"Penyakit jantung koroner (PJK) menyebabkan ketidakseimbangan suplai dan kebutuhan metabolik miokard dalam melakukan fungsi sirkulasi dan homeostasis. Baku emas terapi PJK adalah bedah pintas arteri koroner (BPAK). Prosedur BPAK dengan mesin pintas jantung paru (PJP) dapat mencetuskan cedera miokard tingkat selular sehingga memerlukan aplikasi proteksi miokard. Glutamin adalah asam amino conditionally essential yang berperan dalam proteksi miokard dengan membentuk energi selama periode iskemia, tetapi belum teruji penggunaannya pada pasien dengan fraksi ejeksi rendah. Padahal, pasien fraksi ejeksi (EF) rendah lebih rentan terhadap cedera miokard, sehingga glutamin diharapkan dapat memberi proteksi. Penelitian menggunakan desain double blind randomized controlled trial di Instalasi Bedah Jantung Dewasa RSJPDHK Jakarta pada bulan Januari–Agustus 2021 dengan subjek penelitian 60 pasien sesuai kriteria inklusi dan eksklusi. Alokasi random subjek untuk memilih 30 pasien mendapatkan 500 mL glutamin 0,5 g/kg dalam NaCl 0,9% sebagai kelompok intervensi (glutamin), dan 30 pasien mendapatkan NaCl 0,9% sebanyak 500 mL sebagai kelompok kontrol selama 24 jam pertama. Pengukuran yang dilakukan meliputi kadar glutamin plasma, kadar α-KG, myocardial injury score, indeks apoptosis, ekspresi anti-kardiak troponin I, kadar troponin I, EF, indeks jantung dan kadar laktat. Dua subjek drop out sehingga analisis dilakukan terhadap 58 subjek. Efek proteksi miokard glutamin terlihat pada kadar troponin I, laktat plasma, dan myocardical injury score yang lebih rendah pada kelompok glutamin, serta ekspresi anti-kardiak troponin I jaringan apendiks atrium kanan jantung setelah mesin PJP dilepas lebih tinggi dibandingkan kontrol. Tidak didapatkan perbedaan bermakna indeks apoptosis jaringan apendiks atrium kanan, fraksi ejeksi pasca-operasi, penggunaan vasoaktif dan inotropik pasca-operasi, durasi penggunaan ventilator dan durasi perawatan intensif pasca-operasi pada kedua kelompok. Simpulan: Pemberian preoperatif glutamin 0,5 g/kg secara intravena dalam 24 jam pertama memiliki efek proteksi miokard pada pasien BPAK elektif dengan EF rendah yang menggunakan mesin PJP.

Coronary heart disease (CHD) causes a myocardial metabolic supply and demand imbalance in performing circulatory and homeostatic functions. The gold standard treatment of CHD is coronary artery bypass graft (CABG). The CABG procedure with a cardiopulmonary bypass (CPB) machine can trigger myocardial injury at cellular level due to ischemia and reperfusion. Glutamine is a conditionally essential amino acid in the human body which has a role as myocardial protector through energy production during myocardial ischemia. However, its application has not been tested in low ejection fraction (EF) patients. Meanwhile, patients with low EF are more vulnerable to myocardial injury. Thus, glutamine administration was expected to provide myocardial protection. The study was a double-blind, randomized controlled trial design and was performed at the Adult Cardiac Surgery Installation of the National Cardiovascular Center Harapan Kita, Jakarta from January to August 2021 with a sample size of 60 patients meeting the inclusion and exclusion criteria. Subjects were randomly allocated into intervention (glutamine): 30 patients were administered a solution of glutamine 0.5 g/kg dissolved in 0.9% NaCl up to 500 mL in total volume and control group; 30 patients were administered 500 mL of 0.9% NaCl, both over a period of the first 24 hours. Parameters measured include plasma glutamine levels, α-KG levels, myocardial injury scores, apoptotic index, anti-cardiac troponin I expression, troponin I levels, EF, cardiac index and lactate levels. Two samples were dropped out; hence 58 patients were analyzed in this study. Myocardial protective effects of glutamine are observed in plasma troponin I, lactate levels, and myocardial injury score of right atrial appendage tissue, which were significantly lower in the glutamine group and higher anti-cardiac troponin I expression of right atrial appendage tissue in the glutamine group. Apoptotic index of right atrial appendage tissue, postoperative ejection fraction, postoperative use of vasoactive and inotropic, ventilator time, and duration of intensive care showed no significant differences in both groups. Conclusion: Preoperative administration of intravenous glutamine 0.5 g/kg in the first 24 hours has a cardioprotective effect in low EF patients underwent elective on-pump CABG."

"Diketahui bahwa insidensi infeksi sekunder bakterial cukup tinggi pada pasien ICU COVID-19 yang diduga dapat memengaruhi kondisi kesehatan pasien. Penelitian ini meneliti hubungan antara infeksi sekunder bakterial terhadap lama rawat serta outcome mortalitas pasien ICU COVID-19. Penelitian dilakukan terhadap data rekam medis pasien ICU COVID-19 di RSCM menggunakan desain penelitian cohort retrospektif. Data yang dihimpun berupa data demografis (usia dan jenis kelamin), status infeksi sekunder bakterial, lama rawat, dan outcome mortalitas. Analisis hubungan infeksi sekunder bakterial dengan lama rawat dilakukan dengan analisis regresi linear, sedangkan hubungan infeksi sekunder bakterial dengan mortalitas dilakukan dengan analisis regresi logistik biner. Dari 173 pasien ICU COVID-19 yang memenuhi kriteria inklusi, hampir separuhnya (47,98%; n=83) mengalami infeksi sekunder bakterial. Median usia pasien secara keseluruhan adalah 20 (1–80) tahun, dengan 54,9% pasien berjenis kelamin laki-laki. Adanya infeksi sekunder bakterial dinilai memperpanjang lama rawat pasien selama 12,5 hari (R=0,911; B coefficient=12,486; p=0,000). Selain itu, infeksi sekunder bakterial juga meningkatkan risiko kematian sebesar 3,993 kali lipat (OR=3,993; CI=95%; p=0,020). Sehingga dapat disimpulkan bahwa terdapat hubungan yang signifikan antara infeksi sekunder bakterial dengan lama rawat dan outcome mortalitas pasien.

It is known that the incidence of secondary bacterial infections is quite high among COVID-19 patients in the ICU, which is suspected to affect the patients' health conditions. This study examines the relationship between secondary bacterial infections and the length of stay as well as the mortality outcome of COVID-19 ICU patients. The research was conducted on the medical records of COVID-19 ICU patients at RSCM using a retrospective cohort study design. Data collected included demographic information (age and gender), the status of secondary bacterial infections, length of stay, and mortality outcome. The relationship between secondary bacterial infections and the length of stay was analyzed through linear regression, while the association between secondary bacterial infections and mortality was examined using binary logistic regression. Out of 173 COVID-19 ICU patients who met the inclusion criteria, almost half (47.98%, n=83) experienced secondary bacterial infections. The overall median age of the patients was 20 years (range: 1–80 years), with 54.9% of patients being male. The presence of secondary bacterial infections was found to significantly extend the length of stay for patients by 12.5 days (R=0.911; B coefficient=12.486; p=0.000). Additionally, secondary bacterial infections increased the risk of mortality by 3.993 times (OR=3.993; CI=95%; p=0.020). Hence, it can be concluded that there is a significant relationship between secondary bacterial infections, the length of stay, and the mortality outcome of patients."

"Latar Belakang Anatomi merupakan topik pembelajaran dasar yang penting bagi mahasiswa kedokteran. Namun, pandemi Covid-19 menyebabkan keterbatasan dalam pembelajaran anatomi. Salah satu metode pembelajaran anatomi yang mudah digunakan oleh mahasiswa kedokteran dalam mempelajari anatomi adalah penggunaan aplikasi 3D. Penelitian ini bertujuan untuk membandingkan nilai pre-test dan post-test praktikum anatomi hepatobilier mahasiswa yang menggunakan aplikasi anatomi 3D dibanding metode 2D pada anatomi hepatobilier. Metode Penelitian ini dilakukan dengan desain quasi-eksperimental menggunakan 34 sampel mahasiswa FKUI tingkat 2 yang dibagi menjadi dua kelompok, yaitu kelompok intervensi diberi materi 2D dan 3D dan kelompok 2D yang mendapat materi 2D saja. Penelitian dilakukan pada tahun 2023. Masing-masing kelompok diberikan sejumlah pertanyaanpertanyaan pre-test dan post-test terkait anatomi hepatobiliar. Data dianalisis dengan uji statistik Mann-Whitney dengan SPSS versi 24. Hasil Hasil penelitian ini menunjukkan adanya perbedaan signifikan nilai pre-test dan post-test praktikum anatomi hepatobilier antara kelompok 2D dan kelompok intervensi (p <0,05) dengan kelompok intervensi memiliki peningkatan nilai yang lebih tinggi. Didapatkan distribusi nilai kelompok eksperimental adalah 30 (10–80) dan kelompok 2D adalah 20 (0–80). Kesimpulan Hasil penelitian ini menyimpulkan terdapat perbedaan signifikan pada nilai pre-test dan post-test praktikum anatomi hepatobilier mahasiswa yang menggunakan aplikasi anatomi 3D dibanding metode 2D pada anatomi hepatobilier.

Introduction Anatomy is an important foundation of knowledge for medical student. The Covid-19 pandemic causes limitation in student’s anatomy study. An alternative anatomy learning method that is accessible to medical student is by using 3D anatomy application. This study wants to compare the pre-test and post-test hepatobiliary anatomy practical score between students who learn using 3D anatomy application and students who learn only using 2D anatomy materials. Method This study was conducted on year 2023 using quasi-experimental study with 34 participants from FKUI 2nd year student. The participants were divided into experimental group, who received both 3D and 2D materials, and 2D group that only received 2D materials. Each group were given the same pre-test and post-test question about hepatobiliary anatomy. The data was then analyzed using Mann-Whitney test on SPSS version 24. Result The result of analysis shows that there was a significant difference between pre-test and post-test hepatobiliary anatomy practical score between the experimental group and the 2D group (p <0.05). The experimental group had higher increase in score compared to 2D group. The score distribution for experimental group and 2D group was 30 (10–80) and 20 (0-80) in order. Conclusion This result shows that there was a significant difference in pre-test and post-test hepatobiliary anatomy practical score between students who use 3D anatomy application and students who only learn with 2D anatomy materials."
