"Pelayanan Kefarmasian di Rumah Sakit adalah salah satu bagian penting dalam sistem pelayanan kesehatan Rumah Sakit yang berfokus pada pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinik. Rumah Sakit Universitas Indonesia (RS UI) menyediakan berbagai fasilitas dan pelayanan kesehatan, salah satunya yaitu Unit Central Operation Theater (COT) yang berfokus pada penyediaan layanan prosedur tindakan operasi atau pembedahan, salah satunya yaitu tindakan CAG/PCI. Tindakan CAG/PCI adalah rangkaian tindakan terhadap jantung. Depo Instalasi Farmasi OK (Operatio Kamer) berperan dalam mendukung kegiatan unit COT dengan menyediakan berbagai paket kebutuhan tindakan operasi. Penelitian ini bertujuan untuk mengevaluasi penggunaan standar paket tindakan CAG/PCI yang telah disiapkan untuk mengoptimalkan efisiensi dan efektivitas pelayanan dan pengelolaan sediaan farmasi di unit farmasi OK RS UI. Pengambilan data dilakukan secara retrospektif menggunakan data periode Juni – Agustus 2023. Data kemudian diolah menggunakan Microsoft Excel dan dinilai kesesuaiannya per pasien, per item, dan per bulan. Hasil analisis menunjukkan bahwa penggunaan item tindakan CAG/PCI pada 77 tindakan pasien tidak sesuai dengan paket standar. Penggunaan 14% item paket standar telah sesuai dengan jumlah pada paket standar, sedangkan 51% item melebihi paket standar dan 35% lainnya di bawah paket standar. Selama bulan Juni – Agustus 2023, penggunaan item untuk tindakan CAG/PCI melebihi dari paket standar.

Pharmaceutical services in hospitals are an important part of the hospital health service system which focuses on the management of pharmaceutical supplies, medical devices and consumable medical materials as well as clinical pharmacy services. The University of Indonesia Hospital (Rumah Sakit Universitas Indonesia) provides various health facilities and services, one of which is the Central Operation Theater (COT) Unit which focuses on providing surgical procedures or surgical procedures, one of which is CAG/PCI procedures. The OK (Operatio Kamer) Pharmacy Unit plays a role in supporting the activities of the COT unit by providing various packages of operational needs. This study aims to evaluate the use of the standard CAG/PCI action package that has been prepared to optimize the efficiency and effectiveness of service and management of pharmaceutical preparations in the OK UI Hospital pharmacy unit. Data collection was carried out retrospectively using data for the period June – August 2023. The data was then processed using Microsoft Excel and assessed for suitability per patient, per item and per month. The results of the analysis showed that the use of CAG/PCI action items in 77 patient procedures was not in accordance with the standard package. The use of 14% of standard package items is in accordance with the amount in the standard package, while 51% of items exceed the standard package and the other 35% are below the standard package. During June – August 2023, item usage for CAG/PCI actions exceeds that of the standard package."

"Air merupakan bahan yang paling banyak digunakan dalam kehidupan, termasuk proses produksi berbagai jenis industri. Air memiliki beberapa tingkatan kualitas yang dibedakan berdasarkan proses pemurniannya, salah satu jenisnya adalah air murni. PT Forsta Kalmedic Global menggunakan air murni untuk keperluan sanitasi dan bahan pendukung pembuatan kultur media yang terdistribusi dalam beberapa ruangan produksi. Pemantauan kualitas air murni secara rutin dilakukan untuk memastikan bahwa air murni yang digunakan dalam rangkaian proses produksi memenuhi persyaratan kimia dan mikrobiologis yang ditetapkan. Pada tugas akhir ini, evaluasi terhadap hasil pemantauan air murni, khususnya pada ruangan Mixing selama periode bulan Agustus – Desember 2022 dilakukan untuk menilai kualitas dan kesesuaiannya terhadap persyaratan air murni yang telah ditentukan. Evaluasi yang dilakukan meliputi uji total karbon (TOC), uji konduktivitas, uji pH, dan uji microbial limit. Pengambilan data dilakukan secara retrospektif dengan mengumpulkan data hasil pemantauan air murni bulan Agustus – Desember 2022 yang kemudian dievaluasi berdasarkan persyaratan USP. Hasil evaluasi terhadap total karbon, konduktivitas, pH, dan total mikroba berturut-turut adalah 0.60 – 44.90 ppb; 0.086 – 0.982 uS/cm;, 6.30 – 6.90; dan <1 CFU/mL. Berdasarkan hasil tersebut, kualitas air murni PT Forsta Kalmedic Global yang dihasilkan oleh mesin pada bulan Agustus – Desember 2022 telah memenuhi spesifikasi.

Water is the most widely used material in life, including the production processes of various types of industry. Water has several levels of quality which are differentiated based on the purification process, one type of which is purified water. PT Forsta Kalmedic Global uses purified water for sanitation purposes and as supporting material for making culture media which is distributed in several production rooms. Purified water quality monitoring is routinely carried out to ensure that the purified water used in the production process meets the specified chemical and microbiological requirements. In this research, an evaluation of the results of purified water monitoring, especially in the Mixing room during the period August – December 2022 was carried out to assess its quality and suitability to the specified purified water requirements. Evaluations carried out include total carbon (TOC) tests, conductivity tests, pH tests and microbial limit tests. Data collection was carried out retrospectively by collecting purified water monitoring data from August – December 2022 which was then evaluated based on USP requirements. The evaluation results of total carbon, conductivity, pH and total microbes were respectively 0.60 – 44.90 ppb; 0.086 – 0.982 uS/cm;, 6.30 – 6.90; and <1 CFU/mL. Based on these results, the quality of PT Forsta Kalmedic Global's purified water produced by the machine in August – December 2022 has met specifications."

"Puskesmas adalah fasilitas kesehatan tingkat pertama yang menjadi salah satu pilihan terdekat untuk mendapatkan layanan kesehatan. Puskesmas Kecamatan Kalideres memiliki beberapa pelayanan kesehatan, salah satunya adalah Poli Penyakit Tidak Menular (PTM). Pasien penyakit tidak menular pada umumnya memerlukan pengendalian kondisi yang lama atau pengobatan jangka panjang serta umumnya mendapatkan lebih dari satu macam obat. Kondisi ini menyebabkan pasien rentan mengalami Drug Related Problem (DRPs). Unit farmasi puskesmas bertanggung jawab terhadap pelayanan farmasi kepada pasien untuk memaksimalkan efikasi dan meminimalkan efek samping. Pada penelitian ini, dilakukan identifikasi pada lima pasien rawat jalan Poli PTM Puskemas Kalideres bulan Mei 2023 untuk mengetahui jenis dan angka kejadian DRP. Pengambilan data dilakukan secara retrospektif dan penilaian DRPs dilakukan berdasarkan klasifikasi Hepler-Strand. Dari hasil kajian, jenis kejadian DRPs yang ditemukan meliputi dosis subterapetik (2), dosis berlebih (1), dan interaksi obat bersifat major (6).

Distrct health center is a first level health facility which is one of the closest options for getting health services. The Kalideres District Health Center has several health services, one of which is the Non-Communicable Diseases polyclinic. Patients with non-communicable diseases generally require long-term condition control or long-term treatment and generally receive more than one type of medication. This condition makes patients vulnerable to experiencing Drug Related Problems (DRPs). The pharmacy unit in district health center is responsible for providing pharmaceutical services to patients to maximize efficacy and minimize side effects. In this study, five outpatient with non-communicable diseases were identified at the Kalideres District Health Center in May 2023 to determine the type and incidence of DRP. Data collection was carried out retrospectively and DRPs assessment was carried out based on the Hepler-Strand classification. From the results of the study, the types of DRPs found included subtherapeutic doses (2), overdoses (1), and major drug interactions (6)."

"Hipertensi adalah penyakit kronis yang menjadi salah satu masalah kesehatan baik secara nasional maupun global. Penyakit ini bersifat progresif dan tidak memiliki gejala sehingga efeknya pada kesehatan pasien dapat terjadi setelah bertahun-tahun. Dengan sifat asimtomatik penyakit ini menyebabkan kepatuhan pengobatan pasien hipertensi menjadi aspek penting dalam mencapai keberhasilan terapi pengobatan berupa tekanan darah yang terkontrol. Apoteker sebagai tenaga kesehatan bertugas memberikan pelayanan farmasi klinik untuk mencapai keberhasilan terapi pasien. Penelitian ini dilakukan dengan metode Literature Review dengan tujuan untuk mengetahui pemantauan terapi obat pasien hipertensi di fasilitas apotek. Pemantauan terapi pengobatan hipertensi dilakukan untuk memastikan pasien menerima pengobatan hipertensi yang aman, efektif dan rasional untuk mencapai tekanan darah yang terkontrol. Proses kegiatan ini merupakan proses yang komprehensif dimulai dari seleksi pasien, pengumpulan data pasien, identifikasi masalah terkait obat, rekomendasi terapi, rencana pemantauan sampai dengan tindak lanjut atau komunikasi. Pada tahap perencanaan pemantauan, apoteker juga harus menetapkan interval pemantauan, pemantauan fungsi ginjal dan hati, dan target terapi hipertensi. Selama proses pelaksanaan, apoteker harus memiliki pemahaman menyeluruh tentang pengobatan dimulai dari informasi kontraindikasi obat, interaksi obat, hingga efek samping yang dapat muncul dari obat-obat terapi antihipertensi serta manajemen yang harus dilakukan. Kepatuhan pasien hipertensi perlu dinilai, dipantau dan dianalisis untuk mencapai tujuan pengobatan. Penilaian dapat dilakukan melalui metode pelaporan mandiri atau kuesioner, wawancara atau menghitung jumlah pil sisa. Implementasi pelayanan telefarmasi hipertensi di apotek sesuai standar pelayanan kefarmasian juga dapat dilakukan untuk memantau dan meningkatkan kepatuhan pengobatan pasien hipertensi.

Hypertension is a chronic disease that is a health problem both nationally and globally. It is a progressive disease and has no symptoms so its effects on the patient's health can occur after many years. With the asymptomatic nature of this disease, compliance with treatment for hypertensive patients becomes an important aspect in achieving successful treatment therapy in the form of controlled blood pressure. Pharmacists as health workers are tasked with providing clinical pharmacy services to achieve successful patient therapy. This research was conducted using the Literature Review method with the aim of knowing the monitoring of drug therapy for hypertensive patients in pharmacy facilities. Hypertension treatment therapy monitoring is carried out to ensure patients receive safe, effective and rational hypertension treatment to achieve controlled blood pressure. This activity process is a comprehensive process starting from patient selection, patient data collection, identification of drug-related problems, therapy recommendations, monitoring plans to follow-up or communication. At the monitoring planning stage, pharmacists must also determine monitoring intervals, monitoring kidney and liver function, and hypertension therapy targets. During the implementation process, pharmacists must have a comprehensive understanding of medication starting from information on drug contraindications, drug interactions, to side effects that can arise from antihypertensive therapy drugs and the management that must be carried out. Hypertensive patient compliance needs to be assessed, monitored and analyzed to achieve treatment goals. Assessment can be carried out through self-reporting methods or questionnaires, interviews or counting the number of remaining pills. Implementation of hypertension telepharmacy services in pharmacies according to pharmaceutical service standards can also be carried out to monitor and improve medication compliance for hypertension patients."

Mesin Chest Freezerdigunakan untuk menyimpan ice packyang berguna untuk pengiriman produk rantai dingin. Kualifikasi mesin terdiri dari beberapa tahap kualifikasi yang dilakukan sebelum mesin digunakan untuk operasional keseharian. Pada penelitian ini dilakukan kualifikasi operasional mesin sesuai dengan prosedur kualifikasi perusahaan. Kualifikasi operasional (OQ) dilakukan untuk menetapkan bahwa instalasi dan semua sistem dan subsistem beroperasi secara efektif dan konsisten ketika area penyimpanan kosong. Hasil kualifikasi operasional untuk Chest FreezerAB-1200-T-X ID. P05 keadaankosongtelah dilaksanakan dengan hasil memenuhi kriteria penerimaan dengan adanya justifikasi. Oleh karena itu, pengujian kualifikasi dapat dilanjutkan ke tahap kualifikasi kinerja.

Pharmaceutical distributors are an important role in the pharmaceutical product supply chain. Pharmaceutical distributors must meet the requirements for Good Distribution Practice (CDOB) to guarantee the quality of the products distributed by ensuring that the quality throughout the process meets the requirements. PT Enseval Putera Megatrading, Tbk is a distributor that distributes various pharmaceutical products, including cold chain products. Hence, it is equipped with cold chain product support facilities. Chest Freezer machines are used to store ice packs which are useful for sending cold chain products. Machine qualification consists of several qualification stages which are carried out before the machine is used for daily operations. In this research, operational qualification was carried out in accordance with the company's qualification procedures. Operational qualification (OQ) is performed to establish that the installation and all systems and subsystems operate effectively and consistently when the storage area is empty. Operational qualification results for Chest Freezer AB-1200-T-X ID. P05 empty condition has been implemented with the results meeting the acceptance criteria with justification. Therefore, qualification testing can proceed to the performance qualification stage."
