"ABSTRAKKegiatan praktek kerja profesi di Apotek Kimia Farma dilakukan agar calon apoteker mampu memahami peranan, tugas, dan tanggung jawab apoteker di apotek sesuai dengan ketentuan standar pelayanan farmasi di apotek. Selain itu juga, diharapkan agar calon apoteker memahami tugas dan tanggung jawab Apoteker Pengelola Apotek baik secara teknis maupun non teknis farmasi sesuai dengan ketentuan peraturan perundang-undangan dan etika yang berlaku. Dalam melakukan pelayanan kefarmasian, petugas wajib mengikuti rangkaian tahap pelayanan resep sesuai prosedur. Pada tugas khusus, dilakukan kegiatan pelayanan kefarmasian di rumah Home Pharmacy Care pada pasien asma. Pelayanan kefarmasian di rumah Home Pharmacy Care dilakukan dengan melakukan konseling dan telefarma terlebih dahulu kepada pasien kronis yang terpilih dan selanjutnya dilakukan Home Pharmacy Care.

"ABSTRAKKegiatan praktek kerja profesi di Puskesmas Kecamatan Pancoran dilakukan agar calon apoteker mampu memahami peranan, tugas, dan tanggung jawab apoteker di puskesmas sesuai dengan ketentuan standar pelayanan farmasi di puskesmas. Selain itu juga, diharapkan agar calon apoteker mempelajari mengenai strategi dan pengembangan praktek profesi di puskesmas. Dalam melakukan pelayanan kefarmasian, petugas depo farmasi wajib mengikuti rangkaian tahap pelayanan resep sesuai prosedur. Pada tugas khusus, dilakukan pelayanan penyuluhan Diabetes Melitus tipe 2. Penyuluhan dilakukan dengan cara mengumpulkan pasien Diabetes Melitus Tipe 2 dan dengan menggunakan powerpoint untuk menjelaskan materi Diabetes Melitus tipe 2.

The counseling is done by gathering Diabetes Mellitus Type 2 patient and by using powerpoint to explain the material of Type 2 Diabetes Mellitus."

"ABSTRAKKegiatan praktek kerja profesi di RSAB Harapan Kita dilakukan agar calon apoteker mampu memahami peranan, tugas, dan tanggung jawab apoteker di rumah sakit sesuai dengan ketentuan standar pelayanan farmasi di rumah sakit. Selain itu juga, diharapkan agar calon apoteker mempelajari mengenai struktur rumah sakit, instalasi farmasi, serta jenis pelayanan yang tersedia di rumah sakit. Dalam melakukan pelayanan kefarmasian, petugas depo farmasi wajib mengikuti rangkaian tahap pelayanan resep sesuai prosedur. Pada tugas khusus, dilakukan analisis Drug Related Problem DRP pada neonatus prematurium yang menderita Respiratory Distress Syndrome. Analisa dilakukan dengan melihat rekam medis dan catatan perkembangan pasien, mengamati perkembangan pasien selama 5 hari dan melakukan analisis Drug Related Problem DRP pada pasien dengan cara melakukan SOAP.

In doing pharmaceutical care, pharmacy officer must follow the stages in prescription service procedure. On special assignment, Drug Related Problem on Prematorium Neonates who suffer from Respiratory Distress Syndrome was analyzed. Analysis was done by view medical records and patient development records, observe patient progress for 5 days and Drug Related Problem analysis with SOAP."
