dimer ≥500 ng/mL lebih banyak mengalami kondisi sedang dan berat. Di sisi lain, kadar D-dimer yang lebih tinggi (≥500 ng/mL) berhubungan dengan durasi rawat inap yang lebih lama (≥14 hari) dengan p=0,044, menunjukkan perbedaan signifikan antara kadar D-dimer dan lama rawat inap pasien COVID-19.
Kesimpulan
Penelitian ini menunjukkan bahwa tidak ada hubungan signifikan antara kadar D-dimer awal dengan tingkat keparahan COVID-19 (p=0,364), namun kadar D-dimer ≥500 ng/mL berhubungan dengan durasi rawat inap yang lebih lama (≥14 hari) (p=0,044).

Introduction
The COVID-19 pandemic, caused by SARS-CoV-2, has had a global impact, including in Indonesia, with major complications such as thrombosis associated with elevated D- dimer levels, a biomarker of blood coagulation. High D-dimer levels are linked to poor prognosis, including the risk of thrombosis, prolonged hospital stays, and mortality. This study aims to explore the relationship between D-dimer levels, disease severity, and length of hospitalization in COVID-19 patients at RSUP Persahabatan.
Method
This study is an observational-analytical research with a cross-sectional design conducted on COVID-19 patients with moderate, severe, and critical conditions at RSUP Persahabatan, Jakarta, from January to December 2021. Data were collected from medical records and analyzed using consecutive sampling methods, involving 57 samples. The study includes measurements of D-dimer levels, disease severity, and length of hospitalization. Data analysis was performed using SPSS 25.0 with univariate and bivariate tests, including Chi-square or Fisher’s exact tests to examine the relationships between variables.
Results
The results of this study indicate that there is no significant relationship between initial D-dimer levels and the severity of COVID-19 (p=0.364), although subjects with D-dimer levels ≥500 ng/mL were more likely to experience moderate and severe conditions. On the other hand, higher D-dimer levels (≥500 ng/mL) are associated with longer hospital stays (≥14 days) with a p-value of 0.044, indicating a significant difference between D- dimer levels and the duration of hospitalization in COVID-19 patients.
Conclusion
This study shows that there is no significant relationship between initial D-dimer levels and COVID-19 severity (p=0.364), but D-dimer levels ≥500 ng/mL are associated with longer hospital stays (≥14 days) (p=0.044).

"Latar belakang. Coronavirus disease-2019 (COVID-19) memiliki spektrum penyakit yang sangat luas dari gejala ringan sampai berat, hingga kematian. Reaksi inflamasi berat akibat dari COVID-19 ini menimbulkan gangguan hemostasis yang disebut dengan COVID-19 associated coagulopathy. Penelitian ini bertujuan untuk menilai profil koagulasi pada pasien dalam pemantauan (PDP) ataupun terkonfirmasi COVID-19 serta hubungannya terhadap mortalitas 30-hari pasien. Metode. Studi ini merupakan studi kohort retrospektif di RS Cipto Mangunkusumo (RSCM) selama Maret 2020 hingga Juni 2020. Sebanyak 106 subjek yang sesuai kriteria inklusi dianalisis dari data rekam medis. Dilakukan pengambilan data berupa data demografik, klinis atau hemodinamik pasien, profil koagulasi saat subjek ditentukan sebagai PDP atau terkonfirmasi COVID-19, pemberian terapi tromboprofilaksis heparin, dan status mortalitas 30 hari setelah admisi. Perhitungan statistik dilakukan dengan menggunakan Statistical Package of Social Science (SPSS) versi 24.0. Profil koagulasi subjek penyintas 30 hari dibandingkan dengan subjek yang mengalami mortalitas. Variabel profil koagulasi yang bermakna kemudian dianalisis dengan analisis bivariat dan regresi logistik multivariat. Hasil. Pada kelompok yang mengalami mortalitas 30-hari ditemukan adanya peningkatan jumlah leukosit (p: 0,022), penurunan kadar trombosit (p: 0,016), dan waktu protrombin (PT) dan waktu activated partial thromboplastin time (APTT) yang lebih panjang (p: 0,002 dan p: 0,018) dibandingkan pada kelompok penyintas 30-hari. Tidak ditemukan perbedaan fibrinogen dan d-Dimer yang bermakna secara statistik. PT merupakan suatu profil koagulasi tunggal yang dapat digunakan sebagai prediktor mortalitas 30-hari dengan odds ratio (95% CI) sebesar 1,407 (1,072 – 1,846), nilai p: 0,014.
Simpulan. Terdapat hubungan antara faktor koagulasi pasien COVID-19 dengan mortalitas 30 hari di RSCM, khususnya PT yang dapat digunakan sebagai prediktor mortalitas 30-hari.

Background. Coronavirus disease-2019 (COVID-19) has a very broad spectrum of disease from mild to severe symptoms, to death. The severe inflammatory reaction as a result of COVID-19 infection causes a hemostasis disorder called COVID-19 associated coagulopathy. This study aims to assess the coagulation profile of patients under monitoring (PDP) or confirmed COVID-19 and its relationship with 30-day mortality.
Method. This retrospective cohort study was conducted at RS Cipto Mangunkusumo (RSCM) from March 2020 to June 2020. A total of 106 subjects who met the inclusion criteria were analyzed from medical record data. Data were collected in the form of patient demographic, clinical or hemodynamic data, coagulation profile when the subject was determined as PDP or confirmed as COVID-19, administration of heparin thromboprophylaxis therapy, and mortality status 30 days after admission. Statistical calculations were performed using the Statistical Package of Social Science (SPSS) version 24.0. We compared the coagulation profiles of the survivor group in contrast to the non-survivor group. Significant coagulation profile variables were analyzed using bivariate analysis and multivariate logistic regression.
Results. There was elevated number of leukocytes (p: 0.022), reduced platelet levels (p: 0.016), and longer prothrombin time (PT) as well as activated partial thromboplastin time (APTT) (p: 0.002 and p: 0.018, consecutively) in non-survivor group. There were no statistical differences in fibrinogen and d-Dimer levels in both groups. Additionally, PT is a single coagulation profile which predicted 30- day mortality with an odds ratio (95% CI) of 1.407 (1.072 - 1.846), and p value: 0.014.
Conclusion. This present study shows abnormal coagulation results are associated with 30-day mortality in COVID-19 patients at RSCM. Prolonged PT was an independent predictor for 30-day mortality in COVID-19 patients.

"CT-scan abdomen merupakan opsi dalam penegakan diagnosis terkait dengan dugaan penyakit yang diderita pasien menggunakan radiasi pengion, namun resiko radiasi pada organ sensitif di sekitar area abdomen dapat menimbulkan kekhawatiran tersendiri. Maka dari itu, dilakukan penelitian untuk menunjukkan seberapa besar dosis radiasi yang diterima organ sensitif (gonad, payudara, tiroid dan mata) pada pelaksanaan pemeriksaan CT abdomen, dengan fantom rando sebagai objek pemeriksaan dan TLD rod sebagai penangkap radiasi. Pelaksanaan pemeriksaan dilakukan dengan memvariasikan kV (80,120 dan 140) dan nilai pitch (4,6 dan 8). Dosis radiasi terbesar didapatkan pada gonad dengan 7,67 mGy dan terendah pada tiroid kanan dengan 0,01 mGy. Sehingga, dapat disimpulkan bahwa pemeriksaan CT-scan abdomen tidak menimbulkan efek langsung.

CT-scan for abdomen area is an examination option in diagnosis that related to patient’s disease, but the radiation risk that appears on sensitive organs near abdomen area need to be concern. Therefore, a research was done to show how much radiation dose for organs received (gonad, breasts, thyroids, and eyes) in CT-scan examination for abdomen, using rando phantom as an object and TLD rod as dosemeter. The variation of examination was done for kV (80, 120, and 140) and pitch (4, 6, and 8). The result show that gonad had received the highest radiation dose with 7,674 mGy (tube’s voltage was 140 kV, pitch 6). So, it can be concluded that examination with CT-scan did not give deterministic effect to sensitive organs."

"Tujuan penelitian ini adalah untuk melakukan pengukuran CTDI dan dosis efektif pada 3 mesin CT dengan pengujian abdominal terhadap pasien pediatrik dan dewasa menggunakan phantom CT yang dibuat dan detector pensil CT serta electrometer RADCAL. Phantom CT dibuat berdasarkan standar US-FDA untuk CT. Pengujian Computed Tomography (CT) yang dilakukan menggunakan 2 CT phantom yang berbeda, diameter 32 cm (pasien dewasa) dan diameter 16 cm (pasien pediatrik). Pengukuran CTDI dan perhitungan dosis efektif radiasi menunjukkan bahwa terdapat dosis pasien pediatrik yang lebih tinggi signifikan dibandingkan dengan dosis pasien dewasa pada perlakuan yang sama. Hal ini menandakan pentingnya penentuan secara hati-hati parameter teknis scan dan justifikasi yang kuat terhadap penggunaan pengujian CT pada pasien pediatrik.

A study has been performed to measure CTDI and effective dose on 3 CT machines pertinent to abdominal examination to adult and pediatric patients using fabricated CT phantom and RADCAL pencil detector and electrometer. The CT phantom was tailored according US-FDA standard. Computed tomographic (CT) examinations have been performed using two different CT phantoms, 32 cm (adult) and 16 cm (pediatric) diameter. Computed tomographic (CT) examinations and radiation effective dose showed significant higher pediatric dose as compared to adult patient dose at the same examinations. This indicates the importance of careful selection of technical scan parameters and strong justification of the use of CT examination on pediatric patient."

"Citra Cone Beam CT (CBCT) sangat berperan dalam menentukan keberhasilan verifikasi posisi pasien radioterapi, oleh karena itu jaminan kualitas sistem CBCT sangat diperlukan. Percobaan ini dilakukan dengan menggunakan pesawat Linear accelerator yang dilengkapi dengan CBCT dan CT Simulator GE Bright Speed Edge. Fantom Catphan® 600 dan CBCT Electron DensityTM digunakan untuk menilai kualitas dari citra CBCT dan linearitas CT Number. Sesuai dengan uji kualitas, citra pada CBCT hanya dapat membedakan kontras rendah dan kontras tinggi (udara, jaringan dan tulang).
Hasil uji ketebalan slice menunjukkan nilai yang didapat masih dalam batas toleransi ±0.5 mm. Pada uji kontras rendah bagian supra-slice untuk target kontras 1%, 0.5%, dan 0.3% nilai konstantanya sebesar 3, 2.5, dan 4.5, sedangkan pada bagian sub-slice untuk target kontras jarak 7, 5, dan 3 mm memiliki nilai konstanta 5 mm. Hasil pengujian resolusi tinggi pada CBCT dan CT Simulator adalah 3 lp/cm dan 7 lp/cm. Hasil pengujian uniformitas pada CBCT tidak memenuhi standar dari batas toleransi rata-rata CT Number tepi dan tengah kurang dari 5 HU, dan nilai setiap titik tepi dan tengah ±2 HU.

Cone Beam Computed Tomography (CBCT) image is very important in verification of patient positioning in the treatment couch radiotherapy machine so quality control of the system is required. The experiment was performed using the Linear accelerator with equipped with CBCT and CT simulator GE Bright Speed Edge. Catphan® 600 and CBCT Electron DensityTM phantom was used to evaluate the quality of CBCT and CT Number linearity. According to the image quality test, the CBCT image only be able to distinguish low contrast and high contras for air, tissue and bone.
Quantitavely, the slice thickness was in tolerance limit ±0.5 mm, low contrast with constant value of 3, 2.5, dan 4.5 for supra-slice contrast targets 1%, 0.5%, dan 0.3% whereas sub-slice targets axis lenghts for 3, 5, and 7 mm with constant value of 5 mm, the high resolution appear in 3 lp/cm and 7 lp/cm for CBCT and CT simulator, respectively. On the one hand, CBCT uniformity was out of tolerance limit with average CT number edge and central less than 5 HU, and ±2 HU for the edge and center point.

"Latar Belakang: Penggunaan heparin inhalasi pada beberapa penelitian COVID-19
memberikan hasil dalam perbaikan klinis pasien, baik dalam menurunkan lama rawat,
perbaikan oksigenasi paru, dan mortalitas. Dosis harian total heparin inhalasi yang
bervariasi terutama bila diberikan bersamaan dengan antikoagulan sistemik, memiliki
resiko komplikasi perdarahan yang memerlukan kajian terhadap keefektifan dan
keamanannya.
Tujuan: Meneliti keefektifan dan keamanan inhalasi heparin dosis 150,000 IU/hari
dengan dosis 100.000 IU/hari dinilai dari AaDO2, aPTT dan d-Dimer dalam 7 hari
pengamatan pada pasien ICU COVID-19.
Metode: Penelitian ini merupakan penelitian observasional dengan studi kohort
retrospektif menggunakan data sekunder rekam medis pasien ICU COVID-19 bulan
September 2020 – September 2021. Terdapat 300 sampel menggunakan consecutive
sampling. Pasien dikelompokkan menjadi kelompok heparin dosis 150.000 IU/hari dan
100.000 IU/hari. Pencatatan dilakukan dalam 7 hari pengamatan. Uji Statistik
menggunakan uji Mann Whitney untuk menilai tingkat keparahan, Uji Wilcoxon rank test
untuk melihat perbedaan variabel dependen hari pertama dengan hari ketujuh pada
masing-masing dosis.
Hasil: Heparin inhalasi baik dosis 150.000 IU/hari dan 100.000 IU /hari bermakna
menurunkan AaDO2 pada 7 hari pengamatan (p 0.001). Nilai aPTT tidak memanjang
pada kedua kelompok, dan kedua dosis heparin sama- sama menurunkan nilai d-Dimer
pada 7 hari pengamatan (p 0.001).
Simpulan: Heparin Inhalasi dosis 150.000 IU/hari sama efektif dinilai dari AaDO2, dan
sama amannya terhadap aPTT dan d-Dimer dibandingkan heparin inhalasi dosis 100.000
IU/hari.

Rationale: The use of inhaled heparin in several COVID-19 studies has resulted in
clinical improvements in patients, both in reducing length of treatment, improving
pulmonary oxygenation, and reducing mortality. The varying total daily dose of inhaled
heparin, especially when given together with systemic anticoagulants, poses a risk of
bleeding complications that require review of its effectiveness and safety.
Objective: To analyze effectiveness and safety of heparin inhalation dosage 150,000
IU/day compare to 100,000 IU/day assessed from AaDO2, aPTT and d-Dimer from 7
days observation in ICU COVID-19 patients with invasive and non-invasive ventilator
patterns.
Methods: An observational cohort retrospective study used secondary data from medical
records ICU COVID-19 patients with invasive and noninvasive ventilator patterns from
September 2020 – September 2021. There were 300 samples using consecutive sampling.
Patients divided into 2 groups, one received dosage 150,000 IU / day heparin inhalation,
the other received heparin inhalation dosage 100,000 IU / day. Recording of the research
from medical records is carried out at 7 days of ICU treatment. Statistical tests were
carried out using Mann Whitney to assess severity, Wilcoxon rank test to see the
difference in dependent variables day 1 and day 7 to dose.
Measurements and Main Results: Heparin inhalation at dose of 150,000 IU/day and
100,000 IU/day both significantly decreased AaDO2 at 7 days of observation (p 0.001).
The aPTT on both groups at 7 days of observation are within normal limits. Both doses
of heparin inhalation decreased d-dimer at 7 days of observation (p 0.001).
Conclusion: Inhaled heparin doses of 150,000 IU/day as effective and as safe as inhaled
heparin doses of 100,000 IU/day.

"Pemeriksaan yang paling tepat dalam menentukan volume jaringan lemak visceral dilakukan dengan menggunakan modalitas CT-scan. Namun karena setiap slice citra memiliki bentuk dan lokasi lemak visceral yang berbeda-beda, maka penentuan volume menjadi tidak mudah. Sehingga, Computer Aided Diagnosis (CAD) dapat dijadikan salah satu solusi untuk membantu tenaga ahli dalam pembacaan citra dan menganalisa area lemak terutama area lemak visceral pada citra abdomen dengan lebih akurat. Dalam penelitian ini, sistem CAD dikembangkan dengan menggunakan metode segmentasi Thresholding, ekstrasi ciri berbasis Gray Level Co-Occurrence Matrix (GLCM) dan klasifikasi citra lemak visceral menggunakan Multilayer Perceptron (MLP). Penelitian ini mengolah data 665 citra CT-scan abdomen dari 38 pasien yang diperoleh dari Rumah Sakit Persahabatan Jakarta. Data tersebut dibagi menjadi 70% citra sebagai data pelatihan dan 30% citra sebagai data pengujian. Hasil performa sistem CAD yang direpresentasikan sebagai tingkat keakurasian dengan nilai sebesar 98.73% untuk data pelatihan dan 95.58% untuk data pengujian. Selain itu, juga diperoleh informasi bahwa hasil kalkulasi volume area jaringan lemak visceral dengan nilai terbesar yaitu sebesar 1238.89 dengan tebal slice sebesar 5 mm. Sedangkan ketebalan 10 mm diperoleh volume sebesar 1072.91 Sementara untuk hasil kalkulasi volume area jaringan lemak visceral terkecil sebesar 107.57 pada ketebalan 5 mm. Sedangkan ketebalan 10 mm diperoleh volume sebesar 47.43 . Evaluasi pada proses segmentasi dilakukan menggunakan metode SSIM dengan mengahasilkan nilai rata-rata SSIM untuk keseluruhan data sebesar 0.843 pada data latih dan 0.838 pada data uji. Dari hasil penelitian ini, sistem CAD berhasil dikembangkan untuk membantu dalam proses mengestimasi volume area jaringan lemak visceral. Namun, tingkat keakurasian antara kalkulasi volume lemak visceral menggunakan sistem CAD dan software CT-scan belum dapat diperoleh dengan baik.

The most precise examination in determining the volume of abdominal fat tissue is using a CT-scan modality. However, because each slice image has a different shape and location of visceral fat, it is not easy to determine the volume. So that, Computer Aided Diagnosis (CAD) can be used as a solution to assist experts in reading images and analyzing fat areas, especially visceral fat areas on abdominal images more accurate. In this study, a CAD system was developed using the Thresholding segmentation method, feature extraction based on Gray Level Co-Occurrence Matrix (GLCM) for the identification of abdominal fat. Next, in the classification process, the visceral fat area is separated from the subcutaneous fat area using Multilayer Perceptron (MLP). This study processed data from 665 abdominal CT-scan images from 38 patients obtained from Persahabatan Hospital. The data is divided into 70% images as training data and 30% images as test data. The results of the CAD system performance are represented as the level of accuracy with a value of 98.73% for training data and 95.58% for test data. In addition, information was also obtained that the calculation of the volume of visceral fat tissue areas with the largest value of 1238.89 with a slice thickness of 5 mm. While the thickness of 10 mm obtained a volume of 1072.91 Calculation of the volume of the volume area of ​​the smallest visceral fat tissue of 107.57 at 5 mm thickness.
