"ABSTRAK
Praktik Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak
dan Bunda Harapan Kita. Kegiatan ini berlangsung selama 6 minggu dari tanggal
7 September sampai dengan 16 Oktober 2015. PKP di rumah sakit bertujuan agar
mahasiswa apoteker mengerti peranan apoteker dalam pengelolaan sediaan
farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi
klinik, serta bekerjasama dengan profesi kesehatan lain. Berdasarkan kegiatan
PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di Rumah Sakit Anak
dan Bunda Harapan Kita memiliki peran, tugas, dan tanggung jawab dalam
pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai serta
pelayanan farmasi klinik. Mahasiswa apoteker telah berhasil memperoleh
wawasan mengenai kegiatan rutin yang dilakukan di rumah sakit. Kegiatan
pengelolaan meliputi pemilihan, perencanaan kebutuhan, pengadaan, penerimaan,
penyimpanan, pendistribusian, pemusnahan, pengendalian dan administrasi, serta
kegiatan farmasi klinis meliputi pengkajian dan pelayanan resep, pelayanan
informasi obat, visite, dan dispensing sediaan steril (penanganan obat sitostatik).
Kerjasama dengan tenaga kesehatan lain bertujuan untuk menjamin keselamatan
pasien demi terciptanya pelayanan kefarmasian yang bermutu.
ABSTRACT Profession Internship at pharmacy was held at Women?s and Children?s Hospital
Harapan Kita, West Jakarta. This activity was held for six weeks from September

2015. Profession Internship at pharmacy was intended to
make apothecary student understand the role of pharmacists, have insight into the
implementation of pharmaceutical practice, learning strategies and the
development of professional practice of pharmacists, have real description of the
issues of pharmaceutical practice in health centers, as well as to communicate and
interact with other health professionals. Based on the activities, it can be
concluded that the pharmacist at Women's and Children's Hospital Harapan Kita
have a role, duties and responsibilities in the management of pharmaceutical,
medical devices, and medical materials and consumables pharmaceutical care
clinic. Apothecary student have obtained insight into routine activities in
pharmacy. Pharmaceutical technical activities include selecting, requirement
planning, procurement, reception, storage, extermination, control and
administration, and activities include assessment of clinical pharmacy services
and prescription drug information service, visite, and dispensing sterile
preparations (handling cytostatic drugs). Cooperation with other health
professionals aims to ensure patient safety in order to create quality
pharmaceutical services.

"ABSTRAK
Praktik Kerja Profesi (PKP) di apotek bertempat di Apotek Kimia Farma No.96
Jalan Letjen S.Parman Kav.G/12 Jakarta Barat. Kegiatan ini berlangsung selama
empat minggu dari tanggal 3 sampai dengan 30 Agustus 2015. PKP di apotek
bertujuan agar mahasiswa apoteker mengerti peranan, tugas, dan tanggung jawab
Apoteker Pengelola Apotek (APA), memiliki wawasan, pengetahuan, serta
pengalaman praktis tentang pelaksanaan pekerjaan kefarmasian, dan memiliki
gambaran nyata akan permasalahan pekerjaan kefarmasian yang terjadi di apotek.
Berdasarkan kegiatan PKP yang dilakukan, APA di Apotek Kimia Farma No.96
Jalan Letjen S.Parman Kav.G/12 Jakarta Barat secara umum telah menjalankan
fungsi pengawasan dan pengendalian terhadap semua komponen obat di apotek dan
juga melaksanakan fungsinya sebagai apoteker dalam menjamin penggunaan obat
yang rasional. Pengelolaan pelayaan kefarmasian yang dilakukan di apotek meliputi
perencanaan kebutuhan obat, pengadaan obat, pendistribusian obat, penyimpanan
obat serta pelayanan informasi obat untuk obat dengan resep dan obat non resep
atau swamedikasi. Praktek pelayanan kefarmasian di Apotek Kimia Farma No. 96
secara keseluruhan telah berjalan sesuai dengan peraturan perundang-undangan
yang berlaku. Masalah yang terjadi di Apotek Kimia Farma No.96 adalah
kurangnya kedisiplinan petugas farmasi dalam melakukan pencatatan barang di Apotek.

"ABSTRAK
Praktik Kerja Profesi (PKP) di puskesmas bertempat di Puskesmas Kecamatan
Kembangan, Jakarta Barat. Kegiatan ini berlangsung selama tiga minggu dari
tanggal 30 Juni hingga 24 Juli 2015. PKP di puskesmas bertujuan agar mahasiswa
apoteker mengerti peranan apoteker, memiliki wawasan tentang pelaksanaan
pekerjaan kefarmasian, mempelajari strategi dan pengembangan praktik profesi
apoteker, memiliki gambaran nyata akan permasalahan pekerjaan kefarmasian di
puskesmas, serta berkomunikasi dan berinteraksi dengan tenaga kesehatan lain.
Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di
Puskesmas Kecamatan Kembangan telah melaksanakan kewajibannya dengan baik.
Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin
yang dilakukan di puskesmas. Kegiatan manajerial meliputi perencanaan
kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pengendalian,
pencatatan, pelaporan, dan pengarsipan obat, serta pemantauan dan evaluasi
pengelolaan obat dan bahan medis habis pakai, dan kegiatan farmasi klinis meliputi
pelayanan resep (dispensing), Pelayan Informasi Obat (PIO), konseling, homecare,
dan penyuluhan kesehatan. peningkatan mutu pelayanan kefarmasian dilakukan
dengan memanfaatkan perkembangan teknologi seperti penggunaan perangkat
lunak dan website. Masalah yang terjadi di puskesmas adalah keterbatasan sarana
dan prasarana pendukung kegiatan pelayanan kefarmasian.
